302. Outcomes of radical cystectomy in pT4 bladder cancer frail patients: A high-volume single center study.

Abstract

Objectives: This study aims to evaluate the effect of frailty in patients undergoing radical cystectomy (RC) for locally advanced bladder cancer. Methods: In this retrospective, single center study we evaluated 51 patients with pT4 bladder cancer treated with radical cystectomy between 2016-2020. Patient frailty was assessed with the Clinical Frailty Scale (CFS). Furthermore, six separate parameters (early mortality index within 30 days after surgery, death after one year, length of stay, respiratory complications, readmission index, total hospital charges) were also evaluated. The patients were categorized on three groups (Group 1, 2, 3) based on the CFS. Results: A total of 51 pT4 RC patients were included in the study. Mean age was 75.6 years. Early mortality rate at 30 days after surgery was low all the groups. One year mortality rate was higher in Group 2 (22%) and 3 (69%). The length of stay and the number of patients with respiratory complications were also higher in the frailer groups. 30 days readmission rate was 22% in Group 2 and 38% in Group 3. Conclusions: Preoperative frailty is associated with worse postoperative results after RC. CFS is an objective tool for patient risk stratification and can predict postoperative complications and mortality. [ABSTRACT FROM AUTHOR]

Abstract

International audience; The emergence of the Internet of Things (IoT) has made wireless connectivity ubiquitous and necessary. Extending the IoT to the Industrial Internet of Things (IIoT) places significant demands in terms of reliability on wireless connectivity. The Institute of Electrical and Electronics Engineers (IEEE) Std 802.15.4-2015 standard was designed in response to these demands, and the IPv6 over Low power Wireless Personal Area Networks (6LoWPAN) adaptation layer was introduced to address (among other issues) its payload size limitations by performing packet compression and fragmentation. However, the standardised method does not cope well with low link-quality situations and, thus, we present the state-of-the-art Forward Error Correction (FEC) methods and introduce our own contribution, Network Coding FEC (NCFEC), to improve performance in these situations. We present and analyse the existing methods as well as our own theoretically, and we then implement them and perform an experimental evaluation using the 6TiSCH simulator. The simulation results demonstrate that when high reliability is required and only low quality links are available, NCFEC performs best, with a trade-off between additional network and computational overhead. In situations where the link quality can be guaranteed to be higher, simpler solutions also start to be feasible, but with reduced adaptation flexibility.

Abstract

National audience; The IPv6 Routing Protocol for Low-Power and Lossy Networks (RPL) networks is designed for Internet of Things (IoT) networks to generate routes between devices with minimal processing. This protocol creates a DODAG (Destination Oriented Directed Acyclic Graph) network topology through the use of DODAG Information Object (DIO) control packets. The DODAG routes the data packets upstream to the destination device. In order to obtain a reliable network, we implement Packet Replication and Elimination (PRE) to perform multi-path data transmission via multiple parent devices. However, there is no standard way to select an alternative path. This document presents three types of Alternative Parent (AP) selection following a braided model. We focus on analyzing its performance in terms of delay and compromise between network traffic and reliability.; Le protocole de routage IPv6 pour les réseaux à faible puissance et fort taux de pertes (RPL) est conçu pour les réseaux Internet des objets (IoT) afin de générer des itinéraires entre les appareils avec un traitement minimal. Abstract

Bladder Cancer (BC) is11th most common malignancy. In terms of pathology, the vast majority of patients suffer from transitional cell carcinoma. Apart from this common type of BC, there are many morphological subtypes with different oncological characteristics. Plasmacytoid BC is a well-recognized subtype of BC with great diagnostic importance as it usually presents in locally advanced or even metastatic stage.The objective of this study was to evaluate our experience in diagnosing and treating patients with this rare BC subtype.A retrospective analysis of 5 patients diagnosed with plasmacytoid BC in our department during they ears 2014-2016 was performed. Transurethral resection of the tumors was performed in all patients and pathology diagnosis of plasmacytoid variant was based on several morphologic and immunohistochemical parameters. Staging included abdominal and thoracic CT.3 of 5 patients were diagnosed with metastatic disease. These patients were referred to the oncology department. 2 patients presented with non-metastatic BC after initial staging and thus a radical cystectomy was performed. Follow up of all patients was carried out and their survival was recorded.We concluded that despite the fact that the plasmacytoid variant of BC is rare, it is important to take into account the pathologic and clinical features of this tumor in order to manage the optimal treatment of this poor prognosis cancer.INTRODUCCIÓN: El cáncer de vejiga es el 11o cáncer mas común. Del punto de vista de la patología, la mayoría de pacientes tienen carcinoma urotelial de vejiga.A parte de la forma mas común, el cáncer de vejiga plasmocitoide es un subtipo ampliamente reconocido de cáncer de vejiga con una gran importancia diagnóstica ya que habitualmente se presenta como localmente avanzadoo en estadio metastático.OBJETIVO: El objetivo de este estudio fue evaluar nuestra experiencia en el diagnóstico y tratamiento en pacientes con esta entidad.MÉTODOS: Análisis retrospectivo de 5 pacientes diagnosticados con cáncer de vejiga plasmocitoide en nuestro departamento entre 2014 y 2016. Se realizó una resección transuretral de vejiga en todos los pacientes y un análisis histopatológico que demostró la variante plasmocitoide del punto de vista morfológico e immunohistoquímico. El estadiaje incluyó TAC toracoabdominopélvico.RESULTADOS: 3 de los 5 pacientes fueron diagnosticado sen estadio metastático. Estos pacientes pasaron a cargo del departamento de oncología medica. 2 pacientes se diagnosticaron en estadio localizado por lo que se realizó una cistectomía radical. Se realizó el seguimiento y se determinó la supervivencia de estos pacientes. CONCLUSIONES: Se concluye que todo y que el diagnóstico de cáncer de vejiga plasmocitoide es raro; es importante reconocer esta variedad histológica para poderdar un manejo optimo a esta variedad con muy mal pronostico.

Abstract

Due to uncontrolled interferences, including the self-induced multipath fading, deterministic networking can only be approached on wireless links. The radio conditions may change-way-faster than a centralized routing can adapt and reprogram, in particular when the controller is distant and connectivity is slow and limited. RAW separates the routing time scale at which a complex path is recomputed from the forwarding time scale at which the forwarding decision is taken for an individual packet. RAW operates at the forwarding time scale. The RAW problem is to decide, within the redundant solutions that are proposed by the routing, which will be used for each individual packet to provide a DetNet service while minimizing the waste of resources.

Implementing Packet Replication and Elimination from/to the RPL root requires the ability to forward copies of packets over different paths via different RPL parents. Selecting the appropriate parents to achieve ultra-low latency and jitter requires information about a node's parents. This document details what information needs to be transmitted and how it is encoded within RPL control packets to enable this functionality. The design and construction of this standard device was carried out at the Technical University of Crete and is described in detail., Στην συγκεκριμένη διπλωματική εργασία, πραγματοποιείται ανάλυση κύκλου ζωής ηλιακών συλλεκτών κενού και μελέτη περιβαλλοντικής απόδοσης με χρήση του λογισμικού SimaPro, καθώς επίσης γίνεται και αναλυτική μελέτη και ανάπτυξη λειτουργίας ενός πειραματικού ηλιακού θερμικού συστήματος για θέρμανση ζεστού νερού χρήσης. In order to support 1280-byte packets, as required for IPv6, SCHC includes a fragmentation functionality, since relevant LPWAN technologies offer very short data unit sizes and do not provide native fragmentation mechanisms. SCHC offers 3 fragmentation modes: No-ACK, ACK-Always, and ACK-on-Error, the latter being especially promising due to its reliability and high efficiency. In this article, we develop a mathematical model to compute the most critical performance parameters for the SCHC ACK-on-Error mode, namely, the acknowledgment traffic incurred by a fragment receiver for the successful delivery of a fragmented packet. The model is used to evaluate the SCHC ACK-on-Error mode performance, as well as to optimally tune its main parameters when used over LoRaWAN and Sigfox, for different packet sizes. Abstract

In this paper, a comparative experimental assessment of computer vision-based methods for sign language recognition is conducted. By implementing the most recent deep neural network methods in this field, a thorough evaluation on multiple publicly available datasets is performed. The aim of the present study is to provide insights on sign language recognition, focusing on mapping non-segmented video streams to glosses. For this task, two new sequence training criteria, known from the fields of speech and scene text recognition, are introduced. Furthermore, a plethora of pretraining schemes is thoroughly discussed. Finally, a new RGB+D dataset for the Greek sign language is created. To the best of our knowledge, this is the first sign language dataset where sentence and gloss level annotations are provided for a video capture.

Abstract

IEEE std 802.15.4-2015 Time Slotted Channel Hopping (TSCH) extends and replaces multiple proprietary industrial standards to support ultra-reliable and ultralow power operation in Industrial Internet of Things. Under TSCH, all devices must remain continuously synchronised with their time source neighbour. To do so, the nodes either periodically exchange control packets or data packets to compensate for clock drift. However, single time source neighbour scheme endangers de-synchronisation in the wireless network. In this letter, we propose using multiple time source parents to improve the synchronisation of the the network.

323. Towards open and expandable cognitive AI architectures for large-scale multi-agent human-robot collaborative learning

Abstract

Learning from Demonstration (LfD) constitutes one of the most robust methodologies for constructing efficient cognitive robotic systems. Despite the large body of research works already reported, current key technological challenges include those of multi-agent learning and long-term autonomy. Towards this direction, a novel cognitive architecture for multi-agent LfD robotic learning is introduced, targeting to enable the reliable deployment of open, scalable and expandable robotic systems in large-scale and complex environments. In particular, the designed architecture capitalizes on the recent advances in the Artificial Intelligence (AI) field, by establishing a Federated Learning (FL)-based framework for incarnating a multi-human multi-robot collaborative learning environment. The fundamental conceptualization relies on employing multiple AI-empowered cognitive processes (implementing various robotic tasks) that operate at the edge nodes of a network of robotic platforms, while global AI models (underpinning the aforementioned robotic tasks) are collectively created and shared among the network, by elegantly combining information from a large number of human-robot interaction instances. Regarding pivotal novelties, the designed cognitive architecture a) introduces a new FL-based formalism that extends the conventional LfD learning paradigm to support large-scale multi-agent operational settings, b) elaborates previous FL-based self-learning robotic schemes so as to incorporate the human in the learning loop and c) consolidates the fundamental principles of FL with additional sophisticated AI-enabled learning methodologies for modelling the multi-level inter-dependencies among the robotic tasks. The applicability of the proposed framework is explained using an example of a real-world industrial case study for agile production-based Critical Raw Materials (CRM) recovery.

Abstract

Introduction and objective: Non muscle invasive, high-risk, bladder cancer is an entity which is usually treated with radical cystectomy. Incidental prostate cancer refers to prostate cancer detected in radical cystectomy specimens in patients with no signs of the disease. Objective of this study is to report the prevalence, characteristics, and clinical significance of incidental prostate cancer in non-muscle invasive bladder cancer patients treated with radical cystectomy in our department. Material and methods: We retrospectively reviewed data from 41 patients who underwent radical cystectomy for non-muscle invasive, high risk, bladder cancer during the years 2016–2020 in our department. Prostate cancer was described as clinically significant when there were positive surgical margins, extraprostatic extension, Gleason score >6, or tumor volume ⩾0.5 cm3. Two groups of patients were formed according to the presence or absence of clinically significant prostate cancer. Results: Incidental prostate cancer in the cystectomy specimens was detected in 21 of the 35 patients investigated. Clinically significant prostate cancer was detected in five patients. Positive surgical margins and extraprostatic extension were present in one patient, respectively. Gleason score was more than six in four of the five patients and PCa tumor volume was above 0.5 cm3 in three patients. Two patients with clinically significant prostate cancer were diagnosed with biochemical recurrence during their follow up. Conclusions: In non-muscle invasive, high-risk patients undergoing radical cystectomy, clinically significant incidental PCa is an important issue as it may affect prognosis, quality of life, metastasis free survival, and overall survival.
