Analysis of nine patients with intracranial arachnoid cysts surgically treated from 1974 to 1995. The treatment consisted of cystoperitoneal shunt in seven patients and craniotomy with excision of cyst membrane in the other two cases. After surgery, bacterial meningitis and failure shunt device were observed in three cases. Both the infectious process and the shunt device were respectively treated and corrected at once. It has been observed total clinical improvement in all nine cases. A review of the literature has been made and some pathophysiological, diagnostic and therapeutic aspects are discussed.

The potential of an immunofluorescence test for detection of IgM antibodies against Schistosoma mansoni gut-associated antigens (IgM-IFT) was evaluated as a tool for studying aspects related to the schistosomiasis transmission in Ribeirão Pires, in the metropolitan area of the capital of the State of São Paulo, Brazil. Children from a school with about 400 students, 6 to 18 years, were followed-up for two years. In the five surveys, carried out at 6-month intervals, from October 92 to October 94, serological (IgM-IFT) prevalence indices of 5.3%, 5.8%, 6.2%, 2.9% and 3.3% were obtained. These indices were 7 to 10 times higher than the parasitological prevalence indices of 0.5%, 0.5%, 0.7%, 0.4% and 0% determined by the Kato-Katz method. Seroconversion from IFT negative to positive was indicating possible newly acquired S. mansoni infection in three children. But confirmation of infection by fecal examination was possible in only one child. The IgM-IFT can constitute a valuable tool for the improvement of the vigilance program in low endemic areas for schistosomiasis, better characterizing the S. mansoni transmission in such areas.

The experience with the surgical treatment of cerebellar astrocytomas in 25 children is reported. The clinical presentation, incidence, CT-scan diagnostic studies, pathology, recurrence and treatment aspects are discussed. The series included children until 10 years old with peak (7 cases) in the 7th year of age. The more frequent opening symptoms were: headache, vomit and gait disturbances. No surgical mortality occurred in the series. The authors conclude that surgical radical resection is the best therapeutics for this type of tumor and that radiotherapy is indicated only for tumors with malignant histology.

The albumin and globulin fractions from lentil seeds were isolated and characterised by gel filtration. The latter was shown to be homogeneous and the former heterogeneous on PAGE. The amino acid analysis revealed high values of amidic amino acids for both fractions with great differences in the sulphur-containing amino acids. Native albumin, globulin and salt-soluble proteins were markedly resistant to trypsin hydrolysis compared to casein. The SDS-PAGE of native salt-soluble proteins indicated that the globulin fragments (20 to 30 kD) were slowly digested in the presence of albumin. The heating increased the hydrolysis of the proteins in the order: salt-soluble, albumin and globulin. The facilitated hydrolysis of the heated salt-soluble fraction seemed to be due to protein-protein interactions induced by heat.

The authors present the results observed with large bifrontal decompressive craniotomy performed on 12 patients with severe cerebral edema, 10 of them related to cerebral contusion, which did not respond to conventional methods of therapy. All patients had before surgery very bad prognosis, with severe neurological signs of higher brain stem compression. Bilateral carotid angiography was sistematically performed before and after surgery, constituting as a matter of fact the decisive element indicating cerebral decompression. Six patients (50 per cent) survived and 5 of them (41.6 per cent) had an excellent neurological and mental improvement. Considering these results, we think that a large bifrontal decompressive craniotomy is the best method of treatment in such cases, specially when performed precociously. These patients, however, need very special care after surgery, if possible in units of intensive therapy, owing to the large incidence of complications.

After brief considerations on the symptomatology and ethiopathogeny of megadolichobasilar, five cases of this vascular anomaly are presented. Agreeing with the majority of the papers on the subject, the age of the patients was between the fourth and the sixth decades, all of them presenting arterial hypertension of long duration and advanced atherosclerosis; the neurological findings were varied, having in common the fact of an abrupt (ictal) beggining. The angiographic study of the vertebrobasilar system was basic in the diagnosis in every case.
