Abstract

Introduction: Laparoscopic radical nephrectomy is the gold standard for treatment of renal cell carcinoma. However, previous abdominal surgery is generally regarded as a contraindication to the laparoscopic approach because it causes intraperitoneal adhesions, which are thought to interfere with subsequent laparoscopic procedures inside the abdominal cavity. Few studies have examined the influence of prior surgery on laparoscopic nephrectomy. Therefore, the aim of this study was to evaluate the impact of previous laparotomy on laparoscopic nephrectomy. Methods: The records of 251 consecutive patients who had undergone laparoscopic nephrectomy for renal cell carcinoma at our hospital between 2005 and 2015 were reviewed retrospectively. Results: Of the 251 laparoscopic nephrectomy patients, 76 patients (30%) had undergone prior abdominal surgery (surgery group), whereas the remaining 175 patients (70%) had not previously had abdominal surgery (control group). There were no significant differences between the control group and the surgery group with regard to pneumoperitoneum time (143 vs 135 min, P = 0.241) or blood loss (39 vs 36 mL, P = 0.763). Next, we divided the patients into two cohorts based on surgeon experience: 98 patients had been treated by an expert (i.e. someone who performs more than 50 laparoscopic procedures per year) and 153 patients had been treated by a non‐expert. There was no significant difference in pneumoperitoneum time between the control and surgery groups in each cohort (treated by experts, 108 vs 103 min; treated by non‐experts: 162 vs 166 min). Conclusion: We conclude that laparoscopic nephrectomy may be feasible after previous abdominal surgery and could be one of the surgical options. [ABSTRACT FROM AUTHOR]

311. Evaluation of prostate-specific antigen density in the diagnosis of prostate cancer combined with magnetic resonance imaging before biopsy in men aged 70 years and older with elevated PSA.

Abstract

There is an increasing proportion of individuals aged 70 years and older, as well as an increasing life expectancy worldwide. The present study may guide the management of older patients with elevated prostate specific antigen (PSA). The medical records of 241 older men aged >70 years who underwent multiparametric magnetic resonance imaging (mpMRI) before prostate biopsy (PBx) at our institution were reviewed retrospectively. Multiple variables were evaluated as predictors for the diagnosis of prostate cancer (PCa). The variables included serum PSA level, digital rectal examination, size of region of interest on mpMRI, prostate volume and PSA density. PCa was positive in 162 (67.2%). Prostate volume and PSA density were significant PCa predictors (P<0.001). In patients aged 70–75 and >75 years, PSA density was significantly higher in patients with PCa (0.21 ng/ml/cc, P=0.014 and 0.24 ng/ml/cc, P<0.001, respectively). Similarly, PSA density was significant higher in patients with significant PCa (0.24 ng/ml/cc, P=0.004 and 0.29 ng/ml/cc, P<0.001, respectively). The cut-off value of PSA density was calculated using receiver operating characteristic curves. Area under curve of PSA density was 0.698, and the best cut-off value was 0.20 ng/ml/cc. These results indicate that the combination of PSA density with mpMRI before PBx is a helpful method and can be a decision-making model for a selection of PBx. [ABSTRACT FROM AUTHOR]

Abstract

著者らが行った体腔鏡下副腎摘除術27例(L群)の手術成績を, 開放性副腎摘除術21例(O群)と比較検討した.その結果, 手術時間は両群間で有意差を認めなかったが, 出血量は有意にL群で少なかった.O群と比べL群は, 術後の歩行開始, 経口摂取開始までの期間, 術後在院期間の中央値が有意に短く, 術後の抗疼痛薬の使用回数が有意に少なかった.L群でmajor complicationを認めたのは1例のみであり, その他の合併症はいずれも軽度のもので保存的に軽快した.合併症の発生率はL群, O群で有意差はなかった.L群を前期13例, 後期13例に分け検討したところ, 手術時間ならびに出血量は, ともに後期症例で有意に減少していた, Twenty-seven laparoscopic adrenalectomies (LapAdx) were performed at the National Defense Medical College between 1999 and 2004. We evaluated the results of LapAdx (group L) compared to the results of open adrenalectomy (group O). Twenty-six of the 27 LapAdx were successfully performed, but one patient with a large pheochromocytoma required open surgey because of arterial bleeding in the renal hilus. The mean operating time in group L (185 +/- 19 min) was not significantly different from that in group O (206 +/- 13 min). The mean estimated blood loss in group L (111 +/- 61 g) was significantly lower than that in group O (308 +/- 67 g). The starting time for oral feeding and for ambulation was significantly earlier in group L than in group O. There was a major complication (intraoperative bleeding) in which a group L patient required a blood transfusion. We also compared the surgical results of 26 patients in LapAdx divided chronologically into the first half and the last half to determine the surgical skill acquired. The operating time was significantly shorter and blood loss significantly less for patients in the last half. In addition, the operating time and blood loss for the first-time LapAdx operators were comparable with those of experienced surgeons. Our results support the efficacy and the minimal invasiveness of LapAdx. The accumulated experience and knowledge regarding laparoscopic surgery in our institute were important in improving surgical procedures and results.

Abstract

In men with castration-resistant prostate cancer, it might be beneficial to withdraw antiandrogens from patients who have favorable prognostic factors for a response to such withdrawal, notably receipt of hormone therapy for a long period. Antiandrogens should not be withdrawn in patients without favorable factors and in patients with a high prostate-specific antigen level at the time of cancer diagnosis.

Research team head, Research team member, Publisher, 腎癌において, メタボリックシンドロームへの重要な関与が指摘され始めた脂肪酸受容体GPR120の与える影響を検討した。各種腎細胞癌株において, GPR120の活性化による短期的な抗腫瘍効果をWST assayで調べたが明らかな抗腫瘍効果は認めず, またWestern blot法にてGPR120の発現も変化を認めなかった。agonistを定期投与することによる長期の抗腫瘍効果についても, 様々な条件下でも細胞増殖能やGPR120の発現に変化を認めなかったが, GPR120はその抗炎症化作用から癌との関連が予想されるため, 今回様々な条件の下で行った実験の過程や結果は今後の研究の一助となり得ると考えられた。 In renal cell carcinoma, we evaluated the impact of GPR120, a fatty acid receptor which has been identified as the important substance associated with metabolic syndrome. For examining short-term antitumor effect by stimulating GPR120, cell viability was measured subsequently after the exposure of agonists using WST-assay in a variety of renal cell carcinoma cell lines. We observed no obvious inhibition of proliferation and no change of GPR120 activity over time by western blot method. Although we also evaluated the long-term cytotoxicity by regular GPR120 stimulation, we found no remarkable change concerning the cellular proliferative potential and expression of GPR120 by western blot analysis regardless of conditions. Abstract

89歳男.肉眼的血尿が出現し, 排泄性尿路造影, 逆行性尿路造影, 腹部CTなどで左尿管腫瘍と診断, シスプラチン併用放射線療法を施行し, いったん腫瘍縮小, 血尿が改善したが再発した.排泄性尿路造影で左腎のやや描出不良, 逆行性尿路造影で水腎症, 下部尿管陰影欠損を認めた.また, 左尿管尿細胞診はクラスIV, 腹部CTで左水腎症と左下部尿管壁肥厚を認めた.腫瘍による血尿が著明でコントロール困難なため左腎尿管摘出術を施行した..腎盂腫瘍はpT3, 尿管腫瘍はpT1の癌肉腫で手術経過は良好で血尿は消失し, 術後1年5ヵ月経過観察中で転移, 再発は認めていない, We report a case of carcinosarcoma of the renal pelvis and ureter arising in an 89-year-old man who presented at our hospital with gross hematuria. Abdominal computed tomography, excretory pyelography, and retrograde pyelography demonstrated that left hydronephrosis was caused by an ureteral tumor. Left urine cytology indicated transitional cell carcinoma. The patient underwent chemotherapy and radiation therapy. However, gross hematuria recurred, and the patient underwent left nephroureterectomy. The surgical specimen showed carcinosarcoma in the renal pelvis and ureter histologically. He has been free of cancer for 1.5 years.

Abstract

Research team head, Research team member, Publisher, 転移を伴う腎細胞癌に対しては分子標的薬が治療の中心となり, 治療成績は向上した。しかし, 根治が難しいのは薬に効かないがん細胞が残存するからである。我々はこの残存するがん細胞の特徴として, 形態学的には肉腫瘍の変化を伴い, 上皮間葉転換をきたしており, 機序として癌幹細胞のマーカーであるCD44の高発現とTumor Necrosis Factor (TNF)-αによるCD44発現の誘導を発見した。 Molecular targeted agents are widely used and better survival benefits are shown for the treatment of metastatic renal cell carcinoma (RCC). However, there are still hurdles to conquer RCC because survival cancer cells survive. The phenotype survival cancer cells look like sarcoma suggesting epithelial- mesenchymal transition.
