Internal and international migration increasingly continues to be of global importance for development policies and programmes, but the dearth of data on migration for African countries and the limited focus on the structural conditions that motivate migration from specific localities within the region remain glaring. In this study, we examine the patterns and drivers of migration in Cameroon, focusing on the dynamics of rural-urban migration, migrant circulation, regional economic migrants and refugees, international migration, brain drain and returns from emigration. Consequent upon regional conflicts and instability, we highlight the refugee problem in Cameroon and significant challenges in addressing it. Finally, we underscore the policy and research challenges necessary to harness the potentials of internal and international migration for national development. [ABSTRACT FROM AUTHOR]

Building on evidence of the vulnerability of migrants to disadvantages in morbidity and mortality, we examine the relationships between internal migration and early life mortality. We apply the Cox-proportional hazards model on the nationally representative data from the 2008 Demographic and Health Surveys in Kenya and Nigeria to predict the hazard of dying between birth and age 14. We find more deaths in Nigeria than Kenya, with the highest mortality burdens among rural and urban non-migrants in both countries. We also find urban-rural and rural-urban migrants in Kenya and rural-urban migrants in Nigeria showing elevated mortality burdens than other migrant categories. Consistent with evidence that social and biological disadvantages trail individuals through the life course, we find that distinct mortality patterns displayed after 2 years of life gradually enlarge till age 14 in both countries. Beyond migration, we find commonalities in more deaths among children with shorter birth intervals, multiple than singleton births, children whose mothers have no/preschool education, with shortest duration of stay in current place of residence, divorced/separated, and from the poorest households. On differentials, we find in Kenya higher mortality burdens among ethnic Somali children, in Northeastern and Western regions. In Nigeria, we find more deaths among boys, children of mothers under 20 years old, unemployed, Muslim, and from North-west and North-east regions. Our study is a critical step in providing credible evidence that can guide discourse and actions in addressing the health challenges of migrants and non-migrants in two major African countries. The majority of studies of the birth spacing–child survival relationship rely on retrospective data, which are vulnerable to errors that might bias results. The relationship is re-assessed using prospective data on 13,502 children born in two Nairobi slums between 2003 and 2009. Nearly 48% were first births. Among the remainder, short preceding intervals are common: 20% of second and higher order births were delivered within 24 months of an elder sibling, including 9% with a very short preceding interval of less than 18 months. After adjustment for potential confounders, the length of the preceding birth interval is a major determinant of infant and early childhood mortality. In infancy, a preceding birth interval of less than 18 months is associated with a two-fold increase in mortality risks (compared with lengthened intervals of 36 months or longer), while an interval of 18–23 months is associated with an increase of 18%. During the early childhood period, children born within 18 months of an elder sibling are more than twice as likely to die as those born after an interval of 36 months or more. Only 592 children experienced the birth of a younger sibling within 20 months; their second-year mortality was about twice as high as that of other children. ABSTRACT Building on African migration as a household survival strategy; urban-rural linkages as critical for migrants' continued engagement with origin communities; reintegration in case of return; and safety net for supplementing precarious urban incomes; we examine the role of family ties in rural origin linkages among 1,693 older migrants living in Nairobi informal settlements. Despite the grim of slum residence, 80% of older migrants in Nairobi slums maintained contact with their rural origin homes during a full year of observation. Family-related factors, especially members of the nuclear family residing in rural origin, explained 45% of explained linkages. Religion, ethnicity, land ownership at origin, and current health and economic statuses are other key predictors. The patterns and reasons of linkages are consistent with migrants' positive contributions to the upkeep of rural origin households. Our findings are well-anchored in the larger continental literature that has shown the urban migrant as not a 'disembedded individual' but instead part of rural origin collectives. Against the weakness of state safety net system, the study sheds crucial light on the enduring importance of sociocultural networks in people's everyday lives, particularly the importance of family ties for older migrants. To the extent that poor health status, being aged 60 years or older, and long duration of residence in the slums, which are predictors of low propensities to maintain contacts with rural origin, are also indicators of diminished social engagement, policy interventions among the urban poor may need to include efforts to enhance rural origin reintegration of the most-aged individuals. Improving access to quality-assured artemisinin combination therapies (ACTs) is an important component of malaria control in low- and middle-income countries. In 2010 the Global Fund to Fight AIDS, Tuberculosis, and Malaria launched the Affordable Medicines Facility—malaria (AMFm) program in seven African countries. The goal of the program was to decrease malaria morbidity and delay drug resistance by increasing the use of ACTs, primarily through subsidies intended to reduce costs. We collected data on price and retail markups on antimalarial medicines from 19,625 private for-profit retail outlets before and 6-15 months after the program's implementation. We found that in six of the AMFm pilot programs, prices for quality-assured ACTs decreased by US$1.28-$4.34, and absolute retail markups on these therapies decreased by US$0.31-$1.03. Prices and markups on other classes of antimalarials also changed during the evaluation period, but not to the same extent. In all but two of the pilot programs, we found evidence that prices could fall further without suppliers' losing money. Thus, concerns may be warranted that wholesalers and retailers are capturing subsidies instead of passing them on to consumers. These findings demonstrate that supranational subsidies can dramatically reduce retail prices of health commodities and that recommended retail prices communicated to a wide audience may be an effective mechanism for controlling the market power of private-sector antimalarial retailers and wholesalers. Objective: Diarrhoea remains a leading cause of morbidity and death among under-5 children in Kenya, despite multipronged policy and programme initiatives to increase access to treatment. This study interrogates the comprehensiveness and adequacy of Kenya's policies, frameworks and action plans for diarrheal management and prevention. The study seeks to identify policy and practice gaps that need to be filled to strengthen diarrhoea treatment and prevention among under-5 children in Kenya., Design: Our study is a landscape analysis, which seeks to identify the gaps in the current Kenya diarrheal policy, frameworks and action plans. The critical questions included their comprehensiveness, the availability of elaborate treatment, management and prevention solutions, together with updatedness, building on evidence from extant literature on key pathways to infection relating to man-animal environmental interaction, which are critical in enteric infection prevention initiatives., Data Sources: We conducted an internet search of databases of Government of Kenya's Ministry of Health; relevant websites/publications of international organisations and groups (Centre for Disease Control and Prevention, UNICEF and WHO) and published and grey literature (Google searches, Google Scholar and PubMed)., Eligibility Criteria: Included are publicly available key national diarrheal policy frameworks, plans, strategies, laws, institutional frameworks and operational guidelines that inform pertinent questions on the adequacy of policy and practice and preventive policy updates and actions. Further, peer-reviewed and grey literature on diarrheal morbidity and mortality and diarrheal prevention and management are included. The analysis excluded any information that was not referenced on the internet nor obtained from the internet., Data Extraction and Synthesis: The review team extracted the key provisions of the policy guidelines guided by a checklist and questions around the adequacy of existing national policies in addressing the determinants, prevention and treatment interventions of enteric infections and diarrhoea among under-5 children in the country. The checklist covered Kenyan background and diarrhoea situation analysis, policy objectives, policy strategies and policy implementation., Results: The analysis identified a corpus of strategies for the management of diarrhoea at multiple levels: health facilities, communities and households. The policies highlighted advocacy, health communication and social mobilisation, as well as logistics management and prevention strategies. However, the triangulation of evidence from the policy provisions and extant literature identified critical policy gaps in diarrhoea prevention and management in Kenya, particularly the lack of focus on zoonotic pathways to enteric infection, environment-pathogen linkages and operationalisation of the roles of social determinants of health and related services. The policy documents had limited focus on rapid diagnosis, vaccine development and deployment, together with weak funding commitment towards implementation and unclear pathways to funding responsibilities., Conclusion: Policies are central to guiding programmatic actions towards effective enteric and diarrhoea prevention and management measures in Kenya. This study shows the need for policy updates to reflect pathways to enteric infections not covered in the current policy guidelines. Further, there is a need to strengthen the treatment and management of infection through rapid diagnosis, vaccine development and deployment, and strong funding commitment towards implementation together with clear funding responsibilities. Safeguarding is rapidly rising up the international development agenda, yet literature on safeguarding in related research is limited. This paper shares processes and practice relating to safeguarding within an international research consortium (the ARISE hub, known as ARISE). ARISE aims to enhance accountability and improve the health and well-being of marginalised people living and working in informal urban spaces in low-income and middle-income countries (Bangladesh, India, Kenya and Sierra Leone). Our manuscript is divided into three key sections. We start by discussing the importance of safeguarding in global health research and consider how thinking about vulnerability as a relational concept (shaped by unequal power relations and structural violence) can help locate fluid and context specific safeguarding risks within broader social systems. We then discuss the different steps undertaken in ARISE to develop a shared approach to safeguarding: sharing institutional guidelines and practice; facilitating a participatory process to agree a working definition of safeguarding and joint understandings of vulnerabilities, risks and mitigation strategies and share experiences; developing action plans for safeguarding. This is followed by reflection on our key learnings including how safeguarding, ethics and health and safety concerns overlap; the challenges of referral and support for safeguarding concerns within frequently underserved informal urban spaces; and the importance of reflective practice and critical thinking about power, judgement and positionality and the ownership of the global narrative surrounding safeguarding. The main objective of this paper is to investigate the association between fertility preferences and contraceptive use among 15-49-year-old women living in Korogocho and Viwandani, informal settlements in Nairobi, Kenya. We draw on longitudinal data collected under the Maternal and Child Health project conducted between 2006 and 2010 in the two settlements. There is substantial regularity and stability but also unusual instability in reported fertility preferences over time among women living in these settings. Younger women, aged 15-24 years, are likely to change their preferences over time, passing from limiting to wanting additional children. But women aged 35-49 are likely to change their preferences from desiring more children to limiting their childbearing. The desire to limit childbearing is strongly associated with the use of modern and long-acting contraceptive methods. Findings have major implications for the success of family planning programmes in informal settlements where access to and knowledge about contraception may be limited.

Nigeria is the most populous country in Africa and the eighth most populous country in the world, yet there is a dearth of published research about its demography. As Nigeria enters a period of potentially rapid economic growth due to the increase in the working-age population, it is critical to understand the demographic trends in the country. This paper examines the age and sex composition of Nigeria as it relates to various population characteristics using the two most recent Demographic and Health Surveys for Nigeria (2003 and 2008), as well as some data from the 2006 Census. It also highlights Nigeria's demographic composition and trends using United Nations population projection data, and its implications for the country reaping the demographic dividend. Finally, it draws some conclusions and makes some policy recommendations based on the findings.

Background: The Affordable Medicines Facility - malaria (AMFm), implemented at national scale in eight African countries or territories, subsidized quality-assured artemisinin combination therapy (ACT) and included communication campaigns to support implementation and promote appropriate anti-malarial use. This paper reports private for-profit provider awareness of key features of the AMFm programme, and changes in provider knowledge of appropriate malaria treatment., Methods: This study had a non-experimental design based on nationally representative surveys of outlets stocking anti-malarials before (2009/10) and after (2011) the AMFm roll-out., Results: Based on data from over 19,500 outlets, results show that in four of eight settings, where communication campaigns were implemented for 5-9 months, 76%-94% awareness of the AMFm 'green leaf' logo, 57%-74% awareness of the ACT subsidy programme, and 52%-80% awareness of the correct recommended retail price (RRP) of subsidized ACT were recorded. However, in the remaining four settings where communication campaigns were implemented for three months or less, levels were substantially lower. In six of eight settings, increases of at least 10 percentage points in private for-profit providers' knowledge of the correct first-line treatment for uncomplicated malaria were seen; and in three of these the levels of knowledge achieved at endline were over 80%., Conclusions: The results support the interpretation that, in addition to the availability of subsidized ACT, the intensity of communication campaigns may have contributed to the reported levels of AMFm-related awareness and knowledge among private for-profit providers. Future subsidy programmes for anti-malarials or other treatments should similarly include communication activities.

Transactional sex, or the exchange of money and gifts for sexual activities within nonmarital relationships, has been widely considered a contributing factor to the disproportionate prevalence of HIV/AIDS among young women in sub-Saharan Africa. This study applied social exchange theory to premarital relationships in order to investigate the linkages between a variety of young women's resources-including employment and material transfers from male partners-and sexual behaviors. Data on the first month of premarital relationships (N=551 relationships) were collected from a random sample of young adult women ages 18-24 in Kisumu, Kenya, using a retrospective life history calendar. Consistent with the hypotheses, results showed that young women's income increases the likelihood of safer sexual activities, including delaying sex and using condoms consistently. Material transfers from the male partner displayed the opposite effect, supporting the view that resources obtained from within the relationship decrease young women's negotiating power.
