328. A retrospective and interpretive case study of how Northern Ireland policymakers narratively used evidence when crafting child poverty policy (2010-2017)

- Abstract
Part of the Northern Ireland (NI) political settlement of 1998 seemed to promise the dismantling of the structural drivers buttressing the previous thirty years of conflict. The new devolved Executive committed to a social policy based on social inclusion, cohesion and equality, including the eradication of child poverty by 2020. Nevertheless, despite a raft of apparently evidence-based policies and interventions to end child poverty, it remains an unresolved policy concern. This research aims to contribute to the understanding of why child poverty has proved so intractable to (NI) policy actors by drawing on a growing literature of how evidence is processed in complex or contested policy contexts. It considers the cognitive, linguistic, psychological or behavioural responses of policymakers displayed through their public narrative inclusion of evidence. A retrospective and qualitative case study overarches the pluralistic mixed method research approach including the quality assessment criteria of the researcher's own reflexivity within the inquiry process. A social construction ontology and post-positivist epistemology and axiology is followed within the meta-theory of a qualitative narrative policy framework. Official records of debates within the NI Assembly Chamber and committees in the period 2010-2017 have provided the basis for an interpretation of how those making policy navigated the relationship between knowledge, power and policy in their understanding and response to child poverty. Findings show that policymakers drew on various types, sources and suppliers of evidence. Statistical evidence was typically preferred when addressing solutions to child poverty instead of the nature of child poverty as a problem. Some used evidence of solutions recognised as successful elsewhere as 'symbolic reassurance' or 'coalition glue', attracting into a groupthink those with a similar policy stance rather than just a shared political affiliation. At critical points in policy development, the preference for statistical evidence appeared to monopolise decision-making and restricted consideration of other forms of knowledge, which might have upset the dominant 'groupthink'. It is concluded that rather than providing a basis for avoiding ineffective or trial and error policy development, evidence would appear to have primarily played a role in holding together an unstable coalition of political interests.

329. Novel carbon dioxide wound sensors for pathogen detection and therapeutic monitoring

The treatment and management of chronic wounds presents a significant challenge to modern healthcare. Evidence now supports microbial biofilms as a key deterrent to healing, with their presence occurring in up to 80% of non-healing wounds. In this thesis, a novel sensor was developed to offer 24/7, non-invasive monitoring of wound infection to overcome the shortcomings of current wound care and provide an early warning to infection development and unsuccessful treatment. The sensor monitors the headspace that exists between the wound and dressing for small increases in CO2, which is ubiquitously produced from aerobic microorganisms. A formulation containing LDPE, xylenol blue dye, and tetrabutylammonium hydroxide was 3D printed to form a CO2-sensitive indicator film and tested against a range of common wound pathogens in an ex vivo porcine skin wound dressing model. The film changed colour from blue to yellow in response to wound infection. The time taken to reach an intermediate colour change was found to be directly proportional to the time taken for the microbial load to exceed 106 CFU/mL, regardless of initial inoculum. In addition, this colour change occurred before a measurable increase in biofilm biomass, indicating that this sensor acts as an early warning to biofilm development. The ability of the sensor for monitoring treatment efficacy was also assessed by treating P. aeruginosa biofilms with antibiotic monotherapy, phage cocktail monotherapy, or a combination of both. A greater log-fold reduction in P. aeruginosa following treatment was complemented by longer sensor response times. Sensors monitoring biofilms exposed to suboptimal antibiotic or phage concentrations changed colour, whilst successfully treated biofilms elicited no colour change. This sensor has the potential to aid wound care by allowing successfully treated wounds to heal undisturbed, prompting further intervention for failing treatment, and reducing costs and time dedicated to dressing changes.

Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

This paper presents three participatory methods informed by design techniques for engaging youth and youth services providers in research activities online. We detail two data collection methods: 1) a priming activity completed before a focus group to prompt reflection before direct interaction with the research team; and 2) a focus group activity imagining future organizational goals. We also share our technique for collaboratively analyzing preliminary research findings with a focus group activity. We developed these methods as a part of a larger research project examining how teens and those who provide services to teens through community programs conceptualized success in their experiences, sense of community, and program implementation. This paper discusses work with five organizations in a mid-size community in the midwestern United States during the COVID-19 pandemic (July 2021–February 2022). We aimed to amplify the voices and perspectives of teens in both research and the organizations they engaged in, bringing their participation to our data collection and analysis and to the adults and decision-makers of community organizations. We wanted to understand how youth or teens, defined as 13–19 years old, describe successful experiences in these out-of-school settings, how adults define success, and to identify connections and differences between these two perspectives. This paper details the techniques we developed and tested, shares examples from our research, and discusses the impacts and implications of these methods for future participatory work. We contribute evidence for the value of drawing on design methods for participatory methods, articulate benefits and considerations for intentionally doing so online, and share techniques that emphasize voice in both research and organizational partners’ program development and management.

Background PRECISE-DYAD is an observational cohort study of mother-child dyads running in urban and rural communities in The Gambia and Kenya. The cohort is being followed for two years and includes uncomplicated pregnancies and those that suffered pregnancy hypertension, fetal growth restriction, preterm birth, and/or stillbirth. Methods The PRECISE-DYAD study will follow up ~4200 women and their children recruited into the original PRECISE study. The study will add to the detailed pregnancy information and samples in PRECISE, collecting additional biological samples and clinical information on both the maternal and child health. Women will be asked about both their and their child’s health, their diets as well as undertaking a basic cardiology assessment. Using a case-control approach, some mothers will be asked about their mental health, their experiences of care during labour in the healthcare facility. In a sub-group, data on financial expenditure during antenatal, intrapartum, and postnatal periods will also be collected. Child development will be assessed using a range of tools, including neurodevelopment assessments, and evaluating their home environment and quality of life. In the event developmental milestones are not met, additional assessments to assess vision and their risk of autism spectrum disorders will be conducted. Finally, a personal environmental exposure model for the full cohort will be created based on air and water quality data, combined with geographical, demographic, and behavioural variables. Conclusions The PRECISE-DYAD study will provide a greater epidemiological and mechanistic understanding of health and disease pathways in two sub-Saharan African countries, following healthy and complicated pregnancies. We are seeking additional funding to maintain this cohort and to gain an understanding of the effects of pregnancies outcome on longer-term health trajectories in mothers and their children.

333. Pilot feasibility and efficacy of a strategy to sustain A1C improvement among diverse adults with type 2 diabetes completing a diabetes care management program

Introduction Evidence-based strategies are needed to sustain improvements in outcomes following diabetes care management (DCM) programs. We examined the impact of Boot Camp-Plus (BC-Plus), an innovative sustaining strategy, on A1C among adults with type 2 diabetes completing a 3-month Diabetes Boot Camp (DBC). This health system sponsored program consisted of diabetes self-management education and support, medical nutrition therapy and antihyperglycemic medications management.Research design and methods From March 2019 to July 2021, adult DBC completers with Medicare or a health system Medicaid or employee commercial plan were enrolled in BC-Plus for 9 months. DBC completers not meeting insurance eligibility or who declined to participate in BC-Plus acted as controls. During the first 3 months, BC-Plus participants received ongoing daily remote blood glucose (BG) monitoring; and during all 9 months, they received monthly check-in calls with BG review by a medical assistant who addressed needs for supplies/drugs, whether participants were checking BGs, and self-care encouragement. Escalation to a nurse practitioner occurred if the monthly BG trend was >200 mg/dL and/or several BG 9.0% were identified. A1C was followed for an additional 9 months post-BC-Plus. A longitudinal mixed effects analysis was used to assess change in A1C from month 0 to month 21 of follow-up between BC-Plus participants versus controls.Results A total of 838 DCM completers were identified, among whom 281 joined the BC-Plus intervention and 557 acted as controls. Mean age was 55.9 years; 58.2% were women; 66.2% were black; and 30.6% insured by Medicare. BC-Plus participants experienced significantly lower A1C compared with controls and remained below 8.0% to month 18.Conclusions Among completers of a 3-month DCM program, a low intensity 9-month sustaining strategy maintained A1C under 8.0% (HEDIS (Healthcare Effectiveness Data and Information Set) threshold for diabetes control) compared with controls for 15 months after completion of the initial DCM intervention.

334. A digital health intervention to support patients with chronic pain during prescription opioid tapering: a pilot randomised controlled trial

Abstract. Introduction:. Recent changes in opioid prescribing guidelines have led to an increasing number of patients with chronic pain being recommended to taper. However, opioid tapering can be challenging, and many patients require support. Objectives:. We evaluated the feasibility, acceptability, and potential efficacy of a codesigned digital health intervention to support patients with chronic pain during voluntary prescription opioid tapering. Methods:. In a pilot randomised controlled trial, participants received a psychoeducational video and 28 days of text messages (2 SMS/day) in addition to their usual care (intervention) or usual care alone (control). The feasibility, acceptability, and potential efficacy of the intervention were evaluated. The primary outcome was opioid tapering self-efficacy. Secondary outcomes were pain intensity and interference, anxiety and depression symptom severity, pain catastrophising, and pain self-efficacy. Results:. Of 28 randomised participants, 26 completed the study (13 per group). Text message delivery was high (99.2%), but fidelity of video delivery was low (57.1%). Most participants rated the messages as useful, supportive, encouraging, and engaging; 78.5% would recommend the intervention to others; and 64.2% desired a longer intervention period. Tapering self-efficacy (Cohen d = 0.74) and pain self-efficacy (d = 0.41) were higher, and pain intensity (d = 0.65) and affective interference (d = 0.45) were lower in the intervention group at week 4. Conclusion:. First evidence supports the feasibility, acceptability, and potentially efficacy of a psychoeducational video and SMS text messaging intervention to support patients with chronic pain during voluntary prescription opioid tapering. Definitive trials with longer intervention duration are warranted.

Background There are few qualitative studies of firearm injury survivors and hospital-based violence intervention programme (HVIP) participants. The original study aimed to identify facilitators and barriers to survivors’ utilisation of mental health services. This secondary analysis aimed to identify factors that may impact engagement with an HVIP.Study design This study was a subanalysis of an original qualitative study that used a community-based participatory research approach to conduct semistructured interviews with English-speaking, intentional firearm injury survivors aged 13 and older within Indianapolis, Indiana between 2021 and 2022. Participants were recruited by a community organisation through a snowball sampling method. Interviews were analysed using manual thematic analysis. Themes were analysed and discussed in relation to HVIPs.Results A total of 18 interviews were completed. The majority of participants identified as black (17/18, 94.4%). Nearly all (75%) participants were between the ages of 13 and 24 years of age at the time of their shooting. During content analysis, five themes were identified: (1) delayed readiness to change, (2) desire for independence, (3) lack of trust, (4) persistent emotional and physical effects of trauma and (5) unawareness of HVIP resources.Conclusions This qualitative analysis of firearm injury survivor experiences provided insights for improved engagement with HVIPs. Continued pursuit of survivors for several years after their injury, improved dissemination of resources, establishing trust, and addressing persistent physical and psychological symptoms while respecting participants’ desire for independence may lead to increased engagement of firearm injury survivors with HVIPs.

Birth-death models play a key role in phylodynamic analysis for their interpretation in terms of key epidemiological parameters. In particular, models with piecewise-constant rates varying at different epochs in time, to which we refer as episodic birth-death-sampling (EBDS) models, are valuable for their reflection of changing transmission dynamics over time. A challenge, however, that persists with current time-varying model inference procedures is their lack of computational efficiency. This limitation hinders the full utilization of these models in large-scale phylodynamic analyses, especially when dealing with high-dimensional parameter vectors that exhibit strong correlations. We present here a linear-time algorithm to compute the gradient of the birth-death model sampling density with respect to all time-varying parameters, and we implement this algorithm within a gradient-based Hamiltonian Monte Carlo (HMC) sampler to alleviate the computational burden of conducting inference under a wide variety of structures of, as well as priors for, EBDS processes. We assess this approach using three different real world data examples, including the HIV epidemic in Odesa, Ukraine, seasonal influenza A/H3N2 virus dynamics in New York state, America, and Ebola outbreak in West Africa. HMC sampling exhibits a substantial efficiency boost, delivering a 10- to 200-fold increase in minimum effective sample size per unit-time, in comparison to a Metropolis-Hastings-based approach. Additionally, we show the robustness of our implementation in both allowing for flexible prior choices and in modeling the transmission dynamics of various pathogens by accurately capturing the changing trend of viral effective reproductive number.

337. Tuberculosis infection and hypertension: prevalence estimates from the US National Health and Nutrition Examination Survey

Objectives Tuberculosis infection (TBI) is marked by dynamic host–pathogen interactions with persistent low-grade inflammation and is associated with increased risk of cardiovascular diseases (CVD) including acute coronary syndrome, myocardial infarction and stroke. However, few studies assess the relationship between TBI and hypertension, an intermediate of CVD. We sought to determine the association between TBI and hypertension using data representative of the adult US population.Methods We performed cross-sectional analyses using data from the 2011–2012 US National Health and Nutrition Examination Survey (NHANES). Eligible participants included adults with valid QuantiFERON-TB Gold In-Tube (QFT-GIT) test results who also had blood pressure measures and no history of TB disease. TBI was defined by a positive QFT-GIT. We defined hypertension by either elevated measured blood pressure levels (ie, systolic ≥130 mm Hg or diastolic ≥80 mm Hg) or known hypertension indications (ie, self-reported previous diagnosis or use of antihypertensive medications). Analyses were performed using robust quasi-Poisson regressions and accounted for the stratified probability sampling design of NHANES.Results The overall prevalence of TBI was 5.7% (95% CI 4.7% to 6.7%) and hypertension was present among 48.9% (95% CI 45.2% to 52.7%) of participants. The prevalence of hypertension was higher among those with TBI (58.5%, 95% CI 52.4% to 64.5%) than those without TBI (48.3%, 95% CI 44.5% to 52.1%) (prevalence ratio (PR) 1.2, 95% CI 1.1 to 1.3). However, after adjusting for confounders, the prevalence of hypertension was similar for those with and without TBI (adjusted PR 1.0, 95% CI 1.0 to 1.1). The unadjusted prevalence of hypertension was higher among those with TBI versus no TBI, especially among individuals without CVD risk factors including those with normal body mass index (PR 1.6, 95% CI 1.2 to 2.0), euglycaemia (PR 1.3, 95% CI 1.1 to 1.5) or non-smokers (PR 1.2, 95% CI 1.1 to 1.4).Conclusions More than half of adults with TBI in the USA had hypertension. Importantly, we observed a relationship between TBI and hypertension among those without established CVD risk factors.Summary The prevalence of hypertension was high (59%) among adults with TBI in the USA. In addition, we found that the prevalence of hypertension was significantly higher among adults with positive QFT without established hypertension risk factors.

Background Long‐term (visit‐to‐visit) blood pressure variability (BPV) and heart rate variability (HRV) outside pregnancy are associated with adverse cardiovascular outcomes. Given the limitations of relying solely on blood pressure level to identify pregnancies at risk, long‐term (visit‐to‐visit) BPV or HRV may provide additional diagnostic/prognostic counsel. To address this, we conducted a systematic review to examine the association between long‐term BPV and HRV in pregnancy and adverse maternal and perinatal outcomes. Methods and Results Databases were searched from inception to May 2023 for studies including pregnant women, with sufficient blood pressure or heart rate measurements to calculate any chosen measure of BPV or HRV. Studies were excluded that reported short‐term, not long‐term, variability. Adjusted odds ratios were extracted. Eight studies (138 949 pregnancies) reporting BPV met our inclusion criteria; no study reported HRV and its association with pregnancy outcomes. BPV appeared to be higher in women with hypertension and preeclampsia specifically, compared with unselected pregnancy cohorts. Greater BPV was associated with significantly more adverse pregnancy outcomes, particularly maternal (gestational hypertension [odds ratio range, 1.40–2.15], severe hypertension [1.40–2.20]), and fetal growth (small‐for‐gestational‐age infants [1.12–1.32] or low birth weight [1.18–1.39]). These associations were independent of mean blood pressure level. In women with hypertension, there were stronger associations with maternal outcomes but no consistent pattern for perinatal outcomes. Conclusions Future work should aim to confirm whether BPV could be useful for risk stratification prospectively in pregnancy, and should determine the optimal management path for those women identified at increased risk of adverse outcomes.

339. Understanding and responding to racism and the provision of culturally safe care by interdisciplinary health professionals in the aged care sector in regional, rural and remote areas: a scoping review

Introduction: This scoping review was undertaken to obtain conceptual clarification about how racism and cultural safety are understood by interdisciplinary health professionals globally in the aged care sector in regional, rural and remote areas. There is evidence in Australia and internationally that racism is a factor impacting significantly on the health of First Peoples and other racialised minorities. Recent policy changes in Australia have required health professionals to integrate cultural safety into their practice to mitigate racism and improve the health of older First Nations Australians and older people from diverse ethnic and cultural groups. Methods: This review consisted of literature published in English from 1990, including published primary studies; systematic, integrative and narrative reviews; meta-analyses; theses; policy documents; guidelines; position statements; and government literature. Ovid (MEDLINE), CINAHL Plus with Full Text, Scopus, Proquest Nursing and Allied Health Database, and Informit were used in the full search. The most recent search of all databases was undertaken on 9 May 2022. Ten papers were included in the review, following the exclusion of 376 papers. A title and abstract search of the reference lists of papers included in the review identified no additional papers. Results: Ten papers were included in the review from Australia, Canada, the US, Norway and England. The literature reviewed suggests that health professionals in the aged care sector in regional, rural and remote areas in Australia, Canada, the US, Norway and England use alternative terms to 'racism' and 'racist', such as 'institutional marginalisation'. Discussion: The absence of explicit reference to racism aligns with critical race research that argues implicit bias and institutional racism are often separated from an individualised understanding of racism. That is, practitioners may understand racism as something that is perpetrated by individuals in an otherwise 'neutral' health setting. There is also a lack of clarity on how culturally safe care is understood, even though individual care plans are viewed as instrumental in establishing the needs and preferences of the consumers. Within the literature surveyed, barriers to providing quality and culturally inclusive care include disengaged management, insufficient human and material resources, language barriers and a lack of education focused on the needs of older individuals and groups with various cultural and spiritual needs. Additionally, the review does not clearly illuminate what health professionals understand to be racist thinking or behaviour and how it is responded to in practice. Likewise, there is limited information about health professionals' understanding of cultural safety and how to provide culturally inclusive care. Conclusion: While work is beginning on developing standards for cultural safety training in an Australian context, there are also opportunities to consider how these should be applied or adapted to residential and community aged care to best meet the needs of a diverse consumer base and workforce.

The coronavirus disease 2019 (COVID-19) pandemic has impacted health-related behaviors that influence fatty liver disease (FLD) management. We evaluated the impact of the pandemic on FLD management and satisfaction with care delivery in this population. In the San Francisco safety-net hepatology clinics, we evaluated health-related behaviors and factors associated with self-reported weight gain during the COVID-19 pandemic as well as satisfaction with telemedicine in adults with FLD by using multivariable modeling. From June 1, 2020, to May 5, 2021, 111 participants were enrolled. Median age was 52 years, 30% were men, 63% were Hispanic, 21% were Asian/Pacific Islander, and 9% were White. Eating habits were unchanged or healthier for 80%, physical activity decreased in 51%, 34% reported weight gain, and 5% reported increased alcohol intake. Forty-five percent had severe depressive symptoms, 38% in those without diagnosed depression and 60% of individuals with heavy alcohol use. We present new precision measurements of the elastic electron-proton scattering cross section for momentum transfer (Q^{2}) up to 15.75 (GeV/c)^{2}. Combined with existing data, these provide an improved extraction of the proton magnetic form factor at high Q^{2} and double the range over which a longitudinal or transverse separation of the cross section can be performed. The difference between our results and polarization data agrees with that observed at lower Q^{2} and attributed to hard two-photon exchange (TPE) effects, extending to 8 (GeV/c)^{2} the range of Q^{2} for which a discrepancy is established at >95% confidence. We use the discrepancy to quantify the size of TPE contributions needed to explain the cross section at high Q^{2}.

We present the 30-min cadence Kepler/K2 light curve of the Type Ia supernova (SN Ia) SN 2018agk, covering approximately one week before explosion, the full rise phase and the decline until 40 days after peak. We additionally present ground-based observations in multiple bands within the same time range, including the 1-day cadence DECam observations within the first $\sim$5 days after the first light. The Kepler early light curve is fully consistent with a single power-law rise, without evidence of any bump feature. We compare SN 2018agk with a sample of other SNe~Ia without early excess flux from the literature. We find that SNe Ia without excess flux have slowly-evolving early colors in a narrow range ($g-i\approx -0.20\pm0.20$ mag) within the first $\sim 10$ days. On the other hand, among SNe Ia detected with excess, SN 2017cbv and SN 2018oh tend to be bluer, while iPTF16abc's evolution is similar to normal SNe Ia without excess in $g-i$. We further compare the Kepler light curve of SN 2018agk with companion-interaction models, and rule out the existence of a typical non-degenerate companion undergoing Roche-lobe overflow at viewing angles smaller than $45^{\circ}$., Comment: 20 pages, 14 figures, 5 tables. General Relativity and Quantum Cosmology - Abstract
The second Gravitational-Wave Transient Catalog reported on 39 compact binary coalescences observed by the Advanced LIGO and Advanced Virgo detectors between 1 April 2019 15:00 UTC and 1 October 2019 15:00 UTC. We present GWTC-2.1, which reports on a deeper list of candidate events observed over the same period. We analyze the final version of the strain data over this period with improved calibration and better subtraction of excess noise, which has been publicly released. We employ three matched-filter search pipelines for candidate identification, and estimate the astrophysical probability for each candidate event. While GWTC-2 used a false alarm rate threshold of 2 per year, we include in GWTC-2.1, 1201 candidates that pass a false alarm rate threshold of 2 per day. We calculate the source properties of a subset of 44 high-significance candidates that have an astrophysical probability greater than 0.5. Of these candidates, 36 have been reported in GWTC-2. If the 8 additional high-significance candidates presented here are astrophysical, the mass range of events that are unambiguously identified as binary black holes (both objects $\geq 3M_\odot$) is increased compared to GWTC-2, with total masses from $\sim 14 M_\odot$ for GW190924_021846 to $\sim 182 M_\odot$ for GW190426_190642. The primary components of two new candidate events (GW190403_051519 and GW190426_190642) fall in the mass gap predicted by pair instability supernova theory. We also expand the population of binaries with significantly asymmetric mass ratios reported in GWTC-2 by an additional two events (the mass ratio is less than $0.65$ and $0.44$ at $90\%$ probability for GW190403_051519 and GW190917_114630 respectively), and find that 2 of the 8 new events have effective inspiral spins $\chi_\mathrm{eff} > 0$ (at $90\%$ credibility), while no binary is consistent with $\chi_\mathrm{eff} < 0$ at the same significance., Comment: 8 figures, 8 tables, including updates to parameter estimates of events from GWTC-1 and GWTC-2 in an Appendix

Let $\Gamma_g$ be the fundamental group of a closed connected orientable surface of genus $g\geq2$. We introduce a combinatorial structure of "core surfaces", that represent subgroups of $\Gamma_g$. These structures are (usually) 2-dimensional complexes, made up of vertices, labeled oriented edges, and $4g$-gons. They are compact whenever the corresponding subgroup is finitely generated. The theory of core surfaces that we initiate here is analogous to the influential and fruitful theory of Stallings core graphs for subgroups of free groups., Comment: 23 pages, 7 figures. After the detection of gravitational waves from compact binary coalescences, the search for transient gravitational-wave signals with less well-defined waveforms for which matched filtering is not well-suited is one of the frontiers for gravitational-wave astronomy. Broadly classified into "short" $ \lesssim 1~$\,s and "long" $ \gtrsim 1~$\,s duration signals, these signals are expected from a variety of astrophysical processes, including non-axisymmetric deformations in magnetars or eccentric binary black hole coalescences. In this work, we present a search for long-duration gravitational-wave transients from Advanced LIGO and Advanced Virgo's third observing run from April 2019 to March 2020. For this search, we use minimal assumptions for the sky location, event time, waveform morphology, and duration of the source. The search covers the range of $2~\text{--}~ 500$~s in duration and a frequency band of $24 - 2048$ Hz. We find no significant triggers within this parameter space; we report sensitivity limits on the signal strength of gravitational waves characterized by the root-sum-square amplitude $h_{\mathrm{rss}}$ as a function of waveform morphology. These $h_{\mathrm{rss}}$ limits improve upon the results from the second observing run by an average factor of 1.8.
