Ineffective esophageal motility is the most frequent esophageal motility disorder. It is currently defined by 50% or more ineffective esophageal contractions and a normal lower esophageal sphincter relaxation on high-resolution esophageal manometry. Although reported in patients without symptoms, it is typically associated with gastro-esophageal reflux disease, and might be a consequence of the reflux. Ineffective esophageal motility can account for a certain degree of dysphagia, and is associated with a more severe gastroesophageal reflux. Todate, no specific endoscopic or pharmacologic treatment is available, and proton pump inhibitor are advisable when gastro-esophageal reflux is associated.

What is the association between blood progesterone level alone, progesterone to mature oocytes index (PMOI) and IVF outcomes?Clinical data from 960 couples undergoing their first fresh embryo transfer after an intracytoplasmic sperm injection attempt carried out between September 2012 and July 2017 were analysed. All patients underwent ovarian stimulation combining recombinant FSH and gonadotrophin releasing hormone antagonist. Progesterone was measured on the day on which ovulation was triggered. The PMOI was divided into four groups based on 25th, 50th and 75th percentiles, and live birth and implantation rates were compared between the groups.A negative association was found between PMOI levels, live birth and implantation rates. When adjusting for age, ovarian stimulation index (OSI) and number of embryos transferred, the PMOI remained negatively correlated to live birth rate (LBR) (OR = 0.147 [0.031 to 0.701]; P = 0.0161), whereas total blood progesterone was no longer evident. Moreover, the LBR (18.5% versus 28.4%; P0.01) and implantation rate (12.4% versus 21.0%; P0.01) were significantly decreased only when PMOI was 0.167 ng/ml or greater, irrespective of progesterone concentration (or ≥1.08 ng/ml). The opposite was not true, however. Similar PMOIs were recorded in the same patient from one attempt to the next, and were partially linked to basal FSH, anti-Müllerian hormone, antral follicle count and OSI.PMOI seems to be more predictive than total progesterone level of IVF outcome and reflects the ability of embryos to develop as it is linked to the implantation rate.

To test if and how chemotherapy-induced peripheral neurotoxicity (CIPN) is perceived differently by patients and physicians, making assessment and interpretation challenging. We performed a secondary analysis of the CI-PeriNomS study which included 281 patients with stable CIPN. We tested: (a) the association between patients' perception of activity limitation in performing eight common tasks and neurological impairment and (b) how the responses to questions related to these daily activities are interpreted by the treating oncologist. To achieve this, we compared patients' perception of their activity limitation with neurological assessment and the oncologists' blind interpretation. Distribution of the scores attributed by oncologists to each daily life maximum limitation ("impossible") generated three groups: Group 1 included limitations oncologists attributed mainly to motor impairment; Group 2 ones mainly attributed to sensory impairment and Group 3 ones with uncertain motor and sensory impairment. Only a subset of questions showed a significant trend between severity in subjective limitation, reported by patients, and neurological impairment. In Group 1, neurological examination confirmed motor impairment in only 51%-65% of patients; 76%-78% of them also had vibration perception impairment. In Group 2, sensory impairment ranged from 84% to 100%; some degree of motor impairment occurred in 43%-56% of them. In Group 3 strength reduction was observed in 49%-50% and sensory perception was altered in up to 82%. Interpretation provided by the panel of experienced oncologists was inconsistent with the neurological impairment. These observations highlight the need of a core set of outcome measures for future CIPN trials.

Lichens are useful biomonitors for atmospheric polycyclic aromatic hydrocarbons (PAHs). Different sample preparation techniques were explored in this regard, including ultrasound-assisted solvent extraction, microwave-assisted extraction, Soxhlet, and the quick, easy, cheap, effective, rugged, and safe (QuEChERS) technique. It was found that a QuEChERS technique using hexane:acetone (1:1, v/v), never reported before for application to lichens, provided the best recoveries of internal standards, the highest total peak area for all PAHs of interest, and %RSDs comparable with the other preparation techniques tested. The optimized sample preparation technique was found to be a comparatively fast method (45 min), with good recoveries (96%), using less solvents and minimal energy consumption. Strong matrix effects were found: both strong enhancement (for the lighter PAHs) and strong suppression (for the heavier PAHs). The use of matrix-matched standards is thus imperative for the accurate determination of PAH concentrations in the lichen samples. Graphical abstract "Note: This data is mandatory. Please provide."

BACKGROUND Cryopreservation is used for infertility treatment and for fertility preservation. The results of the use of frozen spermatozoa for ART (Assisted Reproductive Technology) are lower than those of fresh spermatozoa. The phospholipase C Zeta (PLCζ) protein is involved in oocyte activation. OBJECTIVES The aim of this study was to compare the percentage of spermatozoa expressing phospholipase C Zeta protein before and after a frozen-thawing cycle. MATERIALS AND METHODS Samples were provided after at least 2 days of sexual abstinence. A part of the fresh ejaculate (200 μL) was recovered for the study. Fifty microliters was necessary to carry out the technique before freezing. The remaining 150 μl was frozen according to a slow manual freezing technique. The samples were treated based on the procedure described by Yelumalai et al. (Fertil. Steril., 104, 2015, 561-568.e4) and Grasa et al. (Hum. Reprod. Oxf. Engl. 23, 2008, 2513-2522). Lithium-ion-free tris(2,2′-bipyridine) Co(ii/iii)-mediated electrolytes have previously been proposed for long-term stable dye-sensitized solar cells (DSSCs). Such redox systems also offer an impressive DSSC performance improvement under light soaking exposure, manifested by an increase in photocurrent and fill factor without the expense of decreasing photovoltage. Kinetic studies show that charge transfer and ion diffusion at the electrode/electrolyte interface are improved due to the light exposure. Control experiments reveal that the light effect is unambiguously associated with electrolyte components, [Co(bpy)3]3+ and the Lewis-base additive tert-butylpyridine (TBP). Electrochemical and spectroscopic investigation of the [Co(bpy)3]3+/TBP mixtures points out that the presence of TBP, which retards the electrolyte diffusion, however causes an irreversible redox reaction of [Co(bpy)3]3+ upon light exposure that improves the overall conductivity. This discovery not only provides a new strategy to mitigate the typical Jsc-Voc trade-off in Co(ii/iii)-mediated DSSCs but also highlights the importance of investigating the photochemistry of a photoelectrochemical system. Anti-sperm antibodies (ASA) have been shown to reduce male fertility but consensus about the precise situations in which tests should be carried out are lacking. In infertility investigations, should the mixed antiglobulin reaction (MAR) test be a first-line test? Should it be carried out systematically before assisted reproductive technology (ART)? What are the risk factors for ASA?All infertile patients (n = 1364) were tested with SpermMar (modified MAR test) between July 2013 and June 2017. Intra-patient variability of the MAR test was also assesed by comparing two tests within the same year in selected patients (n = 101).The main factor that influenced the percentage of ASA was the presence or absence of sperm agglutination. In the presence of agglutinations, 27 out of 72 (37.5%) patients were positive for ASA compared with 33 out of 1292 (2.6%) in the absence of agglutinations (P0.0001). When one risk factor was present (spontaneous sperm agglutination, history of scrotal trauma or inguinal surgery), 33 out of 179 (18.44%) tests were positive for ASA (≥50% coated spermatozoa), whereas only 27 out of 1242 (2.2%) were positive when no risk factor was present (P0.0001).ASA detection should not be systematically recommended in investigations of fertility status and before ART but reserved for when sperm agglutination is found during conventional sperm examination, or if the patient has a history of scrotal trauma or has undergone inguinal surgery.

La centralización y el impulso a la “educación patriótica”, diseñados para lograr una rápida homogeneidad cultural entre la población fueron las características clave del sistema público de educación elemental que se consolidó en Argentina en la segunda mitad del siglo XIX. Ellas auparon hacia su cúspide a un sector de funcionarios “normalistas” que tendieron a considerarse como una elite intelectual más apegada a los destinos del Estado que a los de la profesión de la que formaban parte. Sin embargo, en contra de sus expectativas, no pudieron dejar de ser un grupo intelectual subordinado. Los motivos deben buscarse en el monopolio que los “doctores”, pertenecientes en general a los sectores sociales más encumbrados, ejercieron sobre los espacios en los que se definían las abstracciones académicas y las políticas educativas y en las estrategias de distinción que los propios “normalistas” aplicaron sobre los demás docentes, que los debilitó como sector profesional.

Hydrogen is a major industrial chemical whose manufacture is responsible for ∼3% of global carbon dioxide emissions. >95% of hydrogen is made via reforming fossil fuels which typically co-produces hydrogen and waste carbon dioxide. Nearly all other hydrogen is co-produced with other commodity chemicals. Unfortunately, many alternative, clean hydrogen production processes are small-scale because they require major reductions in capital cost or energy prices to be economical enough for industry. Because the climate problem is urgent, and the economics of future energy is uncertain, this paper seeks to expand the options for producing industrial-scale, clean hydrogen under common, present-day economic conditions. First, we build a model to understand the economic and carbon dioxide emissions constraints of sulfur electrolysis which is an emerging process that cogenerates hydrogen and co-salable sulfuric acid and has the potential to produce up to 36% of the world's current hydrogen demand under present-day, average US economic conditions. We also use our model to evaluate water electrolysis, which cogenerates hydrogen and waste oxygen, but is not economical under present-day average US economic conditions. We then propose criteria for identifying clean hydrogen production chemistries. Using these criteria, we find enough reactions to have the combined potential to make over 150% of the world's industrial hydrogen needs under present-day, average US economic conditions while reducing cost and reducing or eliminating CO2 emissions. Given the urgency of the climate problem, we believe that an economic analysis, such as this is crucial to near-term CO2 emissions reductions.

328. Chert procurement in Corsica during the Neolithic: Inferring social territories in the Tyrrhenian islands

From the Neolithic, foreign siliceous materials were imported into Corsica as the island lacks local chert and obsidian. Such a context constitutes a real opportunity to investigate the relationship of the island with surrounding areas, in perspective with cultural evolutions. For 20 years, chert sourcing studies were carried out. We took into account 26 sites, dating from the Ancient Neolithic to the Final Neolithic. The work is based on non-destructive petro-archaeological observations of the artefacts. and on the survey and characterization of Sardinian sources (320 samples collected and 60 different lithotypes characterized). The study aims to better understand the place of chert among the lithic assemblages throughout time and identify the provenance of most of the chert material introduced into Corsica. It reveals procurement evolution in terms of preferred facies and of stages of introduction, depending on the chronology and geographical situation of the sites. The results confirm connections with Sardinia, among which Perfugas basin constitutes a major source of raw material for Corsica. It also shows relations with the Italian Peninsula for some Middle Neolithic sites in Northern Corsica. Comparisons with data from Sardinia show the affinities and differences between the two islands and open further avenues for research.

In this paper an overview of commercial hydrogen separation technologies is given. These technologies are discussed and compared—with a detailed discussion on membrane-based technologies. An emerging and promising novel hydrogen separation technology, namely, electrochemical hydrogen separation (EHS) is reviewed in detail. EHS has many advantages over conventional separation systems (e.g., it is not energy intensive, it is environmentally-friendly with near-zero pollutants, it is known for its silent operation, and, the greatest advantage, simultaneous compression and purification can be achieved in a one-step operation). Therefore, the focus of this review is to survey open literature and research conducted to date on EHS. Current technological advances in the field of EHS that have been made are highlighted. In the conclusion, literature gaps and aspects of electrochemical hydrogen separation, that require further research, are also highlighted. Currently, the cost factor, lack of adequate understanding of the degradation mechanisms related to this technology, and the fact that certain aspects of this technology are as yet unexplored (e.g., simultaneous hydrogen separation and compression) all hinder its widespread application. In future research, some attention could be given to the aforementioned factors and emerging technologies, such as ceramic proton conductors and solid acids.

BackgroundIn contrast with the setting of acute myocardial infarction, there are limited data regarding the impact of diabetes mellitus on clinical outcomes in contemporary cohorts of patients with chronic coronary syndromes. We aimed to investigate the prevalence and prognostic impact of diabetes according to geographical regions and ethnicity.Methods and resultsCLARIFY is an observational registry of patients with chronic coronary syndromes, enrolled across 45 countries in Europe, Asia, America, Middle East, Australia, and Africa in 2009–2010, and followed up yearly for 5 years. Chronic coronary syndromes were defined by ≥1 of the following criteria: prior myocardial infarction, evidence of coronary stenosis >50%, proven symptomatic myocardial ischaemia, or prior revascularization procedure.Among 32 694 patients, 9502 (29%) had diabetes, with a regional prevalence ranging from below 20% in Northern Europe to ∼60% in the Gulf countries. In a multivariable-adjusted Cox proportional hazards model, diabetes was associated with increased risks for the primary outcome (cardiovascular death, myocardial infarction, or stroke) with an adjusted hazard ratio of 1.28 (95% confidence interval 1.18, 1.39) and for all secondary outcomes (all-cause and cardiovascular mortality, myocardial infarction, stroke, heart failure, and coronary revascularization). Differences on outcomes according to geography and ethnicity were modest.ConclusionIn patients with chronic coronary syndromes, diabetes is independently associated with mortality and cardiovascular events, including heart failure, which is not accounted by demographics, prior medical history, left ventricular ejection fraction, or use of secondary prevention medication. This is observed across multiple geographic regions and ethnicities, despite marked disparities in the prevalence of diabetes.ClinicalTrials identifierISRCTN43070564

STUDY QUESTION Do daily manipulations of preimplantation embryos with polycarbonate (PC)—made bisphenol A (BPA)—releasing strippers influence embryo development? SUMMARY ANSWER Compared to glass strippers, PC strippers enhance the blastocyst development rate but this does not seem to be BPA-related. WHAT IS KNOWN ALREADY PC strippers have been shown to release tiny amounts (around 0.5 ng/ml BPA) of BPA in routine human IVF procedures. A chronic exposure to BPA either in vivo or in vitro during the preimplantation period can impact post-implantation and post-natal development. BPA can act rapidly by binding to membrane receptors and inducing rapid non-genomic effects. STUDY DESIGN, SIZE, DURATION This experimental study using mouse embryos had a balanced design and blinded evaluations of the endpoints. PARTICIPANTS/MATERIALS, SETTING, METHODS In vivo fertilized zygotes were obtained from outbred Swiss CD1 mice crossings after an ovarian stimulation. The zygotes were allocated to three daily handling conditions (HCs) and cultured until Day 4 in a single human commercial medium. Each day, the embryos were handled for 20 s either in a PC stripper (HC1) or in a glass stripper (HC2). In HC3, the embryos were pre-exposed to 0.5 ng/ml BPA before being handled for 20 s in a glass stripper. Handling operations were repeated on Days 1, 2 and 3. Embryo development was assessed blindly on Day 4. Expanded blastocysts were selected for a transcriptomic analysis using Agilent Sureprint G3 Mouse GE v2 microarrays and the retrotransposon LINE1-Orf2 expression was analysed using qRT-PCR, as a proxy for a global evaluation of the epigenetic status. MAIN RESULTS AND THE ROLE OF CHANCE Compared to the embryos manipulated in HC2 (n = 243), those in HC1 (n = 228) developed significantly more often to the blastocyst stage (55 vs 46%; P LARGE SCALE DATA https://www.ncbi.nlm.nih.gov/geo/query/acc.cgi?acc=GSE148868. LIMITATIONS, REASONS FOR CAUTION Our results using a mouse model designed to mimic human conditions (outbred strain, human commercial IVF dishes and a unique commercial human embryonic culture media) are reassuring since no gene was found to be differentially expressed, including LINE-1 genes, as a proxy for a global evaluation of the epigenetic status. However, no global epigenetic analysis of the genome has been performed. Furthermore, we did not evaluate post-implantation events, although BPA exposure during peri-conception could affect foeto-placental and post-natal development. WIDER IMPLICATIONS OF THE FINDINGS Based on the precautionary principle, several European countries banned the use of BPA in baby bottles and food packaging several years before European Agencies took an official position. The question of applying this principle to plastics in closed contact with human embryos is raised. Further studies are needed for a decision to be made. Summary The Cantrell complex, located in the continental shelf in the Southern Gulf of Mexico, is a naturally fractured carbonate field discovered in the late 70’s by PEMEX. The main field known as Akal is the largest with 32 Bboe OOIP. Underlying the Akal filed, the namely Sihil field (1.2 Bboe OOIP) constitutes the thrusted footwall block of a complex fault propagation fold and thrust belt structure. Even if the Cantarell complex has been studied for several decades and its exploitation reaches a mature declination stage, several questions remain related to the physical and geochemical processes occurring during the trap fold thrust evolution and the synchronous thermo-fluid dynamics: How was the thermal-pressure evolution between the overlying Akal and the Sihil footwall during fold thrust formation? What was the timing of HC expulsion and the fluid migration interactions between both blocks? How was the evolution of the fluid composition and PVT phase behavior during HC charge? What are the implications for near field exploration (NFE) and field development? The applied methodology is a 2D structural kinematic restoration meshing tool (KronosFlow), coupled with a fault-related petroleum system modelling (TemisFlow) and detailed 1D reservoir fluid PVT phase behavior modeling.

This paper proposes a methodology based on Artificial Neural Networks (ANNs) for integrating products derived by three on-ground Non-Destructive high-performance Techniques (NDTs) to estimate the International Roughness Index (IRI) of flexible road pavements. About that, IRI of 93 two-lane road sections has been detected by a Laser Profiler (LaP) and considered as output target to be predicted. Structural and geometrical road pavement parameters recognized by Falling Weight Deflectometer (FWD) and Ground Penetrating Radar (GPR) have been considered as input features, along with climate and rainfall information. Accordingly, different ANNs architectures have been trained (by using 70% of samples), validated (15% of samples), and tested (15% of samples) by the Levenberg-Marquardt (LM) backpropagation learning algorithm. We chose the LM since it allows handling limited data and avoiding overfitting issues. Outcomes reveal that a Deep Neural Network (DNN) recognizes hidden patterns between different road surveys and made the integration of NDTs possible and reliable. Specifically, a DNN architecture composed of two hidden layers containing 23 and 12 artificial neurons, respectively, shows a Determination Coefficient (R2) of 0.813 for the training phase, 0.761 for the validation phase, and 0.741 for the test phase. Also, the residual distribution is Gaussian with zero mean. Supported by these findings, we have deployed the DNN including all road sections, obtaining an R2 parameter of 0.762 and a Root Mean Square Error (RMSE) of 0.450 mm/km. Road authorities can consider ANNs and the LM backpropagation learning algorithm for appropriately predicting IRI and efficiently integrating NDT-based outcomes.

The addition of rejuvenators agents (RJ) into bitumen allows increasing the amount of recycled asphalt in new mixtures. In the preliminary part of this study, three different type of oils were tested as potentials RJ: a mineral oil from the automotive industry, a waste domestic cooking vegetable oil (VO), and a biodiesel. Several mixtures were prepared by adding different percentage of oils and VO was individuated as the best RJ. Therefore, VO was used in a further investigation, to better characterize the properties of the rejuvenated binder and its resistance to aging during a second life cycle. Rejuvenated binders with different oil content were prepared and evaluated. The rheological characterization included frequency sweep tests to obtain the binder master curves and linear amplitude sweep test to evaluate the cumulative damage resistance. Moreover, the molecular weight distributions of the binders were investigated by both gel permeation chromatography and rheology by application of the delta-method.

International audience; In birds, maternal transfer is a major exposure route for several contaminants, including poly-and perfluoroalkyl substances (PFAS). Little is known, however, about the extent of the transfer of the different PFAS compounds to the eggs, especially for alternative fluorinated compounds. In the present study, we measured legacy and emerging PFAS, including Gen-X, ADONA, and F-53B, in the plasma of prelaying black-legged kittiwake females breeding in Svalbard and the yolk of their eggs. We aimed to (1) describe the contaminant levels and patterns in both females and eggs, and (2) investigate the maternal transfer, that is, biological variables and the relationship between the females and their eggs for each compound. Contamination of both females and eggs were dominated by linPFOS then PFUnA or PFTriA. We notably found 7:3 fluorotelomer carboxylic acid-a precursor of long-chain carboxylates-in 84% of the egg yolks, and provide the first documented finding of ADONA in wildlife. Emerging compounds were all below the detection limit in female plasma. There was a linear association between females and eggs for most of the PFAS. Analyses of maternal transfer ratios in females and eggs suggest that the transfer is increasing with PFAS carbon chain length, therefore the longest chain perfluoroalkyl carboxylic acids (PFCAs) were preferentially transferred to the eggs. The mean n-ary sumation Sigma(PFAS) in the second-laid eggs was 73% of that in the first-laid eggs. Additional effort on assessing the outcome of maternal transfers on avian development physiology is essential, especially for PFCAs and emerging fluorinated compounds which are under-represented in experimental studies.

Interpretation of averaged evoked potentials is difficult when the time relationship between stimulus and response is not constant. Later components are more prone to latency jitter, making them insufficiently reliable for routine clinical use even though they could contribute to greater understanding of the functioning of polysynaptic components of the afferent nervous system. This study is aimed at providing a simple but effective method of identifying and quantifying latency jitter in averaged evoked potentials. Autocorrelation techniques were applied within defined time windows on simulated jittered signals embedded within the noise component of recorded evoked potentials and on real examples of somatosensory evoked potentials. We demonstrated that the technique accurately identifies the distribution and maximum levels of jitter of the simulated components and clearly identifies the jitter properties of real evoked potential recording components. This method is designed to complement the conventional analytical methods used in neurophysiological practice to provide valuable additional information about the distribution of latency jitter within an averaged evoked potential. It will be useful for the assessment of the reliability of averaged components and will aid the interpretation of longer-latency, polysynaptic components such as those found in nociceptive evoked potentials.

This paper describes a procedure for improving the resilience of roadway networks. A methodology is outlined that develops a time-dependent and performance-based resilience index. This methodology was applied to an Italian road, with the aim of optimizing intersections that are critical due to inadequate baseline capacity. The methodology uses a calibrated microscopic traffic model (using Aimsun™) whereby average delay at intersection approaches are estimated by an analytical model. From the simulation, average speed over time is obtained for each approach. These values in turn are used as inputs for calculating each intersection’s resilience index. The procedure allows the identification of less resilient intersections, and provides design solutions for each of them. Lastly, a safety assessment is tested for one of the intersections.
