Abstract

To discover novel patterns in pathology co-occurrence, we have developed algorithms to analyze and visualize pathology co-occurrence. With access to a database of pathology reports, collected under a single protocol and reviewed by a single pathologist, we can conduct an analysis greater in its scope than previous studies looking at breast pathology co-occurrence. Because this data set is unique, specialized methods for pathology co-occurrence analysis and visualization are developed. Primary analysis is through a co-occurrence score based on the Jaccard coefficient. Density maps are used to visualize global co-occurrence. When our co-occurrence analysis is applied to a population stratified by menopausal status, we can successfully identify statistically significant differences in pathology co-occurrence patterns between premenopausal and postmenopausal women. Genomic and proteomic experiments are planned to discover biological mechanisms that may underpin differences seen in pathology patterns between populations. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Many differences between U.S. military beneficiaries and the U.S. general population, including differences in health care access, are known factors affecting invasive breast cancer outcomes. Thus, comparing the two populations for any outcome differences and their contributing factors may provide insights to breast cancer prognosis.Methods: Using a marginal Cox proportional hazards regression model, we compared disease-specific survival (DSS) and 5-year DSS rates between 418 patients from the Clinical Breast Care Project at the Walter Reed National Military Medical Center (CBCP-WR) and a set of 1:5 randomly matched patients from the Surveillance, Epidemiology, and End Results program. Patients were compared in the "demographic model" (adjusted by diagnosis year, age, and race) and the "overall model" (further adjusted by estrogen receptor, progesterone receptor, stage, and grade).Results: In the "overall model," CBCP-WR patients were less likely overall to die from breast cancer (hazard ratio [HR] = 0.631, 95% confidence interval [CI] = 0.437-0.911; p = 0.014). This increase in survival was also significant in African American patients (HR = 0.524, 95% CI = 0.277-0.992; p = 0.047) and patients older than 50 (HR = 0.511, 95% CI = 0.306-0.854; p = 0.010). The advantage in 5-year DSS rate for CBCP-WR patients was 5.3% (93.1% vs. 87.8%; p < 0.001) in the "demographic model" and 3.4% (91.3% vs. 87.9%; p = 0.018) in the "overall model."Conclusion: CBCP-WR patients demonstrated significantly better DSS over matched SEER patients. Although a portion of the outcome disparity, i.e., 36% of the 5.3% DSS rate difference, could be explained by differences in tumor characteristics, the cause(s) behind the majority of the disparity has yet to be identified. Identification and further analysis of contributing factors to survival differences have the potential to improve clinical practice and outcomes for invasive breast cancer patients. Abstract

The heterogeneity of breast cancer requires the discovery of more incisive molecular tools that better define disease progression and prognosis. Proteomic analysis of homogeneous tumor cell populations derived by laser microdissection from formalin-fixed, paraffin-embedded (FFPE) tissues has proven to be a robust strategy for conducting retrospective cancer biomarker investigations. We describe an MS-based analysis of laser microdissected cancerous epithelial cells derived from twenty-five breast cancer patients at defined clinical disease stages with the goal of identifying protein abundance characteristics indicative of disease progression and recurrence. Comparative analysis of stage 0 and stage III patients revealed 113 proteins that significantly differentiated these groups and included known factors associated with disease pathogenesis, such as CDH1 and CTNNB1, as well as those previously implicated in breast cancer, such as TSP-1. Similar analyses of patients presenting with stage II disease that did or did not exhibit recurrence two years postdiagnosis revealed 42 proteins that significantly differentiated these subgroups and included IRS-1 and PARK7. These data provide evidence supporting the utility of FFPE tissues for functional proteomic analyses and protein biomarker discovery and yielded protein candidates indicative of disease stage and recurrence in breast cancer that warrant further investigation for diagnostic utility and biological relevance.

Abstract

Background: Heat shock protein HYOU1, alternatively known as Orp150, plays an important role in hypoxia/ischemia and angiogenesis. Preliminary studies demonstrated increased HYOU1/Orp150 expression in prostate, bladder and invasive breast cancer. This study further evaluates HYOU1/Orp150 expression in different stages of breast cancer such as benign, pre-malignant and malignant lesions, and correlates it with clinical and pathological data., Material/methods: Sixty-six paraffin-embedded breast tissue sections were stained by immunohistochemistry using HYOU1/Orp150 antibody. Antibody staining was reviewed and scored 0 (lowest) to 3+ (highest). Clinico-pathological data was compared with HYOU1/Orp150 staining using ANOVA or Student's t-Test as appropriate with significance determined by P<0.05., Results: There was a small but significant increase (P<0.01) in HYOU1/Orp150 expression as the tumor transformed from benign (mean 1.52+/-0.12) to ductal carcinoma in situ (mean 2.05+/-0.14) and invasive carcinoma (mean 2.1+/-0.17). HYOU1/Orp150 was partially lost in the benign tissue adjacent to purely invasive cancer (0.88+/-0.22) and it was less (P=0.046) than what was found in benign only lesions (1.52+/-0.12) or in benign components adjacent to DCIS (1.47+/-0.16). Overexpression of HYOU1/Orp150 was associated with tumor lymph node involvement, presence of lymphovascular invasion, and absence of estrogen receptor expression (P<0.01). A trend was observed between HYOU1/Orp150 and nuclear p53 overexpression (P<0.04), but no correlation was detected with age, TNM stage, PR status, HER2/neu or Ki67., Conclusions: HYOU1/Orp150 is overexpressed in some invasive breast cancers and its overexpression appears to be associated with indicators of poor prognosis. Its association with factors such as lymphovascular invasion and regional nodal involvement suggests a possible role for HYOU1/Orp150 in metastasis.

149. Detecting outlier microarray arrays by correlation and percentage of outliers spots.

Abstract

We developed a quality assurance (QA) tool, namely microarray outlier filter (MOF), and have applied it to our microarray datasets for the identification of problematic arrays. Our approach is based on the comparison of the arrays using the correlation coefficient and the number of outlier spots generated on each array to reveal outlier arrays. For a human universal reference (HUR) dataset, which is used as a technical control in our standard hybridization procedure, 3 outlier arrays were identified out of 35 experiments. For a human blood dataset, 12 outlier arrays were identified from 185 experiments. In general, arrays from human blood samples displayed greater variation in their gene expression profiles than arrays from HUR samples. As a result, MOF identified two distinct patterns in the occurrence of outlier arrays. These results demonstrate that this methodology is a valuable QA practice to identify questionable microarray data prior to downstream analysis.
