Abstract

Leukoplakia is an uncommon diagnosis. Patient usually presents with hematuria (visible and non-visible), dysuria, urgency and frequency. Definitive investigation is rigid cystoscopy. Abnormal bladder wall tissue should be resected and biopsies sent for histopathology to confirm the diagnosis. The condition is said to be pre-malignant it is usually advised to do annual cystoscopy with multiple biopsies is to look for any evidence of malignant transformation. Case Report: We present case report of a 64-year-old male presented to the urology clinic with a hematuria. Cystoscopy revealed a 2-cm white lesion on the right side of the urinary bladder. White plaques were also noted covering some areas of the bladder. Transurethral resection of the lesion and biopsy of the white plaques were performed. Pathological examination confirmed a diagnosis of keratinizing squamous metaplasia of the bladder with no evidence of malignancy. [ABSTRACT FROM AUTHOR]

Abstract

High Dynamic Range (HDR) images are the ones that contain a greater range of luminosity as compared to the standard images. HDR images have a higher detail and clarity of structure, objects, and color, which the standard images lack. HDR images are useful in capturing scenes that pose high brightness, darker areas, and shadows, etc. An HDR image comprises multiple narrow-range-exposure images combined into one high-quality image. As these HDR images cannot be displayed on standard display devices, the real challenge comes while converting these HDR images to Low dynamic range (LDR) images. The conversion of HDR image to LDR image is performed using Tone-mapped operators (TMOs). This conversion results in the loss of much valuable information in structure, color, naturalness, and exposures. The loss of information in the LDR image may not directly be visible to the human eye. To calculate how good an LDR image is after conversion, various metrics have been proposed previously. Some are not noise resilient, some work on separate color channels (Red, Green, and Blue one by one), and some lack capacity to identify the structure. To deal with this problem, we propose a metric in this paper called the Tone Mapping Quality Index (TMQI-3), which evaluates the quality of the LDR image based on its objective score. TMQI-3 is noise resilient, takes account of structure and naturalness, and works on all three color channels combined into one luminosity component. This eliminates the need to use multiple metrics at the same time. We compute results for several HDR and LDR images from the literature and show that our quality index metric performs better than the baseline models.

318. Racial differences in hospitalizations for acute cholangitis: a nationwide time trend analysis, 2008-2018.

Abstract

Background The data on racial epidemiologic trends for acute cholangitis (AC) are scarce. Therefore, we conducted a longitudinal assessment of the racial breakdown of AC-related hospitalizations in the United States (US) over 11 years (2008-2018). Methods Using the National Inpatient Sample, we retrieved adult (>18 years) patients with AC. The adjusted yearly hospitalization rate per 100,000 for each race category was calculated based on the US population estimate for July 1 of the corresponding year obtained from the US Census Bureau. We followed Healthcare Cost and Utilization Project recommendations to: (1) derive a time-interrupted trend (before and after 2015), after determining that the International Classification of Diseases coding change affected AC hospitalizations because of more specific coding in the tenth revision; and (2) generate proportionate estimates using revised trend weights. Results A total of 321,849 patients with AC were included in the analysis. Before 2015, the overall hospitalizations (per 100,000 persons) increased from 16.03 in 2008 to 20.76 in 2014 (P<0.001). Following 2015, the overall hospitalizations increased from 14.34 in 2016 to 14.70 in 2018 (P=0.04). After Whites, Asians represented the ethnic group with the highest race-specific AC hospitalizations per 100,000 persons. Conclusions This cohort study demonstrated an overall rising and disproportionate rate among different races for AC-related hospitalizations. Even though Asians constitute only 6.5% of the US population, they represent the ethnic minority with most hospitalizations for AC. [ABSTRACT FROM AUTHOR]

Abstract

When appendix is found in the inguinal hernia sac whether inflamed or non-inflamed then this type of inguinal hernia is called Amyand's Hernia. It is found mostly on right side in incarcerated inguinal hernia with male gender preponderance. Here we present case of 80-year-old male with incarcerated inguinal hernia and was operated in emergency operation theatre. Intraoperatively type 3 Amyand's hernia was found, operation was converted to midline laparotomy and appendectomy was performed with no mesh placement in inguinal region due pus collection in hernia sac. Amyand's hernia is rare pathology and is found intraoperatively most of the time. It should be kept in mind while operating incarcerated or strangulated inguinal hernia. 320. Clinical outcomes of conduction system pacing compared to biventricular pacing in patients requiring cardiac resynchronization therapy.

Abstract

Background: Cardiac resynchronization therapy (CRT) with biventricular pacing (BVP) is well-established therapy in patients with reduced left ventricular ejection fraction (LVEF) and bundle branch block or indication for pacing. Conduction system pacing (CSP) using His-bundle pacing (HBP) or left bundle branch area pacing (LBBAP) has been shown to be a safe and more physiological alternative to BVP.Objective: The purpose of this study was to compare the clinical outcomes between CSP and BVP among patients undergoing CRT.Methods: This observational study included consecutive patients with LVEF ≤35% and class I or II indications for CRT who underwent successful BVP or CSP at 2 major health care systems. The primary outcome was the composite endpoint of time to death or heart failure hospitalization (HFH). Secondary outcomes included subgroup analysis in left bundle branch block as well as individual endpoints of death and HFH.Results: A total of 477 patients (32% female) met inclusion criteria (BVP 219; CSP 258 [HBP 87, LBBAP 171]). Mean age was 72 ± 12 years, and mean LVEF was 26% ± 6%. Comorbidities included hypertension 70%, diabetes mellitus 45%, and coronary artery disease 52%. Paced QRS duration in CSP was significantly narrower than BVP (133 ± 21 ms vs 153 ± 24 ms; P <.001). LVEF improved in both groups during mean follow-up of 27 ± 12 months and was greater after CSP compared to BVP (39.7% ± 13% vs 33.1% ± 12%; P <.001). Primary outcome of death or HFH was significantly lower with CSP vs BVP (28.3% vs 38.4%; hazard ratio 1.52; 95% confidence interval 1.082-2.087; P = .013).Conclusion: CSP improved clinical outcomes compared to BVP in this large cohort of patients with indications for CRT. Abstract

Teaching strategies are the principles and methods that are instrumented by teachers to attain the required optimal learning by students, which depends not only on the subject that is supposed to be taught but also by the nature and psychology of learner. The appropriation and efficiency of a teaching method lies in its relation with the characteristics of learner and the main goal it has supposed to bring about. The study was carried out in Ayub medical college, Abbottabad from December 2016 to August 2017. The methods primarily employed in this research were In-depth interview (IDI) and Focus Group Discussion (FGD). Both FDGs and Interviews were conducted among fourth year MBBS students by team mates. The sampling technique that we used in this study was non-probability purposive sampling. In all about 8 FGDs and 4 in-depth interviews were conducted. Our study revealed that most of the students were not satisfied with one-way teaching. Teacher does not involve students in learning process. Most teachers are unaware of use of technology, which teachers referred to lack of trainings and seminars. In large class strength, every teacher can’t adopt interactive method of teaching. Project writing and field visits were admired. Students were not satisfied with class tests schedule. With the new advancements in education system around the world, teaching methods are also constantly evolving. The conclusion of this study is that teaching is a multidimensional process, which involves key stakeholders that are the teachers, students, and the administration. For an effective teaching, all the stakeholders have to work optimally. These developments can be enhanced with student's feedback, because they are the direct people being affected from the various teaching methods used in a typical classroom of a medical college.

Abstract

Immunization is a very important process of making a person immune or resistant to an infectious disease. It is of utmost importance for the children under 5 years of age to get immunized against the infectious diseases which are vaccine preventable. The childhood immunization introduced through EPI is the most efficient public health innovation for the prevention of vaccine-preventable common child illnesses provided that the program is uninterrupted and parents are satisfied with the service and adhere to the program. The satisfaction of the accompanying persons depends on different factors which are substantial in making the final decision. The sampling technique that we used in this study was non-probability consecutive sampling. The study was carried out at Immunization center at Ayub Teaching Hospital, Abbottabad from December 2016 to August 2017. The methods primarily employed in this research were based on filling a questionnaire. They were filled by us according to the answers the accompanying persons provided. Our study revealed that most of the accompanying persons were satisfied with the immunization services provided at ATH, Abbottabad. Their education status was at least Primary and secondary and mostly the persons who were graduated were not satisfied with the services. The waiting time was the main concern because the persons who thought waiting time was prolonged were mostly unsatisfied. Greetings affected the behavior of the accompanying persons regarding the services and their satisfaction level was directly affected by the behavior of the health team. Cleanliness of the immunization center didn’t affect the level of satisfaction that much. With the new advancements in health system and education status around the world, everything inconstantly evolves. The conclusion of this study is that satisfaction of the accompanying persons has different factors which affect it directly and indirectly .awaiting time, greetings, distance etc. Satisfying the accompanying persons regarding the immunization services requires teamwork. The government, health professionals and health team workers are all responsible in this regard. The waiting time should be decreased and the accompanying persons should be greeted properly. Cleanliness should be maintained at the immunization center.

334. Prevalence and global trends of polypharmacy among people living with HIV: a systematic review and meta-analysis

Background: There has been a rising prevalence of polypharmacy among people living with HIV (PLWH). Uncertainty however remains regarding the exact estimates of polypharmacy among these cohorts of patients. Methods: We conducted a systematic search of PubMed; EMBASE, CROI, Cochrane Database of Systematic Reviews; Science Citation Index and Database of Abstracts of Reviews of Effects for studies between 1 January 2000 and 30 June 2021 that reported on the prevalence of polypharmacy (ingestion of > 5 non-ART medications) among PLWH on antiretroviral therapy regimen (ART). Prevalence of polypharmacy among HIV-positive patients on ART with Clopper–Pearson 95% confidence intervals were presented. The heterogeneity between studies was evaluated using [Formula: see text] and [Formula: see text] statistics. Results: One hundred ninety-seven studies were initially identified, 23 met the inclusion criteria enrolling 55,988 PLWH, of which 76.7% [95% confidence interval (CI): 76.4–77.1] were male. The overall pooled prevalence of polypharmacy among PLWH was 33% (95% CI: 25–42%) ( I2 = 100%, τ2 = 0.9170, p 2 = 100%, τ2 = 1.0886, p 2 = 100%, τ2 = 0.7944, p Conclusion: The pooled prevalence estimates from this synthesis established that polypharmacy is a significant and rising problem among PLWH. The exact interventions that are likely to significantly mitigate its effect remain uncertain and will need exploration by future prospective and systematic studies. Methods and Results: In this systematic review and meta-analysis of 23 studies comprising 55,988 participants, we have for the first time found an estimated polypharmacy pooled prevalence of 33% among PLWH. There was a relatively higher pooled prevalence of polypharmacy among the America’s compared with European cohorts of PLWH. Conclusion: Polypharmacy among PLWH is a rising morbidity that needs urgent intervention both at policy and patient levels of care.

This letter to the editor discusses the permanent deterioration of fine motor skills that can occur after the resection of tumors in the supplementary motor area (SMA). While advancements in neurosurgical techniques have allowed for successful tumor removal and targeted interventions, damage to the SMA can lead to persistent difficulties in activities requiring precise coordination and dexterity. Rehabilitation efforts may help individuals adapt and compensate for motor deficits, but complete restoration of preoperative function may not always be possible. The psychological impact of motor skill deterioration should also be addressed to promote holistic well-being. [Extracted from the article]

Abstract

Abstract

Global climate change brings environmental quality sensitivity, especially in European countries. In this context, European countries are striving to achieve carbon neutrality by 2050. Renewable energy use can be an effective way to curtail the increasing environmental degradation. How the oil prices shape the energy use pattern and environmental quality remains unclear to date. Therefore, the present research examines the linkage between energy use (non-renewable and renewable energy), oil prices, and CO2 emissions in 30 European countries between 1997 and 2017. The study applied fully modified ordinary least squares (FMOLS), Westerlund (2007) cointegration and the Driscoll-Kraay Regression tests to explore these associations. The result of individual samples and the full sample provides the same intimations for the use of energy in Europe. The outcomes of the research ratify the existence of a long-run relationship among variables and found that non-renewable energy consumption increases CO2 emissions, while renewable energy consumption mitigates CO2 emissions. Furthermore, when the oil prices rise, people switch towards substitute energy sources that reduce CO2 emissions. Based on the results, the study suggests that European countries should increase the share of renewable energy, promote the use of clean energy resources, and discourage fossil fuel energy consumption to ensure sustainability. Abstract

The benefit of endoscopic retrograde cholangiopancreatography (ERCP) for the treatment of primary sclerosing cholangitis (PSC) remains controversial. To identify predictors of jaundice resolution after ERCP and whether resolution is associated with improved patient outcomes, we conducted a retrospective cohort study of 124 patients with jaundice and PSC. These patients underwent endoscopic biliary balloon dilation and/or stent placement at an American tertiary center, with validation in a separate cohort of 102 patients from European centers. Jaundice resolved after ERCP in 52% of patients. Median follow‐up was 4.8 years. Independent predictors of jaundice resolution included older age (P = 0.048; odds ratio [OR], 1.03 for every 1‐year increase), shorter duration of jaundice (P = 0.059; OR, 0.59 for every 1‐year increase), lower Mayo Risk Score (MRS) (P = 0.025; OR, 0.58 for every 1‐point increase), and extrahepatic location of the most advanced biliary stricture (P = 0.011; OR, 3.13). A logistic regression model predicted jaundice resolution with area under the receiver operator characteristic curve of 0.67 (95% confidence interval, 0.5‐0.79) in the validation set. Independent predictors of death or transplant during follow‐up included higher MRS at the time of ERCP (P < 0.0001; hazard ratio [HR], 2.33 for every 1‐point increase), lower total serum bilirubin before ERCP (P = 0.031; HR, 0.91 for every 1 mg/dL increase), and persistence of jaundice after endoscopic therapy (P = 0.003; HR, 2.30). Conclusion: Resolution of jaundice after endoscopic treatment of biliary strictures is associated with longer transplant‐free survival of patients with PSC. The likelihood of resolution is affected by demographic, hepatic, and biliary variables and can be predicted using noninvasive data.
