Background: Recent studies suggest that there is an increase in cardiovascular disease after pneumonia; however, there is little information on cardiac arrhythmias after pneumonia. The aims of this study were to assess the incidence of, and examine risk factors for, cardiac arrhythmias after hospitalization for pneumonia., Methods: We conducted a national cohort study using Department of Veterans Affairs administrative data including patients aged ≥65 years hospitalized with pneumonia in fiscal years 2002-2007, receiving antibiotics within 48 hours of admission, having no prior diagnosis of a cardiac arrhythmia, and having at least 1 year of Veterans Affairs care. We included only the first pneumonia-related hospitalization, and follow-up was for the 90 days after admission. Cardiac arrhythmias included atrial fibrillation, ventricular tachycardia/fibrillation, cardiac arrest, and symptomatic bradycardia. We used a multilevel regression model, adjusting for hospital of admission, to examine risk factors for cardiac arrhythmias., Results: We identified 32,689 patients who met the inclusion criteria. Of these, 3919 (12%) had a new diagnosis of cardiac arrhythmia within 90 days of admission. Variables significantly associated with increased risk of cardiac arrhythmia included increasing age, history of congestive heart failure, and a need for mechanical ventilation or vasopressors. Beta-blocker use was associated with a decreased incidence of events., Conclusion: An important number of patients have new cardiac arrhythmia during and after hospitalization for pneumonia. Additional research is needed to determine whether use of cardioprotective medications will improve outcomes for patients hospitalized with pneumonia. At-risk patients hospitalized with pneumonia should be monitored for cardiac arrhythmias during the hospitalization., (Published by Elsevier Inc.)

Cisplatin (Cs) is a chemotherapeutic agent able to generate reactive oxygen species (ROS) which are linked to several side effects of the drug. Even when it is known that Cs produces Leydig cell dysfunction, it is unknown whether this particular side effect is mediated by ROS. The aim of this study was to evaluate the in vitro effects of Cs on testosterone production and the participation of ROS in this effect. We demonstrate that Cs promotes the generation of ROS in a time-, and concentration-dependent fashion, not only in mouse testicular interstitial cells but also in MA-10 Leydig cells. Also, Cs inhibits testosterone synthesis in a concentration-dependent fashion (5-50 μM for 4 h) and to a similar extent, in cells exposed to human chorionic gondadotropin hormone (hCG), to an analog of the second messenger cAMP (8Br-cAMP) or to a freely diffusible cholesterol analog (22R-hydroxycholesterol). However, this treatment does not inhibit the conversion of pregnenolone to testosterone. These data suggest that Cs exerts its inhibitory action on testosterone synthesis by an action at the level of P450scc. We also demonstrated that an antioxidant impairs the inhibitory effect of Cs on the conversion of the cholesterol analog into pregnenolone and that Cs does not change the expression level of P450scc mRNA. Therefore, it is concluded that Cs inhibits testosterone synthesis by a mechanism that includes the inhibition of P450scc by ROS., (Copyright © 2012 Elsevier Ireland Ltd. All rights reserved.)

Electrospun fibers have been fabricated for wide use as artificial tissue engineering scaffolds. In particular, fibers smaller than a cell body have been extensively employed to mimic natural extracellular matrix (ECM) and to explore specific responses by various cell types. We investigated the effects of various poly(lactic acid-co-glycolic acid) (PLGA) fiber features on embryonic hippocampal neurons in the early developmental stages in terms of initial axon formation (i.e., polarization) and axon orientation. We produced PLGA fibers that have average diameters ranging from 0.44 microm to 2.2 microm and different degrees of fiber alignment (16-58 degrees in angular standard deviation). After 22 h in culture, embryonic hippocampal neurons grown on PLGA fibers exhibited more axon formation with a 30-50% increase over those on spin-coated smooth PLGA films. This improvement was independent of fiber diameter and alignment; however, slightly more polarization was observed on the smaller fibers and the more aligned fibers. In addition, average axon length of the polarized embryonic hippocampal neurons was not significantly different among the PLGA fibers when compared with cells grown on spin-coated PLGA films. These findings suggest that fibers of subcellular diameters stimulate initial axon establishment and guide the direction of axonal extension; however, these fibers do not appear to affect the overall axon length. In the present study, we sought to establish the effect of diverse structural-related hydroxycoumarins on the proliferation, cytotoxicity, and induction of apoptosis in promonocytic leukemic cells (U-937). The dihydroxylated coumarins, 7,8-dihydroxy-coumarin and esculetin, induced DNA fragmentation as well as characteristic morphological changes of programmed cell death in U-937 cells. With the aim to perform a structure-activity relationship study, the correlation between the physicochemical properties of the molecules and their pro-apoptotic activity was carried out. Results showed that the presence of two adjacent phenolic hydroxyl groups was the most important factor in terms of the SAR. The exposure of leukemic cells to 7,8-dihydroxy-coumarin evoked a phenoxyl radical generation that was detected by electron spin resonance spectroscopy. The present study suggests that reactive oxygen species generation plays a critical role in dihydroxycoumarin-induced apoptosis in U-937 cells. These findings further suggest that these compounds may have a potential therapeutic role in the treatment of hematological malignancies.

The search for new drugs requires a deep understanding of the molecular basis of drug action, being necessary the elucidation of the mechanism of action with the understanding of the relationship between structure and activity. In the present study, we evaluated the pro-apoptotic activity of 7,8-dihydroxy-4-methylcoumarin (DHMC) and its underlying mechanisms in human leukemic cells. Here, we present evidence that DHMC induced selective and concentration-dependent apoptosis in human leukemic cells. The pro-apoptotic effect of DHMC was mediated by activation of the JNKs and inhibition of the ERK1/2 and PI3K/Akt pathways, with no participation of the p38 cascade after 24h of treatment. Indeed, down-regulation of the proto-oncogene c-myc as well as induction of the cell cycle inhibitor p21(WAF1/CIP1) through a p53 independent mechanism were observed in U-937 cells. These findings suggest that DHCM may have a potential therapeutic role in the future treatment of hematological malignancies.

Electrically conducting polymers such as polypyrrole (PPy) are important biomaterials in neural engineering applications, including neural probes, nerve conduits, and scaffolds for tissue and nerve regeneration. Surface modification of these polymers can introduce other valuable characteristics for neural interfacing in addition to electrical conductivity, such as topographical features and chemical bioactivity. Here, the patterning of PPy to create topographical cues for cells is reported. In particular, 1 and 2 µm wide PPy microchannels are fabricated using electron-beam (e-beam) lithography and electropolymerization. A systematic analysis of parameters controlling PPy micropatterning is performed, and finds that microchannel depth, roughness, and morphology are highly dependent on the e-beam writing current, polymerization current, PPy/dopant concentrations, and the polymerization time. Embryonic hippocampal neurons cultured on patterned PPy polarize (i.e., defined an axon) faster on this modified material, with a twofold increase in the number of cells with axons compared to cells cultured on unmodified PPy. These topographical features also have an effect on axon orientation but do not have a significant effect on overall axon length. This is the first investigation that studies controlled PPy patterning with small dimensions (i.e., less than 5 µm) for biological applications, which demonstrates the relevance of expanding microelectronic materials and techniques to the biomedical field.
