Background: Hepatitis B virus infection is a global health problem with the highest prevalence in East Asia and Sub-Saharan Africa. The majority of infected people, including healthcare workers are unaware of their status. This study is aimed to determining seroprevalence of hepatitis B virus infection and associated factors among healthcare workers in northern Tanzania.Methods: This cross-sectional study included 442 healthcare workers (HCWs) from a tertiary and teaching hospital in Tanzania before the nationwide hepatitis B vaccination campaign in 2004. Questionnaire- based interviews were used to obtain detailed histories of the following: demographic characteristics; occupation risks such splash and needle stick injuries or other invasive procedure such as intravenous, intramuscular or subcutaneous injections; history of blood transfusion and surgeries, as well as HCWs'knowledge of HBV. Serological markers of HBV were done using Laborex HBsAg rapid test. Serology was done at zero months and repeated after six months ( bioscienceinternational.co.ke/rapid-test-laborex.html HBsAg Piazzale-milano-2, Italy [Accessed on November 2017]). Chi-square (χ2) tests were used to compare proportion of HBV infection by different HCWs characteristics. Multivariable logistic regression was used to determine factors associated with HBV infection.Results: A total of 450 surveys were sent out, with a 98.2% response rate. Among the 442 HCWs who answered the questionnaire, the prevalence of chronic hepatitis B virus infection was 5.7% (25/442). Only 50 (11.3%) of HCWs were aware of the HBV status. During the second HBsAg testing which was done after six months one participant sero-converted hence was excluded. Adjusted for other factors, history of blood transfusion significantly increased the odds of HBV infection (OR = 21.44, 95%CI 6.05, 76.01, p < 0.001) while HBV vaccine uptake was protective against HBV infection (OR = 0.06, 95%CI 0.02, 0.26, p < 0.001). The majority of HCWs with chronic HBV infection had poor to fare knowledge about HBV infection but this was not statistically significant when controlled for confounding.Conclusions: Prevalence of HBV among health care workers was 5.7% which is similar to national prevalence. Although the response rate to take part in the study was good but knowledge on HBV infection among HCWs was unsatisfactory. History of blood transfusion increased risks while vaccine uptake decreased the risk of HBV infection. This study recommends continues vaccinating HCWs together with continues medical education all over the country. We also recommend documentation of vaccination evidence should be asked before employment of HCWs in order to sensitize more uptakes of vaccinations. Although we didn't assess the use of personal protective equipment but we encourage HCWs to abide strictly on universal protections against nosocomial infections. [ABSTRACT FROM AUTHOR]

We have analyzed the recent seismicity of Mt. Vesuvius, with particular emphasis on the period 1999-2012. Since 1972 continuous observations with electromagnetic seismometers allowed the compilation of a detailed earthquake catalogue for the station OVO. Furthermore since 1999 another, more complete, catalogue for the station BKE, closer to the crater, is available. The Gutenberg-Richter distribution of magnitudes shows a temporal decrease of the b-value since 1985, with current values close to 1.0. The temporal pattern of the strain release shows a non-stationary behavior with periods of increased release rates (as in 1989-1990, 1995-1996 and 1999-2000). The spatial distribution of the seismicity consists in two main seismogenic volumes, one with hypocenters clustered below the Mt. Vesuvius crater at depths mostly between 1-6 km, and another with hypocenters clustered within the Gran Cono volcanic edifice, with depths above the sea level. We compare the statistical properties of the seismicity occurring within these two volumes and their spatial and temporal patterns. Moreover we analyze the statistical distribution of focal mechanisms for each volume. Our results point to gravity-induced stresses as the source of the shallow seismicity and of a combined effect of crustal heterogeneities, regional stress and hydrothermal dynamics for the deeper seismicity. Finally we discuss possible future developments of the seismic monitoring system in the light of the past and current seismicity.

Mt. Vesuvius (southern Italy) is one of the most hazardous volcanoes in the world. Its activity is currently characterized by moderate seismicity, with hypocenters located beneath the crater zone with depth rarely exceeding 5 km and magnitudes generally less than 3. The current configuration of the seismic monitoring network of Mt. Vesuvius consists of 18 seismic stations and 7 infrasound microphones. During the period 2006-2010 a seismic array with 48 channels was also operative. The station distribution provides appropriate coverage of the area around the volcanic edifice. The computing techniques currently available for the seismic monitoring allow advanced analysis. However, the correct event classification remains a critical aspect for the reliability of real time automatic analysis. Among the existing methods, neural networks may be considered efficient tools for detection and discrimination, and may be integrated into intelligent systems for the automatic classification of seismic events. In this work we apply an unsupervised technique for analysis and classification of seismic signals recorded in the Mt. Vesuvius area in order to improve the automatic event detection. The examined dataset contains about 1500 records divided into four typologies of events: earthquakes, landslides, artificial explosions, and “other” (any other signals not included in the previous classes). First, the Linear Predictive Coding (LPC) and a waveform parametrization have been applied to achieve a significant and compact data encoding. Then, the clustering is obtained using a Self-Organizing Map (SOM) neural network which does not require an a-priori classification of the seismic signals, groups those with similar structures, providing a simple framework for understanding the relationships between them. The resulting SOM map is separated into different areas, each one containing the events of a defined type. This means that the SOM discriminates well the four classes of seismic signals. Moreover, the system will classify a new input pattern depending on its position on the SOM map. The proposed approach can be an efficient instrument for the real time automatic analysis of seismic data, especially in the case of possible volcanic unrest.

Recent investigations have revealed that the genetic deletion of P2X7 receptors (P2rx7) results in an antidepressant phenotype in mice. However, the link between the deficiency of P2rx7 and changes in behavior has not yet been explored. In the present study, we studied the effect of genetic deletion of P2rx7 on neurochemical changes in the hippocampus that might underlie the antidepressant phenotype. P2X7 receptor deficient mice (P2rx7-/-) displayed decreased immobility in the tail suspension test (TST) and an attenuated anhedonia response in the sucrose preference test (SPT) following bacterial endotoxin (LPS) challenge. The attenuated anhedonia was reproduced through systemic treatments with P2rx7 antagonists. The activation of P2rx7 resulted in the concentration-dependent release of [(3)H]glutamate in P2rx7+/+ but not P2rx7-/- mice, and the NR2B subunit mRNA and protein was upregulated in the hippocampus of P2rx7-/- mice. The brain-derived neurotrophic factor (BDNF) expression was higher in saline but not LPS-treated P2rx7-/- mice; the P2rx7 antagonist Brilliant blue G elevated and the P2rx7 agonist benzoylbenzoyl ATP (BzATP) reduced BDNF level. This effect was dependent on the activation of NMDA and non-NMDA receptors but not on Group I metabotropic glutamate receptors (mGluR1,5). An increased 5-bromo-2-deoxyuridine (BrdU) incorporation was also observed in the dentate gyrus derived from P2rx7-/- mice. Basal level of 5-HT was increased, whereas the 5HIAA/5-HT ratio was lower in the hippocampus of P2rx7-/- mice, which accompanied the increased uptake of [(3)H]5-HT and an elevated number of [(3)H]citalopram binding sites. The LPS-induced elevation of 5-HT level was absent in P2rx7-/- mice. In conclusion there are several potential mechanisms for the antidepressant phenotype of P2rx7-/- mice, such as the absence of P2rx7-mediated glutamate release, elevated basal BDNF production, enhanced neurogenesis and increased 5-HT bioavailability in the hippocampus.

Rab7 is a small GTPase that belongs to the Rab family and controls transport to late endocytic compartments such as late endosomes and lysosomes. The mechanism of action of Rab7 in the late endocytic pathway has been extensively studied. Rab7 is fundamental for lysosomal biogenesis, positioning and functions, and for trafficking and degradation of several signaling receptors, thus also having implications on signal transduction. Several Rab7 interacting proteins have being identified leading to the discovery of a number of different important functions, beside its established role in endocytosis. Furthermore, Rab7 has specific functions in neurons. This review highlights and discusses the role and the importance of Rab7 on different cellular pathways and processes.

This article is a study of the contrast between the Danish law concerning reduced economic benefits for newly arrived refugees and immigrants (known as Start Help or as introductory benefit) and the idea of recognition as the condition for individual self-realization and justice. Our assumption is that Start Help both implies economic discrimination against newly arrived persons in Denmark (especially refugees and their families under family reunification rules) and symbolizes a lack of recognition. We have chosen to adopt the theories of recognition (and redistribution) propounded by Axel Honneth and Nancy Fraser to explore our queries about Start Help and the discriminatory impact on its recipients. Empirically the article is based on in-depth qualitative interviews with six refugees who all receive Start Help. The article begins by providing an introduction to the debate about integration legislation in Denmark. Subsequently we present and discuss our main theoretical concepts, namely the theories of recognition and justice which function as clues for our empirical analysis. From this theoretical perspective we present finally an empirical analysis of Start Help recipients’ experiences.

Background: Post-operative nausea and vomiting (PONV) is one of the most important causes of patient discomfort after laparoscopic surgeries despite the use of a multimodal pharmacological approach. This study assessed whether the addition of aprepitant to a multimodal regimen would further decrease the incidence of PONV in high-risk patients.Methods: Apfel-score three or four patients, scheduled for laparoscopic procedures to treat abdominal or pelvic cancer, were randomized to receive oral starch (control group) or 80 mg of oral aprepitant (treatment group) before induction of anaesthesia in a double-blind study. All patients received 4-8 mg of intravenous dexamethasone (at induction) and 4-8 mg of ondansetron (at the end) and a standardized total intravenous anaesthesia (TIVA) technique combined with neuraxial blockade. PONV was defined as any episode of nausea, vomiting or retching in the first 24 h after anaesthesia.Results: Sixty-six patients completed the study. Vomiting occurred in 13/32 (40.6%) patients in the control group and in 1/34 (2.9%) patients in the treatment group (P = 0.0002, 95%CI: 18-54%) in the first 24 h after anaesthesia. Severe nausea occurred in two (6.3%) patients, and severe vomiting occurred in four (12.5%) patients in the control group. One patient presented with severe vomiting in the treatment group in the first 24 post-operative hours.Conclusion: Eighty milligrams of aprepitant added to a three-drug multimodal prophylaxis strategy can bring benefits to a high-risk population by reducing PONV episodes and rescue antiemetic requirements. The article describes experience in studying the effect of a new complex biofertilizer based on natural raw materials - extracts of bird droppings and sapropel, namely: humic and fulvic substances from it, nitrogen-fixing microorganism Azotobacter chroococcum, fungus–ascomycete Trichoderma viride and a source of zinc sulfur - zinc sulfate. on germination and germination energy, seeds of winter wheat (Triticum aestivum) of Tanya variety, oats (Avena sativa L.) of Valdin 765 variety and legume mash (Vigna radiata) of Tajik 1 variety. The purpose of this study was to establish a sustainable positive effect by germinating seeds of winter wheat, oats and mung bean in Petri dishes when treated with a new complex biofertilizer based on sapropel and without treatment, to track the intensity of the germination energy of seeds of these plants on the third day and germination - on the seventh day. On the seventh day of germination of winter wheat in the experimental version, the length of seedlings exceeded the control version by 9.2%. In oats, the length of sprouts in the experimental version is 13.6% longer than in the control version.

The effect of a new feed additive “GLINMOL” in the form of emulsion on productivity of sturgeons was studied. The aim of the research was to study the effect of newly developed feed additives based on fish milt roe and glycerin on sturgeon fish productivity. The experiment consisted of two parts; groups of fish of 3-4 and 4 maturity stages were formed. The first group (control) received complete mixed feed without additives, the second - 2.0% of glycerin of total feed weight; the third - feed additive “GLINMOL” at a dose of 2.0% of total feed weight. The largest gross increase of live weight for the entire rearing period was noted when “GLINMOL” feed additive was used - 11.9% (P
