Background: The treatment of diaphyseal forearm fractures using open reduction and plate fixation is generally accepted as the best choice in many studies. However, periosteal stripping, haematoma evacuation may result in delayed union, nonunion and infection. Refracture after plate removal is another concern. To overcome these problems intramedullary nails (IM) with different designs have been used with various outcomes. However previous IM nails have some shortcomings such is rotational instability and interlocking difficulties. We evaluated the results of newly designed IM nail in the treatment of diaphyseal forearm fractures in adults. Materials and Methods: 32 patients who had been treated with the interlocking IM nail for forearm fractures between 2011 and 2014 were included in this study. There were 23 males and 9 females with mean age of 36 years (range 18-68 years). 22 patients (68.8%) had both bone fractures. Nine patients (28.1%) had open fractures. The remaining ten patients (31.2%) had radius or ulna fractures. Grace and Eversmann rating system was used to assess functional evaluation. Patient reported outcomes were assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire scores. Results: Union was achieved in all patients. The mean followup was 17 months (range 13 - 28 months). According to the Grace-Eversmann criteria, 27 patients (87.5%) had excellent or good results. The mean DASH score was 14 (range 5-36). Overall complication rate was 12.5%. Superficial infection was encountered in two patients. One patient had delayed union, however fracture healed without any additional surgical procedure. One patient who had open grade 3A, comminuted proximal third radius fracture developed radioulnar synostosis. Conclusions: The new design IM interlocking forearm nail provides satisfactory functional and radiological outcomes in the treatment of adult diaphyseal forearm fractures. [ABSTRACT FROM AUTHOR]

Objective: We aimed to evaluate the functional and radiological results of intraarticular distal radius fractures which treated with volar locking plates. Methods: Twenty-eight patient (mean age 44.6; range 19-74) with intraarticular distal radius fracture were treated with volar locking plate. Of these 18 were type B, and 10 were type C. Mean follow-up was 19 months. Patients were evaluated with MAYO score, and DASH (Disabilities of the Arm, Shoulder and Hand) questionnaire. Shortening, radial inclination and palmar tilt were recorded. Uninjured wrist was used for comparison. Results: Union was achieved in all fractures. MAYO score revealed excellent results in 6 patients, good in 7 patients, satisfactory in 14 patients, and poor in 1 patients at the latest follow-up. The mean postoperative DASH score was 16.4 (range, 0-76). The mean radial inclination loss was 0.9 degrees and mean palmar tilt loss was 6.9 degrees when compared to uninjured wrist. Active flexion was 56.3°, extension was 52.5°, supination was 74.2° and pronation was 70.7°. Extensor tenosynovitis was observed in four patients, fleksor pollisis longus tenosynovitis in two, carpal tunnel syndrome in two, superficial infection in one, sudeck atrophy in one. Conclusion: Volar locking plates maintain adequate stability with satisfactory functional results in the treatment of intraarticular distal radius fractures. However, flexor and extensor tendon problems, entrapment of median nerve and reflex sympathetic dystrophy should be kept in mind which might be encountered. Objective: Although foreign body injuries are quite common, there are an insufficient number of publications on this matter. The aim of this study was to evaluate patients with foreign body injuries, and to determine the treatment algorithm. Methods: A total of 97 patients were evaluated, including 53 men and 44 women. Ninety-one patients underwent surgery, and six patients were treated conservatively. Surgery was performed in the operating room in all cases. To ensure that the foreign body was removed, fluoroscopic control was completed during and after the procedure. Antibiotics were administered routinely to all patients, and the need for tetanus prophylaxis was evaluated in each patient. Results: The mean age of the patients was 16.5 (range: 2-58) years. Regarding the type of foreign body, there was injury due to: a needle in 65 patients, glass in 14, wood in 7, iron spit in 1, iron bar in 1, pieces of metal in 3, and other foreign bodies were found in 6 patients. There were 25 injuries of the upper extremity, and 72 of the lower extremity. The time of admission to the hospital after the injury ranged from 1 day to 3 years. Obvious signs of infection were detected in 10 patients. There were no complications after the follow-up of patients who were treated conservatively. Conclusion: Foreign body penetration is a severe injury that may cause significant problems when ignored. Treatment algorithms prepared according to the type of injury would guide the most appropriate approach to these injuries. [ABSTRACT FROM AUTHOR]

Foreign bodies of the urethra and bladder are seen with iatrogenic injury, self-insertioand, rarely, migration from adjacent sites. The most frequent complications of foreign bodies are urethritis, urethral tear with periurethral abscess and or fistula, haemorrhage, and urethral diverticuli. Fournier's gangrene is the result of a highly lethal and rapidly progressive necrotizing infection of the perineal and genital fascia. Anorectal, genitourinary, and traumatic infections are the most common causes of Fournier's gangrene. In this case report, we present a 57 year old patient with mental retardation who was admitted with scrotal swelling, fever, anuria and treated by emergency surgery with the diagnosis of Fournier's Gangrene. Purpose: Neuromuscular control is important for joint stabilization. Supraspinatus muscle plays an essential role in the perception of proprioceptive sense. The aim of this study is to investigate active joint position sense (AJPS) both in patients with partial and full-thickness supraspinatus tears and in healthy participants. Methods: Twenty patients with partial supraspinatus tears, 20 patients with full-thickness supraspinatus tears, and 20 healthy participants, aged 40–65 years, were included in the study. Proprioceptive sense was assessed with AJPS measurement. Absolute error was calculated to evaluate joint position sense. Results: Active joint position sense was decreased in partial and full-thickness tears on both in affected and contralateral shoulders compared to control group (p < 0.05). There was no difference between groups with partial and full-thickness tears on the affected and contralateral shoulders at 40° and 100° elevation (p > 0.05). Conclusion: AJPS was affected after supraspinatus injury. It was seen as proprioceptive deficit in patients with partial and full-thickness tears in both affected and contralateral shoulders. Subperiosteal localization of aneurysmal bone cysts is relatively rare compared to the other localizations in the bones. The mass can cause symptoms dependent upon compression and edema of the surroundings tissues. In this study, we present a 14-year-old girl with a vaguely painful, firm, immobile, enlarging mass measuring 4x4x3 cm on the distal-anterior part of her left humerus. Objectives: The aim of this study was to evaluate the results of reamed and locked intramedullary nailing for tibial diaphysis fractures. Materials and methods: The study included 38 patients (26 males, 12 females) who were treated with reamed and locked intramedullary nailing for tibial diaphysis fractures. Fractures were classified according to Gustilo-Anderson classification and functional results were assessed using the Johner-Wrush criteria. Results: The mean age was 36 years (range 18-61). There were 21 AO/ASIF type A, 16 type B, and 1 type C fractures. Twenty-four fracture were closed (63.1%) and 14 (36.9%) were open fractures. According to the Gustilo-Anderson classification, 9 were grade I, 4 patients grade II, and one grade IIIA open fractures. Intramedullary nailing was performed following open reduction in 18 patients, and closed reduction in 20. The mean time to surgery was 9.4 days and the mean follow-up was 29 months. Union was achieved in all patients within a mean of 17.6 weeks. Anterior knee pain developed in 18 patients and infection developed in three patients. Angular deformity less than 10° was developed 12 patients (31.6%). There were screw breakacge and synostosis in four and two patients respectively. According to the Johner-Wrush criteria, functional results were very good in 23 patients (60.5%), good in 12 patients (31.6%) and fair in 3 (7.9%) patients. Conclusion: Treatment of tibial diaphysis fractures with reamed and locked intramedullary nailing gives satisfactory results.
