105. THE SENSORIMOTOR CONTROL OF HUMAN STANDING POSTURE: AN INVESTIGATION INTO THE RELATIONSHIP AMONG ATTENTION, VISUAL FEEDBACK AND AGE

Abstract

Maintaining upright posture is seemingly an automatic task in younger adults, but it may require additional resources in late adulthood due to decreases in sensorimotor and cognitive functions. The thesis used a dual-task paradigm to investigate age-related changes in relation to the secondary task and context-dependent factors attributes to postural control. The postural task involved visuomotor tracking. Successfully performing the visuomotor task necessitated proper sensory feedback, motor response, and sensorimotor integration. Moreover, we used silent counting as a cognitive task to investigate attentional demands on postural control and age-related difference in cognitive processing. We first investigated the relative contributions of visual feedback delay and cognitive task load on postural dynamics as well as age-related difference in this effect. Our results supported distinct timescale mechanisms for postural control. Moment-to-moment center of pressure fluctuations are dependent on cognitive performance during delayed visual feedback postural control. Also, we demonstrated the increased role of vision with age in postural control. Next, we investigated whether postural control improved when performing a cognitive task with an internal focus of attention. We found that devoting less attention internally by performing a cognitive dual-task enhanced postural control in young adults. Yet, the age-related declines diminish the attentional allocation ability. Lastly, we investigated how older and younger adults differ in employing sensorimotor strategies in a dual-task situation. Our results suggested that age-related changes in postural control may degrade the flexible coordination of the sensory feedback and motor execution. Furthermore, diminished cognitive and attentional capacities may alter postural performance in dual-task conditions. 113. Differences in gaze anticipation for locomotion with and without vision.

Abstract

Previous experimental studies have shown a spontaneous anticipation of locomotor trajectory by the head and gaze direction during human locomotion. This anticipatory behavior could serve several functions: an optimal selection of visual information, for instance through landmarks and optic flow, as well as trajectory planning and motor control. This would imply that anticipation remains in darkness but with different characteristics. We asked 10 participants to walk along two predefined complex trajectories (limaçon and figure eight) without any cue on the trajectory to follow. Two visual conditions were used: (i) in light and (ii) in complete darkness with eyes open. The whole body kinematics were recorded by motion capture, along with the participant's right eye movements. We showed that in darkness and in light, horizontal gaze anticipates the orientation of the head which itself anticipates the trajectory direction. However, the horizontal angular anticipation decreases by a half in darkness for both gaze and head. In both visual conditions we observed an eye nystagmus with similar properties (frequency and amplitude). The main difference comes from the fact that in light, there is a shift of the orientations of the eye nystagmus and the head in the direction of the trajectory. These results suggest that a fundamental function of gaze is to represent self motion, stabilize the perception of space during locomotion, and to simulate the future trajectory, regardless of the vision condition. Abstract

Information at a distance provided by vision is critical for adaptive human locomotion. In this paper we focus on which visually observable environmental features from the visual images on the retina are extracted and how they are coupled to changes in appropriate locomotion patterns. Studies related to environmental features that pose a danger to the mobile agent are described: these include obstacles; sliding doors and undesirable foot landing area in the travel path. Both static and dynamic environmental features result in changing optic flow patterns: environmental features that change independently pose an added challenge. Key results from these studies are discussed in terms of issues that are important for the implementation of visually guided adaptable biped robot. [ABSTRACT FROM AUTHOR]

116. Abstract 11860: Recurrent Appropriate Shocks in a Patient With Twiddler?s Syndrome

Abstract

Introduction:Most reported cases of Twiddler?s Syndrome have been in patients with pacemakers and manifest as pacing failure, syncope, symptomatic bradycardia as well as phrenic and vagus nerve stimulation. AICD Twiddler?s Syndrome is rare and dangerous as it may manifest as inappropriate shocks or failed appropriate therapy.Case Presentation:An 84-year-old female with ischemic cardiomyopathy received a St. Jude Assura VR model 1357-40Q single lead AICD for primary prevention. A year later, the patient presented with multiple, repetitive shocks within a 2 hour period. Device interrogation revealed over 50 anti-tachycardia pacing (ATP) and 22 36J shocks. Chest x-ray showed dislodgment of the RV lead to the tricuspid valve (TV) caused by looping of the lead at the generator (Fig. 1). Dislodgement of the RV lead to the TV caused oversensing of the atrium and ventricle activation triggering inappropriate ATP/VT programmed protocol. Multiple shocks were delivered by 2 mechanisms: (1) inappropriate ATP-induced recurrent sustained VT treated appropriately (Fig. 2A) (2) inappropriate ATP failed therapy ending by multiple inappropriate subsequent shocks due to persistent double counting of electrograms (Fig. 2B). The patient underwent subpectoral implantation of the generator.Conclusion:AICD Twiddler?s Syndrome is a rare, potentially fatal phenomenon where an AICD is manipulated by the patient and may manifest as inappropriate shocks, abrupt increased lead impedance, and increase in pacing thresholds. Risk factors for Twiddler?s Syndrome include female gender, obesity, weight loss, psychiatric disorders and a larger pocket size relative to the implant. In patients at high risk as well as in those where the device is likely to migrate due to lack of subcutaneous tissue support, a submuscular device implant is recommended. The use of newer pulse generators without sharp edges decrease the patient discomfort that may mitigate device twiddling and manipulation.
