Abstract

BackgroundCirculating tumor DNA (ctDNA) is used to select initial targeted therapy, identify mechanisms of therapeutic resistance, and measure minimal residual disease (MRD) after treatment. Our objective was to review private and Medicare coverage policies for ctDNA testing.MethodsPolicy Reporter was used to identify coverage policies (as of February 2022) from private payers and Medicare Local Coverage Determinations (LCDs) for ctDNA tests. We abstracted data regarding policy existence, ctDNA test coverage, cancer types covered, and clinical indications. Descriptive analyses were performed by payer, clinical indication, and cancer type.ResultsA total of 71 of 1,066 total policies met study inclusion criteria, of which 57 were private policies and 14 were Medicare LCDs; 70% of private policies and 100% of Medicare LCDs covered at least one indication. Among 57 private policies, 89% specified a policy for at least 1 clinical indication, with coverage for ctDNA for initial treatment selection most common (69%). Of 40 policies addressing progression, coverage was provided 28% of the time, and of 20 policies addressing MRD, coverage was provided 65% of the time. Non-small cell lung cancer (NSCLC) was the cancer type most frequently covered for initial treatment (47%) and progression (60%). Among policies with ctDNA coverage, coverage was restricted to patients without available tissue or in whom biopsy was contraindicated in 91% of policies. MRD was commonly covered for hematologic malignancies (30%) and NSCLC (25%). Of the 14 Medicare LCD policies, 64% provided coverage for initial treatment selection and progression, and 36% for MRD.ConclusionsSome private payers and Medicare LCDs provide coverage for ctDNA testing. Private payers frequently cover testing for initial treatment, especially for NSCLC, when tissue is insufficient or biopsy is contraindicated. Coverage remains variable across payers, clinical indications, and cancer types despite inclusion in clinical guidelines, which could impact delivery of effective cancer care.

Abstract

Half metals are ferromagnets hosting spin-polarized conducting carriers and are crucial for spintronics applications. The chromium spinel HgCr2Se4 represents a unique type of half-metal, which features a half-metal to semiconductor transition (HMST) and exhibits colossal magnetoresistance (CMR) across the ferromagnetic-paramagnetic (FM-PM) transition. Using angle-resolved photoemission spectroscopy, we find that the Fermi surface of n-type HgCr2Se4 (n-HgCr2Se4) consists of a single electron pocket which moves above the Fermi level (EF) upon the FM-PM transition, leading to the HMST. Such a Lifshitz transition manifests a giant band splitting which originates from the exchange interaction unveiled with a specific chemical nonstoichiometry. The exchange band splitting and the chemical nonstoichiometry are two key ingredients to the HMST and CMR, consistent with our ab initio calculation. Our findings provide spectroscopic evidence of the electronic origin of the anomalous properties of HgCr2Se4, which address the unique phase transition in half-metals.

Abstract

Spontaneous symmetry breaking underlies much of our classification of phases of matter and their associated transitions1-3. The nature of the underlying symmetry being broken determines many of the qualitative properties of the phase; this is illustrated by the case of discrete versus continuous symmetry breaking. Indeed, in contrast to the discrete case, the breaking of a continuous symmetry leads to the emergence of gapless Goldstone modes controlling, for instance, the thermodynamic stability of the ordered phase4,5. Here, we realize a two-dimensional dipolar XY model that shows a continuous spin-rotational symmetry using a programmable Rydberg quantum simulator. We demonstrate the adiabatic preparation of correlated low-temperature states of both the XY ferromagnet and the XY antiferromagnet. In the ferromagnetic case, we characterize the presence of a long-range XY order, a feature prohibited in the absence of long-range dipolar interaction. Our exploration of the many-body physics of XY interactions complements recent works using the Rydberg-blockade mechanism to realize Ising-type interactions showing discrete spin rotation symmetry6-9.

Abstract

Leber hereditary optic neuropathy (LHON) is an important example of mitochondrial blindness with the m.11778G>A mutation in the MT-ND4 gene being the most common disease-causing mtDNA variant worldwide. The REFLECT phase 3 pivotal study is a randomized, double-masked, placebo-controlled trial investigating the efficacy and safety of bilateral intravitreal injection of lenadogene nolparvovec in patients with a confirmed m.11778G>A mutation, using a recombinant adeno-associated virus vector 2, serotype 2 (rAAV2/2-ND4). The first-affected eye received gene therapy; the fellow (affected/not-yet-affected) eye was randomly injected with gene therapy or placebo. The primary end point was the difference in change from baseline of best-corrected visual acuity (BCVA) in second-affected/not-yet-affected eyes treated with lenadogene nolparvovec versus placebo at 1.5 years post-treatment, expressed in logarithm of the minimal angle of resolution (LogMAR). Forty-eight patients were treated bilaterally and 50 unilaterally. At 1.5 years, the change from baseline in BCVA was not statistically different between second-affected/not-yet-affected eyes receiving lenadogene nolparvovec and placebo (primary end point). A statistically significant improvement in BCVA was reported from baseline to 1.5 years in lenadogene nolparvovec-treated eyes: -0.23 LogMAR for the first-affected eyes of bilaterally treated patients (P < 0.01); and -0.15 LogMAR for second-affected/not-yet-affected eyes of bilaterally treated patients and the first-affected eyes of unilaterally treated patients (P < 0.05). The mean improvement in BCVA from nadir to 1.5 years was -0.38 (0.052) LogMAR and -0.33 (0.052) LogMAR in first-affected and second-affected/not-yet-affected eyes treated with lenadogene nolparvovec, respectively (bilateral treatment group). A mean improvement of -0.33 (0.051) LogMAR and -0.26 (0.051) LogMAR was observed in first-affected lenadogene nolparvovec-treated eyes and second-affected/not-yet-affected placebo-treated eyes, respectively (unilateral treatment group). The proportion of patients with one or both eyes on-chart at 1.5 years was 85.4% and 72.0% for bilaterally and unilaterally treated patients, respectively. The gene therapy was well tolerated, with no systemic issues. Intraocular inflammation, which was mostly mild and well controlled with topical corticosteroids, occurred in 70.7% of lenadogene nolparvovec-treated eyes versus 10.2% of placebo-treated eyes. Among eyes treated with lenadogene nolparvovec, there was no difference in the incidence of intraocular inflammation between bilaterally and unilaterally treated patients. Overall, the REFLECT trial demonstrated an improvement of BCVA in LHON eyes carrying the m.11778G>A mtDNA mutation treated with lenadogene nolparvovec or placebo to a degree not reported in natural history studies and supports an improved benefit/risk profile for bilateral injections of lenadogene nolparvovec relative to unilateral injections.

Abstract

As Wi-Fi becomes ubiquitous in public and private spaces, it becomes natural to leverage its intrinsic ability to sense the surrounding environment to implement groundbreaking wireless sensing applications such as human presence detection, activity recognition, and object tracking. For this reason, the IEEE 802.11bf Task Group is defining the appropriate modifications to existing Wi-Fi standards to enhance sensing capabilities through 802.11-compliant devices. However, the new standard is expected to leave the specific sensing algorithms open to implementation. To fill this gap, this article explores the practical implications of integrating sensing into Wi-Fi networks. We provide an overview of the physical and medium access control layers sensing enablers, together with the application layer perspective. We analyze the impact of communication parameters on sensing performance and detail the main research challenges. Abstract

The rise of intelligent reflecting surfaces (IRSs) is opening the door for unprecedented capabilities in visible light communication (VLC) systems. By controlling light propagation in indoor environments, it is possible to manipulate the channel conditions to achieve specific key performance indicators. In this paper, we investigate the role that IRSs can play in boosting the secrecy capacity of non-orthogonal multiple access (NOMA) VLC systems. More specifically, we propose an IRS-based physical layer security (PLS) mechanism that mitigates the information leakage risk inherent in NOMA. Our results demonstrate that the achieved secrecy capacity can be enhanced by up to 105% for a number of 80 IRS elements. To the best of our knowledge, this is the first paper that examines the PLS of NOMA-based IRS-assisted VLC systems.

347. Observation of self-patterned defect formation in atomic superfluids -- from ring dark solitons to vortex dipole necklaces

Abstract

Unveiling nonequilibrium dynamics of solitonic and topological defect structures in a multidimensional nonlinear medium is a current frontier across diverse fields. One of the quintessential objects is a ring dark soliton (RDS), whose dynamics are expected to display remarkable interplay between symmetry and self-patterned topological defect formation from a transverse (snake) instability, but it has thus far evaded full experimental observations. Here, we report an experimental realization of RDS generation in a two-dimensional atomic superfluid trapped in a circular box. By quenching the confining box potential, we observe an RDS emitted from the edge and its peculiar signature in the radial motion. As an RDS evolves, we observe transverse modulations at discrete azimuthal angles, which clearly result in a patterned formation of a circular vortex dipole array. Through collisions of the vortex dipoles with the box trap, we observe vortex unbinding, vortex pinning to the edge, and emission of rarefaction pulses. Our box-quench protocol opens a new way to study multidimensional dark solitons, structured formation of topological defects, and potentially the dynamics of ordered quantum vortex matter.

348. Improving Named Entity Recognition in Telephone Conversations via Effective Active Learning with Human in the Loop

Abstract

Telephone transcription data can be very noisy due to speech recognition errors, disfluencies, etc. Not only that annotating such data is very challenging for the annotators, but also such data may have lots of annotation errors even after the annotation job is completed, resulting in a very poor model performance. In this paper, we present an active learning framework that leverages human in the loop learning to identify data samples from the annotated dataset for re-annotation that are more likely to contain annotation errors. In this way, we largely reduce the need for data re-annotation for the whole dataset. We conduct extensive experiments with our proposed approach for Named Entity Recognition and observe that by re-annotating only about 6% training instances out of the whole dataset, the F1 score for a certain entity type can be significantly improved by about 25%., Comment: The final version of this paper will be published in the Proceedings of the DaSH Workshop @ EMNLP 2022. 349. Multi-rate adaptive transform coding for video compression

Abstract

Contemporary lossy image and video coding standards rely on transform coding, the process through which pixels are mapped to an alternative representation to facilitate efficient data compression. Despite impressive performance of end-to-end optimized compression with deep neural networks, the high computational and space demands of these models has prevented them from superseding the relatively simple transform coding found in conventional video codecs. In this study, we propose learned transforms and entropy coding that may either serve as (non)linear drop-in replacements, or enhancements for linear transforms in existing codecs. These transforms can be multi-rate, allowing a single model to operate along the entire rate-distortion curve. To demonstrate the utility of our framework, we augmented the DCT with learned quantization matrices and adaptive entropy coding to compress intra-frame AV1 block prediction residuals. Abstract

The sensitivity of the optical wireless communication (OWC) can be effectively improved by employing the highly sensitive single-photon avalanche diode (SPAD) arrays. However, the nonlinear distortion introduced by the dead time strongly limits the throughput of the SPAD-based OWC systems. Optical orthogonal frequency division multiplexing (OFDM) can be employed in the systems with SPAD arrays to improve the spectral efficiency. In this work, a theoretical performance analysis of SPAD-based OWC system with asymmetrically-clipped optical OFDM (ACO-OFDM) is presented. The impact of the SPAD nonlinearity on the system performance is investigated. In addition, the comparison of the considered scheme with direct-current-biased optical OFDM (DCO-OFDM) is presented showing the distinct reliable operation regimes of the two schemes.
