Background: Neighborhood socioeconomic status (SES) is associated with worse health outcomes, yet its relationship with in-hospital heart failure (HF) outcomes and quality metrics are underexplored. We examined the association between socioeconomic neighborhood disadvantage and in-hospital HF outcomes for patients from diverse neighborhoods in the Get With The Guidelines-Heart Failure registry. Methods: SES-disadvantage scores were derived from geocoded US census data using a validated algorithm, which incorporated household income, home value, rent, education, and employment. We examined the association between SES-disadvantage quintiles with all-cause in-hospital mortality, adjusting for demographics and comorbidities. Results: Of 593 053 patients hospitalized for HF between 2017 and 2020, 321 314 (54%) had residential ZIP Codes recorded. Patients from the most compared with least disadvantaged neighborhoods were younger (mean age 67 versus 76 years), more often Black (42% versus 9%) or Hispanic (14% versus 5%), and had higher comorbidity burden. Demographic-adjusted length of stay increased by ≈1.5 hours with each increment in worsening SES-disadvantage quintiles. Adjusted-mortality odds ratios increased with worsening SES-disadvantage quintiles (P trend=0.003), and was 28% higher (adjusted OR=1.28 [1.12–1.48]) for the most compared with least disadvantaged neighborhood groups. Conclusions: Patients hospitalized for HF from disadvantaged neighborhoods were younger and more often Black or Hispanic. SES disadvantage was independently associated with higher in-hospital mortality. Further research is needed to characterize care delivery patterns in disadvantaged neighborhoods and to address social determinants of health among patients hospitalized for HF. BACKGROUND Systemic arterial compliance is sometimes derived by echocardiographic stroke volume to pulse pressure ratios. Few studies have assessed echocardiographic arterial compliance in blacks or its associations with explicit, rather than composite, cardiovascular disease (CVD) outcomes. METHODS We analyzed a subset (n = 1,887) of blacks (mean age = 59±6 years; 65% women) in the Atherosclerosis Risk in Communities study who were free of prevalent CVD and were imaged by echocardiography in 1993–1995. Arterial compliance was calculated by the aortic velocity time integral and brachial pulse pressure ratio (VTI/PP). Associations between VTI/PP and subsequent CVD (defined as first incident stroke, coronary event, or heart failure) were modeled by Cox regression after controlling for demographics, anthropometry, and cardiac risk factors. For comparison, CVD hazard ratios were also calculated for pulse pressure quartiles. RESULTS Over a mean follow-up of 13±4 years, 237 subjects (12%) developed coronary disease, 322 (16%) developed heart failure, and 180 (9%) experienced a stroke. Hazard ratios contrasting lowest with highest quartiles of VTI/PP were 2.3 (95% confidence interval (CI) = 1.7–3.1) for composite CVD, 2.1 (95% CI = 1.3–3.2) for coronary disease, 2.5 (95% CI = 1.7–3.6) for heart failure, and 2.7 (95% CI = 1.6–4.5) for stroke. Hazard ratios contrasting widest with narrowest pulse pressure quartiles were 1.7 (95% CI = 1.3–2.2) for composite CVD, 1.6 (95% CI = 1.0–2.4) for coronary heart disease, 1.8 (95% CI = 1.2–2.6) for heart failure, and 2.3 (95% CI = 1.3–3.9) for stroke. CONCLUSIONS In blacks, the VTI/PP ratio has stronger associations with both composite and individual CVD outcomes than does pulse pressure. [ABSTRACT FROM PUBLISHER]

Background: Carotid intima-medial thickness (IMT) is an indicator of subclinical atherosclerosis and medial hypertrophy, and is easily quantified by ultrasonography. Although carotid IMT has been associated with systemic cardiovascular disease, its relation to silent brain infarctions (SBI) is not well established. The etiology of SBI is multifactorial; likely causes include atherosclerotic and thrombotic obstructions, microthrombi, and lipohyalinosis. Because arterial IMT reflects both atherogenesis and hypertensive remodeling, we hypothesized that elevated carotid IMT would be associated with SBI. Methods: The Atherosclerosis Risk in Communities (ARIC) study is an observational cohort of 15,792 black and white participants from 4 US communities. At visit 3 (1993-1995) a subset of stroke-free study participants (N=1,346) was screened by brain MRI and carotid ultrasound. IMT was averaged bilaterally, from 12 posterior wall measurements along a 1 cm segment of the distal common carotid artery up to the bifurcation. Consistent with European Society of Cardiology recommendations, IMT > 0.9 mm was considered elevated. Asymptomatic brain lesions ≥3 mm in diameter were considered SBI. Prevalence ratios (PRs) for number of SBI lesions associated with elevated IMT were analyzed using Poisson regression, adjusted for age, race-center, sex, current smoking, hypertension, diabetes, and hyperlipidemia. Model fit was ascertained by the deviance to degree of freedom ratio. Results: Of 1,346 individuals, 170 (13%) were classified with elevated IMT. Mean IMT was 1.0 ± 0.2 mm vs. 0.7 ± 0.1 mm for those with and without elevated IMT. Study participants with elevated IMT were more often male (49% vs. 38%; p=0.006), black (60% vs. 46%; p=0.0007), and older (64 vs. 62 years; p=0.0001); with a greater prevalence of both hypertension (65% vs. 42%; p<0.0001) and diabetes (27% vs. 13%; p<0.0001). SBI were detected in 157 individuals with a collective total of 247 lesions. A total of 30 (18%) participants with elevated IMT had at least one SBI lesion, compared to 127 (11%) with normal IMT (p=0.009). Elevated IMT was associated with 70% greater number of SBI lesions (PR=1.7; 95% CI: 1.3 - 2.3; p=0.0008). After adjustment for cardiovascular risk factors, elevated IMT remained associated with a 40% greater SBI count (PR= 1.4; 95% CI: 1.0 - 1.9; p=0.05). Conclusions: Although elevated IMT is often classified by a conservative 0.9 mm cut-point, it is a continuum, with advanced wall thickening (>1.5 mm) recognized as plaque. In this analysis from the ARIC Study, asymptomatic individuals classified with elevated IMT had mild wall thickening (mean IMT = 1.0 mm), which nonetheless was associated with greater number of SBI lesions.
