Background: The imperative to improve global health has prompted transnational research partnerships to investigate common health issues on a larger scale. The Global Alliance for Chronic Diseases (GACD) is an alliance of national research funding agencies. To enhance research funded by GACD members, this study aimed to standardise data collection methods across the 15 GACD hypertension research teams and evaluate the uptake of these standardised measurements. Furthermore we describe concerns and difficulties associated with the data harmonisation process highlighted and debated during annual meetings of the GACD funded investigators. With these concerns and issues in mind, a working group comprising representatives from the 15 studies iteratively identified and proposed a set of common measures for inclusion in each of the teams' data collection plans. One year later all teams were asked which consensus measures had been implemented. Result(s): Important issues were identified during the data harmonisation process relating to data ownership, sharing methodologies and ethical concerns. Measures were assessed across eight domains; demographic; dietary; clinical and anthropometric; medical history; hypertension knowledge; physical activity; behavioural (smoking and alcohol); and biochemical domains. Identifying validated measures relevant across a variety of settings presented some difficulties. The resulting GACD hypertension data dictionary comprises 67 consensus measures. Of the 14 responding teams, only two teams were including more than 50 consensus variables, five teams were including between 25 and 50 consensus variables and four teams were including between 6 and 24 consensus variables, one team did not provide details of the variables collected and two teams did not include any of the consensus variables as the project had already commenced or the measures were not relevant to their study. Introduction Delirium is a common, serious and potentially preventable condition with devastating impact on the quality of life prompting a proliferation of interventional trials. Core outcome sets aim to standardise outcome reporting by identifying outcomes perceived fundamental for measurement in trials of a specific interest area. Our aim is to develop international consensus on two core outcome sets for trials of interventions to prevent and/or treat delirium, irrespective of study population. We aim to identify additional core outcomes specific to the critically ill, acutely hospitalised patients, palliative care and older adults. Methods and analysis We will conduct a systematic review of published and ongoing delirium trials (1980 onwards) and one-on-one interviews of patients who have experienced delirium and family members. These data will inform Delphi round 1 of a two-stage consensus process. In round 2, we will provide participants their own response, summarised group responses and those of patient/family participants for rescoring. We will randomise participants to receive feedback as proportion scoring the outcome as critical or as group mean responses. We will hold a consensus meeting using nominal group technique to finalise outcomes for inclusion. We will repeat the Delphi process and consensus meeting to select measures for each core outcome. We will recruit 240 Delphi participants giving us 80% power to detect a 1.0-1.5 point (9-point scale) difference by feedback method between rounds. We will analyse differences for subsequent scores, magnitude of opinion change, items retained and level of agreement. Objectives The aim of this study was to describe clinical and virological outcomes in therapy-naive HIV-1-positive patients treated in a routine ART programme in rural Cameroon. Methods In a prospective cohort, 300 consecutive patients starting first-line ART were enrolled and followed for 12 months. Among 238 patients with available viral load data at Month 12, logistic regression was used to analyse risk factors for virological failure (≥1000 HIV RNA copies/mL) including clinical, immunological and virological parameters, as well as data on drug adherence. Population sequencing was performed to detect the presence of drug-resistance mutations in patients with virological failure at Month 12; minority drug-resistance mutations at baseline were analysed using next-generation sequencing in these patients and matched controls. Results At Month 12, 38/238 (16%) patients experienced virological failure (≥1000 HIV RNA copies/mL). Patients with virological failure were younger, had lower CD4 cell counts and were more often WHO stage 3 or 4 at baseline. Sixty-three percent of patients with virological failure developed at least one drug-resistance mutation. The M184V (n = 18) and K103N (n = 10) mutations were most common. At baseline, 6/30 patients (20%) experiencing virological failure and 6/35 (17%) matched controls had evidence of minority drug-resistance mutations using next-generation sequencing (P = 0.77). Lower CD4 count at baseline (OR per 100 cells/mm3 lower 1.41, 95% CI 1.02-1.96, P = 0.04) and poorer adherence (OR per 1% lower 1.05, 95% CI 1.02-1.08, P < 0.001) were associated with a higher risk of virological failure. Unavailability of ART at the treatment centre was the single most common cause for incomplete adherence. Conclusions Virological failure after 1 year of ART was not associated with minority drug resistance at baseline but with incomplete adherence. Local government plays a vital role in providing infrastructure, services and employment to rural and regional communities. Indeed, threats to the fiscal viability of regional councils may well jeopardise the sustainability of an entire community. In December 2013 the New South Wales (NSW) Minister for Local Government suspended Central Darling Shire (in far-western NSW) and appointed an interim Administrator in response to an unprecedented liquidity crisis. In October 2014 a public inquiry recommended extension of the period of administration until September 2020. This paper considers the processes leading up to this extraordinarily lengthy period of financial administration. In particular, we examine the claim that an inequitable allocation of Financial Assistance Grants (FAGs) was a major factor in bringing about the Shire's liquidity crisis. We conclude our analysis with some recommendations for changes to FAG allocations which will help ensure sustainable futures for rural communities.

© 2016 Australian Political Studies Association. Albert Hirschman's rhetoric of reaction is a potentially powerful typology of the arguments made by both proponents and opponents of reform (1991. The rhetoric of reaction: Perversity, futility, jeopardy. Cambridge: Belknap Press of Harvard University Press). However, scholars have identified a number of gaps in the typology, in particular that it has struggled to explain the lines of rhetoric associated with disputed empirical evidence. This paper reviews Hirschman's typology before applying it to the contentious municipal amalgamation debates currently unfolding in New South Wales, Australia. We then examine the lines of attack open to progressives and reactionaries on the basis of empirical data. We conclude that the use of empirical data opens new lines of rhetoric for both ‘progressives’ and ‘reactionaries’ generally, but that both information costs and complexity significantly affect the timing and penetration of the arguments.

Generally, in the history of religion, the interrelation of religion and society has produced numerous invented traditions. Hence, not only a specific religion has had various appearances during its history, but also it may be different in the same time in different parts of the world. This study provides a discussion to clarify that to what extent the history of Islam has been invented and how. For this purpose, two different viewpoints of the insiders or Muslims and the outsiders or non-Muslims will be examined. In fact, it is clear that each side looks at this historical scene through its specific eyeglasses. This study also indicates how Islamic history and tradition could be seen differently by different groups of Muslims themselves.
