Purpose: To analyze hyperreflective foci typically seen in diabetic macular edema (DME) in optical coherence tomography (OCT)., Design: Prospective clinical trial., Participants: Twelve consecutive patients with treatment-naïve, clinically significant DME., Methods: During a same-day examination, a standardized visual acuity assessment (Early Treatment of Diabetic Retinopathy Study protocol), infrared fundus imaging, color fundus photography, and biomicroscopy were performed. Additionally, all patients were scanned using Stratus, Cirrus, and Spectralis OCT and results correlated., Main Outcome Measures: Morphologic changes secondary to DME., Results: In all eyes with DME, distinct hyperreflective foci distributed throughout all retinal layers were found in the OCT scans of all 3 OCT devices. These deposits could not be identified by infrared imaging, fundus photography, or biomicroscopy as long as they were not confluent. Accumulations of such foci at the border of the outer nuclear and in the outer plexiform layer were recognizable clinically as hard exudates showing the same hyperreflective features in OCT. The hyperreflectivity of these foci did not correspond with intraretinal hemorrhage, nor did the lesions cause the characteristic OCT laser beam scattering phenomena typically seen secondary to intraretinal bleedings or microaneurysms. Further, they were detected within the walls of intraretinal microaneurysms., Conclusions: Well-demarcated, hyperreflective foci were identified in the retina of patients with DME. The deposits were located within walls of intraretinal microaneurysms and scattered throughout all retinal layers, forming confluent plaques in the outer plexiform layer. It is suggested that the foci represent extravasated lipoproteins and/or proteins being a very early subclinical barrier breakdown sign in DME.

Purpose: To correlate leakage patterns in fluorescein angiography (FA) images and retinal morphologic features in high-definition optical coherence tomography (HD OCT) images in diabetic macular edema., Design: Prospective pilot study and case series., Participants: Nine consecutive patients (10 eyes) with diabetic macular edema., Methods: All patients were examined using FA (HRA 2; Heidelberg Engineering) and HD OCT (Carl Zeiss Meditec; resolution, 512x128 pixels, 5.8x5.8 mm, and high-resolution scans consisting of 4096 A scans) on the same day. Using Adobe Photoshop (CS2 Version 9.0; Adobe Systems Incorporated, San Jose, CA) a grid containing 15x7 fields was superimposed on the HD OCT en face image and a late-phase FA image according to retinal landmarks. In each patient, a standardized analysis of 105 subfields was performed to provide a characterization of the type of vascular leakage in FA and the associated retinal morphologic changes in corresponding locations., Main Outcome Measures: Angiographic leakage type and structural alteration in retinal morphologic features in OCT., Results: There was a high consistency between FA and OCT in the petaloid pattern of hyperfluorescence that correlated with the presence of large cystic spaces in the outer nuclear layer (ONL) and the outer plexiform layer (OPL) with or without subretinal fluid in 30.4% and 69.6% of graded fields, respectively. A honeycomblike pattern of hyperfluorescence was associated with swelling and cystic spaces in ONL, OPL, the inner nuclear, and the inner plexiform layer in 71.4% of graded fields. Diffuse patterns of hyperfluorescence did not correlate with characteristic retinal changes in HD OCT, but rather showed diversity in the type of morphologic alteration. The presence of central fluid pooling, such as subretinal fluid, could be identified only by HD OCT and not by FA., Conclusions: In the examined patients, a petaloid pattern and a honeycomb pattern of hyperfluorescence observed in FA were found to correlate to characteristic changes in HD OCT, whereas a diffuse leakage pattern was associated with nonuniform changes in retinal morphologic features., Financial Disclosure(s): The author(s) have no proprietary or commercial interest in any materials discussed in this article.

Background: The evaluation of OCT3 during anti-VEGF therapy., Methods: A total of 29 patients with choroidal neovascularization secondary to AMD received 3 intravitreal injections of ranibizumab. OCT examination and visual acuity testing (ETDRS) were performed before therapy onset, after 1 week and after 1, 2 and 3 months., Results: The central retinal thickness (CRT) was assessed correctly by OCT in 128 out of 145 measurements. There was a distinct (84%) but not significant correlation between decrease in central retinal thickness and increase in visual acuity. Incorrect retinal layer segmentation and inadequate CRT measurements were a significant technical limitation., Conclusion: During anti-VEGF therapy, OCT allows documentation and quantification of morphologic retinal changes and in most cases also for an estimation of functional improvement.

By the end of the first year, infants are able to recognize both goal-directed and perceptually guided behavior in the actions of non-human agents, even faceless ones. How infants derive the relevant orientation of an unfamiliar agent in the absence of familiar markers such as eyes, ears, or face is unknown. The current studies tested the hypothesis that infants' calculate an agent's "front" from the geometry of its behavior in the spatial environment. In the first study, 14- to 15-month-old infants observed a symmetrical, faceless agent either interact contingently with a confederate or act randomly. It then turn toward one of two target objects. Infants were more likely to look in the direction the agent turned than the opposite direction, but only in the contingent condition. In the second study, the location of the confederate and target objects was varied, which in turned influenced which end of the agent infants interpreted as the front. Finally, implications for infants' early gaze-following behaviors with humans are tested and implications for theory of mind development more broadly are discussed.

Background: To evaluate the effect of systemic bevacizumab (Avastin) therapy on pigment epithelial detachment (PED) secondary to age-related macular degeneration (AMD) and to identify prognostic factors for PED regression and improvement in best corrected visual acuity (BCVA)., Study Design: Prospective uncontrolled pilot study., Methods: Nine patients (nine eyes) received three systemic bevacizumab treatments at 2 week intervals and were examined at baseline, weeks 1, 2, 4, 6 and month 3 by using optical coherence tomography (Stratus OC, Carl Zeiss Meditec, Dublin, California, USA). Changes in maximum PED height and greatest linear diameter (GLD) were planimetrically analysed by using Adobe Photoshop CS and correlated with retinal morphological changes and changes in BCVA., Results: Systemic bevacizumab therapy was well tolerated. Mean maximum PED height decreased significantly by 21% as early as 1 week (-96 microm (SD 48.8), p<0.01). At 3 months follow-up, two PEDs resolved completely, mean maximum PED height decreased significantly by 39% (-179 microm (SD 178), p = 0.02) and mean PED GLD by 24% (-714 microm (SD 1010), p = 0.07). Mean BCVA improved significantly by week 2 (+8.7 letters (SD 5.7), p<0.01) and at 3 months with 12.7 letters (SD 6.4) (p<0.01)., Conclusion: In the examined nine patients, systemic bevacizumab therapy showed evidence for an effect on PED secondary to neovascular AMD in terms of a decrease in lesion height and diameter. A high PED at baseline was found to be a negative predictive factor for visual outcome.

Purpose: To compare the posterior capsule opacification (PCO) inhibiting effect of the round anterior and sharp posterior optic edge profile of the Clariflex silicone intraocular lens (IOL) [AMO Inc, Santa Ana, California, USA] with that of the double-round edge profile of the SI40 (Phacoflex; AMO Inc, Santa Ana, California, USA) silicone IOL over a period of three years., Design: Prospective, randomized, double-masked, bilateral clinical trial., Methods: The study took place at the Department of Ophthalmology, Medical University of Vienna, Austria. Fifty-two patients with age-related cataracts (104 eyes) were included in the study. Each patient received an SI40 IOL (round edges) in one eye and a Clariflex IOL with OptiEdge (sharp posterior optic edge) in the other eye. Follow-up examinations were at one week, one month, six months, and one, two, and three years. Digital retroillumination images were taken of each eye. The amount of PCO was subjectively assessed with the slit-lamp and objectively assessed by automated image analysis software (the computer program Automated Quantification of After-Cataract [AQUA]; Vienna, Austria) one, two, and three years after surgery., Results: The Clariflex lens showed markedly less PCO at one, two, and three years after surgery. The mean AQUA PCO score was 1.39 for the SI40 and 0.56 for the Clariflex lens after one year, estimated at 1.64 and 0.57 after two years, and at 2.04 and 0.64, respectively, after three years (scale zero to 10; P < .001). The Nd:YAG laser capsulotomy rate was far higher in the SI40 group (Five cases vs one case at three years)., Conclusions: The sharp posterior optic edge profile of the Clariflex silicone IOL led to marked and consistently less PCO than the round-edged SI40 IOL one, two, and three years after surgery.

Previous studies suggested altered sensitivity of the GABA/benzodiazepine receptor system in alcoholic patients. Expanding on these findings, the present functional magnetic resonance imaging (fMRI) study aimed to assess whether a differential modulation of cognitive brain activation by an acute GABAergic drug challenge could be detected in patients with alcoholism. Eight detoxified male patients meeting DSM-IV criteria for alcohol dependence and nine healthy male control subjects were studied with fMRI while performing a 2-back working memory task. The fMRI scans were performed 1 h after intravenous administration of saline and again 1 h after 0.03 mg/kg lorazepam I.V. After saline, a task x group interaction effect with higher task activation in alcoholic patients in the left cerebellum and the right prefrontal cortex emerged. Additionally, a differential task x drug x group interaction was identified in the right cerebellum with more pronounced reduction in cognitive activation after lorazepam in the patient group. A significant correlation between lorazepam sensitivity and duration of alcohol dependence was detected. The present findings are in line with previous studies suggesting disrupted prefrontal-cerebellar activation with potential compensatory hyperactivation of the compromised brain networks in alcoholism. Moreover, the results suggest enhanced responsivity to an acute GABAergic challenge in the right cerebellum with disease-related disruption of cerebellar functional integrity.

Aim: Aim of this study was to investigate the relationship between refractive changes in the eye and biometric changes of the human crystalline lens during accommodation. Furthermore, differences in these relationships between young, healthy emmetropic and myopic subjects were analyzed., Methods: Mean relative change in anterior chamber depth (ACD), lens thickness (LT), anterior segment length (ASL = ACD + LT) and in objective refraction were simultaneously assessed during near-point-induced accommodation in 10 emmetropic and 10 myopic subjects. Via a beam splitter, measurements were performed simultaneously using partial coherence interferometry (PCI) and infrared (IR) photorefraction., Results: On average, for each diopter of accommodation LT increased by 0.063 mm in emmetropic and by 0.072 mm in myopic eyes, and ACD decreased by 0.047 mm and 0.057 mm, respectively. Mean ASL, indicating the position of the posterior lens pole, increased by 0.009 mm in emmetropic and by 0.013 mm in myopic eyes. The correlation between refractive and biometric changes was found to be essentially linear in both subgroups. Differences in ACD between emmetropic and myopic eyes were statistically significant at an accommodative stimulus of -1 D (p<0.04) and -2 D (p<0.02)., Conclusion: The biometric and refractive changes of the human lens are highly correlated and linear in function in both emmetropic and myopic eyes.

Purpose: To assess whether a temporal limbal-corneal incision approach for phacoemulsification cataract surgery induces a gradient in corneal thickening along the horizontal meridian., Setting: Department of Ophthalmology, Medical University of Vienna, Vienna, Austria., Methods: Corneal thickness in 21 eyes of 21 patients was measured preoperatively as well as 1 day, 1 week, and 1 and 3 months after phacoemulsification through a temporal limbal-corneal incision. Measurements were performed using partial coherence interferometry (PCI) with a commercial instrument, the ACMaster (Carl Zeiss Meditec, Jena). Measurements were taken along the horizontal meridian centrally along the visual axis at 1.5 mm, 3.0 mm, and 4.5 mm eccentricity., Results: Preoperatively, there were slight nasal-temporal differences in corneal thickness at all eccentricities. The mean thickness was 522 microm +/- 34 (SD) at 1.5 mm nasally and 513 +/- 36 microm at 1.5 mm temporally (P<.01). On day 1, there was a significant mean increase in corneal thickness (38 +/- 43 microm) along all locations. The thickening was slightly more pronounced in the periphery than in the center, a difference not reaching statistical significance. At 1 week, corneal thickness returned almost to baseline at all locations except for 3.0 mm temporally, where it was slightly, but not significantly, thicker (mean 8 +/- 14 mum). At 1 month, corneal thickness at the 3.0 mm temporal location returned to baseline., Conclusions: A nasal-temporal difference in corneal thickness was found preoperatively in all patients. Phacoemulsification through a temporal limbal-corneal incision caused an increase in corneal thickness along the horizontal meridian 1 day after surgery. The prolonged corneal thickening at 3.0 mm eccentricity temporally could be a result of the proximity to the incision site.

Purpose: To evaluate anterior segment biometry using the ACMaster (Carl Zeiss Meditec AG, Jena, Germany) in regard to precision of measurement and clinical performance compared to the original laboratory prototype of the partial coherence interferometry technique., Methods: Ten phakic (20 eyes) and 27 pseudophakic (44 eyes) patients were included in this study. Anterior segment biometry of phakic and pseudophakic study eyes was performed using the ACMaster and the laboratory prototype of the partial coherence interferometry technique. The ACMaster is a commercially available device based on the partial coherence interferometry technique, which provides high precision anterior segment measurements in a quick and user-friendly fashion. Examination included measurement of central corneal thickness, anterior chamber depth, and lens thickness, the latter only in the phakic eyes., Results: The consistency of anterior segment measurements performed with both units was excellent in phakic as well as in pseudophakic eyes. In pseudophakic eyes, the measurement of anterior chamber thickness and lens thickness with the ACMaster produced several (typical) measurement artifacts in some cases, which partially aggravated the identification of the A-scan peak representing the anterior intraocular lens surface., Conclusions: The ACMaster is a user-friendly device that enables axial anterior segment biometry using the partial coherence interferometry technique with a reproducibility similar to that of the original laboratory prototype.

Purpose: To evaluate the long-term effects of anterior capsule polishing on regeneratory posterior capsule opacification (PCO), anterior capsule opacification (ACO), and fibrotic PCO with a silicone intraocular lens (IOL) with sharp optic edges., Setting: Department of Ophthalmology, Medical University of Vienna, Vienna, Austria., Methods: This prospective bilateral randomized patient- and examiner-masked clinical trial comprised 130 eyes of 65 patients with bilateral age-related cataract. All eyes had implantation of a 3-piece silicone IOL with a truncated, sharp-edged optic (CeeOn Edge 911A, Advanced Medical Optics). In 1 eye, the anterior capsule was extensively polished using an aspiration curette after phacoemulsification and cortex aspiration. Regenerative PCO was quantified objectively, while ACO and fibrotic PCO were graded subjectively 1, 2, 3, and 5 years postoperatively., Results: The mean ACO score was significantly lower in the eyes in which the anterior capsule had been polished (1 year, P<.02; 2 years, P<.03; 3 years, P<.01; 5 years, P<.01). The mean difference in regeneratory PCO and fibrotic PCO scores between the 2 groups was not statistically significant., Conclusions: Three years after cataract surgery, eyes in which the anterior capsule had been polished had significantly less ACO. However, polishing did not lower PCO intensity when a sharp-edged CeeOn 911A IOL was implanted in the bag. Although results indicate that anterior capsule polishing may enhance the development of regeneratory PCO, this trend did not reach statistical significance.

The general practitioner called upon to provide medical care to the older woman for diseases other than gynecological or obstetric illness, should not neglect to recommend gynecological cancer screening, which is of particular importance in this group. In many differential diagnoses, the general practitioner must also give consideration to gynecological-obstetric aspects. Thus, for example, when a patient presents with lower abdominal pain, gynecological pathology must always be taken into account. Numerous preparations with differing actions and applications used in contraception or hormone therapy in the post-menopausal woman, require individual counseling by the gynecologist.

Background/aim: Owing to the complex topographical aspects of ophthalmic surgery, teaching with conventional surgical videos has led to a poor understanding among medical students. A novel multimedia three dimensional (3D) computer animated program, called "Ophthalmic Operation Vienna" has been developed, where surgical videos are accompanied by 3D animated sequences of all surgical steps for five operations. The aim of the study was to assess the effect of 3D animations on the understanding of cataract and glaucoma surgery among medical students., Method: Set in the Medical University of Vienna, Department of Ophthalmology, 172 students were randomised into two groups: a 3D group (n=90), that saw the 3D animations and video sequences, and a control group (n=82), that saw only the surgical videos. The narrated text was identical for both groups. After the presentation, students were questioned and tested using multiple choice questions., Results: Students in the 3D group found the interactive multimedia teaching methods to be a valuable supplement to the conventional surgical videos. The 3D group outperformed the control group not only in topographical understanding by 16% (p<0.0001), but also in theoretical understanding by 7% (p<0.003). Women in the 3D group gained most by 19% over the control group (p<0.0001)., Conclusions: The use of 3D animations lead to a better understanding of difficult surgical topics among medical students, especially for female users. Gender related benefits of using multimedia should be further explored.

Introduction: Unilateral ovarian agenesis is a rare event and only a few case have been reported., Case Report: We present three additional cases, where patients presented with diffuse lower abdominal pain. During laparoscopy, an unilateral ovarian agenesis was observed in the three cases., Discussion: There are two possible explanations of a unilateral ovarian absence, involving an asymptomatic adnexal torsion or congenital absence. Unknown environmental factors or genetic predisposition could contribute to this kind of anomaly.

A primary carcinoma of the Bartholin's gland is a rarely type of vulva-carcinoma. The patients median age are 55 years and the etiology of the carcinoma are still unknown. Viral influences are discussed. Most frequent types of carcinomas of Bartholin's gland are those of squamous cells and adenocarcinomas. This article reports on a 55 year old patient with a primary carcinoma of Bartholin's gland. The human papillomavirus 18 was detected by a polymerase chain reaction. The correct diagnosis was delayed because of an initial misinterpretation of the carcinoma as a Bartholin cyst, marsupialization and bad healing of the wound. The patient underwent a radical vulvectomy with bilateral inguinofemoral lymphonodectomy in combination with radiation therapy.

World-wide intensive efforts have been made for more than 20 years to reduce the rate of preterm delivery, but so far without remarkable success. Preterm delivery accounts for 6-9% of all deliveries and very-low-birth-weight infants (< or = 1000 g) total up to about 60% of perinatal mortality. This study reports on a retrospective analysis of fetal outcome with 33 very-low-birth-weight-infants (< or = 1000 g) delivered at the Women's Hospital (University of Rostock) during the time from 1986 to 1992. The rate of early mortality was at 45.2%, survival rate was at 33% (n = 11). Caesarean section with preservation of fetal membranes proved to offer better results of fetal outcome than spontaneous delivery. Comparing the fetal outcome of all patients concerned, it became obvious that newborns whose mothers enjoyed intensive prenatal care had better chances than those newborns whose mothers came to hospital not before onset of preterm labour. From our experiences we conclude that intensive antenatal care with very early assessment of all possible risk factors for preterm labour is the prerequisite to reduce the rate of preterm delivery.

A report is given on the levels of c-reactive protein (crp) and haptoglobin (hpt) of 168 puerperae at the second and fourth day post partum. All of the patients had a afebrile puerperium. We conclude that the value of crp (< 5 mg/l) is grown up during the puerperium. That ist why the sole analysis of crp is not to be a criteria for decision during the puerperium.

A report is given on a 28 years old women with congenital aplasia of the thyroid gland. She was substituted with thyroxine (300 micrograms per day). Her first pregnancy was complicated by gestational hypertension and pre-eclampsia. Delivery was by forceps. During the first trimester of her second pregnancy, bleedings occurred. The thyroid-stimulating hormone level (TSH-level) was increased (18.3 microU/ml). The patient did not show clinical signs of manifested hypothyroidism. The thyroxine dosis was increased. Bleedings disappeared. Labour was terminated and induced. Labour intra partum was hypoactive. The delivery was again by forceps. The newborn did not show any signals of hypothyroidism. Dysfunction of thyroid gland is associated with reduced fertility. Hypothyroidism in pregnancy is associated with an adverse outcome in fetal health as well as an increase in obstetric complications. Thyroid hormones play a vital role in fetal development and maturation of brain. Women with a hypothyroidism have a lower rate of pregnancy and a higher rate of spontaneous miscarriages compared to a normal population. Recognition and treatment of thyroid disorders in reproductive age occur before conception. Iodoprophylaxis is necessary for prevention of congenital hypothyroidism (cretinism). Iodoprophylaxis is necessary to prevent endemic goiter in pregnancy. Euthyroid goiter is an indication for a combined treatment with jodid and levothyroxine. Treatment should be individualized. Assessment of efficacy of treatment is based on measurement of TSH- and free thyroid hormone (fT4)-levels.

Systemic LE is a rare complication in puerperium. A case of an acute exacerbation post partum is reported. The described case brings to attention the fact, that behind febrile conditions in puerperium could stand SLE.

To determine the role of the histamine H3-receptor on basophils, different specific H3-antagonists were investigated. Incubation of washed leukocytes with N alpha-acylated histamine-derivatives (N alpha-ahd) induced elevated histamine levels. This process turned out to be dependent on dose, time and temperature, but independent of Ca2+ and Mg2+ ions. IgE-mediated histamine release was not modulated. [3H]-L-histidine was not decarboxylated into [3H]-histamine in spite of the observed histamine increase. Highly purified basophils did not show any histamine elevation but purified neutrophils and eosinophils were found to have increased histamine levels even after disintegration and subsequent incubation with N alpha-ahd. It seems that the increased histamine levels result from the cleavage of the applied histamine amides. Other potent H3-antagonists (e.g. thioperamide) neither produced increased histamine levels nor influenced IgE-mediated release from basophil leukocytes. The existence of H3-receptors on human basophils therefore seems unlikely.
