Abstract

The placement of a stent in coronary arteries that have become blocked again after coronary artery bypass seems to be better than balloon angioplasty in some ways. A stent is a small cylinder placed in the artery to keep it open. In a group of 220 patients who had received a coronary bypass, those who received a stent were more likely to benefit than those who received balloon angioplasty. This is another technique to open blocked arteries. However, the stent was more likely to cause hemorrhage and it did not reduce the rate of re-blockage in the arteries six months later compared to angioplasty.

309. Synthesis of 4a-hydroxytetrahydropterins and the mechanism of their nonenzymatic dehydration to quinoid dihydropterins

Abstract

The 2'-substituted 2-amino-6-[{aminoethyl}amino]-4,5-pyrimidinediones undergo a intramolecular Schiff base condensation of derivatives of quinoid divicine to form 4a-hydroxytetrahydropterins. The 4a-hydroxytetrahydropterins undergo dehydration through proton and general acid catalysis to form quinoid dihydropterins. The dehydration rate is very slow at pH 7.4 and 37 degrees Celsius for efficient cofactor regeneration during the in vivo phenylalanine hydroxylase reaction.

Abstract

A study confirms that either the (6R)- or the (6S)-isomer of 5,10-CH+ -H4folate can bind to the photolyase of E. coli. The chromophore of the folate acts as an antenna, gathering light energy that is translocated to the reduced flavin where DNA repair takes place. The interchromophore energy transfer is effective in native enzyme but is reduced 2-folds in enzyme reconstituted with (6R,S)-5,10-CH+ -H4folate. This reduction may be due to the replacement of the polyglutamate moiety in the natural folate with monoglutamate or because the synthetic folate is racemic.

Abstract

Research shows that the enzyme 4a-hydroxytetrahydropterin dehydratase plays an important part in the metabolism of the aromatic amino acids. This enzyme regenerates the tetrahydrobiopterin cofactor necessary for the hydroxylation of aromatic amino acids. Although this regeneration can occur in the absence of the enzyme, the slower pace could cause the accumulation of 4a-hydroxytetrahydrobiopterin, which could inhibit the hydroxylases.

Abstract

A C6-stereospecific synthesis of tetrahydropteridines was developed. Condensation of 2-amino-6-chloro-5-nitro-4(3H)-pyrimidinone with a chiral diamine and subsequent oxidative cyclization, hydrolysis and reduction produced 6(R)- and 6(S)-propyl-5,6,7,8-tetrahydropterin from L- and D-norvaline, respectively. Likewise, the reductive alkylation of (para-aminobenzoyl)-L-glutamate with a nitropyrimidine aldehyde from D- or L-serine and subsequent cyclization and reduction yielded (6R)- or (6S)-tetrahydrofolic acid.

339. Plexus-specific retinal capillary avascular area in exudative age-related macular degeneration with projection-resolved OCT angiography

Abstract

ObjectiveTo detect the plexus-specific retinal capillary avascular area in exudative age-related macular degeneration (EAMD) with projection-resolved optical coherence tomography angiography (PR-OCTA).Methods and analysisIn this prospective cross-sectional single centre study, eyes with treatment-nai¨ve EAMD underwent macular 3×3 mm OCTA with AngioVue system. OCTA scans were analysed and processed including three-dimensional projection artefact removal, retinal layer semi-automated segmentation and en face angiogram generation. Automated quantification of extrafoveal (excluding the central 1 mm circle) avascular area (EAA) were calculated on projection-resolved superficial vascular complex (SVC), intermediate capillary plexus (ICP) and deep capillary plexus (DCP), respectively.ResultsNineteen eyes with EAMD and 19 age-matched healthy control eyes were included. There was no significant difference between the EAMD and control eyes in terms of age, sex, axial length and mean ocular perfusion pressure (all p>0.05). Compared with control eyes, EAMD eyes had significantly larger EAA in SVC (median 0.125 vs 0.059 mm2, p=0.006), ICP (0.016 vs 0.000 mm2, p=0.004) and DCP (0.033 vs 0.000 mm2, p<0.001).ConclusionPR-OCTA showed that EAMD is associated with focal avascular area in all the three retinal vascular plexuses.

343. Persistent circulating unmetabolised folic acid in a setting of liberal voluntary folic acid fortification. Implications for further mandatory fortification?

Abstract

Abstract Background Ireland is an example of a country that has extensive voluntary fortification with folic acid. After a public consultation process, in 2006, the Food Safety Authority in Ireland FSAI 1 recommended mandatory fortification. However due to safety considerations this decision is now on hold. Before mandatory fortification goes ahead, existing levels of unmetabolised folic acid and their anticipated increase after fortification needs investigation because of the potential of folic acid to mask pernicious anaemia and possibly accelerate the growth of existing cancers. The aim of this study was to examine the levels of circulatory unmetabolised folic acid in Irish adults (both fasted and un-fasted) and new-born infants (fasted) before the proposed implementation of mandatory folic acid fortification. A secondary aim was to predict the increase in circulatory unmetabolised folic acid levels after fortification. Methods Study 1. Setting: Irish Blood Transfusion Service (IBTS). Whole blood samples were collected from blood donors (n = 50) attending for routine blood donation sessions (representing the general population). Subjects were not fasted prior to sampling. Study 2. Setting: Coombe Women's and Infant's University Hospital, Dublin. Whole blood samples were collected by venipuncture from mothers (n = 20), and from their infant's umbilical-cords (n = 20) immediately after caesarean section. All women had been fasted for at least 8 hours prior to the surgery. A questionnaire on habitual and recent dietary intakes of folic acid was administered by an interviewer to all subjects. The data collection period was February to April 2006. Serum samples were analysed for plasma folate, plasma folic acid and red cell folate. Results Blood Donor Group: Circulatory unmetabolised folic acid was present in 18 out of 20 mothers (fasted) (CI: 68.3%–99.8%) comprising 1.31% of total plasma folate, 17 out of 20 babies (fasted) (CI: 62.1%–96.8%), and 49 out of 50 blood donors (unfasted) (CI: 88.0%–99.9%), comprising 2.25% of total plasma folate, Conclusion While the levels of circulatory unmetabolised folic acid reported are low, it is persistently present in women immediately after caesarean section who were fasting indicating that there would be a constant/habitual exposure of existing tumours to folic acid, with the potential for accelerated growth. Mandatory fortification might exacerbate this. This has implications for those with responsibility for drafting legislating in this area.

344. Associations between calf, thigh, and arm circumference and cardiovascular and all-cause mortality in NHANES 1999-2004.

Abstract

Background and Aims: Prior studies have described an association between calf circumference and cardiovascular disorders. We evaluated the associations between calf, thigh, and arm circumference and cardiovascular and all-cause mortality.Methods and Results: We performed a retrospective cohort study of 11,871 patients in the 1999-2004 National Health and Nutrition Examination Survey (NHANES) to determine the association between calf circumference and cardiovascular and all-cause mortality using univariate and multivariate Cox proportional hazards. We additionally examined the association between thigh and arm circumference and mortality. In the multivariable Cox regression for the female stratum, each centimeter increase in calf circumference was associated with a hazard ratio of 0.88 (95% CI 0.84-0.92), and a hazard ratio of 0.90 (95% CI 0.85-0.95) for cardiovascular death. In the model with males, the hazard ratio for higher calf circumference was 0.92 (95% CI 0.88-0.96) for all-cause mortality and 0.94 (95% CI 0.89-0.99) for cardiovascular death. There was a statistically significant association between higher thigh circumference and lower risk of all-cause and cardiovascular mortality. Arm circumference was not similarly associated with mortality in the multivariate model.Conclusion: Calf and thigh circumference may provide important prognostic information regarding cardiovascular and all-cause mortality. 345. Treatment of pelvic fractures in cats with patellar fracture and dental anomaly syndrome.

Abstract

Objectives: The aim of this study was to describe the treatment and outcome of acetabular and other pelvic fractures in cats with patellar fracture and dental anomaly syndrome (PADS) and to provide advice on how to manage these cases in practice. Methods: Data were collated on cats with PADS that were reported to have sustained pelvic fractures or had fractures or fissures of the pelvis identified on submitted radiographs. The details of the fractures were recorded, in addition to any treatment and outcome information. Results: Of the 215 cases reported with PADS, 58 cats (27%) were found to have pelvic fractures, none of which were known to have resulted from significant trauma. There were 101 fractures in total and of these 15 were treated with surgery, including 10 acetabular fractures, two ilial, two pubic and one ischial fracture. Screw loosening and loss of fracture reduction was seen in four of the surgically treated fractures (two pubic fractures, one ilial and one acetabular fracture). Fourteen cats were euthanased as a direct result of a fracture occurring. While most pelvic fractures healed uneventfully, some cats remained intermittently lame, but it was not always possible to determine the cause of the lameness from the information available and because all cats had concurrent patellar fractures. Conclusions and relevance: Many of the pelvic fractures healed with conservative management. Fractures involving articular surfaces such as acetabular fractures may benefit from surgical stabilisation as surgery may offer the benefits of articular fracture repair with improved joint congruency and a faster return to normal activity. Abstract

Purpose of Review: In this review, we summarize the use of virtual physiologic functional assessments of coronary artery disease and their utility to guide virtual coronary intervention (VCI). Recent Findings: Virtual fractional flow reserve (vFFR), coronary angiography–derived fractional flow reserve (FFRangio), virtual contrast FFR (cFFR), and quantitative flow reserve (QFR) are four technologies that generate computer-based FFR measurements comparable to the gold standard of pressure-wire-based FFR. VCI capitalizes on this technology by utilizing pre- and post-vFFR assessments to predict the physiologic response to stenting. Summary: Physiologic assessment of coronary lesion significance has become a cornerstone of decision-making for revascularization. FFR and non-hyperemic pressure ratio use is limited by the requirement for an intracoronary wire and the additional time required. Virtual physiologic assessments address these shortcomings with accuracy comparable to FFR. Abstract

To identify families of stable planar anchor groups for use in single molecule electronics, we report detailed results for the binding energies of two families of anthracene and pyrene derivatives adsorbed onto graphene. We find that all the selected derivatives functionalized with either electron donating or electron accepting substituents bind more strongly to graphene than the parent non-functionalized anthracene or pyrene. The binding energy is sensitive to the detailed atomic alignment of substituent groups over the graphene substrate leading to larger than expected binding energies for -OH and -CN derivatives. Furthermore, the ordering of the binding energies within the anthracene and pyrene series does not simply follow the electron affinities of the substituents. Energy barriers to rotation or displacement on the graphene surface are much lower than binding energies for adsorption and therefore at room temperature, although the molecules are bound to the graphene, they are almost free to move along the graphene surface. Binding energies can be increased by incorporating electrically inert side chains and are sensitive to the conformation of such chains. [ABSTRACT FROM AUTHOR]

Abstract

Nurses caring for patients undergoing the intracoronary stent procedure as an alternative to coronary angioplasty should educate themselves on the patient's needs both before and after the surgery. Eligibility for the procedure includes de novo lesions within a coronary artery measuring 3.0 to 4.0 millimeters and an ability to tolerate anticoagulation. Patients with extreme hypertension or obesity are not eligible. The nurse should inform the patient of all aspects of intracoronary stent surgery and be assured that the patient is making an informed choice in undergoing the procedure.

Abstract

We report the first organically synthesized sp–sp3 hybridized porous carbon, OSPC‐1. This new carbon shows electron conductivity, high porosity, the highest uptake of lithium ions of any carbon material to‐date, and the ability to inhibit dangerous lithium dendrite formation. The new carbon exhibits exceptional potential as anode material for lithium‐ion batteries (LIBs) with high capacity, excellent rate capability, long cycle life, and potential for improved safety performance.

350. Standard prophylactic versus intermediate dose enoxaparin in adults with severe COVID‐19: A multi‐center, open‐label, randomized controlled trial

Abstract

Coronavirus disease 2019 (COVID‐19) is associated with coagulopathy but the optimal prophylactic anticoagulation therapy remains uncertain and may depend on COVID‐19 severity. To compare outcomes in hospitalized adults with severe COVID‐19 treated with standard prophylactic versus intermediate dose enoxaparin. We conducted a multi‐center, open‐label, randomized controlled trial comparing standard prophylactic dose versus intermediate dose enoxaparin in adults who were hospitalized with COVID‐19 and admitted to an intensive care unit (ICU) and/or had laboratory evidence of coagulopathy. Patients were randomly assigned in a 1:1 ratio to receive standard prophylactic dose enoxaparin or intermediate weight‐adjusted dose enoxaparin. The primary outcome was all‐cause mortality at 30 days. Secondary outcomes included arterial or venous thromboembolism and major bleeding. A total of 176 patients (99 males and 77 females) underwent randomization. In the intention‐to‐treat population, all‐cause mortality at 30 days was 15% for intermediate dose enoxaparin and 21% for standard prophylactic dose enoxaparin (odds ratio, 0.66; 95% confidence interval, 0.30–1.45; P= .31 by Chi‐square test). Unadjusted Cox proportional hazards modeling demonstrated no significant difference in mortality between intermediate and standard dose enoxaparin (hazard ratio, 0.67; 95% confidence interval, 0.33–1.37; P= .28). Arterial or venous thrombosis occurred in 13% of patients assigned to intermediate dose enoxaparin and 9% of patients assigned to standard dose enoxaparin. Major bleeding occurred in 2% of patients in each arm. In hospitalized adults with severe COVID‐19, standard prophylactic dose and intermediate dose enoxaparin did not differ significantly in preventing death or thrombosis at 30 days.
