BACKGROUND: Right hemicolectomy or ileocecal resection are used to treat benign conditions like Crohn's disease (CD) and malignant ones like colon cancer (CC).AIM: To investigate differences in pre- and peri-operative factors and their impact on post-operative outcome in patients with CC and CD.METHODS: This is a sub-group analysis of the European Society of Coloproctology's prospective, multi-centre snapshot audit. Adult patients with CC and CD undergoing right hemicolectomy or ileocecal resection were included. Primary outcome measure was 30-d post-operative complications. Secondary outcome measures were post-operative length of stay (LOS) at and readmission.RESULTS: Three hundred and seventy-five patients with CD and 2,515 patients with CC were included. Patients with CD were younger (median = 37 years for CD and 71 years for CC (P < 0.01), had lower American Society of Anesthesiology score (ASA) grade (P < 0.01) and less comorbidity (P < 0.01), but were more likely to be current smokers (P < 0.01). Patients with CD were more frequently operated on by colorectal surgeons (P < 0.01) and frequently underwent ileocecal resection (P < 0.01) with higher rate of de-functioning/primary stoma construction (P < 0.01). Thirty-day post-operative mortality occurred exclusively in the CC group (66/2515, 2.3%). In multivariate analyses, the risk of post-operative complications was similar in the two groups (OR 0.80, 95%CI: 0.54-1.17; P = 0.25). Patients with CD had a significantly longer LOS (Geometric mean 0.87, 95%CI: 0.79-0.95; P < 0.01). There was no difference in re-admission rates. The audit did not collect data on post-operative enhanced recovery protocols that are implemented in the different participating centers.CONCLUSION: Patients with CD were younger, with lower ASA grade, less comorbidity, operated on by experienced surgeons and underwent less radical resection but had a longer LOS than patients with CC although complications' rate was not different between the two groups.

Aim: Patient- and disease-related factors, as well as operation technique, all have the potential to impact on postoperative outcome in Crohn's disease. The available evidence is based on small series and often displays conflicting results. The aim was to investigate the effect of preoperative and intra-operative risk factors on 30-day postoperative outcome in patients undergoing surgery for Crohn's disease. Method: This was an international prospective snapshot audit including consecutive patients undergoing right hemicolectomy or ileocaecal resection. The study analysed a subset of patients who underwent surgery for Crohn's disease. The primary outcome measure was the overall Clavien–Dindo postoperative complication rate. The key secondary outcomes were anastomotic leak, reoperation, surgical site infection and length of stay in hospital. Multivariable binary logistic regression analyses were used to produce odds ratios and 95% confidence intervals. Results: In all, 375 resections in 375 patients were included. The median age was 37 and 57.1% were women. In multivariate analyses, postoperative complications were associated with preoperative parenteral nutrition (OR 2.36, 95% CI 1.10–4.97), urgent/expedited surgical intervention (OR 2.00, 95% CI 1.13–3.55) and unplanned intra-operative adverse events (OR 2.30, 95% CI 1.20–4.45). The postoperative length of stay in hospital was prolonged in patients who received preoperative parenteral nutrition (OR 31, 95% CI 1.08–1.61) and those who had urgent/expedited operations (OR 1.21, 95% CI 1.07–1.37). Conclusion: Preoperative parenteral nutritional support, urgent/expedited operation and unplanned intra-operative adverse events were associated with unfavourable postoperative outcome. Enhanced preoperative optimization and improved planning of operation pathways and timings may improve outcomes for patients. 315. Electronic transport in nanoscale structures

- Abstract
In this dissertation electronic transport in nanoscale structures is discussed. An expression for the shot noise, a fluctuation in current due to the discreteness of charge, is derived directly from the wave functions of a nanoscale system. Investigation of shot noise is of particular interest due to the rich fundamental physics involved. For example, the study of shot noise can provide fundamental insight on the nature of electron transport in a nanoscale junction. We report calculations of the shot noise properties of parallel wires in the regime in which the interwire distance is much smaller than the inelastic mean free path. The validity of quantized transverse momenta in a nanoscale structure and its effect on shot noise is also discussed. We theoretically propose and show the feasibility of a novel protocol for DNA sequencing based on the electronic signature of single-stranded DNA while it translocates through a nanopore. We find that the currents for the bases are sufficiently different to allow for efficient sequencing. Our estimates reveal that sequencing of an entire human genome could be done with very high accuracy in a matter of hours, e.g., orders of magnitude faster than present techniques. We also find that although the overall magnitude of the current may change dramatically with different detection conditions, the intrinsic distinguishability of the bases is not significantly affected by pore size and transverse field strength. Finally, we study the ability of water to screen charges in nanopores by using all-atom molecular dynamics simulations coupled to electrostatic calculations. Due to the short length scales of the nanopore geometry and the large local field gradient of a single ion, the energetics of transporting an ion through the pore is strongly dependent on the microscopic details of the electric field. We show that as long as the pore allows the first hydration shell to stay intact, e.g., ̃6 nearby water molecules, the electric field of the ion can be well screened. We also discuss the consequences of the formation of hydration layers and of the discrete nature of polarization at atomic length scales for the applicability of continuum dielectric models

Background The objective was to investigate whether gender disparities are found in referrals of patients with acute coronary syndromes to percutaneous coronary interventions (PCIs) or coronary artery bypass grafting (CABG) and, furthermore, to study gender differences in complications and mortality. Methods All consecutive coronary angiographies (CAs) and PCIs performed in Sweden and Iceland are prospectively registered in the Swedish Coronary Angiography and Angioplasty Registry. For the present analysis, data of patients with acute coronary syndromes, enrolled in 2007-2011, were used to analyze gender differences in revascularization, in-hospital complications, and 30-day mortality. Results A total of 106,881 CAs were performed during the study period. In patients with significant coronary artery disease, the adjusted odds ratio (OR) for women to undergo PCI compared with men was 0.95 (95% CI 0.92-0.99) and 0.81 (0.76-0.87) for referrals to CABG. In patients with 1-vessel disease, women were less likely to undergo PCI than men, but women with 2- or 3-vessel or left main stem disease were more likely to undergo PCI. All in-hospital complications after CA followed by PCI were more frequent among women (adjusted OR 1.58 [1.47-1.70]). There was no gender difference in adjusted 30-day mortality after PCI (1.02 [0.92-1.12]) and after CABG (0.97 [0.72-1.31]). Conclusions After CA showing 1-vessel disease, women as compared with men were less likely to undergo PCI. In the group with 2- or 3-vessel disease or left main stem stenosis, women were more likely to undergo PCI but less likely to undergo CABG. However, there was no gender difference in 30-day mortality.

The owlet-nightjar Aegotheles bennettii terborghi was described from a single male specimen given by a local man to Jared Diamond, while he was collecting in the Karimui Basin in the Eastern Highlands of Papua New Guinea, in 1964. There has been no further observation of the taxon, despite extensive field work in 2011-12. In July 2016 we travelled to Karimui with the explicit aim of searching for this taxon, which resulted in the first-ever field observation, including photographs and a possible sound-recording of its call.

Syftet med denna studie har varit att undersöka vilka tankar studenter på två universitet har på ämneslärarprogrammet och specifikt idrott och hälsa utbildningen. Frågeställningarna löd på följande sätt: (i) vilken syn har studenterna från de två universiteten på ämneslärarprogrammets utbildning och specifikt inriktningen idrott och hälsa? Och (ii) skiljer sig synen angående utbildningen beroende på vilket universitet studenterna studerar på? Studien har genomförts med hjälp av till största del en kvantitativ metod; en enkätundersökning som 24 studenter ifrån två universitet har fått genomföra. Studien teoretiska ramverk har utgått ifrån de pragmatiska sanningsteorierna och specifikt konsensusteorin. Studiens resultat visar att det inte går att utläsa några sanningar utifrån ett översiktligt synsätt och utifrån konsensusteorin. Dock visar resultaten att det finns en relativt stor skillnad mellan de två universiteten och när de två universiteten separeras ifrån varandra är det möjligt att se en rad olika sanningar om idrott och hälsa utbildningen hos universiteten. Till exempel på fråga 1 i enkätundersökningen (De praktiska momenten har kännetecknats av god pedagogisk kvalité.), där 46,6 procent av respondenterna från universitet 1 svarade med en 4:a eller 5:a och 100 procent av respondenterna från universitet 2 svarade med en 4:a eller 5:a.

We aimed to investigate whether the concomitant use of tamoxifen with warfarin is associated with higher risk for bleeding among patients with early estrogen-receptor (ER)-positive breast in a population-based nested case-control study. We identified 1787 patients taking warfarin and 92 cases hospitalized for bleeding and found an adjusted odds ratio (OR) of 1.42 (95% confidence interval (CI): 0.84-2.40) for the risk of bleeding in patients treated with warfarin that initiated tamoxifen within the previous 30 days. As a result, we could not definitively rule out a potential association between tamoxifen use during warfarin and bleeding risk in patients with breast cancer.

Hypotension är vanligt förekommande vid anestesi och i den postoperativa fasen. Om blodtrycket sjunker till för låga nivåer eller pågår under för lång tid kan vitala organ som hjärta och njurar ta allvarlig skada. De flesta fall av hypotension behandlas idag retroaktivt. Med möjlighet att kunna upptäcka hypotension innan det uppstår skulle behandling kunna ske i ett förebyggande syfte och på så sätt minska tiden i hypotension för patienten. Med hjälp av maskininlärning går det att analysera blodtryckskurvor och kunna hitta vilka attribut i dem som kan förutse hypotension. Detta projekt har gått ut på att definiera och beräkna attribut för att i framtida arbeten kunna låta en maskininlärningsalgoritm analysera dem. Utöver detta har arbetet med signalbehandling skett för att kunna upptäcka bristande kvalitet på den analyserade signalen och kunna förbättra den vid behov. Målet att definiera och beräkna attribut har lyckats. Målet att kunna avgöra kvaliteten på signalen har lyckats delvis.

Sammanfattning Bakgrund: Alltfler människor är i behov av palliativ vård. Sjuksköterskan har det övergripande ansvaret för omvårdnaden. Att utföra palliativ vård och vård vid livets slut är utmanande och kräver kunskap hos den som vårdar. Syfte: Syftet var att beskriva sjuksköterskor och sjuksköterskestudenters erfarenheter av kompetens i samband med vård i livets slutskede. Metod: En kvalitativ litteraturöversikt har genomförts där 13 kvalitativa artiklar ingår. Resultat: Analysen av materialet resulterade i två teman. Behov av stöd som handlar om kollegialt stöd och emotionellt stöd samt Behov av kunskap och erfarenhet som handlar om behov av utbildning, kommunikativa erfarenheter samt att känna utveckling i professionen. Slutsats: Sjuksköterskor och sjuksköterskestudenter upplever känslomässiga svårigheter att vårda vid livets slutskede. Det föreligger även brist på utbildning för att erhålla färdigheter och kompetens inom palliativ vård. Ökad utbildning under sjuksköterskeutbildningen och stöd av erfarna handledare och kollegor skulle få dem att känna en ökad trygghet i att vårda vid livets slutskede. Despite the growing interest for autistic users within the field of Human-Computer Interaction (HCI), the research is mostly limited to assistive technology and often neglects autistic users' own perspectives. Exploring autistic users’ experiences of different mainstream technologies that they use may give valuable knowledge. Using Instagram as a case for a qualitative study with 30 autistic adults, this paper contributes with such knowledge and in the process, it challenges preconceptions about autism that are common in HCI-research. Data from interviews and a survey (N=30) is analysed in the context of previous research and design concepts, discussing the implications for design and future research. Findings show that autistic users' needs are often similar to those of users in general, but may be more pronounced, and that if design recommendations are to be useful, researchers need to take the diversity within the autistic population into account more than is currently done.

SAMMANFATTNING Bakgrund: Hjärtsvikt är ett livslångt syndrom som anses vara ett globalt växande hälsoproblem. Syndromet har drabbat runt 20 miljoner människor världen över. Hjärtsvikt påverkar hela människans fysiska hälsa och livskvalitet därmed kräver patientens delaktighet i sin egen vård. Hjärtsviktspatienter har ökad risk att utveckla en stillasittande livsstil därför bör fysisk aktivitet utnyttjas som en behandlingsform. Syfte: Syftet med detta arbete var att studera hur fysisk aktivitet kan påverka livskvaliteten hos patienter som lider av syndromet hjärtsvikt. Metod: Studien har en beskrivande design med litteraturöversikt som metod. Studien är baserad på kvantitativa originalartiklar från år 2012–2019. Sökningen skedde med databasen PubMed (Medline) och sökorden var MesH-termer samt fritextsökningar. Resultat: I resultatet av studierna framkommer det att ett strukturerat träningsprogram gav en ökad fysisk prestationsförmåga och förbättrade livskvaliteten hos interventionsgrupperna. Genom information, rådgivning och uppmuntran förbättrade patienter egenvårdshantering men däremot var inte fysisk aktivitet som egenvårdsråd en lika effektiv metod för att förbättra livskvaliteten som ett strukturerat träningsprogram. Träningsmetoder som cykling, styrketräning, promenader, jogging, balansträning och aerobic gav signifikanta förbättringar i livskvaliteten hos hjärtsviktspatienter. Slutsats: Det framgår att olika former av träningsprogram som egenvårdsråd kan förbättra livskvaliteten hos hjärtsviktspatienter. Träning som egenvård ansågs effektfullt, dock visade resultatet att ett strukturerat träningsprogram var mer effektivt. Livskvaliteten förbättrades signifikant i de flesta studierna som använde någon form av kondition, styrketräning eller en kombination av dessa träningsprogram. Vidare studier behövs för att kunna tillämpa fysisk aktivitet som icke-farmakologisk behandling vid hjärtsvikt polikliniskt. ABSTRACT Background: Heart failure is a lifelong syndrome which is considered as a globally growing health problem. The syndrome has affected about 20 million people around the world.The syndrome effects on physical health and quality of life of the patients therefore should patients participate in their self-care. Heart failure increase risk of developing a sedentary lifestyle thus physical activity should be used as a treatment. Aim: The aim of this work was to study how physical activity can affect the quality of life of patients suffering from heart failure syndrome. Method: The study has a descriptive design with literature review as method. The study is based on quantitative original articles from 2012-2019. The research was done with the PubMed (Medline) database and the keywords were MesH terms and free text searches. Results: The results of the studies show that different types of exercise methods can increase the physical performance and quality of life of intervention groups. Through information, counseling and encouragement improved the patient’s self-care but it was not as effective as a structured exercise program to improve quality of life. Exercise methods such as cycling, strength training, walking, jogging, balance training and aerobics provided significant improvements in the quality of life of heart failure patients. Conclusions: Various forms of exercise programs as a self-care advice can improve the quality of life of heart failure patients. Training as self-care was considered effective, however the results showed that a structured exercise program was more effective. The quality of life of patients was significantly improved by using of fitness, strength training or a combination of these exercise programs. Further studies are required to be able to apply physical activity such as non-pharmacological treatment in outpatient heart failure.

A right hemicolectomy is among the most commonly performed operations for colon cancer, but modern high-quality, multination data addressing the morbidity and mortality rates are lacking.This study reports the morbidity and mortality rates for right-sided colon cancer and identifies predictors for unfavorable short-term outcome after right hemicolectomy.This was a snapshot observational prospective study.The study was conducted as a multicenter international study.The 2015 European Society of Coloproctology snapshot study was a prospective multicenter international series that included all patients undergoing elective or emergency right hemicolectomy or ileocecal resection over a 2-month period in early 2015. This is a subanalysis of the colon cancer cohort of patients.Predictors for anastomotic leak and 30-day postoperative morbidity and mortality were assessed using multivariable mixed-effect logistic regression models after variables selection with the Lasso method.Of the 2515 included patients, an anastomosis was performed in 97.2% (n = 2444), handsewn in 38.5% (n = 940) and stapled in 61.5% (n = 1504) cases. The overall anastomotic leak rate was 7.4% (180/2444), 30-day morbidity was 38.0% (n = 956), and mortality was 2.6% (n = 66). Patients with anastomotic leak had a significantly increased mortality rate (10.6% vs 1.6% no-leak patients; p0.001). At multivariable analysis the following variables were associated with anastomotic leak: longer duration of surgery (OR = 1.007 per min; p = 0.0037), open approach (OR = 1.9; p = 0.0037), and stapled anastomosis (OR = 1.5; p = 0.041).This is an observational study, and therefore selection bias could be present. For this reason, a multivariable logistic regression model was performed, trying to correct possible confounding factors.Anastomotic leak after oncologic right hemicolectomy is a frequent complication, and it is associated with increased mortality. Syftet med studien är att bidra med kunskap om fyra förskollärares uppfattningar om trygg anknytning vid inskolning på förskolan. De frågeställningar som vi ville undersöka var hur förskollärare beskriver trygg anknytning vid inskolning men även vilka svårigheter förskollärare upplever med att skapa en trygg anknytning vid inskolning samt hur förskollärare beskriver sin roll som anknytningsperson. Studien utgår från anknytningsteorins centrala begrepp, som är trygg anknytning, anknytningsperson, trygg bas och trygghetscirkeln. Det är dessa begrepp, samt begreppet inskolning, som kommer vara bärande genom studien. Detta småskaliga forskningsprojekt bygger på en kvalitativ ansats. Undersökningsinstrument som har använts för att hjälpa oss att få svar på frågeställningarna är fyra semistrukturerade intervjuer av legitimerade förskollärare som arbetar på småbarnsavdelning med barn i åldern ett till tre år. I studiens resultat framkommer det att samtliga förskollärare visar på en medvetenhet om vikten av en trygg anknytning vid barns inskolning i förskolan. Vidare visar förskollärarna förståelse för hur betydelsefullt arbetet är i deras roll som anknytningsperson. Viktiga framgångsfaktorer som förskollärarna betonar i deras roll är att vara tillgänglig och visa närhet och engagemang. I resultatet lyfter förskollärarna några svårigheter som dem upplever med att skapa en trygg anknytning vid inskolning, så som att vårdnadshavarna inte känner sig trygga med att lämna sitt barn på förskolan respektive när barnet har erfarenhet av otrygg anknytning. Förskollärarna lyfter att anknytningen är viktig utifrån relationen hem och förskola och att anknytningens betydelse är beroende av olika ramfaktorer som finns på förskolan.

When young people travel, they are often very dependent on public transport or parents. This study uses interviews with 16–19 years old teenagers in Stockholm to investigate their everyday experiences of public transit. The paper explores the experiences of buses and subways, here conceptualized as mobile places, to understand how they shape teenagers’ daily life. Understanding teenagers’ experiences of public transportation is part of understanding their everyday life, struggles, and possibilities to be mobile and participate in society. It is also a step towards ensuring that they find public transportation inclusive, safe, and worth traveling with today and in the future. Conceptually, the analysis focuses on how these mobile places are experienced as providing weights or reliefs to the everyday and if, how and when they may be places of interaction or retreat, addressing two needs in teenagers’ personal being and development. The study shows how various experiences of traveling with buses and subways shape how the teenagers feel, and how they make strategic choices in relation to this. A quite manifold, varying, and complex picture of public transportation arises, with stories of wellbeing, comfort, discomfort, and exclusion, and with sharp differences between girls and boys, and between buses and subways. These nuances are essential in planning and evaluation of transport systems in regard to how, when, where, or for whom public transport can be a part of social sustainability, as public policies often assume.

Background Impressive improved one-year survival rates after acute myocardial infarction (AMI) have been achieved the last decades while long-term cardiovascular events still are a challenge particularly when diabetes is present. Recently several glucose lowering drugs with preventive effects on heart failure (HF) and MACE have entered the market however still used in low proportion by cardiologists. Purpose To explore the real-life complication rates of HF after AMI in patients with and without diabetes. Methods All patients with AMI admitted for coronary angiography in Sweden 2012–2017 were followed for time to first hospitalisation with HF diagnosis (ICD-10 code I50) until December 2017. Kaplan-Meier curves were used to estimate the cumulative heart failure event stratified by previous MI. Hazard ratios (HR) were calculated in a Cox proportional hazard regression model adjusting for age, gender, smoking, creatinine, previous CABG/cancer/dementia/dialysis/hypertension/COPD/renal failure/stroke, year, indication, hospital, angiographic findings, primary decision after angiography, cardiac chock, medications at discharge. Results Of 73 303 patients, mean age was 69 years (SD±12), 69% were men and 24% had diabetes. In all, HF occurred in 14% with a higher rate in patients with diabetes than those without (22% vs 12%). The highest HF rates were seen in patients with previous MI (33% if diabetes was present vs. 23% if no diabetes). After adjustments, patients with diabetes without previous MI had about the same HF risk (HR 1.52 [95% CI 1.44–1.61]) as patients without diabetes with previous MI (1.48 [1.40–1.57]) where patients without diabetes and previous MI served as a reference. The same pattern was seen regardless of STEMI/NSTEMI and also after excluding patients with previous HF (n=4567, 6%; Figure; patients with diabetes without previous MI 1.48 [1.40–1.57] and patients without diabetes with previous MI 1.27 [1.19–1.36]). Conclusion Heart failure is a common complication after AMI in patients with diabetes, particularly if previous MI, and regardless of previous reported heart failure. Diabetes increases the risk of heart failure by 30–50% compared to those without diabetes. These data indicate the existence of a large diabetes population at heart failure risk after AMI where heart failure protective glucose lowering drugs could be suitable. Objective: To investigate the association between admission plasma glucose and cardiovascular events in patients with acute myocardial infarction treated with modern therapies including early percutaneous coronary intervention and modern stents. Methods: Patients ( n = 5309) with established diabetes and patients without previously known diabetes with a reported admission plasma glucose, included in the VALIDATE trial 2014-2016, were followed for cardiovascular events (first of mortality, myocardial infarction, stroke, heart failure) within 180 days. Event rates were analysed by four glucose categories according to the World Health Organization criteria for hyperglycaemia and definition of diabetes. Odds ratios were calculated in a multivariate logistic regression model. Results: Mean age was 67 ± 11 years. Previously known diabetes was present in 21.2% ( n = 1124). Cardiovascular events occurred in 3.7%, 3.8%, 6.6% and 15.7% in the four glucose level groups and 9.9% in those with known diabetes ( p < 0.001), while bleeding complications did not differ significantly (9.1%, 8.5%, 8.4%, 12.2% and 8.5%, respectively). After adjustment, odds ratio (95% confidence interval) was 1.00 (0.65–1.53) for group II, 1.62 (1.14–2.29) for group III and 3.59 (1.99–6.50) for group IV compared to the lowest admission plasma glucose group (group I). The corresponding number for known diabetes was 2.42 (1.71–3.42). Conclusion: In a well-treated contemporary population of acute myocardial infarction patients, 42% of those without diabetes had elevated admission plasma glucose levels with a greater risk for clinical events already within 180 days. Event rate increased with increasing admission plasma glucose levels. These findings highlight the importance of searching for undetected diabetes in the setting of acute myocardial infarction and that new treatment options are needed to improve outcome.

The aim of this study was to investigate the outcomes of patients with de novo lesions in small coronary vessels undergoing percutaneous coronary intervention (PCI) with drug-coated balloons (DCBs) or newer-generation drug-eluting stents (n-DES).Notwithstanding the available evidence from a few randomized clinical trials and meta-analyses, the best device for PCI in patients with small-vessel coronary artery disease is not yet established.The study included all consecutive patients with de novo lesions in small coronary vessels undergoing PCI in Sweden from April 2009 to July 2017. A small coronary vessel was defined by a device diameter ≤2.5 mm. The primary outcomes were restenosis and definite target lesion thrombosis at 3-year follow-up. The secondary outcomes were the occurrence of all-cause death and myocardial infarction.The study population included 14,788 patients: 1,154 treated with DCBs and 13,634 with n-DES. Overall, 35,541 PCIs were performed using 2,503 DCBs and 33,038 n-DES. The propensity score-adjusted regression analysis showed a significantly higher risk for restenosis in the DCB group compared with the n-DES group (adjusted hazard ratio [HR]: 2.027; 95% confidence interval [CI]: 1.537 to 2.674). Conversely, no difference in the risk for target lesion thrombosis (adjusted HR: 0.741; 95% CI: 0.412 to 1.331) was detected. The risk for all-cause death (adjusted HR: 1.178; 95% CI: 0.992 to 1.399) and myocardial infarction (adjusted HR: 1.251; 95% CI: 0.960 to 1.629) was comparable between groups.Because of the significantly higher risk for restenosis up to 3 years, this research suggests that DCBs are not an equally effective alternative to n-DES for percutaneous treatment of small coronary vessels.

BACKGROUND: The Genetics of Subsequent Coronary Heart Disease (GENIUS-CHD) consortium was established to facilitate discovery and validation of genetic variants and biomarkers for risk of subsequent CHD events, in individuals with established CHD. METHODS: The consortium currently includes 57 studies from 18 countries, recruiting 185 614 participants with either acute coronary syndrome, stable CHD, or a mixture of both at baseline. All studies collected biological samples and followed-up study participants prospectively for subsequent events. RESULTS: Enrollment into the individual studies took place between 1985 to present day with a duration of follow-up ranging from 9 months to 15 years. Within each study, participants with CHD are predominantly of self-reported European descent (38%-100%), mostly male (44%-91%) with mean ages at recruitment ranging from 40 to 75 years. Initial feasibility analyses, using a federated analysis approach, yielded expected associations between age (hazard ratio, 1.15; 95% CI, 1.14-1.16) per 5-year increase, male sex (hazard ratio, 1.17; 95% CI, 1.13-1.21) and smoking (hazard ratio, 1.43; 95% CI, 1.35-1.51) with risk of subsequent CHD death or myocardial infarction and differing associations with other individual and composite cardiovascular endpoints. CONCLUSIONS: GENIUS-CHD is a global collaboration seeking to elucidate genetic and nongenetic determinants of subsequent event risk in individuals with established CHD, to improve residual risk prediction and identify novel drug targets for secondary prevention. Initial analyses demonstrate the feasibility and reliability of a federated analysis approach. BACKGROUND: Genetic variation at chromosome 9p21 is a recognized risk factor for coronary heart disease (CHD). RESULTS: Meta-analyses revealed no significant association between chromosome 9p21 and the primary outcome of CHD death/myocardial infarction among those with established CHD at baseline (GENIUS-CHD odds ratio, 1.02; 95% CI, 0.99-1.05). Similarly, no clear associations were identified for additional subsequent outcomes, including all-cause death, although we found a modest positive association between chromosome 9p21 and subsequent revascularization (odds ratio, 1.07; 95% CI, 1.04-1.09).CONCLUSIONS: In contrast to studies comparing individuals with CHD to disease-free controls, we found no clear association between genetic variation at chromosome 9p21 and risk of subsequent acute CHD events when all individuals had CHD at baseline. However, the association with subsequent revascularization may support the postulated mechanism of chromosome 9p21 for promoting atheroma development.

European Antimicrobial Resistance Genes Surveillance Network (EURGen-Net) capacity survey group (Portugal): Manuela Caniça, Vera Manageiro, A survey on the epidemiological situation, surveillance and containment activities for carbapenemase-producing Enterobacteriaceae (CPE) was conducted in European countries in 2018. All 37 participating countries reported CPE cases. Since 2015, the epidemiological stage of CPE expansion has increased in 11 countries. Reference laboratory capability, dedicated surveillance and a specific national containment plan are in existence in 33, 27 and 14 countries, respectively. Registry-based randomized clinical trials have emerged as useful tools to provide evidence on the comparative efficacy and safety of different therapeutic strategies. However, it remains unknown whether the results of registry-based randomized clinical trials have a sizable impact on daily clinical practice. We sought, therefore, to describe the temporal trends in thrombus aspiration (TA) use in Sweden before, during, and after dissemination of the TASTE trial (Thrombus Aspiration in ST-Elevation Myocardial Infarction in Scandinavia) results.From January 1, 2006, to December 31, 2017, we included all consecutive patients with ST-segment-elevation myocardial infarction undergoing percutaneous revascularization in Sweden. All patients were registered in the Swedish Coronary Angiography and Angioplasty Registry. A total of 55 809 ST-segment-elevation myocardial infarction patients were included. TA use in Sweden substantially decreased after dissemination of TASTE results (from 39.8% to 11.8% during and after TASTE, respectively). Substantial variability in TA use across treating centers was observed before TASTE (TA use ranging from 0% to 70%), but after TASTE both the interhospital variability and the frequency of TA use were markedly reduced. A constant shift in medical practice was seen about 4 months after dissemination of the TASTE trial results. Time trends for all-cause mortality and definite stent thrombosis at 30 days were not associated with variations in TA use ( P values0.05 using the Granger test).In Sweden, the results of the TASTE trial were impactful in daily clinical practice and led to a relevant decrease in TA use in ST-segment-elevation myocardial infarction patients undergoing percutaneous revascularization.

Background To assess the relationship between radiation doses to the coronary arteries (CAs) and location of a coronary stenosis that required intervention after three-dimensional conformal radiotherapy (3DCRT) for breast cancer (BC). Methods The study population consisted of 182 women treated for BC in Sweden between 1992 and 2012. All women received 3DCRT and subsequently underwent coronary angiography due to a suspected coronary event. CA segments were delineated in the patient’s original planning-CT and radiation doses were recalculated based on the dose distribution of the original radiotherapy (RT) plan. The location of the CA stenosis that required intervention was identified from the Swedish Coronary Angiography and Angioplasty Registry (SCAAR). Logistic regression analysis was used to assess the relationship between CA radiation doses and risk of a later coronary intervention at this specific location. Results The odds ratio (OR) varied by radiation dose to the mid left anterior descending artery (LAD) (p = 0.005). Women receiving mean doses of 1–5 Gray (Gy) to the mid LAD had an adjusted OR of 0.90 (95% CI 0.47–1.74) for a later coronary intervention compared to women receiving mean doses of 0–1 Gy to the mid LAD. In women receiving mean doses of 5–20 Gy to the mid LAD, an adjusted OR of 1.24 (95% CI 0.52–2.95) was observed, which increased to an OR of 5.23 (95% CI 2.01–13.6) for mean doses over 20 Gy, when compared to women receiving mean doses of 0–1 Gy to the mid LAD. Conclusions In women receiving conventional 3DCRT for BC between 1992 and 2012, radiation doses to the LAD remained high and were associated with an increased requirement of coronary intervention in mid LAD. The results support that the LAD radiation dose should be considered in RT treatment planning and that the dose should be kept as low as possible. Minimising the dose to LAD is expected to diminish the risk of later radiation-induced stenosis.
