Abstract

What is known and objective Remifentanil can effectively decrease the sevoflurane concentration to block sympathetic adrenergic response to CO2 pneumoperitoneum stimulus,and liver dysfunction will significantly reduce the MACBAR (minimum alveolar concentration for blocking adrenergic response) of sevoflurane. However, the effects of different remifentanil concentrations on the MACBAR of sevoflurane in patients with liver dysfunction are unclear. The aim of this study was to observe the effects of different remifentanil concentrations by intravenous target-controlled infusion on the MACBAR of sevoflurane in patients with grade B liver dysfunction under carbon dioxide pneumoperitoneum stimulus. Methods Seventy-five patients with grade B liver dysfunction undergoing elective laparoscopic surgery were selected, and randomly divided into three groups with remifentanil plasma target concentrations of 0 (group R0 ), 1 (group R1 ) and 2 (group R2 ) ng/ml. Anaesthesia was induced by intravenous injection of propofol 2-3 mg/kg, remifentanil 2 μg/kg and cisatracurium 0.15 mg/kg. All groups were inhaled different concentrations of sevoflurane. The determination of sevoflurane MACBAR in each group was used a method of sequential-allocation technique, and venous blood samples were taken before and after the creation of carbon dioxide pneumoperitoneum to determine plasma adrenaline and noradrenaline concentrations. Results and discussions The MACBAR of sevoflurane in groups R0 , R1 and R2 was 4.83%, 3.00% and 2.10%, respectively. The MACBAR of sevoflurane was significantly difference among the three groups. When a similar effect of MACBAR had achieved in each group, no significant differences were found in the changes of plasma adrenaline and noradrenaline concentrations before and after the creation of pneumoperitoneum. What is new and conclusion Target-controlled infusion of different concentrations of remifentanil can reduce sevoflurane MACBAR during pneumoperitoneum stimulation in patients with liver dysfunction in some degree. However, the changes of plasma adrenaline and noradrenaline concentrations are consistent in the three groups when patient's stress response was inhibited at the same degree.

Abstract

Background To investigate the effects of different plasma target concentrations of remifentanil on the minimum alveolar concentration (MAC) for blocking adrenergic response (BAR) of sevoflurane in children with laparoscopic herniorrhaphy. Methods Seventy-five children with 3-7 years old scheduled for laparoscopic herniorrhaphy were randomly divided into group R0, group R1, and group R2 according to different remifentanil plasma target concentration (0, 1, and 2 ngml-1), respectively. The MACBAR of sevoflurane was determined by the up-and-down and sequential method in each group. The concentrations of epinephrine and noradrenaline were also determined at corresponding time points. Results A total of 52 child patients were used among the anticipated 75 patients. In groups R0, R1, and R2, the MACBAR of sevoflurane was (3.29 ± 0.17) %, (2.12 ± 0.10) % and (1.29 ± 0.11) %, respectively, and a significant difference was found among the three groups (P Conclusion Remifentanil by target-controlled infusion can effectively reduce the MACBAR of sevoflurane during laparoscopic surgery in children. At a similar effect of MACBAR, both the changes of epinephrine and noradrenaline concentrations are not affected by the infusion of different remifentanil target concentrations. 279. Closed-Form Expressions of Parasitic Parameters for Different Sidewall Roughness of Through-Silicon Vias Interconnects

Abstract

The sidewall roughness of through-silicon vias (TSVs) plays a critical role in the reliability and integrity of signal transmission, particularly at high frequencies. In this study, we introduce a novel approach utilizing generalized closed-form expressions to assess the parasitic RLGC parameters resulting from the sidewall roughness of TSVs under high-frequency conditions. By comparing the calculated results for different sidewall roughness with the outcomes from full-wave electromagnetic (EM) simulations, a high degree of consistency is maintained. Simultaneously, the parasitic parameters showcase systematic variations that correspond to changes in sidewall roughness. Our findings underscore that the adverse impact of TSVs’ parasitic effects becomes more pronounced with heightened sidewall roughness. Importantly, the comparative analysis shows that the correction factor for inductance surpasses that of resistance, underscoring the significance of parasitic inductance. This finding attributes the primary source of parasitic effects to the parasitic inductance resulting from sidewall roughness. In addition, this study meticulously scrutinizes the experimental results, drawing comparisons against both calculated and simulated outcomes, resulting in a good degree of concurrence. This synchronization suggests that the proposed closed-form expressions adeptly capture the nuances of sidewall roughness in TSVs and thus prove highly suitable for scrutinizing a gamut of parasitic effects stemming from sidewall irregularities.

280. Derivation and Analysis of a Hemispherical Physical Model and the Correction Factors for the Sidewall Roughness of Through-Glass Vias

Abstract

The sidewall roughness of through-glass vias (TGVs) at high frequencies significantly impacts the integrity and reliability of signal transmissions via the inductance and resistance-correction factors resulting from the combined effect of the skin depth and sidewall roughness. Here, to further investigate the generation and mechanism of the correction factors, a hemispherical physical model of the sidewall roughness of actual TGVs is proposed, and the ${RL}$ model comprising the skin depth and sidewall roughness is derived; the measured results were compared with the causal hemispherical model and the multilevel causal Huray model, and it was verified that the proposed hemispherical model can calculate the ${RL}$ parasitic parameters of TGV with high accuracy. Concurrently, the proposed hemispherical model confirms that the correction factor for internal inductance is larger than that for resistance; it can explore the main source of sidewall-roughness-induced parasitic effects. Moreover, the model offers a solution for reducing the sidewall-roughness-induced parasitic effects.

281. Analysis and Optimization of Sidewall Roughness on Microwave Performance of Through-Glass Vias in 3-D Integrated Circuits

Abstract

The integrity and reliability of signal transmission in glass 3-D integrated circuits (ICs) can be improved by studying the effect of sidewall roughness of through-glass vias (TGVs) on microwave performance. Thus, in this study, the effect of different sidewall roughness of TGVs on the electrical characteristics, such as loss, delay, and impedance correction factors, are analyzed by extracting S-parameters from the CPW-TGVs-CPW model. An accurate RLGC electrical model of TGVs that considers sidewall roughness is proposed. The correction factor of inductance is verified and found to be greater than that of resistance through simulation, physical measurements, and microscopic characterization analysis. Meanwhile, the sidewall roughness was improved by controlling the laser-induced wet etching (LIWE) process of TGVs. It was greatly reduced from $1.257 \mu \text{m}$ to 25 nm. The proposed electrical model is accurately verified below 40 GHz by extracting the electrical parameters of TGVs with different sidewall roughness.

Abstract

The impact of the contact angle of a droplet, the included angle of a substrate and the droplet volume on the morphology and profile of the droplet is discussed, and the spreading characteristics of lead-free solder on an inverted V-shaped substrate are studied, which provides theoretical guidance and data support for a comprehensive study of the interface reaction and wetting mechanism between solder and substrate, and helps improve the brazing process to adapt to complex welding operations. Based on the method of finite element simulation, different contact angles, included angles of inverted V-shaped substrate, droplet volumes and other variables are entered in the model; the relevant physical parameters are defined according to the surface tension and density of solder Sn-3.0Ag-0.5Cu at temperature of 490K; the theoretical spreading results of the droplet are simulated and calculated by Surface Evolver by using the principle of energy minimization and the method of gradient descent; and the spreading distance, contact area and energy equivalence of the droplet are read out by program, which helps to investigate the spreading behavior and wetting characteristics of the droplet.

298. Students' Pressure, Time Management and Effective Learning

Abstract

Purpose: This paper aims to survey the status quo of the student pressure and the relationship between their daily time management and their learning outcomes in three different types of higher secondary schools at Shenyang, the capital city of Liaoning Province in mainland China. Design/methodology/approach: An investigation was carried out in 14 higher secondary schools (HSS) located in five districts of Shenyang. Both qualitative and quantitative approaches were used, such as interviews and questionnaires. Findings: The important findings include: the students pressure sources in three different types of higher secondary schools, the strongest pressure felt by the HSS students in China was the pressure from national college entrance examinations (NCEE), the rank orders of other pressures were pressures from parents, from society, from others, from schools, from teachers. The findings also include the relationship between student time management (time for sleep, time for getting up, time spent at schools, time for doing homework) and the students learning, the tests frequency in different types of HSS, the relationship between the tests frequency and student learning outcomes, etc. Originality/value: To survey the HSS students' pressure causes, to explore the relationship between their time management and learning outcomes, to find out the effective learning factors and strategies will benefit students, teachers and schools worldwide. Abstract

The existing building stock faces the challenge of low energy efficiency and requires renovation and upgrading to meet society′s goals of carbon reduction and sustainable development. This study presents an optimization framework utilizing genetic algorithms to develop robust retrofit plans that balance the need for improved energy efficiency, cost-effectiveness considerations for householders, and uncertainties regarding climate conditions. A case study of an aged residential building in a hot and humid region of China is used to demonstrate the proposed method. The optimization results show a potential energy demand reduction of 81.5%. However, due to the relatively long time required to realize economic benefits from high investments, short-term optimization tends to favor solutions with high energy demand and low primary costs. To effectively reduce carbon emissions, it is recommended to consider the long-term economic benefits of retrofits and prioritize solutions with high energy efficiency. However, it is important to acknowledge that the expensive nature of retrofit investments may pose barriers to residents. Starting from the inheritance and promotion of Chinese life philosophy and traditional Chinese medicine wisdom by Chinese narrative medicine practice, this paper discusses the inheritance and echo relationship between the "close textual reading ability" in Chinese narrative medicine practice and the "four diagnoses" in traditional Chinese medicine wisdom, narrative mediation and the "mind-body holistic philosophy" in traditional Chinese medicine, and narrative wisdom and Dao Sheng in Chinese life philosophy. Meanwhile, by analyzing the stories of contemporary doctors' practice of narrative intelligence, this paper expounds that Chinese narrative medicine is a new model of medical education and clinical practice based on the absorption of Chinese traditional life wisdom and the essence of traditional Chinese medicine culture, and the integration of western narrative medicine concepts.
