Abstract

Background Intermittent self catheterization questionnaire (ISC-Q) is a questionnaire exploring four issues related to self-intermittent catheterization (ISC): ease of use, convenience, discreteness and psychological well-being. The aim of the study was to develop and validate the Italian version of the ISC-Q. Methods Two independent translations of the ISC-Q were performed and compared by an expert committee. A back translation to English was made by a linguistic expert, blinded to the original version. Patients using ISC to void their bladder were from 19 different spinal cord units; questionnaires were examined anonymously in a single center. A subgroup of 47 patients repeated the test at 2 weeks distance and completed the Qualiveen questionnaire to assess the convergent validity of ISC-Q. The internal consistency was determined from Cronbach's α coefficient. Criterion validity was determined through the concurrent use of Qualiveen. Reliability was performed by Intraclass Correlation Coefficient and test-retest. The sensitivity was determined by the effect size-based estimate for small change (ES) and Minimal Detectable Change 95 (MDC95). In the study 217 subjects (65.6% males, 34.4% females) were enrolled. Mean age: 43±10.1 years. Mean duration of ISC: 3.3±6.9 years. Results Cronbach's α ranged from 0.79 to 0.81. Pearson correlation between the different parts of ISC-Q and total score ranged from 0.24 to 0.78. ICC values were higher than 0.9. Convergent validity was faint to null. ES ranged from 3.8 to 6 points. MDC95 ranged from 4.1 to 12.1. Conclusions The Italian ISC-Q has the same psychometric characteristics of the original questionnaire. ISC-Q is a valid, reliable and sensitive patient related outcome, specifically focused on ISC. our work provides a new validated measure for Italian patients to explore in future studies the role and impact of ISC.

Abstract

Background: Approximately 10-20% of recurrences in patients treated with nephrectomy for renal cell carcinoma (RCC) develop beyond 5 yr after surgery (late recurrence). Objective: To determine features associated with late recurrence. Design, setting, and participants: A total of 5009 patients from a multicenter database comprising 13 107 RCC patients treated surgically had a minimum recurrence-free survival of 60 mo (median follow-up [FU]: 105 mo [range: 78-135]); at last FU, 4699 were disease free (median FU: 103 mo [range: 78-134]), and 310 patients (6.2%) experienced disease recurrence (median FU: 120 mo [range: 93-149]). Interventions: Patients underwent radical nephrectomy or nephron-sparing surgery. Outcome measurements and statistical analysis: Multivariable regression analyses identified features associated with late recurrence. Cox regression analyses evaluated the association of features with cancer-specific mortality (CSM). Results and limitations: Lymphovascular invasion (LVI) (odds ratio [OR]: 3.07; p < 0.001), Fuhrman grade 3-4 (OR: 1.60; p = 0.001), and pT stage >pT1 (OR: 2.28; p < 0.001) were significantly associated with late recurrence. Based on accordant regression coefficients, these parameters were weighted with point values (LVI: 2 points; Fuhrman grade 3-4: 1 point, pT stage >1: 2 points), and a risk score was developed for the prediction of late recurrences. The calculated values (0 points: Late recurrence risk 3.1%; 1-3 points: 8.4%; 4-5 points: 22.1%) resulted in a good-, intermediate- and poor-prognosis group (area under the curve value for the model: 70%; 95% confidence interval, 67-73). Multivariable Cox regression analysis showed LVI (HR: 2.75; p < 0.001), pT stage (HR: 1.24; p < 0.001), Fuhrman grade (HR: 2.40; p < 0.001), age (HR: 1.01; p < 0.001), and gender (HR: 0.71; p = 0.027) to influence CSM significantly. Limitations are based on the multicenter and retrospective study design. Conclusions: LVI, Fuhrman grade 3/4, and a tumor stage >pT1 are independent predictors of late recurrence after at least 5 yr from surgery in patients with RCC. We developed a risk score that allows for prognostic stratification and individualized aftercare of patients with regard to counseling, follow-up scheduling, and clinical trial design. © 2012 European Association of Urology.

Abstract

PURPOSE: We assessed the reporting quality of randomized and nonrandomized, controlled trials presented in abstract form at the European Association of Urology annual meeting in a 10-year period and determined the impact on subsequent publication. MATERIALS AND METHODS: Abstracts presented at the European Association of Urology annual meetings in 1998, 1999, 2008 and 2009 were retrieved and included in analysis. Two 2-year meeting periods were considered, including 1) 1998 and 1999, and 2) 2008 and 2009. Two standardized forms were constructed based on the CONSORT (Consolidated Standards of Reporting Trials) and STROBE (Strengthening the Reporting of Observational studies in Epidemiology) guidelines, each including 15 and 16 items for randomized and nonrandomized, controlled trials, respectively. Reporting quality was assessed by measuring the proportion of items respected by authors when preparing the abstract, defined as the score ratio. Subsequent full-length publication within 2 years after the meeting was also determined by a PubMed® search. Differences between the 2 periods were analyzed by the chi-square and simple t tests. Predictors of subsequent full-length publication were evaluated by multiple logistic regression using meeting period, topic, country of origin, design, multi-institutional study and the proportion of reported items (score ratio). RESULTS: A total of 3,139 abstracts were included in analysis, of which 375 (11.9%) were randomized, controlled trials. Overall oncology represented the main topic (49.2% of all abstracts). The score ratio (proportion of adequately reported items in each abstract) was better for period 1 than 2 for randomized, controlled trials (63% vs 57%) but better for period 2 than 1 for nonrandomized, controlled trials (55.4% vs 53.2%, each p
