Abstract

Endoscopy is a competitive field in clinical practice, in which skilled endoscopists are in high demand. The learning process for Junior Gastrointestinal Endoscopists (JGEs) is difficult, quite long, and technically demanding. This directs JGEs to seek additive learning sources, including online sources. The purpose of this study was to determine the frequency, context, attitudes, perceived benefits, drawbacks, and recommendations for using YouTube videos as an educational platform among JGEs from the uses'prespective. We disseminated a cross-sectional online questionnaire from January 15th to March 17th, 2022, and recruited 166 JGE from 39 different countries. The majority of surveyed JGEs (138, 85.2%) were already using YouTube as a learning tool. The majority of JGEs (97, 59.8%) reported gaining knowledge and applying it in their clinical practice, but 56 (34.6 %) reported gaining knowledge without application in real practice. Most participants (124, 76.5 %) reported missing procedure details in YouTube endoscopy videos. The majority of JGEs (110, 80.9%) reported that YouTube videos are provided by endoscopy specialists. Only one participant, 0.6% out of the 166 JGEs surveyed, disliked video records, including YouTube as a source of learning. Based on their experience, 106 (65.4%) of participants recommended YouTube as an educational tool for the coming generation of JGEs. We consider that YouTube represents a potentially useful tool for JGEs, supplying them with both knowledge and clinical practice tricks. However, many drawbacks could make the experience misleading and time-consuming. Consequently, we encourage educational providers on YouTube and other platforms to upload well-constructed, peer-reviewed, interactive educational endoscopy videos. RESUMEN Antecedentes : La endoscopia es un campo competitivo en la práctica clínica en el que los endoscopistas calificados tienen una gran demanda. El proceso de aprendizaje para los endoscopistas gastrointestinales junior (JE) es difícil, bastante largo y técnicamente exigente. Esto dirige a los JE a buscar fuentes de aprendizaje adicionales, incluidas las fuentes en línea. El propósito de este estudio fue determinar la frecuencia, el contexto, las actitudes, los beneficios percibidos, los inconvenientes y las recomendaciones para el uso de videos de YouTube como una plataforma educativa entre los JE desde la perspectiva de los usuarios. Métodos: Se aplicó un cuestionario transversal en línea difundido del 15 de enero al 17 de marzo de 2022 reclutó a 166 endoscopistas gastrointestinales junior de 39 países diferentes. Resultados : La mayoría de los JE encuestados (138, 85,2%) ya utilizaban YouTube como herramienta de aprendizaje. La mayoría de los JE (97, 59,8 %) refirieron adquirir conocimientos y aplicarlos en su práctica clínica, pero 56 (34,6 %) informaron adquirir conocimientos sin aplicación en la práctica real. La mayoría de los participantes (124, 76,5 %) informó que faltaban detalles del procedimiento en los videos de endoscopia de YouTube. La mayoría de los JE (110, 80,9%) informaron que los videos de YouTube son proporcionados por especialistas en endoscopia. Solo a un participante, el 0,6% de los 166 JE encuestados, le disgustaron los registros de video, incluyendo a YouTube como fuente de aprendizaje. 258. Arab political thought on Arab nationalism and unity : the 1980s and the dialectic of old and new paradigms

Abstract

The Arab World has known an Arab nationalist movement since the nineteenth century with its aims being focused firstly on establishing the existence of an Arab nation and secondly that this nation should be politically united in one pan-Arab nation-state. However, whereas many nationalist movements achieved their goal of establishing a nation-state, the Arab world as a whole was one of frustration for the Arab nationalists. Since the 1967 defeat in the war with Israel and particularly after the deradicalization of Arab politics after the death of Nasser and the rise of Arab petrodollar power, the hold of Arab nationalism on Arab politics has been waning. The event of the Iranian revolution of 1979 had the effect of increasing the power of Islamic fundamentalism in the Arab world. Consequently Pan-Islamism, which pan-Arabism had been able to quieten after the Second World War, returned to compete with Arab nationalism. Since then and especially in the 1980s many would attest to the premise that Arab nationalism was no longer the hegemonic ideology in the Arab world and that raison d'etat was dictating the course of Arab politics. Intellectuals began to question the validity of the pan-Arab ideal. The 1980s are marked by a revived assessment of Arab nationalism. There is what may be seen as a new breed of Arab nationalist thought in the writings on Arabism. The idea is no longer viewed in theological tones. Intellectuals and social scientists are examining the Arabist idea not as an ideal but as useful if realistically approached. The research presents and analyzes Arab thought through the contributions of intellectuals in the 1980s. The investigation establishes the continuation of intellectual efforts by Arab nationalists to articulate the pan- Arab idea. The analysis clearly demonstrates and detects the change in the nature of Arab thought as far as Arab nationalism and Arab unity are concerned and as compared to the kind of thought that was considered dead after 1967. Arab nationalist thought and Arab nationalism are being reconstructed and rejuvenated to accommodate the developments in Arab society and politics since 1967. The debates which Arab thought experienced in the 1980s provide evidence that such development was taking place. The discussion and analysis shows that even though Arab nationalism lost its hegemony over Arab politics, Arab thought has produced an entirely fresh contribution to the ideas of Arab nationalism and Arab unity. The study attempts to qualify previous studies and may play a role in qualifying the death verdict issued upon Arab nationalism. Therefore the study focuses on the debates as far as issues like the Arab nation, Arab unity, Arab state, democracy, and minorities are concerned. The analysis of the debates and contributions, which are largely unknown in the West, determine that a rejuvenation of Arab nationalism is taking place. Arab thought on Arab nationalism and Arab unity is undergoing a dialectic between its old and the newly emerging paradigms.

Abstract

Child sexual abuse is a global issue affecting children. It burdens the entire society physically and mentally. It can cause eating disorders and non-suicidal self-injury in abused people (NSSI). Emotion regulation (ER) is an important etiological link between purging, NSSI, and abusive experiences. We interviewed 80 people, ranged in age from 13 to 20, of whom 62.5% had CSA, versus 30 healthy controls. The Toronto Alexithymia Scale, an eating disorders clinical interview, the Difficulties in Emotion Regulation Scale (DERS) to assess emotion dysregulation, the Self-punishment Scale to assess NSSI, the Mini-Kid for children under the age of 18, and the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID I) for those aged 18 and older were given to victims. CSA was found in 62.5% of the participants. Emotional dysregulation was strongly linked to CSA. Descriptive and identifying difficulties in feelings and externally oriented thinking (p0.001, p0.03, p0.001) were found to be associated with the development of alexithymia. CSA participants had higher NSSI than controls, with 28% having severe self-punishment symptoms (P0.001). Finally, CSA is common in kids and teens. It has negative effects on future generations' mental and physical health. All of these conditions can lead to alexithymia.

Abstract

This thesis presents the CITY benchmark, a database benchmark that fairly represents On-Line Transaction Processing (OLTP) environments. It analyses the most widely used benchmarks in general putting more emphasis on the Wisconsin benchmark and the Transaction Processing Council (TPC) benchmarks (TPC-A, TPC- B and TPC-C) in particular. It also presents an empirical approach to examine the workload of those benchmarks and discovered several technical limitations in their scripts. The thesis also presents an investigation of on-line transactions in large database environments. The tested environments were three of the largest organisations in the UK, those organisations were different in objectives and activities. The investigation identified on-line transaction behaviour and defined the salient characteristics of databases in high-volume transaction environments. The findings from those studies established the basis of a transaction and set of tables that are representative of them. The CITY benchmark design is directly driven from the findings from the empirical studies. The benchmark design took into consideration all the critiques directed towards the TPC benchmarks A, B and C. It is the first benchmark that is designed as a result of studying the behaviour of on-line transactions and databases in large database environments. The CITY benchmark is mainly designed to test and compare database systems performance in high-volume transaction environments (OLTP). The work revealed the salient characteristics of large database environments and identified a typical behaviour of on-line transaction in OLTP environments. This research has clearly shown that the TPC benchmarks are not representative to the domain of high-volume transactions environments (OLTP) and it explained why they could be misleading if used to test database management systems in this domain. Additionally, this thesis presents a database performance evaluation methodology that is based on in-depth studies in large database environments.

265. HEPATIC DICROCOELIASIS IN BEEF CATTLE SLAUGHTERED AT ABU-SIMBEL ABATTOIR: FIRST SLAUGHTER HOUSE RECORD IN EGYPT.

Abstract

Dicrocoelium dendriticum, the small liver fluke, is capable of parasitizing the gall bladder and liver of various mammalian species, especially ruminants. In Egypt, there was no previous abattoir-based research concerned with the incidence of dicrocoeliasis. The objective of our study is to record hepatic dicrocoeliasis in imported beef cattle of Sudanese origin that were slaughtered at Abu-Simbel abattoir, Aswan Governorate, Egypt and describe the detailed hepatic gross as well as histopathological changes induced by it. During a period from December 2020 to October 2021, 1575 liver specimens were examined from which 172 cases showed gross pathological lesions. After collection, affected specimens were fixed in 10% neutral buffer formalin, then they were prepared for histopathological examination. The recorded incidence of hepatic dicrocoeliasis was 7.5% among the affected cases. The affected liver showed fibrosis and paleness with irregular yellowish foci on its surface. Fibrous tissue proliferation was seen surrounding bile ducts. Histopathologically, parasitic eggs were detected in affected liver tissues where they stimulated a granulomatous inflammatory reaction with hepatocellular necrosis and infiltrations of neutrophils, eosinophils and giant cells. Portal fibrosis resulted in the formation of pseudolobules. As a consequence, hepatic dicrocoeliasis is prevalent in imported Sudanese beef cattle and these animals should be protected from snail-infected regions and subjected to careful inspection by veterinarians in slaughterhouses. Abstract

The aim of this study was to elucidate current perspectives of redox status in dairy cows and their effects on energy metabolites and immune-inflammatory status during the close-up period. The study was conducted on dairy cows at various stages of lactation, between November 2019 and January 2020. An observational study was conducted on 36 cows, at ~ 4 weeks (28 day±2 day; means±SD) before the expected time of calving. Cows were proved healthy on both clinical and laboratory examinations. A blood sample was drawn from each cow on the 28th day before the expected time of calving to quantify selected biochemical variables. The studied cows were allocated into two groups based on serum oxidative stress index (OSi) concentrations, the first group included twenty-seven cows with high values of OSi, while the second group included nine cows with low values of OSi and served as negative controls. All cows were clinically healthy and showed no signs of illness throughout the study period. Cows with high OSi had higher serum levels of advanced oxidation protein products, reactive oxygen species (ROS), malondialdehyde, superoxide dismutase, non-esterified fatty acids, ß-hydroxy butyric acid, glucose, serum amyloid A, haptoglobin, and immunoglobulin G than negative controls; while had low levels of serum antioxidant capacity (SAC), glutathione peroxidase, vitamin E, and insulin. The results herein confirmed that cows during the close-up period were likely under oxidative stress, and the latter makes cows vulnerable to the development of negative energy balance and significant immune-metabolic alterations. The results of OSi, ROS, and SAC could be used as reference variables to assess the redox status of transition dairy cattle. [ABSTRACT FROM AUTHOR]

Abstract

The aim of this paper is to identify the effect of PCM as a building material of commercial building envelope on the indoor air temperature within a hot climatic context as Egypt. This paper will expose the potentials of using Phase Change Material (PCM) as an integral part of the building process to save energy, where it first introduces the current environmental challenges and identifies the impact of Thermal Energy Storage as a sustainable approach. Two different types of PCM will be tested in an experimental space in Egypt on different building model orientations and different PCM allocations, to test their effect on indoor air temperature. This will be practically examined by DesignBuilder simulation, to explore the possibilities of improving indoor thermal comfort within the studied types. Finally, conducting a set of recommendations for PCM application as a building material within the Egyptian context.
