Die chronische Pyelonephritis entsteht definitionsgemäß als Folge einer bakteriellen Infektion. Sie wird wegen der Art der morphologischen Organschäden zur Gruppe der interstitiellen Nierenentzündungen gezählt. Es handelt sich offenbar um eine verhältnismäßig häufige Krankheit, da sie bei 40 ~ der Kinder und bei etwa 20 ~ der Er­ wachsenen mit dialysepflichtiger Niereninsuffizienz als Grundleiden angenommen wird (112). Obwohl sich viele Un­ tersuchungen mit dieser Erkrankung befaßt haben, sind wesentliche Fragen der Ätiologie, Pathogenese und Klinik noch ungeklärt geblieben. So ist zum Beispiel noch völlig offen, ob es beim Menschen ebenso wie im Tierexperiment eine chronische bakterielle Nephritis auch ohne die Einwirkung'prädisponierender'Faktoren gibt (12,14,23,24,35,36,38,67,117,118,186,192, 194,293,298). Auch der Einfluß einer bakteriellen Infektion auf die Form der morphologischen Veränderungen in der Niere und den Verlauf ~es Leidens ist umstritten (6,10,11,16,17, 26,37,75,77,84,90,95,103,121,131,157,179,199,200,202,203, 211,229,239,265,271,282,296,313,328,329,332,338,342,349, 350,352). Nach wie vor ist ebenfalls ungeklärt, welche Me­ chanismen dafür verantwortlich sind, daß auch in bakterio­ logisch sterilen Nieren die Entzündung nicht ausheilt und in der chronischen Phase der Pyelonephritis zu einem allmählich fortschreitenden Verlust an Nierenparenchym führt (58,59,84,95,131,182,228). Weiterhin herrscht Un- klarheit über die Bedeutung verschiedener Nierenfunktions­ proben für Diagnose und Verlaufskontrolle der chronischen Pyelonephritis. Ein weiteres klinisch wichtiges Problem stellt die Beziehung zwischen chronischer Pyelonephritis und Hochdruckkrankheit dar. Bei etwa 20% aller Hochdruck­ kranken finden sich Hinweise auf eineNierenerkrankung. Die Hälfte dieser Nierenkranken soll an einer bakteriellen Nephritis leiden. Somit besteht die Möglichkeit einer kau­ salen Beziehung zwischen chronischer Pyelonephritis und arterieller Hypertonie (2,37,48,83,105,129,148,219,263, 309,317,339,356,357).

After more than a decade of successful application of cardiac pace­ makers in the therapy of cardiac rhythm disorders, technological and clinical experience has reached a level, at which a technical survey of this field should be of general interest and might promote the further improvement of pace­ maker therapy. The papers contained in this book were presented at the International Symposium on Advances in Pacemaker Technology, held at Erlangen on Sep­ tember 26 and 27,1974 under the auspices of the Societas Physica Medica Erlangensis. One of the traditional aims of the Societas has been the advance­ ment of diagnosis and therapy by the adaptation of medical skill to modern technology and scientific engineering conceptions. The major objective of this book is to present, in expanded form, the lectures given by internationally known basic and clinical researchers in the field of artificial pacing of the heart and to make that information available to a wider public. The experience discussed covers the principles and main methods of pacing using implantable and external, fixed rate, R-wave or P-wave triggered pacemakers with electrodes placed in the myocardium either surgically or transvenously, and powered by zinc-mercury oxide or rechargeable batteries. Particular emphasis was put on problems of pressing importance at the present time, such as the increase of pacemaker longevity with lithium­ iodide and nuclear-powered batteries or improved electrodes, as well as the postoperative management of a steadily increasing number of pacemaker patients.

International textbooks on infectious diseases and antibacterial chemotherapy are usually written for readers in North America and Europe. In many ways, they are not appropriate for the prob­ lems encountered in developing countries. This book, in contrast, intends to define the rules of antibacterial chemotherapy practised under conditions of limited resources. It is meant for everyone con­ cerned with the use of antibiotics in developing countries, includ­ ing doctors, medical assistants, pharmacists, officials in health mio­ isteries, and medical students. Throughout the book, treatment recommendations are made for 1 antibiotics from the WHO list of essential drugs. For example, em­ phasis has therefore been put upon chloramphenicol as a stable, unexpensive and widely available oral agent suitable for the treat­ ment of severe bacterial infections like septicemia and meningitis. So-called'international chemotherapy'with modem cephalospor­ ins and acylaminopenicillins has been outlined for comparison. Since it is the aim of the book to base treatment recommenda­ tions on data from developing countries, many data on the etiology of common bacterial infections in developing countries have also been included. Most of the data are from African, English-speak­ ing developing countries, but references have been made to the lit­ erature on South East Asia, India or Papua New Guinea, where appropriate. On the other hand, pertinent data were not available in every instance, so that several statements and recommendations had to be made as'best guess'. The authors are aware of these imperfections and will welcome comments from the readers.
