251. Determination of preoperative risk factors for iliopsoas tendonitis after total hip arthroplasty: A simulation study.

Abstract

This study aims to identify preoperative risk factors for iliopsoas tendonitis after total hip arthroplasty, a complication typically attributed to acetabular cup position and orientation, using a validated iliopsoas impingement detection simulation. Analyzing CT scans and X‐rays of 448 patients using a validated preoperative planning protocol, patients were simulated for iliopsoas impingement and categorized into at‐risk and not at‐risk groups based on a prior validation study, with a 23% at‐risk incidence. Implementing a propensity score matching algorithm to reduce covariate imbalance, we identified factors that may exacerbate risk of iliopsoas tendonitis. Parameters that were investigated included standing pelvic tilt, functional femoral rotation, and the difference between the planned acetabular cup diameter and native femoral head diameter (ΔC‐NFH). Comparing pelvic tilt, we found a significant difference between the groups (at‐risk: −6.0°, not at‐risk: −0.7°; p << 0.01). A similar trend was noted for ΔC‐NFH (at‐risk: +5.7 mm, not at‐risk: +5.1 mm; p = 0.01). Additional simulations of at‐risk patients indicated increased anteversion of the acetabular cup reduces impingement risk more effectively than medialisation. These findings suggest that spinopelvic parameters may exacerbate iliopsoas irritation risk, underscoring their importance in preoperative planning and patient expectation management. Similar findings of a greater than 6 mm difference between cup size and native femoral head diameter being a significant risk for iliopsoas tendonitis have been observed before, underscoring its potential veracity. These results may provide surgeons with a simple threshold that can be used in determining a cup size to reduce the risk of iliopsoas tendonitis. [ABSTRACT FROM AUTHOR]

Abstract

Down syndrome regression disorder (DSRD) is a rare condition involving subacute cognitive decline, loss of previously acquired developmental skills, and prominent neuropsychiatric symptoms, particularly catatonia, in people with Down syndrome. It is thought to involve both autoimmune and neuropsychiatric mechanisms. Research, however, is largely restricted to case studies and retrospective case series and is particularly limited in terms of prospective longitudinal follow-up. We report a case study of a person with DSRD who received both immunomodulatory (intravenous immunoglobulin; IVIG) and psychiatric interventions (electroconvulsive therapy, ECT) over two years with regular assessments using caregiver and clinician ratings. This revealed a small, unsustained response to IVIG and a rapid, sustained response once ECT was introduced. The case highlights the importance of multimodal assessment involving multiple medical specialties, the need to trial different therapies due to the condition's complexity, and the significant barriers that patients and their families face in accessing care. Abstract

Registries have significant roles in assessing the comparative performance of devices. Ideally, early identification of outliers should use a time-to-event outcome while reducing the confounding effects of other components in the device and patient characteristics. Machine learning (ML), which contains self-learning algorithms, is one approach to consider many variables simultaneously to reduce the impact of confounding. The principal objective of this study was to investigate the effectiveness of using either random survival forest (RSF) or regularised/unregularised Cox regression to account for patient and associated device confounding factors in comparison with current standard techniques. This study evaluated RSF and regularised/unregularised Cox regression using data from the Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) to detect outlier devices among 213 individual primary total hip components performed in 163,356 primary procedures from 1 January 2015 to the end of 2019. Device components and patient characteristics were the inputs, and time to first revision surgery was the primary outcome treated as a censored case for death. The effectiveness of the ML approaches was assessed based on the ability to detect the outliers identified by the AOANJRR standard approach. In the study cohort, the standardised AOANJRR approach identified three acetabular components and seven femoral stems as outliers. The ML approaches identified some but not all the outliers detected by the AOANJRR. Both the methods identified three of the same femoral stems, and the RSF identified the other five components, including two of the same acetabular cups and three of the same femoral stems. In addition, both the RSF and Cox techniques detected a number of additional device components that were not previously identified by the standard approach. The results showed that ML may be able to offer a supplementary approach to enhance the early identification of outlier devices. Random survival forest was a more comparable technique to the AOANJRR standard than the Cox regression, but further studies are required to better understand the potential of ML to improve the early identification of outliers. 255. Assessing screw length impact on bone strain in proximal humerus fracture fixation via surrogate modelling.

Abstract

A high failure rate is associated with fracture plates in proximal humerus fractures. The causes of failure remain unclear due to the complexity of the problem including the number and position of the screws, their length and orientation in the space. Finite element (FE) analysis has been used for the analysis of plating of proximal humeral fractures, but due to computational costs is unable to fully explore all potential screw combinations. Surrogate modelling is a viable solution, having the potential to significantly reduce the computational cost whilst requiring a moderate number of training sets. This study aimed to develop adaptive neural network (ANN)‐based surrogate models to predict the strain in the humeral bone as a result of changing the length of the screws. The ANN models were trained using data from FE simulations of a single humerus, and after defining the best training sample size, multiple and single‐output models were developed. The best performing ANN model was used to predict all the possible screw length configurations. The ANN predictions were compared with the FE results of unseen data, showing a good correlation (R2 = 0.99) and low levels of error (RMSE = 0.51%–1.83% strain). The ANN predictions of all possible screw length configurations showed that the screw that provided the medial support was the most influential on the predicted strain. Overall, the ANN‐based surrogate model accurately captured bone strains and has the potential to be used for more complex problems with a larger number of variables. [ABSTRACT FROM AUTHOR]

Abstract

Iliopsoas tendonitis, typically caused by impingement with the acetabular cup, occurs in up to 18% of patients after total hip arthroplasty (THA) and up to 30% of patients after hip resurfacing arthroplasty (HRA). We have developed a simulation for detecting iliopsoas impingement and validated it in a previous study of THA patients. However, due to the difference in incidence between HRA and THA, this study had two aims. First, to validate the simulation in a cohort of HRA patients and, second, to comparethe results of the HRA and THA patients to understand any differences in their etiology. We conducted a retrospective search in an experienced surgeon's database for HRA patients with iliopsoas tendonitisand control patients without iliopsoas tendonitis, resulting in two cohorts of 12 patients. Using CT scans, 3D models of the each patient's prosthetic and bony anatomy were generated, landmarked, and simulated. Regarding validation of the simulation for HRA patients, impingement significantly predicted the probability of iliopsoas tendonitis in logistic regression models and the simulation had a sensitivity of 83%, specificity of 100%, and an AUC ROC curve of 0.95. Unexpectedly, the HRA cohort exhibited less impingement than the THA cohort. Our novel simulation has now been demonstrated to detect iliopsoas impingement and differentiate between the symptomatic and asymptomatic cohorts in investigations of THA and HRA patients. This tool has the potential to be used preoperatively, to guide decisions about optimal cup placement, and postoperatively, to assist in the diagnosis of iliopsoas tendonitis. [ABSTRACT FROM AUTHOR]

Abstract

Initiatives that collect and share genomic data to advance health research are widespread and accelerating. Commercial interests in these efforts, while vital, may erode public trust and willingness to provide personal genomic data, upon which these initiatives depend. Understanding public attitudes towards providing genomic data for health research in the context of commercial involvement is critical. A PRISMA-guided search of six online academic databases identified 113 quantitative and qualitative studies using primary data pertaining to public attitudes towards commercial actors in the management, collection, access, and use of biobank and genomic data. The presence of commercial interests yields interrelated public concerns around consent, privacy and data security, trust in science and scientists, benefit sharing, and the ownership and control of health data. Carefully considered regulatory and data governance and access policies are therefore required to maintain public trust and support for genomic health initiatives. [ABSTRACT FROM AUTHOR]

Abstract

The Astrophysics Source Code Library (ASCL; ascl.net) is a free online registry of codes used in astronomy research; it currently contains over 900 codes and is indexed by ADS. The ASCL has recently moved a new infrastructure into production. The new site provides a true database for the code entries and integrates the WordPress news and information pages and the discussion forum into one site. Previous capabilities are retained and permalinks to ascl.net continue to work. This improvement offers more functionality and flexibility than the previous site, is easier to maintain, and offers new possibilities for collaboration. The extensible N-Dimensional Data Format (NDF) was designed and developed in the late 1980s to provide a data model suitable for use in a variety of astronomy data processing applications supported by the UK Starlink Project. Starlink applications were used extensively, primarily in the UK astronomical community, and form the basis of a number of advanced data reduction pipelines today. Abstract

The oxidation of carbon monoxide has been investigated in two different systems, Au alloyed with Pd(111) at room temperature and Cu(110) at 90 to 170 K. All experiments were conducted under UHV conditions using mass spectrometry and temperature-programmed desorption to follow the reaction. For the Au/Pd(111) system the intention was to improve the catalytic activity of the pure Pd(111) crystal by alloying with Au, as bimetallic alloys often display enhanced catalytic activities compared to that of the pure metal due to a combination of both ensemble and ligand effects. The ensemble effects being the arrangement of the active atoms and the ligand effects the electronic modifications or charge transfer associated with these active atoms. This was carried out by vapour deposition of various amounts Au on the Pd(111) crystal and annealing to form a surface alloy. Enhanced catalytic activities of up to 27 % was achieved by alloying with Au coverages of 0.2 0.4 ML on the Pd(111) substrate followed by annealing to temperatures 650 – 750 K compared to that of the pure Pd(111) surface. For the Cu(110) system we aimed to assess the catalytic activity of Cu, the objective being to determine the exact kinetic mechanism of the oxidation reaction at 90 to 170 K. This was done initially by pre-adsorbing O2 on the Cu(110) substrate surface at various coverage, followed by CO and monitoring for CO2 production. The experiment was then reversed with CO pre-dosed followed by O2. The catalytic oxidation reaction was found to proceed by a hot-precursor mechanism when O2 was dosed on a CO pre-covered surface above 130 K. Below this temperature any O2 dosed prior to or after the CO uptake would tend to physisorb molecularly and only reacted during dissociation while the O atom remained in a reactive transient. Here, the atomic O reacted directly with adsorbed molecule CO transiently prior to adsorption to the Cu substrate a catalytic reaction akin to that of an Eley-Rideal mechanism. We have studied the wetting properties, autocatalytic dissociative adsorption and the reactivity of the hydroxyl species originating from the co-adsorption of water and oxygen on Cu(110) at 200 to 300 K. Experiments were conducted by two separate methods; using molecular beam uptakes and by varying the static H2O pressures from 1.0 x 10 11 up to 5.0 x 10 7 mbar in conjunction with LEED and TPD. It has been hypothesized that the strong attractive interaction between mixed OH and H2O slows down the desorption kinetics of H2O sufficiently to produce H2O-OH structures that are stable up to RT. As a result the main focal point of the research was to gain some insight into the bonding mechanisms at elevated water pressures and ambient temperatures, whilst assessing the stability of the H2O-OH species. In addition to the 1H2O:1OH chains structures and dimers observed previous at these temperatures we also observed an additional stable structure during the TPD experiments. This structure appeared particularly when H2O was adsorbed with low (0.06 ML) atomic O coverages and became increasingly prominent when the uptake temperature was increased from 200 up to 240 K. The origin of this structure is discussed in the context of the structure proposed by earlier investigators.

Abstract

We evaluate the cross-sectional predictive ability of a forward-looking monetary policy reaction function, or Taylor rule, in both statistical and economic terms. We find that investors require a premium for holding currency portfolios with high implied interest rates while currency portfolios with low implied rates offer negative currency excess returns. Our forward-looking Taylor rule signals are orthogonal to current nominal interest rates and disconnected from carry trade portfolios and other currency investment strategies. The profitability of the Taylor rule portfolio spread is mainly driven by inflation forecasts rather than the output gap and is robust to data snooping and a wide range of robustness checks.
