252. Underdiagnosis and underreporting of vertebral fractures on chest radiographs in men aged over 50 years or postmenopausal women with and without type 2 diabetes mellitus: a retrospective cohort study

Background Osteoporotic vertebral fractures are often clinically silent and unrecognized. The present study aimed to determine whether routine chest radiographs could be a potential screening tool for identifying missed vertebral fractures in men aged over 50 years or postmenopausal women, especially those with type 2 diabetes mellitus (T2DM). In this study, we aimed to determine the prevalence of undetected vertebral fractures in elderly Chinese patients with and without T2DM. Methods Clinical data and chest radiographs of 567 individuals with T2DM (T2DM group) and 583 without diabetes (nondiabetic group) at a tertiary hospital in central south China were extracted from the records. Vertebral fractures were specifically looked for on chest radiographs and classified using the Genant semi-quantitative scale. Prevalence was compared between the two groups. Results Mean age and sex composition were comparable between the two groups. Mean weight and body mass index were significantly lower in the T2DM group. In both groups, fractures mostly involved the T11–12 and L1 vertebrae. Moderate/severe fractures were identified in 33.3% individuals in the T2DM group (31.4% men and 36.0% women) versus 23.2% individuals (20.9% men and 25.5% women) in the nondiabetic group. Conclusions Routine chest radiographs could be a useful screening tool for identifying asymptomatic vertebral fractures. Trial registration The study was designed as an observational retrospective study, therefore a trial registration was not necessary.

In a dual-bridge series resonant dc-dc converter (DBSRC), improper transient modulation may cause dc bias current and overshoot current/voltage in the resonant tank and transformer during the step load change process. Consequently there is a long duration of oscillation before the converter may enter into the new steady state. Based on the root locus of resonant capacitor current and voltage, a fast transient modulation (FTM) is proposed in this paper. Through the adjustment of the durations of the adjacent switching intervals temporarily, the step load change transition can be a fast and overshoot-free procedure. Both simulation and experimental test prove that this modulation method can effectively eliminate dc bias current and overshoot current/voltage in the DBSRC transient process and obtain a fast transient response.

The dysregulated microRNAs (miRNAs) are implicated in the malignancy of lupus nephritis (LN). This work aims to analyse the effect and mechanism of miR-146b-5p in lipopolysaccharides (LPS)-induced model of LN in vitro. The serum samples of LN patients and normal volunteers were collected. HK-2 cells were challenged via LPS. miR-146b-5p and interferon-induced protein 35 (IFI35) abundances were detected via quantitative real-time polymerase chain reaction (qRT-PCR) or western blot. The inflammatory response was assessed via inflammatory cytokines levels via qRT-PCR and enzyme-linked immunosorbent assay. Cell apoptosis was analysed via flow cytometry and apoptotic protein levels. The protein levels of JAK1/STAT1 signalling were detected via western blot. The relationship of miR-146b-5p and IFI35 was analysed via bioinformatics and dual-luciferase reporter assays. This study revealed that miR-146b-5p level was declined and IFI35 abundance was elevated in serum of LN patients and LPS-challenged HK-2 cells. Functionally, IFI35 overexpression promoted LPS-caused inflammatory response and cell apoptosis, and knockdown of IFI35 caused an opposite trend. Meanwhile, miR-146b-5p targeted IFI35 to suppress inflammatory response and cell inflammatory response and apoptosis via inactivating the JAK1/STAT1 pathway. MiR-146b-5p suppressed inflammatory response and cell apoptosis by IFI35 mediated-JAK1/STAT1 signalling in HK-2 cells, which provided a new mechanism for understanding the pathogenesis of LN.

Instant messaging (IM) is increasingly used for family communication amidst the COVID-19 pandemic. However, evidence remains scarce on how family e-chat groups were used and their associations with family and individual wellbeing amidst the pandemic. The numbers of family e-chat groups, functions used, and messages sent and received daily in groups were reported by 4890 adults in May 2020, and their associations with family wellbeing and personal happiness and the mediation effect of family communication quality were examined. Results showed that sending/receiving text messages was most commonly used, followed by receiving/sending photos/pictures, making voice calls, receiving/sending short videos and voice messages, and making video calls. Women and older people used more non-text functions. Higher levels of family wellbeing and personal happiness were associated with having more groups, receiving/sending photos/pictures, video calls, more IM functions used, and more IM messages received/sent daily. Forty-six point two to seventy-five point five percent of their associations with more groups and more functions used were mediated by family communication quality. People having more family e-chat groups and using more IM functions may be more resilient amidst the pandemic, while those without or with low use of family e-chat groups amidst the pandemic would need more attention and assistance in the presence of social distancing.

Using information communication technologies as information sources of COVID-19 was associated with psychological problems, but mechanisms remain uncertain. We examined associations of COVID-19 information sources and information overload with psychological distress symptoms and explored the mediating effect of information overload in Hong Kong. A random sample of Chinese adults (N = 1501; 52.6% females; 55.0% aged 30–59) from dual landline and online surveys in April 2020 reported frequency of using traditional media, internet websites, social networking sites, instant messaging, and online discussion forums as COVID-19 information sources. Psychological distress symptoms were measured using the Patient Health Questionnaire 4-item ([PHQ-4]; 0–12). Information overload defined as a perception of being overwhelmed was measured (1–6). Linear regressions were performed to analyze the associations, and the mediating effect of information overload was examined. Greater PHQ-4 score was observed for frequent use of internet websites (adjusted b = 0.58, 95% CI 0.29, 0.87, adjusted β = 0.12) and online discussion forums (adjusted b = 0.39, 95% CI 0.08, 0.70, adjusted β = 0.08) and information overload (adjusted b = 0.54, 95% CI 0.44, 0.63, adjusted β = 0.28). Information overload mediated 44.9% and 36.9% of associations of frequent use of internet websites and online discussion forums with PHQ-4 score, respectively. Frequent use of social networking sites was associated with lower PHQ-4 score (adjusted b = −0.37, 95% CI −0.69, −0.04, adjusted β = −0.07) partially through lower information overload (37.5%). Psychological distress symptoms were associated with frequent use of internet websites and online discussion forums as COVID-19 information sources, which were mediated through information overload.

To enhance the efficiency of a semi dual-bridge resonant converter(SDBRC), a control strategy to minimize the root mean square(RMS) current and eliminate the backflow power is proposed. An optimal operating point is precisely selected to reduce the conduction loss due to backflow power and high rms current. The converter is enabled to work with the optimal operating point in the whole load range by adjusting the switch-controlled capacitor (SCC) based tunable resonant tank continuously. Soft-switching of all switches can be maintained along the control route as well. A lab prototype converter designed at 300 W is tested to validate the SCC-based topology and proposed modulation scheme. The experimental result has demonstrated that the proposed topology and modulation scheme can improve the converter efficiency over the whole load range, especially in the light load condition.

The procedure of load transition in a dual-active-bridge converter is expected to be fast and spike free. To achieve such a goal, the total required adjustment of phase-shift angles should be assigned to different switch arms properly. This study discusses all possible combinations of two switch arms and their related phase-shifts, which could result in several different solutions. By forcing a zero algebraic variation of the volt-second product, all solutions can keep the volt-second balance of the power inductor so that no risk of magnetic saturation in the transient state exists. The possibility of temporal overshoot current can be checked by verifying the monotonicity of the intermediate volt-second product. A comparison of all solutions is made to give a suggestion for their feasibility. Experimental tests are included at last which validated the theoretical analysis.

Rock burst, a serious seismic hazard in underground coal mining, is usually induced by the coupling effect of static stress and mining-induced dynamic loading. This paper chosen an underground longwall face (LW10302) in Baodian Coal Mine as the numerical simulation model, selecting measured tremor wave which located about 150 m above the goafing of LW10301 as dynamic source, and the failure effect of dynamic loading on coal seam entry and its influential factors can be simulated systematically by FLAC3D. By changing source energy, source location, coal-rock property, the simulation results show that failure effect of dynamic source is more sensitive to source location, followed closely by source energy, and then the rock property. That is, dynamic disturbance is deeply influenced by source location, which is decreased in exponential attenuation with the increasing of transmission distance, while it also increases largely with the source energy increasing. Meanwhile, Burst effect of dynamic source can also be reduced duo to rock integrity reduction and intensity weakening, which the corresponding dynamic loading can be decreased to even less than 40%, but the influence degree may be reduced in the condition of shorter transmission distance. Moreover, the occurrence of rock burst also depends on static stress field around the coal face. Thus, the corresponding measures should be carried out simultaneously to reduce the disturbance effect of dynamic source and static stress concentration. The conclusions can provide theoretical reference to rock burst controlling in underground mining.

Background eHealth literacy can potentially facilitate web-based information seeking and taking informed measures. Objective This study aimed to evaluate socioeconomic disparities in eHealth literacy and seeking of web-based information on COVID-19, and their associations with COVID-19 preventive behaviors. Methods The COVID-19 Health Information Survey (CoVHIns), using telephonic (n=500) and web-based surveys (n=1001), was conducted among adults in Hong Kong in April 2020. The Chinese eHealth literacy scale (eHEALS; score range 8-40) was used to measure eHealth literacy. COVID-19 preventive behaviors included wearing surgical masks, wearing fabric masks, washing hands, social distancing, and adding water or bleach to the household drainage system. Adjusted beta coefficients and the slope indices of inequality for the eHEALS score by socioeconomic status, adjusted odds ratios (aORs) for seeking of web-based information on COVID-19 by socioeconomic status, and aORs for the high adherence to preventive behaviors by the eHEALS score and seeking of web-based information on COVID-19 were calculated. Results The mean eHEALS score was 26.10 (SD 7.70). Age was inversely associated with the eHEALS score, but education and personal income were positively associated with the eHEALS score and seeking of web-based information on COVID-19 (for all, P for trend Conclusions Chinese adults with a higher socioeconomic status had higher eHealth literacy and sought more web-based information on COVID-19; both these factors were associated with a high adherence to the guidelines for preventive behaviors during the COVID-19 pandemic.

In 2008, we published the first set of guidelines for standardizing research in autophagy. Since then, this topic has received increasing attention, and many scientists have entered the field. Our knowledge base and relevant new technologies have also been expanding. Thus, it is important to formulate on a regular basis updated guidelines for monitoring autophagy in different organisms. Despite numerous reviews, there continues to be confusion regarding acceptable methods to evaluate autophagy, especially in multicellular eukaryotes. Here, we present a set of guidelines for investigators to select and interpret methods to examine autophagy and related processes, and for reviewers to provide realistic and reasonable critiques of reports that are focused on these processes. These guidelines are not meant to be a dogmatic set of rules, because the appropriateness of any assay largely depends on the question being asked and the system being used. Moreover, no individual assay is perfect for every situation, calling for the use of multiple techniques to properly monitor autophagy in each experimental setting. Finally, several core components of the autophagy machinery have been implicated in distinct autophagic processes (canonical and noncanonical autophagy), implying that genetic approaches to block autophagy should rely on targeting two or more autophagy-related genes that ideally participate in distinct steps of the pathway. Along similar lines, because multiple proteins involved in autophagy also regulate other cellular pathways including apoptosis, not all of them can be used as a specific marker for bona fide autophagic responses. Here, we critically discuss current methods of assessing autophagy and the information they can, or cannot, provide. Our ultimate goal is to encourage intellectual and technical innovation in the field.

This study aims to explore the effects of Bifidobacterium spp on intestinal microbial diversity and diarrhea symptoms of Macaca mulatto, in chronic diarrhea with high-throughput sequencing and plate counting experiments. The relative abundance of added bifidobacteria was significantly higher than that of the unaddressed group (P<0.05) by comparative analysis of the 16S rRNA high-throughput sequencing. The feed was fed to the chronic diarrhea macaques, and then the feces were collected for metagenomic sequencing a month later, and compared with the intestinal metagenomic data of the unaddressed group of chronic diarrhea and healthy macaques which were obtained in the previous study. It was found that the relative abundance of Lactobacillus spp in the intestinal flora of the added group was significantly upregulated compared with that of the unaddressed group (P<0.05). And there was no significant difference with the healthy group (P>0.05). Finally, the plate counting experiment of Lactobacillus was performed on the faces of improved macaques and persistent diarrhea. It was found that the number of Lactobacillus in the intestines of macaques increased after feeding, validating the result of metagenomic sequencing analysis. [ABSTRACT FROM AUTHOR]
