251. Intra-aortic balloon counterpulsation in acute myocardial infarction: old and emerging indications

Abstract

Recent evidence questions the role of intra-aortic balloon counterpulsation (IABP) in the treatment of acute myocardial infarction (AMI) complicated by cardiogenic shock (CS). An area of increasing interest is the use of IABP for persistent ischaemia (PI). We analysed the use of IABP in patients with AMI complicated by CS or PI.From 2008 to 2010, a total of 4076 patients were admitted to our hospital for primary percutaneous coronary intervention (PCI) for AMI. Out of those, 239 patients received an IABP either because of CS or because of PI. Characteristics and outcome of those patients are investigated.The mean age of the study population was 64 ± 11 years; 75 % were male patients. Of the patients, 63 % had CS and 37 % had PI. Patients with CS had a 30-day mortality rate of 36 %; 1-year mortality was 41 %. Patients with PI had a 30-day mortality rate of 7 %; 1-year mortality was 11 %.Mortality in patients admitted for primary PCI because of AMI complicated by CS is high despite IABP use. Outcome in patients treated with IABP for PI is favourable and mandates further prospective studies.

255. Transepithelial transport of guinea pig gastric mucous cell monolayers

Abstract

Guinea pig gastric mucous epithelial cells were isolated, enriched, cultured in collagen cups, and put into Ussing chambers for electrophysiological studies. The cultured mucous cell monolayers, which retain the morphology of surface cells found in the intact tissue, had a maximal resistance (R) of 272 +/- 12 omega X cm2 and a potential difference (PD) of -3.8 +/- 0.4 mV (apical negative) between 4 and 10 days later (n = 33). The current-voltage and conductance-concentration relationships of the cultures were both nonlinear (n = 12). In addition, NaCl concentration gradients across the monolayer also gave asymmetrical and nonlinear dilution potentials, with the side of lower chemical potential always becoming electrically negative (n = 10). Calculation of the average Cl(-)-to-Na+ permeability ratio at pH 7.4 was 1.35, indicating a slightly greater conductance of anions over cations. Amiloride (0.1-1.0 microM) had no effect on PD or R when given from the apical or basal side (n = 18), but at higher concentrations (0.1-1.0 mM) there was a decrease in the PD. 4,4'-Diisothiocyanostilbene-2,2'-disulfonic acid at 10(-4) M increased R from the apical side only (n = 14), and acetazolamide at 5 X 10(-4) M reduced the PD to -0.5 mV (n = 8). Only ouabain at 10(-4) M from the serosal side was effective in reducing the monolayer PD to zero. This culture preparation will prove useful for future studies in determining specific functions for this gastric cell type and how those functions relate to barrier function in the stomach.

Abstract

Patient‐specific image‐based computational fluid dynamics (CFD) is widely adopted in the cardiovascular research community to study hemodynamics, and will become increasingly important for personalized medicine. However, segmentation of the flow domain is not exact and geometric uncertainty can be expected which propagates through the computational model, leading to uncertainty in model output. Seventy‐four aortic‐valves were segmented from computed tomography images at peak systole. Statistical shape modeling was used to obtain an approximate parameterization of the original segmentations. This parameterization was used to train a meta‐model that related the first five shape mode coefficients and flowrate to the CFD‐computed transvalvular pressure‐drop. Consequently, shape uncertainty in the order of 0.5 and 1.0 mm was emulated by introducing uncertainty in the shape mode coefficients. A global variance‐based sensitivity analysis was performed to quantify output uncertainty and to determine relative importance of the shape modes. The first shape mode captured the opening/closing behavior of the valve and uncertainty in this mode coefficient accounted for more than 90% of the output variance. However, sensitivity to shape uncertainty is patient‐specific, and the relative importance of the fourth shape mode coefficient tended to increase with increases in valvular area. These results show that geometric uncertainty in the order of image voxel size may lead to substantial uncertainty in CFD‐computed transvalvular pressure‐drops. Moreover, this illustrates that it is essential to assess the impact of geometric uncertainty on model output, and that this should be thoroughly quantified for applications that wish to use image‐based CFD models. 265. Feasibility of utility assessment by rating scale and standard gamble in patients with ankylosing spondylitis or fibromyalgia

Abstract

Objective. To assess the feasibility of utility measurement in patients with ankylosing spondylitis (AS) or fibromyalgia (FMS). Patient derived utilities provide overall estimates of the impact of a disease on patient well being. Methods. The Maastricht Utility Measurement Questionnaire was applied cross sectionally to 57 patients with AS and 86 outpatients with FMS. By means of rating scale and standard gamble techniques, patients were asked to value their own health state. Results. All 143 patients completed the interview. Patients with AS valued their personal health state on the rating scale (0-100) considerably higher than patients with FMS (AS: 69 and FMS: 54). Standard gamble utility values (0-1), however, were about the same at a higher level (AS: 0.86 and FMS: 0.83). Four weeks test-retest reliability was examined in 15 patients with FMS. The intraclass correlation coefficient of the utility score for the patient's own health state was 0.56 for the rating scale and 0.66 for the standard gamble technique. Conclusion. Feasibility of the Maastricht Utility Measurement Questionnaire was generally satisfactory in both patient groups. Utility values obtained by rating scale and standard gamble technique differed considerably. Our data support the view that utility measurement is sensitive to the method chosen to elicit patient well being.

271. A Method to Increase the Pulsatility in Hemodynamic Variables in an LVAD Supported Human Circulation System

Abstract

Left Ventricular Assist Devices (LVADs) generally operate at a constant speed in the human body. This causes a decrease in the pulsatility of hemodynamic variables. To increase the pulsatility a stepwise change was applied to the LVAD operating speed over a cardiac cycle. To do this, a numerical cardiovascular system model and a pump model were used. The model was developed by considering the static characteristics of the MicroMed DeBakey LVAD. First, the simulations were performed at constant operating speeds, 8500 rpm, 9500 rpm and 10500 rpm. Pulsatility indexes were calculated for left ventricular (LV) pressure, aortic pressure, LV volume and LVAD flow. Cardiac output (CO) was calculated at constant operating speed and these values used for comparing the pulsatility indexes with stepwise and constant operating speeds. The LVAD was operated at two different constant speeds in the stepwise operating speed simulations. Low and high operating speeds were adjusted so as to obtain the same cardiac output values with the constant operating speed simulations. The operating speeds in the simulations were 7800-11250 rpm, 9300-11250 rpm and 10300-11250 rpm. The same cardiac output values were obtained with an increase in the pulsatility of the hemodynamic variables without significant changes in their shapes except the LVAD flow. The obtained results show that it is possible to obtain more physiological results by applying a stepwise change to LVAD operating speed over a cardiac cycle.
