The authors examined the relation between recreational physical activity and plasma lipid concentrations in early pregnancy. Between 1996 and 2000, 925 normotensive, nondiabetic pregnant women in Washington State were interviewed at approximately 13 weeks' gestation regarding type, frequency, and duration of physical activity during the previous 7 days. Plasma triglyceride, total cholesterol, and high density lipoprotein cholesterol concentrations were measured in contemporaneous blood samples. After adjustment, mean triglyceride concentration was 12.7 mg/dl lower in women performing any physical activity versus none (95% confidence interval (CI): -22.7, -2.6). Mean triglyceride concentration was lower in women in the highest tertiles of time performing physical activity (-23.6 mg/dl, 95% CI: -34.9, -12.2), energy expenditure (-23.6 mg/dl, 95% CI: -35.1, -12.2), and peak intensity (-18.1 mg/dl, 95% CI: -29.5, -6.8) versus inactive women. Reductions in mean total cholesterol were also observed for women with the highest levels of time performing physical activity, energy expenditure, and peak intensity. Linear relations were observed across levels of physical activity measures for triglyceride and total cholesterol. No association was found between physical activity and high density lipoprotein cholesterol. These data suggest that habitual physical activity may attenuate pregnancy-associated dyslipidemia.

Aim: Subjects, 28 recreational male and female novice and experienced Tae Kwon Do practitioners (age 19-42 years), were examined on 6 physiological parameters: body composition (BF%, skinfold measures), flexibility (sit-and-reach and leg-splits tests), lower and upper-body dynamic muscular strength (leg press and bench press), abdominal strength and endurance (1-minute timed, bent-knee sit-ups test), lower extremity explosive power (vertical jump-and-reach test), and cardiovascular endurance (graded exercise treadmill test)., Methods: Subjects were assigned to 1 of the 4 following groups: Tae Kwon Do experienced and trained men (MT), Tae Kwon Do experienced and trained women (FT), novice Tae Kwon Do men (MN), and novice Tae Kwon Do women (FN)., Results: Results of multiple testing procedures and comparison across groups indicated that Tae Kwon Do black belts were more athletically fit as compared with that of novice Tae Kwon Do practitioners of the same sex in spite of the fact that male and female black belts were older than their novice counterparts. Experienced Tae Kwon Do subjects were stronger as measured by lower body strength and showed better aerobic performance capacity as well as lower percent body fat. Additionally, MT subjects demonstrated higher flexibility., Conclusion: The highly diverse training, repeated and continuous use of the legs and arms alone or combined with maximal stretching, and high intensity exercise may account for observed differences among groups.

Background: There is a public health need to monitor habitual participation in physical activity (PA). This report evaluates the usefulness of routine sport and recreation sector collected 'Exercise Recreation and Sport Survey (ERASS)' data from a public health perspective., Methods: The ERASS telephone surveys collected information about participation in any PA undertaken for exercise, recreation or sport and the frequency of participation during the previous 12 months from a random sample of 3,400 Australians (aged > or = 15) each quarter. Activities were categorised as Health Enhancing Physical Activities (HEPA) if the activity was of sufficient intensity (METs > or = 3.5). 'Sufficiently active' people reported HEPA > or = 5 days/week and the sedentary group reported HEPA < 0.5 day/week. The total person-days of PA were calculated and main PA types that contributed to HEPA were highlighted., Results: In 2001, walking accounted for 58% of all HEPA person-days. For females walking (66%), swimming (5.7%) and gym (4.7%) combined accounted for 75% of HEPA days while for males, walking (50%), cycling (8.1%), football (6.2%), jegging (5.6%) and swimming (4.5%) were main contributors. The mean days walked was 2.20 days/week (95% CI; 2.16-2.25) while all other HEPA combined totalled 1.56 days/week (95% CI; 1.52-1.60). Over 12-month period 15.8% of Australians were defined as sedentary and 31% were 'sufficiently active' including 16.8% who reported frequent walking (> or = 5 days/week)., Conclusion: ERASS could supplement informational gaps in current PA surveillance. It provides estimates of long-term PA behavioural habits and informs policy makers about the types of activities that contribute to population health.

Aim: Numerous studies report an association between muscle strength and bone mineral density (BMD) in young and older women. However, the participants are generally non-athletes, thus it is unclear if the relationship varies by exercise status. Therefore, the purpose was to examine the relationships between BMD and muscle strength in young women with markedly different exercise levels., Experimental Design: cross-sectional., Setting: a University research laboratory., Participants: 18 collegiate gymnasts and 22 age- and weight-matched recreationally active control women., Measures: lumbar spine, femoral neck, arm, leg and whole body BMD (g/cm(2)) were assessed by dual X-ray absorptiometry. In addition, lumbar spine and femoral neck bone mineral apparent density (BMAD, g/cm(3)) was calculated. Handgrip strength and knee extensor and flexor torque (60 degrees /s) were determined by dynamometry, and bench press and leg press strength (1-RM) using isotonic equipment., Results: BMD at all sites and bench press, leg press and knee flexor strength were greater in gymnasts than controls (p<0.001). In controls, knee extensor torque was significantly correlated to femoral neck, limb and whole body BMD (r=0.47-0.55, p<0.05), leg press strength was associated with limb and whole body BMD (r=0.52-0.74, p<0.05), and bench press strength with arm BMD (r=0.50, p=0.019). In partial correlations controlling for weight, leg press strength was related to leg and whole body BMD (r=0.46-0.63, p<0.05). There was no association between muscle strength and BMD in gymnasts., Conclusion: These results suggest that the association between muscle strength and BMD in young women is dependent on exercise status. The osteogenic effect of increased mechanical loading associated with gymnastics training likely contributes to the dissociation of the relationship in gymnasts.

Objective: This study describes the associations between sedentary behaviour (television viewing) and participation in vigorous recreational activity with obesity and with biomarkers of cardiovascular disease (CVD) risk profile., Design: Cross-sectional analysis of the EPIC-Norfolk cohort study., Setting: The study is a population-based study of participants living in Norfolk, UK., Subjects: A total of 15 515 men and women aged between 45 and 74 y, recruited through General Practice lists, who completed the detailed physical activity questionnaire., Results: Following exclusion of those with self-reported myocardial infarction, stroke and diabetes, 14 189 participants remained for the analysis. Self-reported television viewing was positively and participation in vigorous activity negatively associated with markers of obesity, blood pressure and plasma lipids. In multiple regression analysis, adjusting for age, alcohol, smoking, treatment for hypertension, vigorous and total physical activity, these associations remained significant. For women who participated in more than 1 h/week of vigorous activity and who watched fewer than 2 h of television each day, the adjusted mean body mass index was 1.92 kg/m(2) less than for women who reported participating in no vigorous activity and who watched more than 4 h of television each day (P<0.001). The equivalent figure for men was 1.44 kg/m(2) (P<0.001). In a similar analysis, with blood pressure as the outcome, mean diastolic blood pressure difference between the extreme groups of vigorous activity and television viewing was 3.6 mmHg in men (P<0.001) and 2.7 mmHg (P=0.001) in women., Conclusions: These data suggest that time spent participating in vigorous recreational physical activity and television viewing, an indicator of a sedentary lifestyle, are associated with obesity and markers of CVD disease risk independent of total reported physical activity. Whether these observations represent the true underlying aetiological relations or are a manifestation of the different precision with which the subdimensions of activity are measured remains uncertain.

Background: Obesity in the United States has reached epidemic proportions and is a major cause of morbidity and mortality., Methods: We describe the activity levels of South Carolina adults on the basis of data derived from the Centers for Disease Control and Prevention Behavioral Risk Factor Surveillance System., Results: Overweight and obese men and women reported less leisure time physical activity than did people of normal weight, with women found to be less active than men., Conclusion: Physical inactivity is more prevalent among obese and overweight men and women than among people of normal weight. Visiting the physician's office offers a unique opportunity to educate patients about the health benefits and appropriate amount of physical activity.

Environmental determinants of health are receiving growing attention in the literature, although there is little empirical research in this area. The Study on Environmental and Individual Determinants of Physical Activity (known as the SEID project) was a social ecological project that examined the relative influence of individual, social environmental and physical environmental determinants of recreational physical activity. It involved a community survey of 1803 healthy workers and home-makers aged 18-59 years living in a 408 km2 area of metropolitan Perth, Western Australia. Physical environmental determinants were mainly conceptualised as spatial access to popular recreational facilities. Overall, 59% of respondents exercised as recommended. Recreational facilities located near home were used by more respondents than facilities located elsewhere. The most frequently used facilities were informal: the streets (45.6%); public open space (28.8%) and the beach (22.7%). The physical environment's directs the influence on exercising as recommended was found to be secondary to individual and social environmental determinants. Nevertheless, accessible facilities determined whether or not they were used and in this way, support and enhance the achievement of recommended levels of physical activity behaviour by providing opportunities. The results suggest that access to a supportive physical environment is necessary, but may be insufficient to increase recommended levels of physical activity in the community. Complementary strategies are required that aim to influence individual and social environmental factors. Given the popularity of walking in the community, it is recommended that greater emphasis be placed on creating streetscapes that enhance walking for recreation and transport.

Background: There have been repeated calls for research on the factors that promote the spread of successful local health promotion initiatives from one community to another. We examined the factors that affected the uptake of an initiative designed in one community to improve the health of women living below the poverty line through increased access to community recreation., Methods: Workshops were held in three other communities and uptake efforts were tracked for one year through follow-up site visits and telephone interviews with workshop participants., Results: Making the issue a priority, actively involving the women in planning, pooling resources, sharing responsibility through partnerships, and addressing the structural dimensions of poverty were factors that enabled uptake. Factors that inhibited uptake included an emphasis on revenue generation, professionally led planning, inadequate attention to structural barriers, the undervaluing of certain resources, and an over-reliance on one idea champion., Conclusion: A shift in how municipal recreation departments view their role as partners in community health promotion is required if programs are to promote health and be accessible to under-served populations.

This article presents a literature review of the current recommendations regarding sports after total joint replacement and also suggests scientifically based guidelines. Patients should be encouraged to remain physically active for general health and also for the quality of their bone. There is evidence that increased bone quality will improve prosthesis fixation and decrease the incidence of early loosening. To recommend a certain activity after total knee or hip replacement, factors such as wear, joint load, intensity and the type of prosthesis must be taken into account for each patient and sport. It has been shown that the reduction of wear is one of the main factors in improving long-term results after total joint replacement. Wear is dependent on the load, the number of steps and the material properties of total joint replacements. The most important question is, whether a specific activity is performed for exercise to obtain and maintain physical fitness or whether an activity is recreational only. To maintain physical fitness an endurance activity will be performed several times per week with high intensity. Since load will influence the amount of wear exponentially, only activities with low joint loads such as swimming, cycling or possibly power walking should be recommended. If an activity is carried out on a low intensity and therefore recreational base, activities with higher joint loads such as skiing or hiking can also be performed. It is unwise to start technically demanding activities after total joint replacement, as the joint loads and the risk for injuries are generally higher for these activities in unskilled individuals. Finally, it is important to distinguish between suitable activities following total knee and total hip replacement. To recommend suitable physical activities after total knee replacement, it is important to consider both the load and the knee flexion angle of the peak load, while for total hip replacement, which involves a ball and socket joint, the flexion angle does not play an important role. During activities such as hiking or jogging, high joint loads occur between 40 and 60 degrees of knee flexion where many knee designs are not conforming and high polyethylene inlay stress will occur. Regular jogging or hiking produces high inlay stress with the danger of delamination and polyethylene destruction for most current total knee prostheses. Based on these design differences between hip and knee replacements it is prudent to be more conservative after total knee arthroplasty than after total hip arthroplasty for activities that exhibit high joint loads in knee flexion.

Objective: Exercise has been hypothesized to influence cancer risk through a variety of mechanisms including hormonal, metabolic and immunologic effects, yet its relation with the risk of thyroid cancer has not been examined. We conducted a population-based case-control study in women aged 18-64 in three counties of western Washington State to assess the relation of recreational physical activity with risk of papillary thyroid cancer., Methods: Of 558 women with thyroid cancer of the follicular epithelium diagnosed during 1988-1994 who were identified as eligible, 468 (83.9%) were interviewed; this analysis was restricted to women with papillary histology (n = 410). Controls (n = 574) were identified by random digit dialing, with a response proportion of 73.6%. Logistic regression was used to calculate odds ratios (OR) and associated confidence intervals (CI) estimating the relative risk of papillary thyroid cancer associated with various aspects of recreational exercise., Results: Risk of thyroid cancer was reduced among women who reported that they engaged in regular recreational exercise during the 2 years before diagnosis relative to women who did not report exercise during that time period (OR = 0.76, 95% CI 0.59-0.98). A similar risk reduction was noted among women who reported having exercised regularly between ages 12 and 21 (OR = 0.83, 95% CI 0.64-1.1). However, no clear associations with aspects of recreational activity, including average hours exercised per week or weekly energy expenditure, were observed., Conclusions: These results provide some initial support for the hypothesis that physical activity may reduce risk of thyroid cancer.

Objective: Children with Down syndrome (DS) are living longer and are increasingly participating in recreational activities. When a child with DS was diagnosed with high-altitude pulmonary edema (HAPE), this study was undertaken to determine whether and under what circumstances children with DS develop HAPE., Design: A retrospective review of the medical records of Children's Hospital, Denver, Colorado was performed for children with a discharge diagnosis of HAPE. Diagnostic criteria for HAPE included the presence of crackles or frothy sputum production on examination, hypoxemia, chest radiograph findings consistent with pulmonary edema, and rapid clinical improvement after descent or oxygen therapy., Results: A total of 52 patients with HAPE were found of whom 6 also had DS. The age range of the children with DS was 2 to 14 years. HAPE developed at altitudes ranging from 1738 to 3252 m. Four children developed HAPE within 24 hours of arrival to altitude. Three children had chronic pulmonary hypertension, and 4 had either an existing cardiac defect with left-to-right shunt or previously had a defect with left-to-right shunt that had been repaired. One child had Eisenmenger syndrome with chronic right-to-left shunting of blood. Five children had preexisting illnesses before travel to altitude., Conclusion: Children with DS often have medical problems such as chronic pulmonary hypertension, frequent infections, and pulmonary vascular overperfusion and injury from existing or previous cardiac defects. These problems all may be viewed as risk factors for HAPE and thus result in the rapid development of HAPE at low altitudes. Care should be taken when traveling to even moderate altitudes with children with DS.

A 1 year prospective study was conducted in France in 1993-1994 to examine the extent of menstrual cycle impairment in a sample of recreational French runners, and to relate the extent of impairment to dietary characteristics. The runners provided 7-day weighed food and fluid intake at intervals of 3 months during the year, as well as full details of their menstrual cycles and training schedules. The food survey commenced with 34 runners and 21 provided information on all four occasions. About 6% of the women, at any session, unrealistically report their dietary intakes. However, there was no evidence of any relationship between extent of under-reporting and menstrual status. The sample was divided into three menstrual cycle groups of eumenorrhoeic, irregular and amenorrhoeic (including oligomenorrhoeic). No significant differences in mean energy intake, carbohydrate, fat and protein intakes were found between groups. However, there were very marked differences in fluid and fibre intake, with amenorrhoeic and oligomenorrhoeic runners consuming about twice the mean daily fluid intake of eumenorrhoeic or irregularly menstruating groups.

There is limited information on the anthropometry, strength, endurance and flexibility of female rock climbers. The aim of this study was to compare these characteristics in three groups of females: Group 1 comprised 10 elite climbers aged 31.3 +/- 5.0 years (mean +/- s) who had led to a standard of 'hard very severe'; Group 2 consisted of 10 recreational climbers aged 24.1 +/- 4.0 years who had led to a standard of 'severe'; and Group 3 comprised 10 physically active individuals aged 28.5 +/- 5.0 years who had not previously rock-climbed. The tests included finger strength (grip strength, finger strength measured on climbing-specific apparatus), flexibility, bent arm hang and pull-ups. Regression procedures (analysis of covariance) were used to examine the influence of body mass, leg length, height and age. For finger strength, the elite climbers recorded significantly higher values (P < 0.05) than the recreational climbers and non-climbers (four fingers, right hand: elite 321 +/- 18 N, recreational 251 +/- 14 N, non-climbers 256 +/- 15 N; four fingers, left hand: elite 307 +/- 14 N, recreational 248 +/- 12 N, non-climbers 243 +/- 11 N). For grip strength of the right hand, the elite climbers recorded significantly higher values than the recreational climbers only (elite 338 +/- 12 N, recreational 289 +/- 10 N, non-climbers 307 +/- 11 N). The results suggest that elite climbers have greater finger strength than recreational climbers and non-climbers.

Various factors have contributed to this phenomenon, including an increased public interest in sports and fitness as well as improvements in disability awareness. An even more essential element has been a consumer-driven demand for advances in prosthetic technology and design. Whether the activity is a music performance, a friendly round of golf, or a high-level track-and-field competition, the benefits of participation in sports and recreation are numerous both at the individual and at the societal level. This article provides an overview of the development and scope of sport and recreational opportunities available to persons with limb deficiency. In addition, specific prosthetic considerations for several common sport and recreational activities are presented in a case-discussion format., Overall Article Objective: To review the development and scope of sport and recreational opportunities available to persons with limb deficiency.

This paper presents the results of a 12-week single-sex, family-based physical activity intervention grounded in Social Cognitive Theory. Mother/daughter pairs and triads (n = 20) attended physical activity and classroom sessions twice weekly. Physiological data (VO2peak, height, and weight), psychological data (physical self-perception profile subscale scores), information about physical activity participation (PAP, d x wk(-1)) and qualitative impressions (QI) of the program were collected pre- and post-intervention. PAP and QI were also collected 6-months after completing the intervention. Although no significant increases in physical activity were reported, significant improvements in perceived sport competence, physical condition, and strength and muscularity were reported over time. The social cognitive theory, as used to plan this physical activity intervention, offered a promising theoretical perspective for facilitating improved physical self-perception in adolescent girls and their mothers.

In any consideration of cultural diffusion, what matters is not only what happens to a cultural form when it arrives, but that it arrives. In the twentieth century, Latin America, like too many other areas of the world, has been a place of diplomatic turmoil, social inequality, political paranoia, capitalist exploitation and class conflict. However, despite all this, and through all this, it may be stated factually and without sentimentality, that it has also been a place where people have survived and thrived, worked, loved and played. Modern sport has brought to their play both unhappy moments of disillusion and disappointment and marvellous opportunities for illusion and pleasure. Modern sport, with its beauty spots and warts, is the reality and while there are things to criticize, there are also many things to applaud. Along with others, the English middle class played a not insignificant part in the arrival of modern sport in Latin America.

Data on 1,501 control women from a multi-center, population-based, case-control study of breast cancer were used to examine characteristics associated with recreational exercise during the year prior to the interview among women 20 to 44 years of age. In a univariate analysis, higher levels of recreational exercise were associated with: higher education; higher family income; white race; previous participation in recreational exercise above the median level at ages 12 to 13 and at age 20; being nulliparous; ever lactating; being a never or past smoker; having a low current Quetelet's index (QI: weight in kilograms divided by height in meters squared); and living in Atlanta or Seattle (compared to New Jersey). In a multiple linear regression model, independent predictors of higher levels of recreational exercise were: participation in higher levels of exercise at 20 years of age; having a low current QI; and never having smoked. Though all women should be encouraged to participate in exercise, these findings identity subgroups of women that may need targeting when developing exercise intervention programs.

More than 800 sports-related concussions occur in the United States each day, sometimes involving high-profile athletes whose injuries reach public awareness through sports broadcasts and news media. Although nonphysicians are often present and relied upon for the detection of concussion in the sports setting, the proper diagnosis and management of this neurological problem require a physician's thoughtful attention to the athlete's signs and symptoms. This article offers a diagnostic protocol and treatment recommendations as well as a useful grading scale and management strategy for return to competition.
