266. Heterosexuality and Race in the Australian Same-Sex Marriage Postal Survey

Abstract

This article argues that race and class are central aspects of sexual citizenship in a Australia. It does so by investigating representations of heterosexuality that were produced and circulated during the 2017 same-sex marriage postal survey. Engaging with feminist and critical race theorists, we position same-sex marriage as not exceptional but part of a wider distribution of sexual citizenship within Australia's ongoing settler colonial history. We do so by introducing a number of illustrative examples of representations of heterosexuality produced during the survey. These representations reveal how same-sex marriage perpetuated heterosexual authority by asserting claims to authenticity and the occupation of space. We observe how heterosexuality in the survey material reproduced fantasies linking these three themes, for example, in an authentic white heterosexual family who speaks from their suburban backyard. It reveals that ceding to a bifurcated view of either progressive or conservative voices forestalls rather than advances other visions which may exceed the limited imaginings of sexual citizenship offered by the white liberal settler colonial state.

267. Safety, efficacy and drug survival of biologics and biosimilars for moderate-to-severe plaque psoriasis

Abstract

BACKGROUND: Real-life data on newer biological and biosimilar agents for moderate-to-severe psoriasis are lacking.OBJECTIVES: To examine safety, efficacy and time to discontinuation (drug survival) of biologics (adalimumab, etanercept, infliximab, secukinumab and ustekinumab) and compare originators with biosimilars (i.e. Enbrel with Benepali, and Remicade with Remsima).METHODS: The DERMBIO registry contains data on all Danish patients with moderate-to-severe plaque psoriasis treated with biologics. We examined patients treated between 1 January 2007 and 31 March 2017. We used Kaplan-Meier survival curves and Cox regression to examine drug survival patterns.RESULTS: A total of 3495 treatment series (2161 patients) were included (adalimumab n = 1332; etanercept n = 579; infliximab n = 333; ustekinumab n = 1055 and secukinumab n = 196). Secukinumab had the highest number of PASI 100 (100% improvement from baseline Psoriasis Area and Severity Index) respondents, but also the lowest drug survival among all the biologics. Ustekinumab had the highest drug survival overall. There were no significant differences in discontinuation risk between originator and biosimilar versions of infliximab or etanercept. Treatment with higher than approved dosages was frequent for all drugs except for adalimumab and secukinumab. Adverse events (predominantly infections) were most frequent for secukinumab compared with the other agents.CONCLUSIONS: Ustekinumab was associated with the highest drug survival, and secukinumab with the lowest, although most patients on secukinumab were non-naïve. Switching from originator to biosimilar had no significant impact on drug survival, and the safety profiles were comparable. Adverse events occurred most frequently with secukinumab. Future studies are warranted to assess the long-term safety of novel biologics for psoriasis.

Abstract

Identification of fetal kidney anomalies invites questions about underlying causes and recurrence risk in future pregnancies. We therefore investigated the diagnostic yield of next-generation sequencing in fetuses with bilateral kidney anomalies and the correlation between disrupted genes and fetal phenotypes. Fetuses with bilateral kidney anomalies were screened using an in-house-designed kidney-gene panel. In families where candidate variants were not identified, whole-exome sequencing was performed. Genes uncovered by this analysis were added to our kidney panel. We identified likely deleterious variants in 11 of 56 (20%) families. The kidney-gene analysis revealed likely deleterious variants in known kidney developmental genes in 6 fetuses and TMEM67 variants in 2 unrelated fetuses. Kidney histology was similar in the latter 2 fetuses-presenting a distinct prenatal form of nephronophthisis. Exome sequencing identified ROBO1 variants in one family and a GREB1L variant in another family. GREB1L and ROBO1 were added to our kidney-gene panel and additional variants were identified. Next-generation sequencing substantially contributes to identifying causes of fetal kidney anomalies. Genetic causes may be supported by histological examination of the kidneys. This is the first time that SLIT-ROBO signaling is implicated in human bilateral kidney agenesis.

Abstract

The long-term reactions, experiences and reflections of simulation educators have not been explored. In a semistructured, exploratory interview study, the experiences of simulation educators in either Advanced Life Support (ALS) or Crisis Resource Management (CRM) courses in Denmark, Norway and the USA were analyzed. Three overarching themes were identified: (1) general reflections on simulation-based teaching, (2) transfer of knowledge and skills from the simulation setting to clinical settings and (3) more overarching transformations in simulation educators, simulation participants, and the healthcare system. Where ALS was deemed as high on the efficiency dimension of learning, CRM courses were described as high on the innovation dimension. General reflections, transfer and transformations described were related to differences in course principles. The results are relevant for career planning, faculty development and understanding simulation as social practice.

Abstract

Aim: The aim of this study was to validate the clinical quality database of the Danish Colorectal Cancer Group. The validation is meant to focus on core data regarding staging of the disease, treatment provided, patient‐related factors and key complications. Method: This was a database validation study assessing the completeness of the database and the accuracy of the data by re‐entering core variables into an online module in a blinded fashion and comparing re‐entered data with the original database data. A sample of 5% of patients from the years 2014–2017 was randomly selected. Results: The sample of 936 patients was identified and data were re‐entered. The completeness of the data retrieved was a median of 96%, 100% and 99% for preoperative, intra‐operative and postoperative variables, respectively. The overall accuracy was a median of 95%. The least accurate variable was date of diagnosis (50% perfect agreement), with agreement rising to 96% when near matches defined as correct date ± 30 days were included. Intra‐operative variables were of high quality, as were data on surgical complications including anastomotic leakage, where agreement was 97%. Conclusion: This was the first major validation of the Danish Colorectal Cancer Group's database. Overall, the completeness and quality of data were high, but the validation process also identified weaknesses, which can be crucial for future users to acknowledge and consider. Abstract

BACKGROUND: Bullous pemphigoid (BP) is a disease of the elderly, and may be associated to neurological, cardiovascular diseases and diabetes. Mortality rates strongly exceed that of the background population.OBJECTIVES: We aimed to investigate the frequency of comorbidities and their temporal relation to BP.METHODS: A register-based matched cohort study on all Danish patients with a hospital-based diagnosis of BP (n = 3,281). Main outcomes were MS, Parkinson's disease (PD), Alzheimer's disease (AD), stroke (ST), diabetes (DM I and II), malignancies (MAL), ischaemic heart disease (IHD) and hypertension (HYP) and eventually death.RESULTS: At baseline, BP patients had an increased prevalence of MS, odds ratio (OR)MS 9.7 (95% confidence interval (CI) 6.0-15.6), ORPD 4.2 (95% CI 3.1-5.8), ORAD 2.6 (95% CI 1.8-3.5) and ORST 2.7 (95% CI 2.4-2.9). Furthermore, MAL, cardiovascular disease and DM were overrepresented among BP patients, ORDMI 3.1 (95% CI 2.5-3.8), ORDMII 2.3 (95% CI 2.0-2.6), ORMAL 1.3 (95% CI 1.1-1.4), ORIHD 1.7 (95% CI 1.5-1.8) and ORHYP 2.0 (95% CI 1.8-2.2). During follow-up, the risk of MS was significantly higher among BP patients, HRMS 9.4 (95% CI 4.9-18.0), even if events during the first year after diagnosis of BP were excluded HRMS-1 5.1 (95% CI 2.3-11.3). BP patients had an average increased mortality rate of 2.04 (95% CI 1.96-2.13).CONCLUSIONS: We discovered a significantly increased frequency of MS among BP patients. 274. Computational analysis of off-design waveriders

Abstract

Comprehensive inviscid and viscous numerical simulations of hypersonic flow past nonconical rounded-nose waveriders are presented. The flow fields and aerodynamic forces at off-design conditions are determined inviscidly by a space-marching CFD code with the initial-data plane provided by a time-marching Navier-Stokes CFD code. Off-design conditions include off-design Mach numbers, angles of attack, and rounded leading edges. A wide range of waverider configurations is investigated and compared. These calculations show the effects of viscous interactions, which are influential near the leading edges, and determine the viscous drag. The inviscid calculations show that L/D decreases as freestream M increases (with alpha = 0). At the on-design Mach numbers, the maximum L/D may occur at slight positive or negative alpha, depending on the shape of the waverider, and zero lift occurs at a negative alpha approximately equal to half of the body thickness. The effects of slight leading-edge blunting produce only local effects in the flow field and small losses in L/D.

Abstract

Research efforts related to the development of a unified aerospace plane analysis based on waverider technology are summarized. Viscous effects on the forebodies of cone-derived waverider configurations were studied. A simple means for determining the average skin friction coefficient of laminar boundary layers was established. This was incorporated into a computer program that provides lift and drag coefficients and lift/drag ratio for on-design waveriders when the temperature and Reynolds number based on length are specified. An effort was made to carry out parabolized Navier-Stokes (PNS) calculations for cone-derived waveriders. When the viscous terms were turned off (in the Euler mode) computations for elliptic cone-derived waveriders could be carried out for a wide range of on-design and off-design situations. Work related to waveriders derived from power law shocks is described in some detail.

Abstract

To support shifting to high mix/low volume production, manufacturers in high wage countries aim for robotizing their production operations – with a special focus on the late production phases, where robotic assembly cells are then confronted with any complexities resulting from part and product varieties. The 'World Robot Challenge 2018' (WRC 2018) emulated such high mix/low volume production scenarios in a competition taking place in Tokyo, Japan. As part of our activities in SDU's newly founded I4.0 Lab, we integrated and advanced our experiences and developments from our various R & D projects in a novel robotic assembly cell design to compete in the WRC 2018. This article describes the system architecture as well as main aspects of its implementation regarding robot control, robot programming and computer vision and how they contributed to winning the challenge. Due to the application of collaborative robots, the cell design allows for operation without fences. Hence, multiple copies of the cell can be arranged in a highly reconfigurable, highly adaptable matrix structure in which several production flows can be handled concurrently. This concept was demonstrated by the installation of a duplicate cell that allowed for parallel developments on two cells and prolonged development also after shipping the first cell to Japan. Abstract

Density functional theory calculations are performed for the adsorption of O2, coadsorption of CO, and the CO+O2 reaction at the interfacial perimeter of nanoparticles supported by rutile TiO2(110). Both stoichiometric and reduced TiO2 surfaces are considered, with various relative arrangements of the supported Au particles with respect to the substrate vacancies. Rather stable binding configurations are found for the O2 adsorbed either at the trough Ti atoms or leaning against the Au particles. The presence of a supported Au particle strongly stabilizes the adsorption of O2. A sizable electronic charge transfer from the Au to the O2 is found together with a concomitant electronic polarization of the support meaning that the substrate is mediating the charge transfer. The O2 attains two different charge states, with either one or two surplus electrons depending on the precise O2 adsorption site at or in front of the Au particle. From the least charged state, the O2 can react with CO adsorbed at the edge sites of the Au particles leading to the formation of CO2 with very low (≈0.15 eV) energy barriers. © 2004 American Institute of Physics. Abstract

The properties of reduced rutile TiO[sub 2](110) surfaces, as well as the adsorption, diffusion, and dissociation of molecular oxygen are investigated by means of density functional theory. The O[sub 2] molecule is found to bind strongly to bridging oxygen vacancies, attaining a molecular state with an expanded O–O bond of 1.44 Å. The molecular oxygen also binds (with somewhat shortened bond lengths) to the fivefold coordinated Ti atoms in the troughs between the bridging oxygen rows, but only when vacancies are present somewhere in the surface. In all cases, the magnetic moment of O[sub 2] is lost upon adsorption. The expanded bond lengths reveal together with inspection of electron density and electronic density of state plots that charging of the adsorbed molecular oxygen is of key importance in forming the adsorption bond. The processes of O[sub 2] diffusion from a vacancy to a trough and O[sub 2] dissociation at a vacancy are both hindered by relative large barriers. However, we find that the presence of neighboring vacancies can strongly affect the ability of O[sub 2] to dissociate.
