Abstract

A multiresponse 25 full factorial experiment is performed to investigate the effects of growth conditions (temperature, power, pressure, total flow rate, partial precursor flow rate) on the chemical, mechanical, dielectric, electronic, and charge transport properties of thin-film amorphous hydrogenated boron carbide (a-BxC:Hy) grown by plasma-enhanced chemical vapor deposition (PECVD) from ortho-carborane. The main and interaction effects are determined and discussed, and the relationships between properties are investigated via correlation analysis. The process condition with the strongest influence on growth rate is pressure, followed by partial precursor flow rate, with low pressure and high partial flow rate conditions yielding the highest growth rates. The atomic concentration of hydrogen (at.% H) and density are controlled primarily by temperature and power, with low temperature and power conditions leading to relatively soft, hydrogen-rich, low-density, porous films, and vice versa. The B/C ratio is controlled by temperature, power, pressure, and the power*pressure interaction, and is uncorrelated to hydrogen concentration. Thin-film dielectric and electronic structure properties, including high-frequency dielectric constant (e1), low-frequency/total dielectric constant (κ), optical band gap (ETauc/E04), and Urbach energy (EU), are correlated strongly with at.% H, and weakly to moderately with B/C ratio. These properties are dominated by the influence of temperature, with a second significant influence from the power*pressure interaction. The interaction of power and pressure leads to two opposite growth regimes—high power and high pressure or low power and low pressure—that can produce a-BxC:Hy films with similar dielectric or electronic structure properties. Charge transport properties also show a correlation with at.% H and B/C, but not with the electronic structure and disorder parameters, which suggests a complicated relationship between the two. The range of properties measured highlights the potential of thin-film a-BxC:Hy for low-κ dielectric and neutron detection applications, and suggests clear pathways for future material property optimization.

Abstract

Purpose Circulatory failure (CF) influences management of out-of-hospital cardiac arrest (OHCA) patients and the decision of circulatory assistance. We performed a study to identify on hospital admission patients at risk for CF-related death. Materials and methods This is a single-center study including OHCA patients without obvious extracardiac cause and sustained return of spontaneous circulation, in a retrospective derivation (RC) and prospective validation cohort (PC). Univariate/multivariate logistic regression was used in the RC to determine a score predicting CF-related death (due to rearrest or persistent shock despite adequate fluid and catecholamine treatment). The score was validated in the PC. Results We included 207 patients in the RC and 96 in the PC. Circulatory failure occurred in 59% of RC and 63% of PC patients ( P = .70); 35% in both cohorts died of CF. In multivariate regression, correlates of CF-related death making up the logistic score were arterial pH ( P P = .0045). In the PC, for a logistic score cut-off of 0.5, sensitivity for CF-related death was 50%; specificity, 92%. Patients with shock and arterial pH less than or equal to 7.11 had a CF-related death probability greater than 0.5. Conclusion A logistic score based on arterial pH and shock requiring catecholamines on admission can predict CF-related death in OHCA patients.

Abstract

In New Seeds of Contemplation, Thomas Merton writes that “To live in communion, in genuine dialogue with others, is absolutely necessary, if man is to remain human.” Yet, as the works of Primo Levi and Elie Wiesel demonstrate, communion and dialogue were precisely what was forbidden in the “concentrationary universe,” where men survived on an ethics of complete selfishness. What strikes this reader of Charlotte Delbo’s Auschwitz et apres, however, is the presence of dialogue, human words, listening ears. The intractable camp laws of anonymity, conformity, and self-absorption have given way to the reality of community which defines itself in the most remarkable ways and explains why and how the women imprisoned here survived together. A final section deals with the simultaneity of disaster and community and the difficulty of returning from an elsewhere that is unimaginable to others.

Highlights • 161 lower limb reconstruction surgeries for pathological or impending pathological fractures were included in the study. • 46.6% of patients did not receive radiation within 12 weeks of surgery. • Among patients not receiving post-operative radiation, 6.7% required a second operation to the index surgical site. • Among patients not receiving post-operative radiation, 16.0% later received radiation to the index surgical site."?>, Purpose Pathological metastatic fractures in lower-extremity weight bearing bones often require surgical reconstruction. Post-operative radiation is routinely recommended following surgical reconstruction. This study evaluated the clinical outcomes of patients that undergo surgical fixation of an established or an impending pathologic lower extremity fracture without post-operative radiation. Materials and methods A retrospective chart review of patients at Sunnybrook Health Sciences Center between 2007 and 2019 was performed. Descriptive statistical analyses were performed. Results A total of 161 surgical reconstruction procedures were identified. Among these cases, 86/161 (53.4%) received post-operative radiation, 75/161 (47%) did not receive post-operative radiation within 12 weeks of their index surgery. Of the 75 patients not receiving post-operative radiation, 40 patients had prior radiation to the surgical site and 35 patients were radiation naïve. 5 patients (6.7%) required a second operation to the index surgical site, with 4 patients (5.3%) requiring a second fixation surgery to stabilize hardware at a median of 6.0 months post-surgery. Post-surgical radiation to the surgical site (at >12 weeks) was administered to 12 patients (16.0%) at a median of 9.1 months post-surgery. Conclusions The surgical revision rate was low despite absence of immediate post-operative radiation therapy and was similar to prior reports in patients receiving post-operative radiation.

Abstract

Background Physiotherapy is essential for the successful rehabilitation of common shoulder injuries and following shoulder surgery. Patients may receive some training and supervision for shoulder physiotherapy through private pay or private insurance, but they are typically responsible for performing most of their physiotherapy independently at home. It is unknown how often patients perform their home exercises and if these exercises are performed correctly without supervision. There are no established tools for measuring this. It is, therefore, unclear if the full benefit of shoulder physiotherapy treatments is being realized. Objective The proposed research will (1) validate a smartwatch and machine learning (ML) approach for evaluating adherence to shoulder exercise participation and technique in a clinical patient population with rotator cuff pathology; (2) quantify the rate of home physiotherapy adherence, determine the effects of adherence on recovery, and identify barriers to successful adherence; and (3) develop and pilot test an ethically conscious adherence-driven rehabilitation program that individualizes patient care based on their capacity to effectively participate in their home physiotherapy. Methods This research will be conducted in 2 phases. The first phase is a prospective longitudinal cohort study, involving 120 patients undergoing physiotherapy for rotator cuff pathology. Patients will be issued a smartwatch that will record 9-axis inertial sensor data while they perform physiotherapy exercises both in the clinic and in the home setting. The data collected in the clinic under supervision will be used to train and validate our ML algorithms that classify shoulder physiotherapy exercise. The validated algorithms will then be used to assess home physiotherapy adherence from the inertial data collected at home. Validated outcome measures, including the Disabilities of the Arm, Shoulder, and Hand questionnaire; Numeric Pain Rating Scale; range of motion; shoulder strength; and work status, will be collected pretreatment, monthly through treatment, and at a final follow-up of 12 months. We will then relate improvement in patient outcomes to measured physiotherapy adherence and patient baseline variables in univariate and multivariate analyses. The second phase of this research will involve the evaluation of a novel rehabilitation program in a cohort of 20 patients. The program will promote patient physiotherapy engagement via the developed technology and support adherence-driven care decisions. Results As of December 2019, 71 patients were screened for enrollment in the noninterventional validation phase of this study; 65 patients met the inclusion and exclusion criteria. Of these, 46 patients consented and 19 declined to participate in the study. Only 2 patients de-enrolled from the study and data collection is ongoing for the remaining 44. Conclusions This study will provide new and important insights into shoulder physiotherapy adherence, the relationship between adherence and recovery, barriers to better adherence, and methods for addressing them. Abstract

Background Acute Coronary Syndrome (ACS) remains a leading cause of mortality worldwide. Several studies have demonstrated a role for Coronary Computed Tomographic Angiography (CCTA) to predict the vulnerability of the plaque. Recent works suggest a role for calcification in this vulnerability. Purpose To assess if the CCTA study of the calcium component of plaques can predict plaque vulnerability defined by intracoronary OCT analysis in patients with non-ST elevation myocardial infarction (NSTEMI). Methods Prospective study of consecutive patients with NSTEMI and elevated high-sensitivity cardiac troponin I level > 50 ng/ml from January to October 2018. CCTA was performed before coronary angiography. When CCTA demonstrated significant lesions, coronary angiography was performed with OCT study of three coronary arteries. Apart culprit plaques, vulnerable plaques were defined in OCT by a fibrous screed thickness Results Of 1478 patients with chest pain, 257 (17%) had NSTEMI. Among them 44(17%) were without known CAD and among these patients 29(66%-mean age 59 ± 13 years, 73% men) had received coronary angiography and OCT. A total of 123 calcified plaques were identified. Among them, OCT allowed to identify 77 (63%) SP and 47 (37%) calcified vulnerable plaques with 28 VNCP and 19 VCP. After CCTA analysis, predictive factors of vulnerability were identified: longer calcification length, larger calcification volume, higher plaque's Agatston score, lower sphericity index, more spotty calcifications, a more intimal position in the wall (P Fig. 1 ). Conclusion CCTA study of the calcium component of plaque allows to identify predictors of plaque vulnerability defined by OCT in patients with NSTEMI.

Abstract

Cardiogenic shock (CS) complicating acute myocardial infarction (AMI) occurs more frequently in women, but little is known about its potential specificities according to sex.To analyse the incidence, management and 1-year mortality of CS according to sex using the FAST-MI programme.The FAST-MI programme consists of four nationwide French surveys carried out 5 years apart from 1995 to 2010, including consecutive patients with AMI over a 1-month period, and with a 1-year follow-up.Among the 10,610 patients included in the surveys, the incidence of CS was 4.8% in men and 8.2% in women (P0.001). Absolute incidence of CS decreased from 1995 to 2010 in both sexes. Mean age in patients with CS tended to decrease in men (from 72±12 to 69±13 years) and to increase in women (from 78±10 to 80±9 years). One-year mortality decreased significantly in men (from 70% in 1995 to 48% in 2010) and in women (from 81% to 54%). Using Cox multivariable analysis, female sex was not an independent correlate of 1-year mortality [hazard ratio (HR): 0.98, 95% confidence interval (CI): 0.78-1.22]. Early use of percutaneous coronary intervention was, however, an independent predictor of 1-year survival in women (HR: 0.55, 95% CI: 0.37-0.81), but showed only a non-significant trend in men (HR: 0.85, 95% CI: 0.61-1.19).The incidence of CS-AMI has decreased in both men and women, but remains higher in women. One-year mortality has significantly decreased for both men and women, and the role of early percutaneous coronary intervention as a potential mediator of decreased mortality seems greater in women than in men.

Abstract

Background:While it is believed that good results can be achieved by arthroscopic debridement of partial-thickness tears (PTTs) of Purpose:To compare the postoperative outcomes of patients with articular- versus bursal-sided PTTs of Study Design:Cohort study; Level of evidence, 3.Methods:An analysis was performed with data from 76 consecutive patients diagnosed with a PTT Results:Between 2001 and 2010, there were 40 (53%) articular- and 36 (47%) bursal-sided tears treated with debridement and selective acromioplasty. The mean patient age was 55 years (range, 36-77 years) for the bursal group and 56 years (range, 33-81 years) for the articular group. The mean follow-up was 24 months (range, 22-26 months). Both groups showed significant improvement in the short version of the Western Ontario Rotator Cuff index, American Shoulder and Elbow Surgeons score, and relative Constant-Murley score 2 years after surgery ( P < .0001) as well as in strength ( P < .0001 for bursal tears, P = .006 for articular tears). There was no statistically significant difference between groups in any of the postoperative outcome measures at 2 years.Conclusion:The results of this study demonstrate that good outcomes can be achieved with arthroscopic debridement and selective acromioplasty among patients with articular- or bursal-sided PTT of

Abstract

Background— Monitoring anticoagulation with activated clotting time (ACT) has been proposed to reduce ischemic or bleeding events. However, the value of using ACT to improve outcomes is uncertain. This study sought to determine the relationship between ACT and outcomes during percutaneous coronary intervention in patients with non–ST-segment–elevation acute coronary syndrome (NSTE-ACS) treated by unfractionated heparin with GPIs (glycoprotein IIb/IIIa inhibitors). Methods and Results— From the randomized TAO trial (Treatment of Acute Coronary Syndromes With Otamixaban), we analyzed the value of ACT to predict ischemic and bleeding outcomes in the 3275 patients receiving unfractionated heparin plus eptifibatide. Ischemic and safety outcomes were analyzed according to ACT to determine the best threshold. Median peak ACT was 225 s. There was no correlation ( r =−0.02; P =0.24) between the unfractionated heparin dose received and the ACT value before percutaneous coronary intervention. There was no evidence of a nonlinear association between ACT and either ischemic or bleeding events ( P =0.66; P =0.07). No threshold was found to predict ischemic complications. Conversely, increased bleeding was observed with ACT >230 s with an optimal threshold of ACTs ≥250 s (4.53% versus 6.17%; odds ratio, 1.46; 95% confidence interval, 1.04–2.06; P =0.028). This optimal threshold varied according to access site: ≥250 s (6.86% versus 10.18%; odds ratio, 1.57; 95% confidence interval, 1.00–2.45; P =0.047) by femoral approach and ≥290 s (2.86% versus 5.43%; odds ratio, 2.24; 95% confidence interval, 1.05–4.44; P =0.027) by radial approach. Conclusions— In the TAO trial, peak procedural ACT ≥250 s was associated with increased bleeding risk in non–ST-segment–elevation acute coronary syndrome patients treated with unfractionated heparin plus GPIs. Abstract

Objective To systematically evaluate the scientific literature examining the efficacy of nonoperative management for midshaft clavicular fractures, specifically looking at the effect of immobilization and/or functional rehabilitation. TYPE: Systematic review. Literature survey MEDLINE (PubMed), EMBASE, CINAHL databases were searched. Methodology Inclusion criteria included adult patients enrolled in a controlled study evaluating bony healing, pain and/or function-related improvements after a strict nonoperative treatment plan for management of midshaft clavicle fractures. Synthesis A total of 10 articles were included in the study. Only four studies contained level I evidence; four studies were prospective case series without a control, and one was a retrospective case series without a control. Half of studies used a strict immobilization period, and most utilized a period of functional rehabilitation. There were no direct comparisons between rehabilitation protocols. Studies without a strict immobilization period had a nonunion rate of 5% to 24% and residual pain in 35% to 83% of patients compared to studies with a strict immobilization period with a nonunion rate of 3% to 29% and residual pain in 14% to 49% of patients. Studies including functional rehabilitation protocol reported functional as measured by the Constant Shoulder Score of 87.8 to 96, out of a maximum of 100 representing no functional limitations, and nonunion rates of 12% to 25% compared to 81 to 85 and 3% to 29% in those without a rehabilitation protocol. Conclusions No studies directly examine the effect of immobilization and functional rehabilitation on clinical outcomes for midshaft clavicular fractures. Future studies are needed to better elucidate the most effective treatment. Level of evidence I.

Abstract

Northern Ireland is emerging from a 30-year local conflict, with new democratic local structures and a new landscape for civic engagement having been established. Community youth workers are well positioned to nurture these new political and civic structures; however, after decades of insular living and thinking, global skills and attitudes are acutely needed. This study gauges the extent to which the international placement of Ulster University (UU) students can help build a population who is “forward- and outward-looking” and how these students’ new perspectives, skills and knowledge can be used for and beyond a “new” Northern Ireland. This mixed-methods study explores the impact of international student placements and their effectiveness in fostering “global-ready citizens”. Findings from this study suggest that the preparation phase must attend equally to both the fear and the opportunities of internationalisation that face the departing student. The study also points to key elements needed in the preparation of students to develop their role as cultural nesters rather than cultural visitors (in their new context). The study illustrates an alignment between the development of intercultural competence and the vocational competence of the community youth work profession. This natural alignment suggests that community youth work students and professionals could further embrace and extend their intercultural skills abroad and their multicultural competence at home for wider and deeper impact.

Abstract

Objective Participation in a physical therapy program is considered one of the greatest predictors of successful conservative management of common shoulder disorders. However, adherence to these protocols is often poor and typically worse for unsupervised home exercise programs. Currently, there are limited tools available for objective measurement of adherence in the home setting. The goal of this study was to develop and evaluate the potential for performing home shoulder physiotherapy monitoring using a commercial smartwatch. Approach Twenty healthy adult subjects with no prior shoulder disorders performed seven exercises from an evidence-based rotator cuff physiotherapy protocol, while 6-axis inertial sensor data was collected from the active extremity. Within an activity recognition chain (ARC) framework, four supervised learning algorithms were trained and optimized to classify the exercises: k-nearest neighbor (k-NN), random forest (RF), support vector machine classifier (SVC), and a convolutional recurrent neural network (CRNN). Algorithm performance was evaluated using 5-fold cross-validation stratified first temporally and then by subject. Main results Categorical classification accuracy was above 94% for all algorithms on the temporally stratified cross validation, with the best performance achieved by the CRNN algorithm (99.4%). The subject stratified cross validation, which evaluated classifier performance on unseen subjects, yielded lower accuracies scores again with CRNN performing best (88.9%). Significance This proof of concept study demonstrates the technical feasibility of a smartwatch device and supervised machine learning approach to more easily monitor and assess the at-home adherence of shoulder physiotherapy exercise protocols.

Abstract

Background: \ud Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.\ud \ud Methods: \ud We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515.\ud \ud Findings: \ud Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. Abstract

Objectives The objective of this study was to assess the 12-month clinical outcomes in patients with drug-eluting stent in-stent restenosis (DES-ISR) who were either predilated with non-compliant balloons (NCBA) and with additional scoring balloons (NCBA + SBA) prior to drug coated balloon (DCB) angioplasty. Methods This monocentric, retrospective study included patients with DES-ISR who were routinely treated over a 2-year time span. Patients with stable angina and documented ischemia or selected forms of unstable angina due to a culprit DES-ISR lesion were analyzed. The primary endpoint was the clinically driven target-lesion revascularization (TLR) rate at 12 months. Secondary endpoints included postinterventional lumen gain and late lumen loss (LLL) at 6 months. Results The 12-month TLR rates in 124 patients who underwent either NCBA + SBA or NCBA only group were not different (17.3%, 9/52 vs. 11.6%, 8/69, P = 0.371) and low as compared to other comparable studies. The use of SBA led to equally high post minimal lumen diameters (MLD) in both treatment arms (NCBA 2.21 ± 0.33 vs. NCBA + SBA 2.18 ± 0.41, P = 0.868). We did not find a significant difference in late lumen loss (LLL) between both groups (0.50 ± 0.62 mm vs. 0.40 ± 0.46 mm, P = 0.468). Conclusions Scoring Balloon Angioplasty can safely and effectively prepare DES-ISR lesions to render them suitable for DCB angioplasty with acceptable TLR and MACE rates.

Abstract

Background Epidemiologic data about cardiogenic shock (CS) are still poor and focused on ischemic CS, forgetting all part of the CS encountered in clinical practice. Methods FRENSHOCK registry (NCT02703038) was a large prospective multicenter registry of non-selected CS patients admitted in critical care units realized between April and October 2016 in France. Patients were included if they met the following three criteria: – low cardiac output defined by SBP – elevation of left and/or right heart pressures defined by clinic/radiology/biology/echocardiography/Swan-Ganz; – clinical and/or biological hypoperfusion. Results In total, 772 patients were included in 48 centers (male 72%, mean age of 66y). Comorbidities associated previous coronary revascularization 26%, peripheral arterial disease 15%, renal failure 21%, COPD 6%, diabetes 28%, tobacco 28%, dyslipidemia 35% and hypertension 47%. 56% were known for previous cardiomyopathy (30% ischemic, 10% idiopathic, 8% valvular). CS etiology often associated several triggers: ischemic was retained for 36% (type 1 infarction 17%). Non-ischemic trigger factors were predominant (64%): supra-ventricular (13%) and ventricular arrythmia (13%), infection (12%), iatrogenic (6%), conductive disorders (2%), non-observance (4%), and others (14%). At admission median SBP was 101.2 mmHg. Right heart failure signs were present in 49% and left signs in 72% (Killip IV 49%). Biological analysis found high lactate (3.0 mmol/L), renal (eGFR 49.6 ml/min) and hepatic alteration (ASAT 90.0 UI/ml; PT 57.1%). Median LVEF was 26.3% and TAPSE 13.4 mm. When realized (52%) coronarography was pathological in 81% (monotroncular 31%, bitroncular 35% and tritroncular 34%). A culprit lesion (79%) concern LVA 48%, RCA 23% and left main 15%. Conclusion CS presentation and etiology are heterogeneous with a predominance of non-ischemic CS in practice.

277. Irreducible Tibial Pilon Fracture Secondary to Posterior Tibial Tendon Entrapment: A Case Report

Abstract

The term pilon fracture was coined in 1911 after a description of this type of injury was published by the French radiologist Destot1. The complication rate from this fracture is high, ranging from 30% to 70%, and includes infection, wound breakdown, chronic ankle pain, and poor patient satisfaction2-6. At our institution, a recent patient with a pilon fracture had the posterior tibial tendon (PTT) trapped within the fracture site, lateral to the central axis of the tibial shaft, preventing reduction and leading to persistent deformity and soft-tissue complications. To the best of our knowledge, this is the first case report in the literature of a PTT that prevented reduction in this manner. The patient was informed that data concerning the case would be submitted for publication, and he provided consent. A twenty-nine-year-old man with a “closed but irreducible pilon fracture” was transferred to our hospital from a community center. At the time of presentation, the injury was sixteen days old. An external fixator was in place and intact. There was a 1 × 1-cm region of stage-2 skin breakdown caused by pressure necrosis from a splint near the medial malleolus. Imaging demonstrated a tibial pilon fracture as well as medial and lateral malleoli fractures and a lateral tibiotalar dislocation (Figs. 1-A and 1-B). Abstract

The oxidative stress element of unhealthy scalp leads to compromised pre-emergent hair formation and poorly formed hair as it grows. Only cosmetic solutions can minimize the impact of unhealthy hair and to achieve healthy looking and feeling hair, the scalp health must be normalized first. The objectives of this research were to both investigate whether oxidative stress was a relevant aetiological element in scalp dandruff and seborrhoeic dermatitis and whether scalp condition affects the quality of hair that grows from it. Further, this research was designed to determine whether an effective anti-dandruff shampoo would repair and protect the scalp and pre-emergent hair from oxidative stress. This study demonstrated that oxidative stress is an aetiological element relevant to the dandruff condition and that a potentiated ZPT shampoo effectively improves scalp condition, including a reduction in oxidative stress. The compromised hair condition associated with dandruff is concomitantly improved when the scalp condition is improved. It appears that there is a direct link between hair quality and scalp health.

Abstract

Background: Clinical studies of shoulder dislocations typically include adult patients (>16 years of age). Only small case series of patients aged 10 to 16 years are available to guide management. Purpose: Using a cohort of patients aged 10 to 16 years, this study sought to determine (1) the incidence density rate (IDR) of primary anterior shoulder dislocations requiring closed reduction (CR) and (2) the rate of and risk factors for repeat shoulder CR. Study Design: Cohort study (prognosis); Level of evidence, 2. Methods: With use of administrative databases, patients aged 10 to 16 years who underwent CR of a primary anterior shoulder dislocation in Ontario, Canada, between April 2002 and September 2010 were gathered. IDRs for the entire cohort and demographic subgroups were calculated. The main outcome, repeat shoulder CR, was sought until September 2012. The cumulative incidence of repeat CR was calculated at multiple time points for the entire cohort and age subgroups. A competing risk model identified risk factors for repeat CR (reported as hazard ratios [HRs] with 95% CIs). Results: There were 1937 patients aged 10 to 16 years who underwent primary CR (median age, 15.0 years; 79.7% male). The incidence of primary CR was highest among male patients aged 16 years (164.4 per 100,000 person-years), but primary dislocations were rare in 10- to 12-year-old children (n = 115; 5.9% of all dislocations). Repeat CR was observed in 740 patients (38.2%) after a median of 0.8 years; however, the rate of repeat CR was age dependent: it was highest among 14- to 16-year-old patients (37.2%-42.3%) and considerably lower among 10- to 13-year-old patients (0%-25.0%). Male sex (HR, 1.2 [95% CI, 1.0-1.5]; P = .04) and older patient age (HR, 1.2 [95% CI, 1.1-1.3]; P < .001) significantly increased the odds of repeat CR. Conclusion: Among 14- to 16-year-old patients, the rate of primary and recurrent shoulder CR mirrors that of high-risk adults (17-20 years of age) from previously published data; however, the rate of shoulder CR (primary or recurrent) is considerably lower among 10- to 13-year-olds. In addition to older patient age, male sex increased the odds of repeat shoulder CR. Going forward, clinicians should counsel male patients and those aged 14 to 16 years regarding their increased risk of recurrence after the nonoperative management of a primary anterior shoulder dislocation.

Abstract

Introduction Simple elbow dislocations treated by closed reduction are thought to result in a satisfactory return of function in most patients. Little, however, is known about how many patients ultimately proceed to subsequent surgical treatment due to the low patient numbers and significant loss to follow-up in the current literature. The purpose of this study was to establish the rate of and risk factors for subsequent surgical treatment after closed reduction of a simple elbow dislocation at a population level. Patients and methods All patients aged 16 years or older who underwent closed reduction of a simple elbow dislocation between 1994 and 2010 were identified using a population database. Subsequent procedures performed for joint contractures, instability or arthritis were recorded. Outcomes were modelled as a function of age, sex, income quintile, co-morbidity, urban/rural status, physician speciality performing the initial reduction and whether orthopaedic consultation and/or post-reduction radiograph was performed within 28 days of the injury, in a time-to-event analysis. Results We identified 4878 elbow dislocations with a minimum 2-year follow-up: stabilisation surgery was performed in 112 (2.3%) at a median time of 1 month, contracture release in 59 (1.2%) at median 9 months and arthroplasty in seven (0.1%) at median 25 months. Admission to hospital for the initial reduction was associated with an increased risk of undergoing stabilisation (hazard ratio (HR), 2.50; 95% confidence interval (CI), 1.67–3.74) and contracture release (HR, 1.93; CI, 1.08–3.44). Multiple reduction attempts increased the risk of requiring contracture release (HR, 3.71; CI, 1.22–11.29). Survival analysis demonstrated that all subsequent procedures had taken place by 4–5 years. Conclusion Few patients with simple elbow dislocations develop complications requiring surgery, but those that do most commonly undergo soft-tissue stabilisation or contracture release within 4 years of the injury. Contrary to current thinking, surgery for instability is performed more often than joint contracture release, albeit with slightly different time patterns.

Abstract

Aspirin is the key treatment in the secondary prevention of atherothrombosis. Interindividual variability of response has been linked to a higher risk for ischemic events. The aim of this study was to identify clinical and biologic factors predicting high on-aspirin platelet reactivity (HPR) in a high-risk, "real-world" population of vascular patients. All platelet testing performed from 2011 to 2013 in consecutive patients receiving long-term treatment with aspirin for coronary or cerebrovascular disease was retrospectively analyzed. Indications for platelet testing were recurrent ischemic events or high-risk angioplasty. HPR was defined as aggregation intensity≥20% using light-transmission aggregometry with arachidonic acid 0.5 mg/ml. Collagen-epinephrine platelet function analysis was also performed (threshold165 seconds). Cardiovascular risk factors, usual biologic parameters, and antiplatelet treatment were recorded. A total of 1,508 patients were included (mean age 63 years, 71% men, 23% with diabetes). Antiplatelet treatment was aspirin alone in 333 patients and dual-antiplatelet therapy in 1,175 patients. HPR was found in 11.1% of patients. In multivariate analysis, independent predictive factors of HPR on light-transmission aggregometry with arachidonic acid were diabetes mellitus (odds ratio [OR] 2.10, 95% confidence interval [CI] 1.39 to 3.16), age (OR 1.25, 95% CI 1.06 to 1.47), fibrinogen level (OR 1.20, 95% CI 1.02 to 1.42), and von Willebrand factor level (OR 1.06, 95% CI 1.03 to 1.09). On light-transmission aggregometry with arachidonic acid and collagen-epinephrine platelet function analysis, fibrinogen remained the main factor associated with HPR (OR 1.33, 95% CI 1.19 to 1.61). Similar results were found in patients treated with aspirin alone or dual-antiplatelet therapy. A fibrinogen level4.0 g/L was associated with a 3.9-fold increased risk for HPR in patients aged75 years. In conclusion, fibrinogen level was the major predictor of HPR on aspirin in this large population of high-risk vascular patients.

Abstract

SYD985 is a HER2-targeting antibody–drug conjugate (ADC) based on trastuzumab and vc-seco-DUBA, a cleavable linker-duocarmycin payload. To evaluate the therapeutic potential of this new ADC, mechanistic in vitro studies and in vivo patient-derived xenograft (PDX) studies were conducted to compare SYD985 head-to-head with T-DM1 (Kadcyla), another trastuzumab-based ADC. SYD985 and T-DM1 had similar binding affinities to HER2 and showed similar internalization. In vitro cytotoxicity assays showed similar potencies and efficacies in HER2 3+ cell lines, but in cell lines with low HER2 expression, SYD985 was 3- to 50-fold more potent than T-DM1. In contrast with T-DM1, SYD985 efficiently induced bystander killing in vitro in HER2-negative (HER2 0) cells mixed with HER2 3+, 2+, or 1+ cell lines. At pH conditions relevant for tumors, cathepsin-B cleavage studies showed efficient release of the active toxin by SYD985 but not by T-DM1. These in vitro data suggest that SYD985 might be a more potent ADC in HER2-expressing tumors in vivo, especially in low HER2-expressing and/or in heterogeneous tumors. In line with this, in vivo antitumor studies in breast cancer PDX models showed that SYD985 is very active in HER2 3+, 2+, and 1+ models, whereas T-DM1 only showed significant antitumor activity in HER2 3+ breast cancer PDX models. These properties of SYD985 may enable expansion of the target population to patients who have low HER2-expressing breast cancer, a patient population with still unmet high medical need. Abstract

Antibody-drug conjugates (ADCs) that are currently on the market or in clinical trials are predominantly based on two drug classes: auristatins and maytansinoids. Both are tubulin binders and block the cell in its progression through mitosis. We set out to develop a new class of linker-drugs based on duocarmycins, potent DNA-alkylating agents that are composed of a DNA-alkylating and a DNA-binding moiety and that bind into the minor groove of DNA. Linker-drugs were evaluated as ADCs by conjugation to the anti-HER2 antibody trastuzumab via reduced interchain disulfides. Duocarmycin 3b, bearing an imidazo[1,2-a]pyridine-based DNA-binding unit, was selected as the drug moiety, notably because of its rapid degradation in plasma. The drug was incorporated into the linker-drugs in its inactive prodrug form, seco-duocarmycin 3a. Linker attachment to the hydroxyl group in the DNA-alkylating moiety was favored over linking to the DNA-binding moiety, as the first approach gave more consistent results for in vitro cytotoxicity and generated ADCs with excellent human plasma stability. Linker-drug 2 was eventually selected based on the properties of the corresponding trastuzumab conjugate, SYD983, which had an average drug-to-antibody ratio (DAR) of about 2. SYD983 showed subnanomolar potencies against multiple human cancer cell lines, was highly efficacious in a BT-474 xenograft model, and had a long half-life in cynomolgus monkeys, in line with high stability in monkey and human plasma. Studies comparing ADCs with a different average DAR showed that a higher average DAR leads to increased efficacy but also to somewhat less favorable physicochemical and toxicological properties. Fractionation of SYD983 with hydrophobic interaction chromatography resulted in SYD985, consisting of about 95% DAR2 and DAR4 species in an approximate 2:1 ratio and having an average DAR of about 2.8. SYD985 combines several favorable properties from the unfractionated ADCs with an improved homogeneity. It was selected for further development and recently entered clinical Phase I evaluation.

Abstract

Many anesthesiologists are unfamiliar with the rate of surgical neurological complications of the shoulder and elbow procedures for which they provide local anesthetic–based anesthesia and/or analgesia. Part 1 of this narrative review series on neurological complications of elective orthopedic surgery describes the mechanisms and likelihood of peripheral nerve injury associated with some of the most common shoulder and elbow procedures, including open and arthroscopic shoulder procedures, elbow arthroscopy, and total shoulder and elbow replacement. Despite the many articles available, the overall number of studied patients is relatively low. Large prospective trials are required to establish the true incidence of neurological complications following elective shoulder and elbow surgery. What’s New As the popularity of regional anesthesia increases with the development of ultrasound guidance, anesthesiologists should have a thoughtful understanding of the nerves at risk of surgical injury during elective shoulder and elbow procedures.

Appraisals could be problematic; nevertheless they are deemed as necessary in organizational management. This is because workers might not always view appraisals as a positive measure. When the employees perceive appraisals to be fair, it emits positive attitude and vice versa. This study aimed to understand the perceptions of employees towards performance appraisals. Data (n=100) was gathered through questionnaires and administered to workers in the banks of Northern Cyprus. An overall perception of respondents shows a positive rating towards interpersonal justice, distributive justice and procedural justice in performance appraisal (PA). While the respondents agreed to rating PA system positively, metrics that define high satisfaction with the PA system was statistically low and non-significant. This can be due to the fact that the majority of these staff perceives PA to be compulsory or organizational routines, and did not evidently see how it affects their job or their career. Furthermore, satisfaction with PA system can only be translated when the employees see that a positive appraisal results in pay rise, promotion, training and development, awards, other monetary incentives such as benefits and insurance. Abstract

The COnstrain Dark Energy with X-ray clusters (CODEX) sample contains the largest flux limited sample of X-ray clusters at 0.35 < z < 0.65. It was selected from ROSAT data in the 10 000 square degrees of overlap with BOSS, mapping a total number of 2770 high- z galaxy clusters. We present here the full results of the CFHT CODEX programme on cluster mass measurement, including a reanalysis of CFHTLS Wide data, with 25 individual lensing-constrained cluster masses. We employ lensfit shape measurement and perform a conservative colour–space selection and weighting of background galaxies. Using the combination of shape noise and an analytic covariance for intrinsic variations of cluster profiles at fixed mass due to large-scale structure, miscentring, and variations in concentration and ellipticity, we determine the likelihood of the observed shear signal as a function of true mass for each cluster. We combine 25 individual cluster mass likelihoods in a Bayesian hierarchical scheme with the inclusion of optical and X-ray selection functions to derive constraints on the slope α, normalization β, and scatter σln λ|μ of our richness–mass scaling relation model in log-space: |${\langle {\rm In}\,\, \lambda\!\!\mid\!\!\mu\rangle = \alpha\mu + \beta,} $| with μ = ln (M 200 c / M piv), and M piv = 1014.81M⊙. We find a slope |$\alpha = 0.49^{+0.20}_{-0.15}$|⁠ , normalization |$\exp (\beta) = 84.0^{+9.2}_{-14.8}$|⁠ , and |$\sigma _{\ln \lambda | \mu } = 0.17^{+0.13}_{-0.09}$| using CFHT richness estimates. In comparison to other weak lensing richness–mass relations, we find the normalization of the richness statistically agreeing with the normalization of other scaling relations from a broad redshift range (0.0 < z < 0.65) and with different cluster selection (X-ray, Sunyaev–Zeldovich, and optical). Therefore, any complete model of human intelligence must be able to understand stories. Understanding stories requires commonsense knowledge. Many story understanding systems rely on rules to express and manipulate common sense, but the amount of common sense is vast and expressing significant amounts of common sense this way is tedious. Commonsense knowledge bases can help address this problem. My work demonstrates the power of combining a story-understanding system and a commonsense knowledge base. Genesis is a story-understanding system that models aspects of story understanding by reading and analyzing stories written in simple English. I have extended Genesis by incorporating ConceptNet, a large knowledge base of common sense, into its processing. Now, Genesis not only has access to the dozens of inference rules provided by Genesis users, but also tens of thousands of goal-related assertions provided by contributors to ConceptNet. Genesis uses ConceptNet's knowledge to infer characters' goals, in some cases allowing Genesis to understand stories with few or even zero user-defined rules. Genesis can now infer relevant events and connections never explicitly stated in the story in a manner unprecedented within the space of story understanding and machine reading. Genesis also uses ConceptNet to apply user-defined rules in a commonsense, more flexible fashion, extending the reach of a rule by factors ranging from ten to one hundred. Abstract

Macrophomina phaseolina is a generalist ascomycetic fungal pathogen, capable of infecting over 500 genera of plants and limiting yield in crops grown in Mississippi. Recent documentation of M. phaseolina on Impatiens × hybrida , a newfound host, has merited multiple experiments to quantify the exact nature of this relationship. Despite M. phaseolina being a soil-borne pathogen, disease symptoms were only reported in aboveground tissue. Mode of infection experiments revealed both above and belowground tissues are susceptible to infection. In vitro experiments identified the optimal temperature for the growth of M. phaseolina to be 26°C, where more than 10x the accumulated biomass resulted compared to samples grown at 37°C. Impatiens × hybrida hosts were particularly prone to infection at temperatures above 27°C. In vitro fungicide assays revealed Banrot and T-Bird to be suitable chemical control agents for limiting M. phaseolina growth.

Abstract

BACKGROUND: Cardiogenic shock (CS) complicating acute myocardial infarction (AMI) remains a major concern. Failure of the left ventricular (LV) pump is the primary insult in most forms of CS, but other parts of the circulatory system and diastolic function contribute to shock. However, little is known of the clinical presentation, management and outcomes according to LV function in these patients.OBJECTIVES: To assess the presentation, management and clinical outcomes in patients admitted for AMI with CS according to early LV ejection fraction (LVEF), using long-term data from the French registry of Acute ST-elevation or non-ST-elevation Myocardial Infarction (FAST-MI) 2010.METHODS: We analysed baseline characteristics, management and 3-year mortality in patients with CS, according to LVEF (≤40% vs>40%). The analyses were replicated in the FAST-MI 2005 cohort.RESULTS: Among 4169 patients with AMI included in the survey, the incidence of CS was 3.3%. LVEF was>40% in 43%. Early PCI (≤24hours) was used more often in patients with LV dysfunction (61% vs 42%), as was the use of optimal medical therapy at discharge (66% vs 40%). CS remained associated with a major increase in 3-year mortality, both in patients with LVEF ≤40% (55%) and in those with LVEF>40% (44%). Using Cox multivariable analysis, LVEF ≤40% was associated with higher 3-year mortality (hazard ratio 2.08, 95% confidence interval 1.15-3.78) in patients with AMI with CS. Consistent results were found in the replication cohort.CONCLUSIONS: Despite the many circulatory system contributors to the physiopathology of CS in patients with AMI, the occurrence of early LV systolic dysfunction is associated with higher long-term mortality., Contexte: Le choc cardiogénique (CC) reste une complication redoutable de l’infarctus du myocarde (IDM). Il résulte le plus souvent d’une dysfonction systolique sévère mais la dysfonction diastolique ainsi que la vasodilatation périphérique conséquence du syndrome de réponse inflammatoire systémique peuvent contribuer aux mécanismes du choc. Pourtant peu de données sont disponibles sur l’épidémiologie des patients présentant un IDM compliqué de CC en fonction de la fonction ventriculaire gauche (VG).Objectifs: Évaluer la présentation clinique, la prise en charge et la mortalité à trois ans des patients avec CC à la phase aiguë d’un IDM en fonction de la présence ou non d’une dysfonction VG (DVG) à partir des données de FAST-MI2010.Méthodes: Les patients avec DVG (fraction d’éjection VG [FEVG] ≤ 40 %) étaient comparés aux patients sans DVG. Les analyses étaient répliquées dans la cohorte FAST-MI 2005.Résultats: Parmi les 4169 patients ayant présenté un IDM, 3,3 % des patients ont présenté un CC dont 43 % avec FEVG > 40 %. L’angioplastie percutanée précoce (≤ 24 heures) était utilisée plus souvent chez les patients avec DVG (61 % vs 42 %). Le CC restait associé à une mortalité élevée à trois ans dans les deux groupes ; 55 % chez le groupe FEVG ≤ 40 % et 44 % dans le groupe FEVG > 40 %. En analyse multivariée, la DVG < 40 % était associée à une surmortalité à trois ans (HR 2,08, IC95 % 1,15–3,78) chez les patients en CC. L’analyse dans la cohorte 2005 retrouvait des résultats similaires.Conclusions: Malgré une physiopathologie complexe du CC compliquant l’IDM, la DVG ≤ 40 % reste associé à une mortalité élevée à trois ans.

Abstract

Background The Intensive Cardiac Care Unit (ICCU) has greatly evolved for decades: it no longer includes only patients with coronary artery disease (CAD). The clinical characteristics and pathological profiles of patients have markedly changed. Detailed data on the topic are critically lacking. Methods We present here a French nation-wide administrative database with an exhaustive description of patients admitted to ICCU throughout a whole year (2014). Results A total of 277,845 patients in 270 centers were admitted to ICCUs at least once in 2014 (exhaustive data). Median age was 71years (IQR: 59–81) and the patients were primarily male (63%). Mean ICCU stay was 2.0days (1.0–4.0). CAD patients (49.0%) represented the major group admitted, followed by patients with arrhythmias (15.2%) and heart failure (HF) (10.0%). Patients admitted with acute CAD were significantly younger (mean age 67.4 y), had better outcomes (mortality 4.0%), and shorter hospital stays (mean stay 6.7 d). Patients with HF were significantly older (mean age 75.2 y), with longer hospital stays (mean stay 12.0 d), and poorer outcomes (mortality 10.5%). Conclusion We present here the largest contemporary administrative database on patients admitted to ICCUs in a developed country. CAD (mainly acute coronary syndromes) remains the primary cause of admission but the population is, by far, more complex than generally considered.
