Avaliou-se a influência do uso de dejetos de suínos e cama de aves sobre o conteúdo de carbono orgânico total (COT) e particulado (COP) e a sua distribuição nas frações granulométricas e em classes de agregados de um Nitossolo Vermelho Eutróférrico. As áreas analisadas foram milho para silagem (M7 anos); milho para silagem (M20 anos); pastagem de azevém (P3 anos); pastagem de azevém (P15 anos); pastagem perene (PP20 anos); erva mate (EM20 anos); mata nativa (MN) e pastagem nativa sem aplicação de dejetos (P0 anos). Os estoques de COT e COP foram maiores na área de mata nativa. A MN teve os maiores teores de C-agregados do que todos os tratamentos nas três classes de agregados avaliados, e M7 teve os menores teores. A aplicação de fertilizantes orgânicos de suínos e aves não aumentou os teores de carbono orgânico do solo.

Blue rubber bleb nevus syndrome is a rare condition characterized by the presence of multiple angiomatic lesions of the skin. These are associated with similar lesions in other organs, namely in the gastrointestinal tract, causing anemia through chronic bleeding. We describe the case of a 72-year-old woman with microcytic anemia. A barium study revealed irregular lacunae in the distal esophagus. A subsequent endoscopy showed blue nodular lesions similar to angiomas of the esophagus and stomach fundus. Endosonography confirmed its angiomatic nature. Exploration of other organs, using magnetic resonance and cranial computed tomography, did not reveal the presence of this type of lesion. In physical examination, two angiomatic lesions were observed on the face and lips, respectively. These were blue in color and compressible, leaving an empty wrinkled sac that rapidly refilled, typical of angiomas.

Dear Editor: Gastrointestinal hemangiomas and vascular malformations are uncommon benign vascular tumors that can occur anywhere in the gastrointestinal tract. They can be distributed throughout the intestinal digestive system or present as a single cavernous hemangioma/malformation, which is often located in the rectosigmoid region [1]. We report a case of a 44-year-old woman who was previously asymptomatic and had a normal physical examination, and laboratory parameters underwent a colonoscopy after a single episode of rectal bleeding. The endoscopic evaluation revealed, in addition to a 30-mm sessile polyp resected at that time, the presence of an extensive, purplish, embossed lesion, with focal aberrant vessels occupying the rectum. Given the suspicion of a vascular origin of the lesion, no biopsy samples were taken. Narrow band imaging (NBI) helped us to establish the boundaries of the polypoid lesion, and with careful positioning of the polypectomy snare, we avoided the resection of other pseudopolypoid hemangiomatous lesions. A contrastenhanced MRI showed a heterogeneous mass of 7.5×3.5 cm in size in the posterior wall of the rectum, with high signal intensity on T2-weighed images and multiple areas of low signal intensity on T1 and T2, possibly related to phleboliths. The mass had the lower extremity at the level of the rectal insertion of the elevator muscle of the anus, avoiding the anal canal and adjacent structures. The study was completed with endoscopic ultrasonography that confirmed the presence of a heterogeneous hypoechoic lesion, containing anechoic and hyperechoic areas, limited by muscularis propria. The lesion had pulsatile flow by Doppler study and was supplied by a small extraluminal vessel. An upper endoscopy and an angiography were performed to exclude the presence of other gastrointestinal vascular lesions. Given the possibility of recurrent bleeding with possible severe hemorrhage (even occult) and the size of the lesion, a surgical procedure was discussed with the patient, who decided to postpone the procedure. Phillips first described cavernous hemangioma of the large intestine in 1839 [2]. Since then, over 200 cases of colonic cavernous hemangiomas and approximately 100 cases of diffuse cavernous hemangioma of the rectum (DCHR) have been reported. An estimated 80 % of patients with DCHR exhibit symptoms, with intraluminal bleeding in the majority of cases. Up to 90 % have recurrent painless bleeding; half will have chronic iron-deficient anemia [3]. Other symptoms stem from a possible compression or invasion of adjacent structures, such as lumbar or perianal pain, metrorrhagia, and hematuria [4]. Misdiagnosis is a major theme in cases of hemangiomas and malformations. A large number of these lesions are nearly missed or falsely diagnosed because symptoms are attributed to hemorrhoids, polyps, or ulcerative colitis, resulting in an average delay of diagnosis of approximately 19 years. Furthermore, approximately 80 % of patients undergo one prior inappropriate surgical procedure, typically hemorrhoidectomy [3, 4]. Our case is an example of an atypical presentation because the patient remained asymptomatic and without anemia until adulthood. Mestrado em Economia e Políticas Públicas Esta dissertação tem como objectivo central analisar o Governo das Sociedades ("Corporate Governance") das empresas do Sector Empresarial do Estado, evidenciando se existem ou não diferenças entre empresas públicas (EPs) e privadas que fazem com que seja necessário formular uma teoria de Governo das Sociedades específica para as primeiras. Este objectivo será alcançado através da elaboração de um estudo de caso sobre a Companhia Carris de Ferro de Lisboa S.A., no qual irão ser analisados tópicos como o problema dos múltiplos principais, indemnizações compensatórias, modelo de financiamento, e processo de nomeação dos gestores públicos. A revisão de literatura pretende destacar a importância e impacto que a performance das EPs, reclassificadas ou não no perímetro das Administrações Públicas, detém na sociedade e nas finanças públicas. Segundo a OCDE (2005), é necessário tratar de forma distinta este tipo de empresas, pois apresentam características e necessidades específicas com consequências económicas relevantes. Aquando do estudo sobre a Carris foi possível confirmar que existem de facto diferenças entre EPs e privadas, como o facto das primeiras terem um estatuto legal diferente, objectivos operacionais mais voláteis, distintos critérios na selecção profissional, restrições orçamentais mais leves e o problema dos múltiplos principais, o que realçou a necessidade de adoptar mudanças na governação destas empresas. Sugere-se assim a aplicação do princípio comply-or-explain às EPs, a adopção de um código de boas práticas na nomeação dos gestores públicos, e a contratualização do serviço público, com a consequente atribuição plurianual de indemnizações compensatórias. This thesis has as main objective to analyze the Corporate Governance of the enterprises within the State Business Sector, indicating whether or not there are differences between private and state-owned enterprises (SOEs) that make it necessary to formulate a theory of Corporate Governance specific for the last ones. This will be achieved through the development of a case study about Companhia Carris de Ferro de Lisboa S.A., in which will be analyzed topics such as the multiple-principals problem, compensatory payments, financing model, and the appointment process of public managers. The literature review aims to highlight the importance and impact that the performance of SOEs, reclassified or not on the perimeter of Public Administrations, holds in society and public finances. According to the OECD (2005), these enterprises should be treated differently, because they present specific characteristics and needs with significant economic consequences. During the study about Carris it was possible to confirm that there are indeed differences between private and SOEs, such as the fact that the second ones have a different legal status, more volatile operating objectives, different criteria for the professional selection, soft budget constraints and the problem of multiple principals, which stressed out the need for changes in the governance of this enterprises. It is therefore suggested the application of the principle of comply-or-explain to SOEs, the adoption of a code of best practices in the appointment of public managers, and the contracting of public service, with the consequent multiannual allocation of compensatory payments.

Crohn's disease (CD) induces cumulative structural damage, initially characterized by a non-stenosing non-penetrating behavior (B1) with progression over time to a fibro-stenosing (B2) and/or penetrating phenotype (B3). Our aim was to assess the long-term evolution of disease behavior of CD and determine what factors predict phenotype progression.This was a study based on prospectively collected data from a CD database in an inflammatory bowel disease outpatient clinic. B1 corresponds to a non-stenosing non-penetrating disease, B2 to a stenosing behavior, and B3 to a penetrating one.Seven hundred and thirty-six patients with CD (368 female) were followed up for 12.3 years (± 8.4), with 87.0% of them exhibiting B1 phenotype at diagnosis. Of these patients, 28.5% progressed to B2 phenotype and 23.5% to B3. Fifty percent of the patients started azathioprine treatment before phenotype change and 13.9% started anti-tumor necrosis factor-α (anti-TNFα) treatment before phenotype change. Monotherapy with azathioprine before phenotype change as well as combination therapy with azathioprine/anti-TNFα before phenotype change delayed disease progression (B1-B2 or B3) in comparison with patients who did not receive treatment (P0.001). The hazard ratio (HR) for disease progression was lower for both monotherapy with azathioprine (HR: 0.15, P0.001) or combination therapy with anti-TNFα (HR: 0.33, P0.001). Upper gastrointestinal tract involvement, male gender, and steroid use were associated with an early progression of phenotype from B1 to B2 or B3 (P0.001). The HR for disease progression was higher in patients who used steroids without criteria of dependence or resistance (HR: 2.67, P0.001) and was even higher in patients with criteria of dependence or resistance (HR: 6.44, P0.001). Longer delays between CD diagnosis and beginning of therapy with azathioprine and/or anti-TNFα were associated with disease progression. The longer the duration of treatment, the less likely the disease progression.Monotherapy with azathioprine before behavior change as well as combination therapy with azathioprine and anti-TNFα before behavior change delays phenotype progression of CD, whereas upper gastrointestinal tract involvement, male gender, and steroid use with or without criteria of steroid dependence are associated with a higher risk for disease progression.

Pseudomonas aeruginosa is a relevant opportunistic pathogen, and worryingly it frequently shows a low antibiotic susceptibility. This bacterium is responsible for 65% of mortality in hospitals all over the world. One of its virulence factors is associated with the ability to adhere to surfaces and form virulent biofilms. This work describes the results from several years of investigation using P. aeruginosa phages alone and combined with antibiotics or other phages against single and mixedspecies biofilms. The mixed species biofilms of P. aeruginosa reported herein were formed with fungi (Candida albicans) since these two microorganisms co-inhabit a wide variety of environments and the interactions between them can result in huge medical and economic impacts; and with Acinetobacter baumannii also an opportunistic pathogen associated with nosocomial infections. In both mixed species biofilms there was observed an inhibitory effect of P. aeruginosa since the levels of C. albicans and A. baumannii were highly reduced in the presence of P. aeruginosa. In P. aeruginosa - C. albicans biofilms, the Pseudomonas phages were able to attack their host population; however, as soon as the phages had killed P. aeruginosa, the numbers of viable C. albicans cells increased rapidly. Furthermore, C. albicans’ morphology and virulence were significantly affected in the presence of P. aeruginosa. In P. aeruginosa – A. baumannii biofilms, phages applied only to one of the hosts decreased, as expected, that specific population already after 6 h. Nevertheless, while after treatment of the mixed species biofilms with P. aeruginosa phages we observed a growth of A. baumannii, the same did not occur when the biofilms were only treated with the A. baumannii phage. The use of both phages was effective and reduced significantly the numbers of viable cells of the mixed population biofilm. Despite the potential of the phages used in this work as antimicrobial agents, it is well known that bacteria can quickly adapt and create new survival strategies and the emergence of phage-resistant phenotypes is inevitable. Indeed, we observed the rapid appearance of Pseudomonas aeruginosa resistant phenotypes following 24h of biofilm contact with phages and those phenotypes exhibited altered LPS structures. Thus, the combination therapy of phages and 4 commonly used antibiotics in the treatment of P. aeruginosa infections was also evaluated. The results obtained proved that certain antibiotics and phages have potentially more benefits and even act synergistically compared to just phages or antibiotics alone.

Antimicrobial photodynamic therapy (aPDT) has been proposed as an adjunctive therapy to scaling and root planing (SRP). The transforming growth factor-β1 (TGF-β) has been considered as an anti-inflammatory cytokine, and its levels in the gingival crevicular fluid (GCF) could monitor the periodontal repair. This study evaluated the adjunct effect of aPDT compared with SRP, analyzing the TGF-β levels in GCF after nonsurgical and surgical therapy in chronic periodontitis patients.Fifteen patients, presenting bilaterally lower molars with class III furcation lesions, were selected. Each pair of teeth was randomly assigned to a control group (CG) or test group (TG). Initially, SRP was performed in the CG, and SRP + aPDT in the TG. Forty-five days later, flap surgery plus SRP, and flap surgery plus SRP + aPDT were performed in CG and TG, respectively. GCF was collected and an enzyme-linked immunosorbent assay (ELISA) test was conducted to determine the amount and concentration of TGF-β in the GCF at baseline, 45 days post-initial therapy, and 21 days after surgery.Statistically significant differences between groups were found in relation to GCF volume 21 days after the surgical procedures (p=0.03) and TGF-β concentration in GCF 45 days post-initial therapy (p=0.04), favoring the TG.There was an additional effect of the aPDT protocol compared with SRP for the TGF-β concentration in GCF 45 days after nonsurgical therapy, and for the GCF volume 21 days after surgical therapy.

The globalization context stimulates many companies to search for new ways to obtain competitive advantage though International Purchasing. Nevertheless, in the global scope of Supply Chain operations, the complexity growth of financial and resources flows have increased the risks related to International Purchasing activities. The use of Risk Management tools for International Supply Chain together with a more strategic view of the Purchasing role for better understanding of the main steps for International Purchasing Cycle can be decisive for one company performance. This study aims to verify how the presence of risk can mediate the relation between International Purchasing and Perceived Performance in the energy and electric/electronics sectors in Brazil. In recent years, ABINEE, association which represents these sectors, has demonstrated growing concern about international crisis repercussion in exportation volume and importation growth. The research was developed in 02 steps. Initially, it was done one exploratory research based on depth interviews with Purchasing or Sourcing Managers which were analyzed using semantic content for an initial view of the risks perception and main ways to manage them. Further, it was done a descriptive research using quantitative method to analyze the simultaneous relation among the constructs based on 149 questionnaires obtained from network professional contacts in the relationship site Linkedin using Factor Analysis and Structural Equation Modeling. The hypothesis which the risks, solely, mediate the relationship with performance was not validated. The results validated the hypothesis which indicates the bigger the likelihood of risks occurrence, the more effective the International Purchasing Management must be and also validated the hypothesis indicating that an effective International Purchasing Management can have positive impact in the Company Performance. The contributions of this study are relevant to the national context of both academic and practical aspects to add the risks for an effective International Purchasing Management in the energy, electric/electronic sectors, which are still dependent on foreign technology. O contexto de globalização impulsiona muitas empresas a buscarem novas formas de obtenção de vantagem competitiva através de Compras Internacionais. Não obstante, no âmbito global de atuação da Cadeia de Suprimentos, o aumento da complexidade dos fluxos financeiros e de recursos, tem repercutido no aumento dos riscos relacionados às atividades das Compras Internacionais. O uso de ferramentas de gestão de riscos em Cadeias de Suprimentos Internacionais, em conjunto com uma visão mais estratégica da função de Compras para melhor conhecimento das principais etapas no Ciclo de uma Compra Internacional, podem ser decisivos para o desempenho de uma empresa. O presente trabalho busca verificar como a presença dos riscos pode mediar a relação entre a Gestão das Compras Internacionais e Desempenho percebido dos Setores de Energia e Eletroeletrônicos no Brasil. Nos últimos anos, a ABINEE, Associação que representa esses setores, tem demonstrado crescente preocupação com repercussão de crises internacionais no volume de exportações e aumento de importações. A pesquisa foi feita em duas etapas. Primeiramente, foi feita uma pesquisa de natureza exploratória constituída de entrevistas em profundidade com gestores de Compras ou Importação, as quais foram tratadas pela técnica semântica da análise de conteúdo para uma visão inicial da percepção desses riscos e principais formas de geri-los. Na sequência, foi feita uma pesquisa de natureza descritiva com método quantitativo para analisar a relação simultânea entre os constructos a partir dos 149 questionários obtidos da rede de contatos do site de relacionamentos profissionais Linkedin com uso de Análise Fatorial e Modelagem em Equações Estruturais. A hipótese de que os riscos, unicamente, mediam a relação com o desempenho não foi validada. Os resultados, contudo, validaram a hipótese que indica que quanto maior a probabilidade de ocorrência de riscos, mais efetiva deverá ser a Gestão de Compras Internacionais e também validaram a hipótese de que uma Gestão efetiva de Compras Internacionais pode ter impacto positivo no Desempenho da empresa. Dissertação de mestrado em Bioengenharia, Pseudomonas aeruginosa e Acinetobacter baumannii são duas bactérias gram-negativas patogénicas altamente problemáticas em infeções hospitalares muito devido à sua capacidade de aderir a variadíssimas superfícies e formar biofilmes que contribuem bastante para o aumento da resistência aos agentes antimicrobianos, o que consequentemente potencia infeções graves em pacientes. Apesar da terapia fágica ter vindo a ser referida como uma estratégia alternativa no controlo de biofilmes, ainda há poucos estudos sobre a interação de fagos com as respetivas bactérias hospedeiras em biofilme mistos. Este trabalho descreve o efeito de fagos em biofilmes simples de A. baumannii AB1 e biofilmes mistos formados por A. baumannii AB1 e P. aeruginosa PAO1. Para este fim, foram desenvolvidos métodos de diferenciação das duas espécies no biofilme recorrendo à técnica de FISH (do inglês, Fluorescent in situ hibridization). Para uma deteção multiplex de bactérias e fagos foram desenhadas “in silico” sondas com base num novo ácido nucleico LNA (do inglês Locked Nucleic Acid), contudo só a sonda específica para a bactéria P. aeruginosa foi sintetizada e aplicada ao estudo. O método de LNA FISH para a identificação desta bactéria foi otimizado e testes laboratoriais em amostras de subespécies de Pseudomonas confirmaram os valores teóricos previstos de especificidade e sensibilidade. Os ensaios de infeção fágica contra biofilme simples de A. baumannii, mostraram que os fagos são eficientes, mesmo usando uma razão de 1 fago para 1 célula hospedeira. Com efeito, verificou-se uma redução de aproximadamente 2 log de células viáveis em biofilmes de 24 h após 6 h de contacto com o fago. Contudo a eficácia dos fagos mostrou-se significativamente superior contra células do biofilme raspado o que indica que a matriz do biofilme constitui uma barreira para a eficiente atuação do fago. Os biofilmes mistos mostraram uma predominância de P. aeruginosa em relação a A. baumannii, mesmo quando P.aeruginosa é adicionada após colonização inicial por A.baumannii. A aplicação de um cocktail formado por fagos específicos contra estas duas espécies presentes em biofilme misto revelou uma redução de 2 log de P. aeruginosa e de aproximadamente 1,5 log de A. baumannii após 6 h. No entanto, após 24 h de infeção, foi possível observar, em todos os biofilmes estudados, um aumento de células viáveis podendo este facto estar relacionado com a aquisição de resistência das bactérias aos respetivos fagos. A técnica de LNA FISH demostrou ser eficiente na discriminação clara das populações de biofilme misto, sendo um complemento na deteção / compreensão na interação dos fagos com os seus hospedeiros em estruturas complexas como os biofilmes. Em suma, este trabalho demonstrou o valor potencial dos fagos no controlo de biofilmes, tornando-se no entanto necessário progredir na presente investigação, para se definirem as condições ótimas de atuação dos fagos e as razões da diminuição da eficácia dos fagos quando em contacto com o biofilme por períodos superiores a 6 h., Pseudomonas aeruginosa and Acinetobacter baumannii are two pathogenic gram-negative bacteria that are highly prevalent in hospital infections due to their natural ability to adhere to surfaces and form biofilms. Cells in biofilms are more tolerant to antimicrobial agents and this enhances serious infections in patients. Phage therapy has been reported as an alternative strategy for biofilm control but the interaction of phages with the respective bacteria in mixed biofilm in poorly understood. The work described herein focuses on the effect of phages in single species of A. baumannii biofilms and in mixed biofilms formed by A. baumannii AB1 and P. aeruginosa PAO1. To this end, methods were developed for the differentiation of the two species in biofilm using the FISH (Fluorescent in situ hibridization) technique. For multiplex detection of bacteria and phage probes based on a novel nucleic acid LNA (Locked Nucleic Acid) were designed "in-silic", but only the probe specific for the bacteria P. aeruginosa was synthesized and applied to the study. The LNA FISH method was optimized, and laboratory testing on representative strains from the Pseudomonas genus subspecies confirmed the predicted theoretical values of specificity and sensitivity. Phage infection of A. baumannii single biofilms shows that phages were efficient even when a ratio of 1 phage to 1 host cell is used. Indeed, there was a reduction of 2 log in viable cells of 24 h biofilms, after 6 h of contact with the phage. Furthermore, the efficiency of phage was significantly higher against biofilm scraped cells indicating that the biofilm matrix is hindering the efficient performance of the phage. The composition of mixed biofilms revealed a predominance of P. aeruginosa compared to that of A. baumannii... The application of a phage cocktail resulted in a reduction of 2 log of P. aeruginosa and 1,5 log of A. baumannii after 6 h. Nevertheless, after 24 hours of infection it was observed the increase of viable cells in all biofilms studied. This result can be explained by the acquisition of resistance of the bacterial hosts towards the respective phages. The LNA FISH technique demonstrated to be efficient in the clear discrimination of mixed biofilm populations, being a complement the detection / understanding the interaction of phages with their hosts in complex structures such as biofilms. Briefly, this study demonstrated the potential use of phages for biofilm control, however become necessary to progress in this research, to define the optimal conditions of phage infections and the reasons for the decreased effectiveness of phages when in contact with the biofilm for periods longer than 6 h.

The impact of pig slurry and poultry litter fertilization on soils depends on the conditions of use and the amounts applied. This study evaluated the effect of organic fertilizers after different application periods in different areas on the physical properties and organic carbon contents of a Rhodic Kandiudox, in Concordia, Santa Catarina, in Southern Brazil. The treatments consisted of different land uses and periods of pig and poultry litter fertilization: silage maize (M7 years), silage maize (M20 years), annual ryegrass pasture (P3 years), annual ryegrass pasture (P15 years), perennial pasture (PP20 years), yerba mate tea (Mt20 years), native forest (NF), and native pasture without manure application (P0). The 0-5, 5-10 and 10-20 cm soil layers were sampled and analyzed for total organic carbon, total nitrogen and soil physical properties such as density, porosity, aggregation, degree of flocculation, and penetration resistance. The organic carbon levels in the cultivated areas treated with organic fertilizer were even lower than in native forest soil. The organic fertilizers and studied management systems reduced the flocculation degree of the clay particles, and low macroporosity was observed in some areas. Despite these changes, a good soil physical structure was maintained, e.g., soil density and resistance to penetration were below the critical limits, whereas aggregate stability was high, which is important to reduce water erosion in these areas with rugged terrain in western Santa Catarina, used for pig and poultry farming. Neste estudo, avaliou-se o efeito da aplicação de fertilizantes orgânicos em diferentes áreas e tempos de aplicação sobre atributos físicos e teores de carbono orgânico de um Nitossolo Vermelho eutroférrico, em Concórdia, SC. Os tratamentos abrangeram diferentes usos do solo com aplicação de dejetos de suínos e cama de aves, a saber: milho para silagem (M7 anos), milho para silagem (M20 anos), pastagem de azevém (P3 anos), pastagem de azevém (P15anos), pastagem perene (PP20 anos), erva-mate (Mt20 anos), mata nativa (NF) e pastagem nativa sem aplicação de dejetos (P0 anos). As amostras de solo foram coletadas nas camadas de 0-5, 5-10 e 10-20 cm e analisadas em relação aos teores de carbono orgânico total e nitrogênio total e às propriedades físicas do solo, como densidade, porosidade, agregação, grau de floculação e resistência à penetração. Os sistemas agrícolas avaliados, mesmo recebendo aplicação de fertilizantes orgânicos, apresentaram menor teor de carbono orgânico em relação ao solo sob mata nativa. O uso de fertilizantes orgânicos e os sistemas de manejo adotados reduziram o grau de floculação das argilas e foi observada baixa macroporosidade em algumas áreas. Apesar disso, observaram-se boas condições físicas ligadas à estrutura do solo, como densidade do solo e resistência à penetração abaixo dos valores críticos e alta estabilidade de agregados, o que é importante para reduzir a erosão hídrica nessas áreas de cultivo associadas à criação de suínos e aves em regiões de relevo acidentado do oeste catarinense.

Introdução: O fenômeno do olho do “sol poente” é um sinal oftalmológico em que os olhos se apresentam dirigidos para baixo, deixando a esclera bem visível, entre a pálpebra superior e a íris, geralmente está associado com hipertensão intracraniana e hidrocefalia na infância. Entretanto, pode ser decorrente de patologias benignas, representadas pela imaturidade dos sistemas de controle reflexo dos movimentos oculares. Pacientes e métodos: Trabalho retrospectivo e descritivo. Foram estudadas 25 crianças de ambos os gêneros, portadoras de hidrocefalia, que foram diagnosticadas através de exames de neuroimagem e tratamento cirúrgico. Resultados: O gênero masculino esteve presente em 16 casos e o feminino em 9 casos. As causas de hidrocefalia foram: estenose do aqueduto de Silvius 14 casos, pós-mielomeningocele lombossacra 8 casos e toxoplasmose congênita 3 casos. Todos foram submetidos a exame de tomografia computadorizada e seis a ressonância magnética. Dos pacientes, 12 apresentavam o fenômeno do “sol poente”. Todos foram submetidos à derivação ventriculoperitoneal. Houve reintervenção cirúrgica em sete casos; dois pacientes foram a óbito. Conclusão: O fenômeno do “sol poente” é um sinal de alerta que indica lesão cerebral grave subjacente. A patogênese deste sinal não está bem esclarecida; acredita-se estar relacionado à distensão do aqueduto cerebral, com compressão de estruturas periaquedutais, secundária ao aumento da pressão intracraniana. Pode estar associado com hidrocefalia e disfunção do sistema de derivação ventriculoperitoneal. O diagnóstico precoce, com a realização de exames de neuroimagem, proporciona uma intervenção cirúrgica imediata e evita complicações neurológicas graves._________________________________________________________________________________________ ABSTRACT: Introduction: The setting-sun phenomenon is an ophthalmologic sign in young children, resulting from upward-gaze paresis. In this condition, the eyes appear driven downward and the sclera may be seen between the upper eyelid and the iris. This sign is usually associated with intracranial hypertension and hydrocephalus in infancy. However, it may be a consequence of benign diseases and represent immaturity of the reflex systems controlling eye movements. Patients and methods: In a retrospective and descriptive study we analyzed 25 children of both genders, all of them with congenital hydrocephalus, that were diagnosed by neuroimaging and surgical treatment. Results: Male: female ratio 16:9. The causes of hydrocephalus were Silvius aqueduct stenosis (14), lumbosacral myelomeningocele (8) and congenital toxoplasmosis (3). All patients were submitted to computorized tomography and six to magnetic ressonance. Twelve of them showed signs of setting sun eye phenomenom. All patients underwent ventriculoperitoneal shunt. There was surgical reintervention in seven cases and three patients died. Conclusions: The phenomenon of the setting sun is a warning sign that indicates severe underlying brain injury. The pathogenesis of this sign is not well understood, but it seems to be related to aqueductal distention, with compression of periaqueductal structures secondary to increased intracranial pressure. It may be associated with hydrocephalus and dysfunction of ventriculoperitoneal shunt. Early diagnosis with the performance of neuroimaging, provides an immediate surgical intervention and prevents severe neurological complications.

O manejo adequado do solo é uma das bases para a sustentabilidade do sistema agrícola. O objetivo do estudo foi avaliar atributos químicos de um Cambissolo Húmico Alumínico após 12 anos em dois sistemas de preparo do solo, com rotação e sucessão de culturas. O experimento foi conduzido em Lages, SC, utilizando preparo convencional (PC) e semeadura direta (SD), sob rotação (r) e sucessão (s) de culturas. O delineamento experimental foi inteiramente casualizado com quatro repetições. As sequências de cultivo foram: feijão-pousio-milho-pousio-soja-pousio no PCr; milho-pousio no PCs; feijão-aveia-milho-nabo-soja-ervilhaca na SDr; e milho-ervilhaca na SDs. As amostras de solo foram coletadas nas camadas 0-2,5; 2,5-5; 5-10; e 10-20cm. Avaliou-se carbono orgânico total (COT), cálcio, magnésio, alumínio trocável, fósforo, potássio, nitrogênio total (NT) e pH em água. A SD aumentou os teores de COT e nutrientes em comparação ao PC, especialmente na camada superficial do solo. O cultivo de milho e ervilhaca em sucessão aumentou os teores de COT e NT na camada superficial do solo em comparação com a rotação sob semeadura direta. Suitable soil management is one of the bases for sustainability in agricultural systems. The study aimed to evaluate chemical properties of a Humic Dystrudept for 12 years under two tillage systems, with crops rotation and succession. The experiment was carried out in Lages, SC, under conventional tillage (CT) and no-till (NT), with rotation (r) and succession (s) cropping systems, using crop sequences of beans-fallow-maize-fallow-soybean in CTr; maize-fallow in CTs; beans-oats-maize-fodder radish-soybean-vetch in NTr; and maize-vetch in NTs. The experimental design was completely randomized with four replicates. The soil samples were collected in the layers 0-2.5, 2.5-5, 5-10 and 10-20cm. The variables assessed were total organic carbon (TOC), calcium, magnesium, exchangeable aluminum, phosphorus, potassium, total nitrogen (TN) and water pH. The no-tillage system increased TOC and nutrient levels in comparison with conventional tillage, especially in the surface soil layer. Maize and vetch crop succession had higher TOC and TN contents in the surface soil layer compared to crop rotation under no-tillage.

As náuseas e vômitos pós-operatórios (NVPO) são complicações que geralmente configuram o período pós-operatório de pacientes neurocirúrgicos. Sabe-se que tem caráter multifatorial, como: sexo, idade, obesidade, dor e medo. As NVPO podem levar o paciente à desidratação, alcalose metabólica, hipocalemia e aspiração pulmonar. Atualmente com a profilaxia através de potentes antieméticos, a incidência de NVPO diminuiu para 20% a 30%. Este trabalho apresenta a incidência de NVPO em 32 pacientes neurocirúrgicos, internados no Hospital de Urgência de Sergipe, referência nesta especialidade. Os autores discutem a fisiopatologia, fatores de risco, complicações e tratamento._________________________________________________________________________________________ ABSTRACT: Nausea and vomiting postoperative (PONV) are complications that often shape the postoperative neurosurgical patients. It is known that is multifactorial, such as gender, age, obesity, pain and fear. The PONV can lead the patient to dehydration, metabolic alkalosis, hypokalemia and pulmonary aspiration. Currently with prophylaxis using potent antiemetics, the incidence of PONV decreased to 20% to 30%. This paper presents the incidence of PONV in 32 neurosurgical patients, the Hospital Emergency Sergipe, reference in this specialty. The authors discuss the pathophysiology, risk factors, complications and treatment.

As fraturas isoladas do processo transverso são consideradas lesões menores. Em geral, são causadas por trauma direto ou avulsão resultante da contração muscular. Radiografias simples são pouco sensíveis para a detecção dessas fraturas, portanto a verdadeira frequência dessas fraturas pode ser subestimada. Com o advento da tomografia computadorizada (TC) helicoidal, tem aumentado à frequência dessa lesão. GS, 53 anos, masculino. Vítima de sequestro e colocado no porta-malas do carro. No setor de emergência, apresentou queixa de dores generalizadas, mais intensa na região lombar. Exame neurológico: normal. Rx simples de abdome: normal. Tomografia da coluna lombar: fratura do processo transverso esquerdo de L5. Tratamento conservador, com evolução satisfatória. Apesar de ser considerada um traumatismo leve, a fratura do processo transverso da vértebra lombar ocorre como resultado de forças intensas. Geralmente, está associado com lesão de vísceras abdominais ou geniturinárias. Seu diagnóstico tem sido feito por meio de exame de TC helicoidal. Seu tratamento inicial é conservador. Fraturas do processo transverso das vértebras lombares cada vez mais têm sido diagnosticadas. Existe associação com lesões de vísceras abdominais e geniturinárias. O tratamento inicial é conservador e cursa com bom prognóstico._________________________________________________________________________________________ ABSTRACT: Isolated fractures of the transverse process are considered minor injuries usually are caused by direct trauma or avulsion resulting from muscle contraction. Plain radiographs are insensitive for the detection of these fractures, so the true frequency of these fractures may be underestimated. With the advent of helical computed tomography, has increased the frequency of this injury. GS, age 53, male. Victim of kidnapping and placed in the trunk of the car. Na chamada era da informação a forma como utilizar e gerenciar as informações tem se tornado um dos grandes desafios para os gestores contemporâneos. A informação ratifica seu caráter estratégico, num ambiente competitivo ao proporcionar o desenvolvimento organizacional. No contexto da gestão das informações e gestão do conhecimento, os arquivos institucionais por meio da Gestão de Documentos podem subsidiar os processos que sustentam a gestão das informações e criação de novos conhecimentos. O presente trabalho tem como foco identificar como a Gestão de Documentos é percebida pelos gestores do Serviço Nacional de Aprendizagem Comercial (SENAC-DF) no contexto da Gestão da Informação e Gestão do Conhecimento. A base metodológica utilizada foi a pesquisa qualitativa, e os instrumentos utilizados na coleta de dados foram a entrevista estruturada e a análise de documentos. Para sua consolidação foi utilizada uma vasta literatura relacionada às temáticas: Gestão de Documentos, Gestão da Informação e Gestão do Conhecimento, merecendo destaque os autores: Couture e Rousseau (1998), Jardim (1988;1992); Duranti (1994), Ponjúan Dante (2004), Tarapanoff (2001;2006), Davenport e Prusak (1998), e Nonaka e Takeuchi (1997). Mediante as perspectivas teóricas, análise do estudo de caso e a percepção dos gestores é possível afirmar que a Gestão de Documentos poderia destacar-se no contexto organizacional como ferramenta de gestão que subsidia a Gestão da Informação e proporcionar a criação de novos conhecimentos. The used methodological base was the qualitative research, and the instruments used in the collection of data had been the structuralized interview and document analysis. For its consolidation a vast literature related to the thematic ones was used: Record Management, Information Management and Knowledge Management, deserving have detached the authors: Couture and Rousseau (1998), Jardim (1988; 1992); Duranti (1994), Ponjúan Dante (2004), Tarapanoff (2001), Davenport and Prusak (1998), and Nonaka and Takeuchi (1997). By means of the theoretical perspectives, analysis of the case study and the perception of the managers, are possible to affirm that the Record Management could be distinguished in the organizational context as management tool that subsidizes the Information Management and provide the creation of new knowledge. For in such a way, one becomes necessary to launch hand of the procedures and tools of the Record Management, for its consolidation in the context of the Information Management and Knowledge Management.

Os dentes escolhidos foram submetidos a tratamento endodôntico, sendo posteriormente realizada a desobstrução mediata com a broca de menor calibre do kit Reforpost, Ângelus® (1,1 mm de diâmetro com ápice medindo 0,7 mm). Eles foram fragmentados e, na porção média, foi analisada a distância entre um ponto externo ao canal e a parte simétrica no seu interior, nas faces mesial, distal, vestibular e lingual, para obtenção dos valores dos remanescentes dentários. Resultados: a média do remanescente vestibular foi de 2.400, do lingual de 2.683, do mesial 1.088 e do distal 0.922mm. A mediana foi, respectivamente, 2.400, 2.650, 1.100 e 0.925. Conclusão: Diante dos dados expostos, fica um alerta que deve motivar a realização de novas pesquisas e gerar mais cautela ao clínico na indicação dos pinos de fibra de vidro em incisivos inferiores. O hematoma subdural crônico (HSDC) é uma coleção bem delimitada entre dura-máter e aracnóide, contendo sangue fluido e coagulado em vários estágios. O presente estudo objetivou avaliar pacientes admitidos no setor de neurocirurgia do Hospital de Urgências de Sergipe (HUSE) com história clínica compatível ao HSDC. Foram analisados quanto à idade, sexo, etiologia e localização do HSDC, quadro clínico, tempo de surgimento dos sinais e sintomas, apresentação da tomografia computadorizada de crânio (TC), conduta instituída e prognóstico. Dos 40 pacientes, 38 foram do gênero masculino e dois feminino, com média 59 anos. A principal causa foi traumatismo cranioencefálico (97,5%). A TC evidenciou desvio de linha média com efeito de massa em 12 pacientes. Dos trinta e oito que foram submetidos à cirurgia, 24 casos foram liberados por alta médica sem déficit e/ou intercorrências (63%), 12 desenvolveram complicações no pós-operatório (32%) e dois tiveram alta com déficit neurológico (5%). Concluiu-se que as complicações pós-cirúrgicas afetam 32% dos pacientes submetidos a craniotomias descompressivas, sendo responsável pelo aumento da morbi-mortalidade destes pacientes._________________________________________________________________________________________ ABSTRACT: The chronic subdural hematoma (HSDC) is a well-defined collection between dura and arachnoid, containing fluid and clotted blood in various stages. This study aimed to evaluate patients admitted to the sector of neurosurgery at the Hospital Emergency Sergipe (Huse) clinical history consistent with the HSDC. Were analyzed for age, sex, etiology and location of the HSDC, clinical presentation, time of onset of signs and symptoms, presentation of computed tomography (CT), conduct established and prognosis. Of the 40 patients, 38 were males and two females, mean age 59. The main cause was head trauma (97.5%). A CT scan showed midline shift with mass effect in 12 patients. Of the thirty-eight who underwent surgery, 24 cases were released on medical discharge without deficit and / or complications (63%), 12 developed postoperative complications (32%) and two were discharged with neurological deficit (5%). It was concluded that postoperative complications affect 32% of patients undergoing decompressive craniotomy, being responsible for increased morbidity and mortality.

La presente investigación tuvo como objetivo analizar los efectos directos e indirectos del apoyo social, la soledad y la vulnerabilidad percibida en la satisfacción de vida (SV) de dos grupos de personas mayores en México, uno de 60 a 69 años de edad y el otro de 70 a 99. Se contó con una muestra no probabilística de 4.433 personas mayores, 61.8 % fueron mujeres y 38.2 % hombres. Se utilizaron los siguientes instrumentos: la escala de soledad de la Universidad de California; la escala de Red Social de Lubben (LSNS-6) para evaluar el apoyo social percibido; la Escala de Vulnerabilidad Percibida de Myall et al.; la escala de Satisfacción con la Vida de Diener et al., así como un cuestionario sociodemográfico. Los resultados mostraron un buen ajuste de los modelos, en los que se aprecia que para ambos grupos es mayor la varianza explicada de SV al relacionar apoyo social con soledad y vulnerabilidad, que si se mide el efecto directo del apoyo sobre SV. Esta investigación aporta elementos para el desarrollo de programas y políticas públicas que contribuyan al bienestar de personas mayores.

298. Antibodies to ribosomal P proteins in lupus nephritis: a surrogate marker for a better renal survival?

Objective To define if antibodies to ribosomal P proteins disclose a better lupus nephritis long-term survival. Methods Sixty consecutive SLE patients with biopsy-proven nephritis (2004 ISN/RPS) were evaluated for renal survival parameters. Inclusion criteria were at least one serum sample at: renal flares, biopsy, and last follow-up until 2008. Anti-P was detected by ELISA/immunoblot and anti-dsDNA by indirect immunofluorescence/ELISA. Results Eleven patients (18%) with anti-P+ (without anti-dsDNA) during renal flare were compared to 49 (82%) persistently negative for anti-P throughout the study. At the final follow-up post-biopsy (6.3 ± 2.5 vs. 6.8 ± 2.4 years, p = 0.36), the comparison of anti-P+/anti-dsDNA− with anti-P− group revealed a trend to lower mean creatinine levels (0.9 ± 0.3 vs. 2.3 ± 2.1 mg/dl, p = 0.07), lower frequency of dialysis (0% vs. 35%, p = 0.025), and higher frequency of normal renal function (91% vs. 53%, p = 0.037). The overall renal survival was significantly higher in anti-P+/anti-dsDNA− compared to anti-P− (11.0 ± 4.5 vs. 9.2 ± 4.5 years, p = 0.033), anti-dsDNA+/anti-P− (vs. 8.7 ± 4.7 years, p = 0.017), and anti-P−/anti-dsDNA− (vs. 9.8 ± 4.3 years, p = 0.09) groups. Conclusion Our data supports the notion that anti-P antibody in the absence of anti-dsDNA during nephritis flares is a valuable marker to predict a better long-term renal outcome in lupus patients.

The aim of this study is to compare the macro- and microsurgery techniques for root coverage using a coronally positioned flap (CPF) associated with enamel matrix derivative (EMD).Thirty patients were selected for the treatment of localized gingival recessions (GRs) using CPF associated to EMD. Fifteen patients were randomly assigned to the test group (TG), and 15 patients were randomly assigned to the control group (CG). The microsurgical approach was performed in the TG, and the conventional macrosurgical technique was performed in the CG. The clinical parameters evaluated before surgery and after 6 months were GR, probing depth, relative clinical attachment level, width of keratinized tissue (WKT), and thickness of keratinized tissue (TKT). The discomfort evaluation was performed 1 week postoperative.There were no statistically significant differences between groups for all parameters at baseline. At 6 months, there was no statistically significant difference between the techniques in achieving root coverage. The percentage of root coverage was 92% and 83% for TG and CG, respectively. After 6 months, there was a statistically significant increase of WKT and TKT in TG only. Both procedures were well tolerated by all patients.The macro- and microsurgery techniques provided a statistically significant reduction in GR height. After 6 months, there was no statistically significant difference between the techniques regarding root coverage, and the microsurgical technique demonstrated a statistically significant increase in WKT and TKT.
