Abstract

1. Introduction Heart transplantation is a common surgical procedure in humans. However, acute rejection and graft arterial diseases (GADs), which are a phenomenon of chronic rejection, have not yet been [...], Heart transplantation has been broadly performed in humans. However, occurrence of acute and chronic rejection has not yet been resolved. Several inflammatory factors, such as cytokines and adhesion molecules, enhance the rejection. The graft arterial disease (GAD), which is a type of chronic rejection, is characterized by intimal thickening comprised of proliferative smooth muscle cells. Specific treatments that target the attenuation of acute rejection and GAD formation have not been well studied in cardiac transplantation. Recent progress in the nucleic acid drugs, such as antisense oligodeoxynucleotides (ODNs) to regulate the transcription of disease-related genes, has important roles in therapeutic applications. Transfection of cis-element double-stranded DNA, named as 'decoy,' has been also reported to be a useful nucleic acid drug. This decoy strategy has been not only a useful method for the experimental studies of gene regulation but also a novel clinical strategy. Abstract

Coronary artery disease and peripheral arterial disease are devastating status of acute vessel occlusion in diseased vessels that are already narrowed enough by atherosclerotic process. People are now focused on therapeutic angiogenesis against the ischemic diseases, to supply and growth of new vessels into the ischemic tissue. Recently, we and others performed autologous transplantation of bone marrow mononuclear cell or endothelial progenitor cell and gene therapy using hepatocyte growth factor (HGF) in clinical trials. Autologous implantation of bone marrow mononuclear cells or endothelial progenitor cells in ischemic limb or heart may induce blood flow and improve function in several pilot clinical studies. The regenerative potential of bone-marrow cells can be explained by any of four mechanisms, transdiffentiaion, cytokine-induced growth, stimulation of endogenous stem cells, and induction of cell fusion. Transdifferentiaion has been described by previous investigators and the concept has been called into question by recent experimental studies. In the pilot clinical study, similar improvements were seen in gene therapy using HGF in peripheral arterial diseases which might be more noninvasive. The clinical significance of the novel therapeutic approach might embrace the large number of patients with peripheral arterial diseases and with chronic coronary artery disease. And we need to figure out the strong angiogenic factor or specific differentiated progenitor cells for therapeutic angiogenesis in the study of bone marrow cell therapy. For the patients, we need to constitute a safe and effective strategy for achievement of therapeutic angiogenesis.

Abstract

Although Lp(a) have been thought to be a cardiovascular risk factor, it is unclear whether lowering Lp(a) levels reduces the risk of cardiovascular diseases. No pharmacological agents which selectively reduce serum Lp(a) levels, and Lp(a) is present in primate but absent in common laboratory animals such as mice and pigs. In the present study we used transgenic mice of human Lp(a) and tested effect a novel Lp(a) lowering drug D-47 on neointima formation after vascular injury. D-47 successfully decreased plasma levels of Lp(a) and possibly inhibited neointima formation in Lp(a) transgenic mice. The results indicate that we can modulate plasma Lp(a) levels by pharmacologic agents and inhibit atherogenic properties of Lp(a) by reducing plasma levels of Lp(a).
