Abstract

We present a case of sclerosing lipogranuloma of the scrotum. A 42-year-old man who had complained of a painless intrascrotal mass of one week's duration visited our clinic. Physical examination revealed a symmetrical Y-shaped hard mass around the penile base in the scrotum. Laboratory examination showed no abnormal findings except for a 14.0% eosinophilia. Magnetic resonance imaging (MRI) with a surface coil showed an atypical mass at the penile base. Percutaneous needle biopsy under ultrasound guidance was performed and histopathological findings revealed lipogranuloma accompanied by infiltrating multinuclear giant cells, eosinocytes and lymphocytes. The tumor gradually disappeared during a 4-week administration of antibiotics and antiphlogistics. We have also reviewed 95 cases of sclerosing lipogranuloma of the scrotum in the Japanese literature.

Abstract

We report a case of adrenal ganglioneuroma. A 33-year-old woman visited our clinic with a complaint of epigastralgia. Abdominal computed tomography (CT) and magnetic resonance imaging (MRI) revealed a round mass, 4 cm in diameter, in the left suprarenal fossa. Laboratory data including endocrinological study were normal except for a mild increase of urinary excretion of dopamine. Under the diagnosis of left non-functioning adrenal tumor, surgery was performed on August 9, 1996 through flank incision. The excised tumor was well-demarcated and weighed 36.6 g. The cut surface revealed an ash-colored round tumor surrounded by thin stretched adrenal tissue. Histological diagnosis of the tumor was a ganglioneuroma originating from the adrenal medulla.

Abstract

Two cases on non-Hodgkin's malignant lymphoma (NHL) of the testis are reported. A 63-year-old man with left painless scrotal swelling underwent orchiectomy. Although prophylactic irradiation was performed under the diagnosis of seminoma, subsequent immunohistochemistry revealed NHL, large diffuse cell type (B type). He had stage IEA disease. Chemotherapy with cyclophosphamide, adriamycin vincristine and prednisolone was performed. The second case was in a 63-year old man with right painless scrotal swelling. Orchiectomy revealed NHL, diffuse medium cell type (B type). Because of tumor in the retrosternum, he had stage IIIEA disease. Chemotherapy and irradiation to the contralateral testis was performed.

Abstract

Two cases of giant hydrocele, having more than 1, 000 ml of contents, are presented. The first patient was a 77-year-old man with the complaint of urination difficulty and a very swollen left scrotum of three years duration. Mild prostatic hypertrophy, mild renal dysfunction and hypertension were seen during hospitalization. A scrotal puncture gave 2, 100 ml of serous fluid. Radical hydrocelectomy and plastic operation for surplus scrotal skin were done, and difficult urination improved markedly. The second patient was a 77-year-old man with the complaint of right scrotal swelling of fifteen years duration. He had developed pollakisuria and urination difficulty three years previously. When he was transferred our hospital on emergency due to engorgement, the diagnosis of giant hydrocele was made. Right orchidectomy was performed because of marked testicular atrophy and old hemorrhagic fluid of 1, 200 ml. Urination difficulty and pollakisuria disappeared postoperatively.

Abstract

A 64-year-old female with hypertension, hypokalemia visited our hospital. Endocrinological examinations showed a low level of plasma renin activity and high level of plasma aldosterone. Circadian rhythmicity of plasma aldosterone level was recognized. No change in the plasma level of aldosterone was observed after loading of standing and administration of furosemide. Adrenal scintigraphy, adrenal venous aldosterone assay and CT scan revealed two tumors in the left adrenal. The diagnosis of primary aldosteronism by left adrenal tumors was made from the above findings. A left adrenalectomy was performed and pathological findings showed two adenomas, which had no capsule either and were surrounded by normal adrenocortical tissue. Blood pressure normalized after surgery and the plasma levels of aldosterone and plasma renin activity were normalized.

Abstract

Non-alcoholic steatohepatitis (NASH) is the progressive phenotype of non-alcoholic fatty liver disease associated with the metabolic syndrome. The existence of autoimmune features in NASH has been reported, but its significance remains unclear. We herein report the autoantibody profile of 54 patients with histologically proven NASH and further determined the development of autoimmunity in three different murine NASH models (monosodium glutamate, CDAA (choline-deficient l-amino acid-defined), and TSOD (Tsumura Suzuki, Obese Diabetes)) at 48 weeks of age. Forty-eight percent (26/54) of NASH cases were positive for antinuclear (ANA) or antimitochondrial antibody and manifested histological signs of overlap with autoimmune hepatitis and primary biliary cirrhosis, respectively. These patients were significantly older (60 ± 10 versus 50 ± 16 years), more frequently women (81 % versus 43 %), and with more severe portal inflammatory infiltrate compared with patients without autoimmunity. In one third of mice, regardless of the model, we observed a marked lymphoid infiltrate with non-suppurative cholangitis, and several cases were ANA-positive, but none AMA-positive. Our data suggest that autoimmunity may share some pathogenetic traits with the chronic inflammation of NASH, possibly related to advanced age. [ABSTRACT FROM AUTHOR]

Abstract

BLUF (sensor of blue light using FAD) domain-containing proteins are one of three types of flavin-binding, blue-light-sensing proteins found in many bacteria and some algae. The other types of blue-light-sensing proteins are the cryptochromes and the light, oxygen, voltage (LOV) domain-containing proteins. BLUF proteins control a wide variety of light-dependent physiological activities including photosystem synthesis, biofilm formation and the photoavoidance response. The BLUF domain photochemical reaction is unique in that only small chromophore structural changes are involved in the light activation process, because the rigid flavin moiety is involved, rather than an isomerizable chromophore (e.g. phytochromobilin in phytochromes and retinal in rhodopsins). Recent spectroscopic, biochemical and structural studies have begun to elucidate how BLUF domains transmit the light-induced signal and identify related, subsequent changes in the domain structures. Abstract

Jasmonates are phytohormones derived from oxygenated fatty acids that regulate a broad range of plant defense and developmental processes. In Arabidopsis, hypocotyl elongation under various light conditions was suppressed by exogenously supplied methyl jasmonate (MeJA). Moreover, this suppression by MeJA was particularly effective under red light condition. Mutant analyses suggested that SCF-mediated proteolysis was involved in this function. However, MeJA action still remained in the coi1 mutant, and (+)-7-iso-JA-L-Ile, a well-known active form of jasmonate, had a weaker effect than MeJA under the red light condition, suggesting that unknown signaling pathway are present in MeJA-mediated inhibition of hypocotyl elongation. EMS mutant screening identified two MeJA-insensitive hypocotyl elongation mutants, jasmonate resistance long hypocotyl 1 ( jal1) and jal36, which had mutations in the phytochrome B ( PHYB) gene.
