252. The preperitoneal (posterior) approach to the repair of recurrent inguinal hernia.

Abstract

The preperitoneal approach to inguinal hernia permits direct access to the innermost aponeuroticofascial layer of the abdominal wall without disturbing the structures in and around the inguinal canal. It is also of exceptional value in the treatment of difficult hernias, specially recurrent hernia. We present here our experience with this approach in the treatment of recurrent hernia. 31 patients were operated for recurrent hernia. A preperitoneal approach with the use of a prosthesis was used in all of them. The mean surgical time was 45 min. The postoperative course was uneventful in 27 patients, 3 patients had urinary retention and one patient (under anticoagulant therapy) had a scrotal hematoma. No infections or mesh rejections were detected. At follow up (from 6 months to 4 years) no recurrence has been found. [ABSTRACT FROM AUTHOR]

Utilizaram-se 14 caprinos, machos, sem raça definida, com idade oscilando entre quatro e seis meses. Os animais foram divididos em dois grupos de sete, permanecendo um grupo semi-confinado em piquetes e outro confinado em baia, recebendo ambos os lotes a mesma alimentação. Após cinco meses de observação, sacrificaram-se três animais de cada grupo, coletando-se amostras de fígado e pâncreas para dosagem dos minerais mencionados. Depois fez-se a inversão dos tratamentos com o restante dos animais, passando os confinados a soltos e estes a confinados, por mais dois meses, quando foram sacrificados e necropsiados. Desses, foram coletados novamente, amostras de fígado e pâncreas, para análise. Não houve diferença estatisticamente significativa, entre os dois grupos, nos níveis dos elementos estudados. This work was performed in order to investigate the levels of copper, iron, manganese and zinc in liver and pancreas goats under confined and semi-confined conditions. Fourteen male goats of from four to six months old and in gool health conditions were used. The animals were divided in two groups of seven. One group was maintained in semi-confined conditions with pasture, while the other group was confined in a stall. Both groups received the same diet. After a period of five months three animals of each group were slaughtered and liver and pancreas samples were collected in order to search for those minerals. During the subsequent two months the remaining animals were swop from their original groups and then slaughtered. Again samples of liver and pancreas were collected for analysis. Statistically, there was no significant difference between the two groups on the studied elements. Os animais confinados apresentaram valores mais elevados para ambos os eletrólitos. As médias de sódio variaram entre 134,85 a 140,28 mEq/l e as de potássio, entre 4,25 a 5,17 mEq/l. The serum levels of sodium and potassium of two groups of goats under confinement and semi-extensive condition were compared in this work. The confined animals showed higher values for both electrolites. The means for sodium ranged from 134,85 to 140,28 mEq/l and the means for potassium ranged from 4,25 to 5,17 mEq/l. Abstract

This work aims to analyse the response of epilithic diatom communities to enviranmental gradients in subtropical temperate southern Brazilian rivers to contribute to the development of a widely applicable methodology for water-quality monitoring. Samples for physical, chemical and biological determination were collected monthly, fram December 2001 to November 2002 and fram March 2003 to February 2004, at 9 stations along the rivers Pardo and Pardinho in the hydrographical basin of Rio Pardo, State of Rio Grande do Sul (RS), Brazil. Physical and chemical variables (water temperature, pH, dissolved oxygen, biochemical oxygen demand, chemical oxygen demand, nitrates, nitrites, phosphates and total dissolved solids) and biological variables (epilithic diatom communities) were used as parameters for water-quality assessment. The data matrix was examined by means of a multivariate ordination using Detrended Correspondence Analysis (DCA). In total, 270 taxa were identified to specific or infra-specific levels. The results showed that of all the physical and chemical variables used, the main gradient along the DCA first axis was eutraphication, which was indicated by its significant correlation with phosphates (p < 0.00l). Hence, the species' scores on the DCA axis were used as an operational criterion for indicating their tolerance to eutrophication. From this analysis, the 10 taxa that were the most tolerant to eutrophication were: Cyclotella meneghiniana, Fallacia monoculata, Nitzschia acicularis, N. clausii, N. nana, N. palea, Nitzschia sp., Pinnularia sp., Sellaphora pupula sensu lato and Ulnaria acuso The present study showed that epilithic diatom assemblages reflect anthropogenic changes in hydrographic basins in subtropical temperate streams, especially pollution by organic enrichment and eutraphication. Este trabajo pretende analizar la respuesta de la comunidad de diatomeas epilíticas a gradientes ambientales en ríos brasileños templados subtropicales, con el objetivo de contribuir al desarrollo de una metodología ampliamente aplicable para la monitorización de la calidad del agua. Diversas muestras para determinaciones físicas, químicas y biológicas fueron recogidas mensualmente, de diciembre de 2001 a noviembre de 2002, y de marzo de 2003 afebrero de 2004, en nueve esta­ciones de muestre o distribuidas a lo largo de los ríos Pardo y Pardinho, localizados en la Cuenca Hidrográfica del Río Pardo, Estado del Río Grande del Sur; Brasil. Variables físicas y químicas (temperatura del agua, pH, oxígeno disuelto, demanda bioquímica de oxígeno, demanda química de oxígeno, nitratos, nitritos, fosfato y sólidos totales disueltos) y biológicas (co­munidad de diatomeas epilíticas) fueron usadas como parámetros para evaluar la calidad del agua. El conjunto de datos fue analizado por medio de una ordenación multivariante usando el Análisis de Correspondencias sin Tendencias (DCA). En total, 270 taxones fueron identificados a nivel especifico o infra-especifico. Los resultados indicaron que, entre todas las variables físicas y químicas usadas, el principal gradiente establecido a lo largo del primer eje DCA fue la eutrojización, señalada por su correlación significativa con la concentración de fosfatos (p < 0.001). De esta forma, los valores alcanzados por las especies en el primer eje DCA fueron usados como un criterio operativo para indicar su tolerancia a la eutrojización. 262. Incidence and lifetime risk of dementia and Alzheimer's disease in a Southern European population

Abstract

To calculate both the incidence rates and the lifetime risk (LTR) of dementia and Alzheimer's disease (AD).A two-phase case-finding procedure was implemented in a cohort of 4057 cognitively intact individuals 55+ years of age living in Zaragoza, Spain, and followed-up at 2.5 and 4.5 years. Age- and sex-specific incidence rates were calculated. A mortality-adjusted, multivariate model was used to document LTRs.The incidence rate of dementia continued to rise after the age of 90 years, but was slightly lower than in North and West European studies. Only a tendency for an increased LTR with age was observed. Thus, LTR was 19.7% for a 65-year-old woman and 20.4% at the age of 85 years, the corresponding figures for AD being 16.7% and 17.6%. The LTR of AD was higher in women and was about twice as high among illiterate individuals when compared with individuals with higher educational levels.The incidence rate of dementia in this Southern European city was slightly lower than in previous studies in North-West Europe. LTR of dementia and AD seems to be slightly increased with age. The association of illiteracy with higher LTR of AD is intriguing.

Abstract

Background. To test the hypothesis that cognitive impairment in older adults is associated with all-cause mortality risk and the risk increases when the degree of cognitive impairment augments; and then, if this association is confirmed, to report the population-attributable fraction (PAF) of mortality due to cognitive impairment. Method. A representative random community sample of individuals aged over 55 was interviewed, and 4557 subjects remaining alive at the end of the first year of follow-up were included in the analysis. Instruments used in the assessment included the Mini-Mental Status Examination (MMSE), the History and Aetiology Schedule (HAS) and the Geriatric Mental State (GMS)-AGECAT. For the standardised degree of cognitive impairment Perneczky et al's MMSE criteria were applied. Mortality information was obtained from the official population registry. Multivariate Cox proportional hazard models were used to test the association between MMSE degrees of cognitive impairment and mortality risk. We also estimated the PAF of mortality due to specific MMSE stages. Results. Cognitive impairment was associated with mortality risk, the risk increasing in parallel with the degree of cognitive impairment (Hazard ratio, HR: 1.18 in the ‘mild' degree of impairment; HR: 1.29 in the ‘moderate' degree; and HR: 2.08 in the ‘severe' degree). The PAF of mortality due to severe cognitive impairment was 3.49%. Conclusions. A gradient of increased mortality-risk associated with severity of cognitive impairment was observed. The results support the claim that routine assessment of cognitive function in older adults should be considered in clinical practice

Abstract

Background: While several drugs are commercially available as the pamoate salts, information on the systemic exposure of pamoic acid (PA) from such drug products is not available in the literature. This study was conducted to determine the pharmacokinetics of PA from a marketed formulation, hydroxyzine pamoate.Methods: The study was conducted as open-label in 6 healthy Caucasian male subjects (25 to 37 years). Each subject received 100 mg hydroxyzine pamoate once on Day 1 and every 6 hours from Days 2 to 4 (9 oral doses). Blood samples were obtained on Days 1 and 4 at 0, 1, 2, 3, 4, 5, 6, 7, 8, 12, and 24 hr after the dose. PA concentrations were measured using a validated HPLC method with fluorescence detection and analyzed using non-compartmental method.Results: Plasma concentration-time profile of PA showed biphasic elimination. PA was rapidly absorbed with time to maximal concentration of 3 hr on Day 1 and 3.5 hr on Day 4. Mean apparent half-life on Days 1 and 4 were 5.7 and 7.0 hr, respectively. Upon multiple dosing, oral clearance ranged from 12 to 115 L/hr and oral volume of distribution ranged from 99 to 1200 L. Maximum average steady state concentrations of PA were 715 ng/mL.Conclusions: PA is rapidly absorbed and eliminated from the systemic circulation on oral administration of hydroxyzine pamoate. The concentrations of PA reported in this study provide information on the systemic exposure of PA. Abstract

Objectives: To investigate whether clinically relevant anxiety increased the risk for developing Alzheimer's disease (AD) while controlling for the presence of depression and other confounders; and to report the population attributable fraction (PAF) associated with anxiety disorder.Method: We used data from the longitudinal, community-based Zaragoza Dementia and Depression (ZARADEMP) study. A random sample of 4057 dementia-free community dwellers aged ≥55 years were followed for 4.5 years. The Geriatric Mental State-Automated Geriatric Examination for Computer Assisted Taxonomy package was used for the diagnosis of clinically significant cases and subcases of anxiety; and AD was diagnosed by a panel of research psychiatrists according to DSM-IV criteria. Multivariate survival analysis with competing risk regression model was performed.Results: We observed a significant association between anxiety cases at baseline and AD risk in the univariate analysis that persisted in the fully adjusted model (SHR: 3.90; 95% CI: 1.59-9.60; p = 0.003), with a PAF for AD of 6.11% (95% CI: 1.30%-16.17%). No significant association between 'subcases' of anxiety at baseline and AD risk was found.Limitations: Data on apolipoprotein E were not available. The hospital-based diagnosis was not completed in all cases of dementia.Conclusion: Late-life, clinically significant anxiety (but not subclinical anxiety) seems to increase the risk of AD, independently of the effect of several confounders, including depression. Taking into account the high prevalence of anxiety among the elderly, future studies are warranted to determine potential risk reduction of AD. [ABSTRACT FROM AUTHOR]

Abstract

Esta disserta??o busca identificar a rela??o de causalidade entre o crescimento do produto interno bruto, varia??o populacional, qualifica??o de m?o de obra e complexidade produtiva no desenvolvimento das regi?es brasileiras. A pesquisa utilizou bases de dados do Minist?rio do Trabalho, Instituto Brasileiro de Geografia e Estat?stica e Instituto Nacional de Estudos e Pesquisas Educacionais. Os dados foram organizados e apresentados em formato de pain?is e mapas cartogr?ficos, permitindo observar quais microrregi?es tiveram maior varia??o entre os anos de 2009 e 2019. Foi criado um modelo de regress?o linear m?ltipla para explicar as varia??es observadas em cada uma das vari?veis. Os resultados apresentaram rela??es de causalidade entre as vari?veis e destacam a influ?ncia de regi?es com alto PIB per capita no crescimento populacional e aumento da qualifica??o de m?o de obra. Ainda, a m?o de obra qualificada exerce influ?ncia sobre a complexidade produtiva nas microrregi?es. Este estudo analisou dados p?blicos de car?ter quantitativo, sem destacar nenhuma influ?ncia espec?fica de pol?ticas p?blicas e incentivos fiscais. This dissertation seeks to identify the causal relationship between the growth of gross domestic product, population variation, labor qualification and productive complexity in the development of Brazilian regions. The research used databases from the Ministry of Labor, the Brazilian Institute of Geography and Statistics and the National Institute of Educational Studies and Research. The data were organized and presented in the form of panels and cartographic maps, allowing us to observe which microregions had the greatest variation between 2009 and 2019. A multiple linear regression model was created to explain the variations observed in each of the variables. The results showed causal relationships between the variables and highlight the influence of regions with high GDP per capita on population growth and increase in the qualification of the workforce. Also, the skilled labor influences the productive complexity in the microregions. This study analyzed public data of a quantitative nature, without highlighting any specific influence of public policies and tax incentives. Abstract

Objective In a background of revision of criteria for states of increased risk for progression to dementia, we compare the conversion rate to dementia and Alzheimer's disease ( AD) of mild cognitive impairment ( MCI) as diagnosed using DSM-5 ( DSM-5- MCI) and Petersen's (P- MCI) criteria. Method A population representative cohort of 4057 dementia-free individuals 55+ years of age was followed up at 2.5 and 4.5 years in Zaragoza, Spain ( ZARADEMP). Using the Geriatric Mental State- AGECAT for assessment, research psychiatrists diagnosed DSM-5- MCI and P- MCI following operationalized criteria. 'Conversion rate' ( CR), 'annual conversion rate' ( ACR), and incidence rate ( IR) were calculated along with incidence rate ratio ( IRR) to compare the performance of the intermediate cognitive definitions. Results At 4.5-year follow-up, in individuals aged 65+ years, ACRs for non-cases, P- MCI, and DSM-5- MCI were 0.8, 1.9 and 3.4, respectively, for global dementia. The IRRs were 2.9 and 5.3 for P- MCI and DSM5- MCI, respectively, being the non-cases the reference category. The corresponding values were slightly lower for AD. Conclusion Conversion rate to dementia and AD was higher using DSM-5- MCI criteria than using Petersen's criteria. However, prediction of the construct still has some way to go, as most MCI individuals did not convert at 4.5-year follow-up. [ABSTRACT FROM AUTHOR]

Abstract

Purpose To describe the organisation of the registry and the preliminary results in terms of characteristics of high-risk pancreatic ductal adenocarcinoma (PDAC) families recruited to date and findings of the screening programme. To compare early onset sporadic cases (⩽50 years), sporadic cases (>50 years) and cases with family history of cancer, for PDAC possible risk factors. Methods/patients Families with hereditary cancer syndromes predisposing to PDAC were recruited from two main sources: Spanish hospitals participating in PanGenEU, a pan-European multicentre case–control study, and their genetic counseling unit. Individuals at high-risk of PDAC were enrolled into a screening programme, consisting of Endoscopic ultrasound, computerised tomography, magnetic resonance imaging. Genetic testing of candidate genes was offered according to each patient’s risk. Results Among 577 consecutive PDAC cases, recruited via PanGenEU, 36 (6%) had ⩾2 first-degree relative with PDAC: Familial pancreatic cancer (FPC). So far PanGen-Fam has recruited 42 high-risk PDAC families; 25 (60%) had FPC. Five index cases with cancer were positive for BRCA2 and one for BRCA1 germline mutations. In the second year of prospective PDAC screening, one neuroendocrine tumour and a high-grade dysplasia lesion suspicious of carcinoma were diagnosed among 41 high-risk individuals. Furthermore EUS detected chronic-pancreatitis-like parenchymal changes in 15 patients. Concluding statement The identification and recruitment of PDAC high-risk families into the PanGen-Fam registry provides an opportunity to detect early onset cancer and precursor pancreatic cancer lesions at a potentially curative stage and to increase the knowledge of the natural history of the disease. Abstract

Objective To contrast the prevalence of mild cognitive impairment ( MCI) as diagnosed using DSM-5 criteria ( DSM5- MCI) with MCI as diagnosed using Petersen's criteria (P- MCI) and to explore the association of both with non-cognitive psychopathological symptoms ( NCPS). Method A two-phase epidemiological screening was implemented in a population-based sample of individuals aged 55+ ( n = 4803). The Geriatric Mental State ( GMS) was the main psychopathological instrument used, and AGECAT was used to make psychiatric diagnoses. Research psychiatrists diagnosed DSM5- MCI and P- MCI using operational criteria. Logistic regression models were then used to investigate the association of MCI with anxiety and depression and with NCPS. Results Weighted prevalence of DSM5- MCI and P- MCI was, respectively, 3.72% and 7.93% for the aged 65+. NCPS were common in both MCI categories, but negative-type symptoms such as 'anergia' and 'observed slowness' were considerably more frequent among persons with DSM5- MCI. Anxiety and depression diagnostic categories were associated with both P- MCI and DSM5- MCI, but affective-type symptoms were mainly associated with P- MCI. Some negative-type symptoms were inversely associated with P- MCI, and no association was observed with DSM5- MCI. Conclusion The prevalence of DSM5- MCI was half that of P- MCI. A literatura mostra a import?ncia do capital humano para reduzir as disparidades entre regi?es e, ciente dessa rela??o, a presente tese compreende tr?s ensaios com ?nfase no crescimento econ?mico regional e setorial, considerando educa??o como proxy de capital humano. No primeiro ensaio, o capital humano ? representado pelo percentual da popula??o de 25 anos ou mais com superior completo, enquanto no segundo e terceiro, utiliza-se a porcentagem de v?nculos da ind?stria de alimentos com ensino superior. No ensaio 1, ? analisada a influ?ncia do capital humano na taxa de crescimento dos munic?pios brasileiros por meio dos padr?es geogr?ficos de aglomera??o espacial. Foi poss?vel observar que, nos munic?pios da regi?o Sul e Sudeste, houve significativa import?ncia do clusters High-High entre a taxa de crescimento e a vari?vel de capital humano, enquanto o modelo econom?trico espacial permite concluir que o capital humano influencia de forma positiva a taxa de crescimento do Produto Interno Bruto (PIB) per capita dos munic?pios. O segundo ensaio verificou os padr?es geogr?ficos das agroind?strias de alimentos dispostos nas microrregi?es do Brasil para o ano de 2016, a fim de apurar se ocorre concentra??o/aglomera??o agroindustrial em torno das microrregi?es metropolitanas. Os resultados mostram que a ind?stria de alimentos favorece um desenvolvimento mais concentrado, com alta densidade populacional de m?o de obra qualificada, acesso ao mercado consumidor e a clusteriza??o para o ganho de produtividade. Por fim, o terceiro ensaio busca analisar como se distribuem espacialmente os v?nculos da ind?stria de alimentos com maior capital humano. Nesse estudo, foi poss?vel identificar que a presen?a de capital humano ? determinante na participa??o dos v?nculos nas microrregi?es que est?o alocadas em todos os estados das regi?es sudeste e sul, assim como em alguns estados do nordeste (Cear? e Bahia) e do norte (Amap? e Roraima). As ind?strias de alimentos que est?o aglomeradas nas microrregi?es do sudeste e sul apresentam v?nculos com maior capital humano, ou seja, nas regi?es brasileiras que j? s?o mais desenvolvidas economicamente e cuja popula??o j? apresenta maior facilidade na aquisi??o de maiores n?veis de escolaridade e compet?ncias. Literature shows the importance of human capital to reduce disparities between regions and, considering this relationship, this thesis comprises three essays emphasizing regional and sectoral economic growth, considering education as a proxy for human capital. In the first essay, human capital is represented by the percentage of the population aged 25 or over with a college degree, while in second and third, the bond percentage in the food industry with higher education is used. In essay 1, the influence of human capital on Brazilian municipalities growth rate is analyzed through the geographic patterns of spatial agglomeration. It was observed that, in southeastern and southern regions, there was significant importance of the High-High clusters between the growth rate and the human capital variable, while the spatial econometric model allows us to conclude that human capital positively influences the rate of Gross Domestic Product (GDP) growth per capita in the cities. The second test verified the food agribusiness geographic patterns arranged in the micro-regions of Brazil in 2016, in order to determine if there is concentration / agribusiness agglomeration around the metropolitan micro-regions. The results show that the food industry favors a more concentrated development, with a high population density of qualified labor, access to consumer market and clustering for productivity gains. Finally, the third essay aimed to analyze the spatial distribution of food industry ties with greater human capital. In this study, it was possible to identify that the presence of human capital is determinant in the participation of ties in the micro-regions that are allocated in all states in the southeastern and southern regions, as well as in some states in the northeast (Cear? and Bahia) and north (Amap? and Roraima). The food industries that are clustered in southeastern and southern micro-regions have links with greater human capital, that is, in the Brazilian regions that are already economically developed and where population are more educated and skilled. Objective: To calculate both the incidence rates and the lifetime risk (LTR) of dementia and Alzheimer's disease (AD). Methods: A two-phase case-finding procedure was implemented in a cohort of 4057 cognitively intact individuals 55+ years of age living in Zaragoza, Spain, and followed-up at 2.5 and 4.5 years. Age- and sex-specific incidence rates were calculated. A mortality-adjusted, multivariate model was used to document LTRs. Results: The incidence rate of dementia continued to rise after the age of 90 years, but was slightly lower than in North and West European studies. Only a tendency for an increased LTR with age was observed. Thus, LTR was 19.7% for a 65-year-old woman and 20.4% at the age of 85 years, the corresponding figures for AD being 16.7% and 17.6%. The LTR of AD was higher in women and was about twice as high among illiterate individuals when compared with individuals with higher educational levels. Conclusions: The incidence rate of dementia in this Southern European city was slightly lower than in previous studies in North-West Europe. LTR of dementia and AD seems to be slightly increased with age. The association of illiteracy with higher LTR of AD is intriguing. [ABSTRACT FROM AUTHOR]

Abstract

The potential use of epilithic diatoms as indicators of organic pollution was evaluated in Gravataí River, RS, (latitude 29°45′–30°12′ S; longitude 50°27′–51°12′ W). The river suffers agricultural impacts in its upper course and urban and industrial organic pollution in its lower course. Epilithic diatoms were sampled eight times from September 2000 to August 2002, at six sites. Species were identified and densities and relative abundances of populations were determined. Simultaneously, physical, chemical and microbiological variables were measured (water temperature, conductivity, turbidity, pH, dissolved oxygen, biochemical oxygen demand (BOD5), chemical oxygen demand, ammonium, organic nitrogen, total nitrogen, ortho-phosphate, total phosphate, chloride and faecal coliforms). In order to interpret the environmental and biological variables, discriminant analysis and the TWINSPAN methods (Two-Way Indicator Species Analysis) were applied. The results indicated that the concentrations of ortho-phosphate, ammonium, total organic nitrogen, BOD5 and faecal coliforms characterized a pollution gradient along the river, where changes in the abundance or species composition were observed. Species were classified into three groups: Group A, including species more tolerant to heavy organic pollution and eutrophication, represented by Luticola goeppertiana, L. mutica, Eolimna subminuscula, Nitzschia palea and Sellaphora pupula; Group B, comprised of tolerant and widely distributed species such as Eunotia bilunaris, Frustulia crassinervia, F. saxonica, Navicula cryptocephala, N. cryptotenella, Nitzschia palea var. tenuirostris, Surirella angusta, Pinnularia microstauron and Ulnaria ulna and Group C, with less pollution tolerant species represented by Eunotia sp. and Gomphonema parvulum.
