Associated factors for infection and in-hospital mortality were analyzed by multivariate logistic regression analysis.Results: A total of 554 patients with PM/DM were included, and 35.6% (197/554) of them developed 404 episodes of infection. Half of the patients developed infection within 4 months after disease onset. Bacterial infection was predominant (249/404, 61.6%). Lung was the most involved (242/404, 59.9%). Gram-negative bacteria the leading pathogens (64/84, 76.2%). Patients with anti-MDA5 positive were prone to develop severe infections (35.1% vs 16.4%, P< 0.001) and had higher mortality (11.7% vs 3.4%, P=0.01). The in-hospital mortality was 6.5% (36/554). Infection was the leading cause of death (20/36, 55.6%). Older age (adjusted odds ratio (OR): 1.05, 95% confidential interval (CI): 1.02– 1.09, P=0.004), ILD (adjusted OR: 2.76, 95% CI: 1.11– 6.84

261. Osteoporosis, (bone) fractures and fracture liaison services: health-related quality of life, clinical and economic outcomes

Abstract

This thesis aims to summarize current evidence relevant to osteoporosis management and to study the clinical and economic outcomes of fracture liaison service (FLS). It suggests that anti-osteoporosis medications are a good way of allocating resources and that FLS is associated with clinical and economic benefits. This thesis also highlights the necessity of increasing the awareness of osteoporosis in both lay and healthcare spheres, the importance of effective communication (using a patient-centered approach) to make patients understand their fracture risk and better adhere to therapy, and the benefits of widely implementing FLS and optimizing the quality of the FLS. The information this thesis provides would be useful for decision-makers (e.g. payers, politicians, clinicians, or other members of decision-making boards) in the healthcare setting. With this information, decision-makers can introduce some positive initiatives in clinical practice to identify more patients at risk of fracture, get more patients treated and adhered to therapy, stimulate policy or financial incentives aiming to support and optimize the osteoporosis management and FLS implementation, lower the fracture risk, and reduce clinical and financial burden.

266. Synthesis of organic–inorganic hybrid flame retardant and its use in polyurethane composite fiber membrane.

Abstract

In this article, we presented an inventive organic–inorganic hybrid flame retardant, MPKG, by the strategic reaction of phytic acid (PA) with melamine (MA), γ‐aminopropyl triethoxysilane (KH550) and graphene oxide (GO). Subsequently, an MPKG/PU fiber membrane was fabricated through electrospinning, wherein MPKG was seamlessly incorporated into the PU spinning solution. The morphology and chemical structures of the flame retardants were characterized by scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR), and x‐ray photoelectron spectroscopy (XPS). The flame retardancy and resilience to soaping of the fiber membranes were assessed via the limiting oxygen index (LOI), thermogravimetric analysis (TGA), and microscale combustion calorimetry (MCC). Impressively, the results demonstrate a significant 38.8% reduction in total heat release (THR) for the MPKG/PU fiber membrane in comparison to pure PU fiber membrane. Moreover, the incorporation of MPKG leads to an increase in LOI from 18.8% to 21.5% for the PU fiber membrane, accompanied by a notable enhancement in char yield at 800°C from 13.5% to 28.5%. These results underscore the augmentative impact of MPKG on the flame retardancy of PU fiber membrane. Remarkably, the MPKG/PU fiber membrane exhibits robust soaping resistance, with the LOI maintaining a level above 20% post‐soaping. [ABSTRACT FROM AUTHOR]

267. Carbon Nanotube-Based High-Current Nanoscale Air-Channel Electronic Devices for Low-Power Ultrafast Electronics.

Abstract

Nanoscale air-channel electronic devices (NAEDs) have gained intensive attention recently due to their great potential as fundamental building blocks in future low-power ultrafast electronics. Despite considerable progress on low-power ultrafast characteristics, practical applications of NAEDs are being hindered by the low working current generally in the range of 0.01–20 μA. This unattainable high current is mainly due to the weak emission of electrons from a seriously limited effective emission area in conventional low-voltage field-emission (LV-FE) electron sources of currently existing NAEDs. Here, we present a high-current LV-FE electron source for an NAED, which consists of high-aspect-ratio carbon nanotubes (CNTs) and large-specific surface-area microstructures. The emission current density is increased exponentially by compensating for the field-screening effect between adjacent CNTs with the additional field-enhancement effect derived from underlying microstructures. Due to the simultaneously enlarged effective emission area, milliampere-level high current is achieved by optimizing the structure of the CNT-based LV-FE electron source. CNTs have been directly grown on well-selected microstructures with the matched feature size and improved interface contact, experimentally demonstrating an optimized LV-FE electron source with a high current of 6 mA. The feasibility of achieving high-current performance with a subpicosecond ultrafast response at a low turn-on voltage has been confirmed by configuring NAED with a CNT-based LV-FE electron source.
