- Abstract
Background Acute heart failure patients are often encountered in emergency departments (ED) from 11% to 57% using emergency medical services (EMS). Our aim was to evaluate the association of EMS use with acute heart failure patients’ ED management and short-term outcomes. Methods This was a sub-analysis of a European EURODEM study. Data on patients presenting with dyspnoea were collected prospectively from European EDs. Patients with ED diagnosis of acute heart failure were categorized into two groups: those using EMS and those self-presenting (non- EMS). The independent association between EMS use and 30-day mortality was evaluated with logistic regression. Results Of the 500 acute heart failure patients, with information about the arrival mode to the ED, 309 (61.8%) arrived by EMS. These patients were older (median age 80 vs. 75 years, p < 0.001), more often female (56.4% vs. 42.1%, p = 0.002) and had more dementia (18.7% vs. 7.2%, p < 0.001). On admission, EMS patients had more often confusion (14.2% vs. 2.1%, p < 0.001) and higher respiratory rate (24/min vs. 21/min, p = 0.014; respiratory rate > 30/min in 17.1% patients vs. 7.5%, p = 0.005). The only difference in ED management appeared in the use of ventilatory support: 78.3% of EMS patients vs. 67.5% of non- EMS patients received supplementary oxygen (p = 0.007), and non-invasive ventilation was administered to 12.5% of EMS patients vs. 4.2% non- EMS patients (p = 0.002). EMS patients were more often hospitalized (82.4% vs. 65.9%, p < 0.001), had higher in-hospital mortality (8.7% vs. 3.1%, p = 0.014) and 30-day mortality (14.3% vs. 4.9%, p < 0.001). The use of EMS was an independent predictor of 30-day mortality (OR = 2.54, 95% CI 1.11–5.81, p = 0.027). Conclusion Most acute heart failure patients arrive at ED by EMS. These patients suffer from more severe respiratory distress and receive more often ventilatory support. EMS use is an independent predictor of 30-day mortality.

Estudou-se o efeito da redução da proteína bruta (PB) e da suplementação de aminoácidos em dietas para suínos, dos 70 aos 100kg. Foram utilizados 48 suínos machos, castrados, híbridos comerciais, com o peso inicial de 70,12±1,56kg, distribuídos em delineamento experimental de blocos ao acaso, com quatro níveis de proteína bruta na dieta, 17,95; 16,45; 14,95; 13,45, seis repetições e dois animais por unidade experimental. Não foi observado efeito dos tratamentos sobre o ganho de peso diário e sobre o consumo de dieta diário. Houve efeito quadrático dos níveis de PB sobre a conversão alimentar, que melhorou até o nível estimado de 15,7% de proteína bruta. Não houve influência dos níveis de PB sobre a espessura de toucinho e sobre o rendimento de carne magra. Conclui-se que a redução da proteína bruta na dieta de até 4,5 pontos percentuais, com suplementação de aminoácidos, para suínos na fase de terminação, não influi no consumo de dieta diário, no ganho de peso diário e nas características de carcaça. Melhor conversão alimentar foi estimada para suínos alimentados com dietas contendo 15,7% de proteína bruta.

Para avaliar dietas de alta densidade nutricional, com ou sem suplementação de aminoácidos industriais, ajustadas com base na energia líquida, utilizaram-se 80 porcas pluríparas, em lactação, do mesmo padrão genético, distribuídas em delineamento experimental de blocos composto por quatro dietas, 20 blocos, considerando cada porca uma unidade experimental. Na formação dos blocos, utilizaram-se como critérios o peso e a ordem de parto das porcas. As dietas foram formuladas com a combinação de quatro níveis de proteína bruta (18,33; 15,23; 19,63; ou 16,59%), quatro de energia metabolizável (3.344; 3.293; 3.480; e 3.440 kcal/kg), dois de lisina digestível (0,85 e 0,95%) e dois de energia líquida (2.519 e 2.636 kcal de EL/kg) e da suplementação com aminoácidos industriais até a relação triptofano:lisina digestíveis de 18%. Não houve efeito das dietas com alta densidade nutricional, com ou sem suplementação de aminoácidos, sobre o consumo médio diário de ração. As dietas também não influenciaram o peso, a perda e variação de peso, o intervalo desmama-estro nem a estimativa da produção de leite das porcas no período de lactação. Dietas de alta densidade nutricional (lisina e energia) para porcas em lactação são eficientes em reduzir a mobilização de reservas corporais e não afetam os desempenhos produtivo e reprodutivo das porcas.In order to evaluate diets with high nutritional density, with or without industrial amino acid supplementation, adjusted at net energy base, it was used 80 pluriparous lactating sows with the same genetic pattern, distributed in an experimental block design composed of four diets, 20 blocks, considering each sow an experimental unity. The criteria used for formation of the blocks were the weight and parity order. Diets were formulated by the combination of four levels of crude protein (18.33; 15.23; 19.63; or 16.59%), four levels of metabolizable energy (3,344; 3,293; 3,480; and 3,440 kcal/kg), two of digestible lysine (0.85 and 0.95%) and two levels of net energy (2,519 and 2,636 kcal of NE/kg) and the supplementation with industrial amino acids up to the digestible triptofano:lysine relationship of 18%. There was no effect of the diets with high nutritional density, with or without amino acid supplementation, on the average daily intake of the ration. The diets did not influence weight, loss and changes on weight, the internal weaning-to-estrus interval neither the estimate of milk production of the sows during lactation period. Diets of high nutritional density (lisyne and energy) for lactating sows are efficient in reducing mobilization of body reserves and they do not affect productive and reproductive performance of the sows.

Com o objetivo de validar as relações de aminoácidos digestíveis com a lisina digestível propostas com base no conceito da proteína ideal, foi realizado um experimento com 120 matrizes suínas em lactação, distribuídas em delineamento em blocos, composto de seis dietas, 20 blocos e uma matriz por unidade experimental. As dietas foram formuladas a partir de uma dieta-referência - contendo 19,5% de proteína bruta (PB) e 0,95% de lisina digestível - suplementada com metionina, treonina, triptofano e/ou valina digestíveis para atingir as relações com a lisina digestível determinadas com base na proteína ideal. As dietas não influenciaram a perda e a variação de peso, a espessura e a variação da espessura de toucinho, a proteína e a variação de proteína corporal, a gordura e a variação de gordura corporal nem alteraram o intervalo desmame-estro ou a produção de leite das porcas. As relações dos aminoácidos metionina + cistina, treonina, triptofano e valina digestíveis com a lisina digestível propostas com base na proteína ideal atendem às exigências de porcas em lactação para melhores desempenhos produtivo e reprodutivo.This experiment aimed at validating the relationship between digestible amino acids with the digestible lysine proposed on the basis of the ideal protein concept. This experiment used 120 lactating sows distributed in a complete randomized block design with six treatments and 20 repetitions and one sow per experimental unity. The diets were formulated based on a reference diet - which contained 19.5% of crude protein (CP) and 0.95% of digestible lysine supplemented with digestible methionine, threonine, tryptophan and/or valine to meet the ideal protein based determined relationships with digestible lysine. The diets did not change weight loss and variation, thickness and thickness variation of backfat, protein and body protein variation, fat and body fat variation, neither they changed the weaning-estrus interval or milk production of the sows. The relationships between metionine + cistine, threonine, tryptophan, and valine digestible amino acids with digestible lysine based proposed on ideal protein meet the requirements of lactating sows for better reproductive and productive performances.

This handbook is designed for school personnel, youth with disabilities, parents, Individualized Education Program/transition team members, service providers, and others who are involved in student-focused transition planning. It provides detailed examples illustrating the process of applying for Social Security Income (SSI) work incentives and demonstrates how students with disabilities can benefit from SSI work incentives when utilized during the transition planning process. The handbook focuses on two of the SSI work incentives: Plan for Achieving Self-support (PASS) and Impairment-related Work Expense (IRWE). PASS provides students with a mechanism for maintaining SSI eligibility while moving toward a career goal by acquiring the necessary training and/or support required to reach that goal. The IRWE provides ongoing support required to maintain employment. Similarities and differences between the two programs are explained, followed by four examples of the PASS initiative and three examples of the IRWE incentive. The PASS examples include a narrative and a completed PASS application form. IRWE examples are presented in narrative format along with calculations that demonstrate how the IRWE incentive will affect the student's SSI benefit rate and earned income. Appendices include hints for completing a PASS application, examples of letters of support, and definitions of SSI related terms. (CR)

268. Climate impact of constructed wetlands treating nitrate-rich agricultural runoff: The methane problem

- Abstract
Constructed wetlands (CW) treating runoff from agricultural catchments can efficiently reduce nitrate contamination, however, most wetlands are inherently net sources of methane (CH4), which is of environmental concern due to its potent global warming capacity. For the mitigation of this negative aspect of CWs we need investigations on the CH4 emission dynamics and environmental conditions governing CH4 production and consumption in CWs.This study integrates results from 4-years (2014-2017) investigations in an off-stream CW in Rampillon, France (0.53 ha, depth 0.3-0.8 m, est. 2010) and from 4-years (2018-2021) studies in an in-stream CW in Vända, Estonia (0.45 ha, depth 0.1-0.6 m, est. 2015). In Rampillon, during four 2-weeks measurement campaigns throughout all seasons CH4 fluxes were measured using floating automated chambers connected to the QCLAS laser system. In addition, gas was sampled twice day from manual floating chambers for further analysis in lab. In Vända in-stream CW, CH4 fluxes were measured twice a month using manual chambers and gas-chromatographs.The average annual CH4 emission in Rampillon for 2014-2015 was 7.7 g CH4-C m-2 yr-1, showing highest values from deeper (0.5-1.0 m) parts in summer and autumn. The highest values reached up to 180 mg CH4-C m-2 h-1, mainly due to ebullition. The emissions in winter and spring were up to 10 times lower, however no negative values were observed. There was an increasing trend in CH4 fluxes: in 2017 the average emission reached to 12.0 g CH4-C m-2 y-1. Differences between the emission values gathered from authomated chambers were about 10% higher than those measured from manual chambers.In Vända in-stream CW, a clear increase in average annual emissions was found: from 0.4 in2018 to 10.5 g CH4-C m-2 yr-1 in 2021. It was correlated with increasing Typha latifolia-dominated vegetation cover. Emissions showed strong correlation with air and water temperature while no clear relationship was found with the depth of various parts.Large CH4 emission from CWs is a major concern and therefore a smart management is needed. Our previous studies in surface flow CWs treating nitrate-contaminated runoff demonstrate that above-ground biomass harvesting of plants can decrease the CH4. The end of growing season is likely the best time for biomass harvesting while avoiding the excessively high CH4 emissions that the summer harvest may produce.

Rapid large-scale development presents challenges for traffic engineers to accurately forecast design-year traffic volumes. Prevailing forecasting methods, such as the ITE Trip Generation Manual, tend to exaggerate local traffic impacts on large developments and do not account for regional influences. This presentation outlines the methodologies for developing the design level forecast of two large freeway projects amid largescale private developments. This approach provides significant improvements over traditional trip generation methods and model-based forecasts.

Väitekirja elektrooniline versioon ei sisalda publikatsioone, Ühiskonna suurenenud toiduvajadus on suurendanud põllumaade osakaalu ning väetiste kasutamist. Intensiivne või halvasti ajastatud väetamine mõjutab pinnavett ja suurendab veekogude eutrofeerumist. Põllumajandusliku hajukoormuse leevendamiseks ja vee kvaliteedi parandamiseks on tõhus meede tehismärgalade rajamine. Need aitavad parandada vee kvaliteeti looduslike protsesside abil ning pakuvad muidki ökosüsteemi hüvesid (suurendavad bioloogilist mitmekesisust, puhverdavad sademevett ja toimivad veereservuaarina). Doktoritöö uuris vahetult voolusängi rajatud Vända avaveelise tehismärgalargala toimet: toitainete vähenemist ja kasvuhoonegaaside (KHG) heitkoguseid mõjutavaid parameetreid. Varasemast on teada, et tehismärgalades sõltuvad toitainete eemaldamise tõhusus ja KHG heitkogused vee parameetritest ja märgalade kujundusest. Vända märgala vähendas tõhusalt fosforit, millel oli selge hooajaline dünaamika. Tõhusamalt eemaldas fosforit soojal perioodil ja külmal perioodil oli tõhusus väiksem. Fosfori eemaldamise efektiivsus oli suurem väikese vooluhulga korral, sest siis aeglane vool soodustas settimis- ja filtreerimisprotsesse. Lämmastiku eemaldamine oli ootustele vastupidine, kuna märgalas suurenes lämmastiku hulk väljavoolu suunas. Lämmastiku lisandumine toimus tõenäoliselt nitraadirikka põhjavee sissevoolu tõttu. Fosfori puhastusefektiivsus sõltus ka taimkattest – mida rohkem taimi, seda tõhusam. Taimestik levis märalal aastate jooksul ning see soodustas fosfori eemaldamist. Metaani (CH4) ja dilämmastikoksiidi (N2O) puhul ilmnes sesoonne dünaamika – soojal perioodil oli lendumine suurem ja külmal perioodil väiksem. N2O heitkogused sõltusid vee sügavusest, voolukiirusest ja vee temperatuurist. Suvel kui mikroobne aktiivsus on suur, täheldati oluliselt suuremaid N2O emissioone., Water quality has degraded due to increased proportion of agricultural lands in some regions, if more fertilizer is used. Intensive or poorly timed fertilization affects surface waters and increases eutrophication in water bodies. Diffuse agricultural pollution is difficult to locate and control due to its diffuse nature, climate conditions and strong anthropogenic influence. In order to maintain or improve surface water quality, it is important to find and use effective measures. One of the best measures for improving water quality is using constructed wetlands. Constructed wetlands help to improve water quality through natural treatment processes, among several other benefits. They increase biodiversity, buffer storm water and act as a water reservoir. The aim of this thesis was to investigate the efficiency of in-stream free water surface flow Vända constructed wetland and study the parameters affecting nutrient reduction and greenhouse gas (GHG) emissions. Long-term monitoring results show that in in-stream CWs, nutrient removal efficiency and GHG emissions are highly dependent on water parameters and wetland design. Vända CW effectively reduced phosphorus that had clear seasonal dynamics. Higher removal efficiency occurred during the warm period and lower values were seen during the cold period. Higher removal efficiency values for phosphorus were seen when the flow rate was lower. With a lower flow rate, sedimentation and filtration processes are favoured. The results for nitrogen removal were contrary to expectations because CW acted more as a source of nitrogen. The addition of nitrogen was suspected to come from groundwater that was contaminated with nitrate. Phosphorus removal had a strong positive correlation with vegetation coverage. Through years’ vegetation spread over the wetland and that promoted phosphorus removal processes. For methane (CH4) and nitrous oxide (N2O), clear seasonal dynamics were seen, with higher emissions during the warm period and lower emissions during the cold period. N2O emissions were strongly related to water depth, flow rate and water temperature. During summer, with high microbial activity, significantly higher N2O emissions were seen.

Background Acute heart failure patients are often encountered in emergency departments (ED) from 11% to 57% using emergency medical services (EMS). Our aim was to evaluate the association of EMS use with acute heart failure patients’ ED management and short-term outcomes. Methods This was a sub-analysis of a European EURODEM study. Data on patients presenting with dyspnoea were collected prospectively from European EDs. Patients with ED diagnosis of acute heart failure were categorized into two groups: those using EMS and those self-presenting (non- EMS). The independent association between EMS use and 30-day mortality was evaluated with logistic regression. Results Of the 500 acute heart failure patients, with information about the arrival mode to the ED, 309 (61.8%) arrived by EMS. These patients were older (median age 80 vs. 75 years, p p = 0.002) and had more dementia (18.7% vs. 7.2%, p p p = 0.014; respiratory rate > 30/min in 17.1% patients vs. 7.5%, p = 0.005). The only difference in ED management appeared in the use of ventilatory support: 78.3% of EMS patients vs. 67.5% of non- EMS patients received supplementary oxygen (p = 0.007), and non-invasive ventilation was administered to 12.5% of EMS patients vs. 4.2% non- EMS patients (p = 0.002). EMS patients were more often hospitalized (82.4% vs. 65.9%, p p = 0.014) and 30-day mortality (14.3% vs. 4.9%, p p = 0.027). Conclusion Most acute heart failure patients arrive at ED by EMS. These patients suffer from more severe respiratory distress and receive more often ventilatory support. EMS use is an independent predictor of 30-day mortality.

The coronavirus disease 2019 (COVID-19) pandemic is a major challenge for global healthcare systems. Early and safe triage in the emergency department (ED) is crucial for proper therapy. However, differential diagnosis remains challenging. Rapid antigen testing (RAT) may help to improve early triage and patient safety. We performed a retrospective study of 234 consecutive patients with suspected COVID-19 who presented to our ED in November 2020. All underwent SARS-CoV-2-nasopharyngeal swab testing using both RAT and reverse transcription polymerase chain reaction (RT-PCR). The inpatient treatment was established according to an empirically developed triage algorithm. The accuracy of the suggested algorithm was analyzed based on the rate of outpatients returning within 7 days and inpatients staying for less than 48 hours. COVID-19 inpatients and outpatients were compared for symptoms, vital signs, and C-reactive protein levels. Of the 221 included patients with suspected COVID-19 infection, the diagnosis could be confirmed in 120 patients (54.3%) by a positive RT-PCR result, whereas only 72% of those had a positive antigen test. Of the 56 COVID-19 outpatients, three returned within 7 days with the need for hospital treatment due to clinical deterioration. Among the 64 COVID-19 inpatients, 4 were discharged within 48 hours, whereas 60 stayed longer (mean duration 10.2 days). The suggested triage algorithm was safe and efficient in the first 234 consecutive patients. RAT can confirm a diagnosis in 72% of PCR proven COVID-19 patients and allows early cohort isolation as an important way to save hospital capacity. CA Fistera

Einleitung Mit weit mehr als 1400 stationär behandelten COVID-19-Patienten ist die Universitätsmedizin Essen der größte COVID-19-Versorger der Region Rhein-Ruhr. Wir präsentieren die Daten unserer Patienten aus den ersten 12 Monaten der Pandemie und die hieraus entstandenen praktischen Konzepte. Methode Retrospektive Analyse aller 1396 stationären COVID-19-Patienten, die zwischen dem 1. März 2020 und 28. Februar 2021 versorgt wurden, im Hinblick auf Komorbiditäten, Überleben und Komplikationen im Verlauf. Es erfolgte ein Gruppenvergleich zwischen Patienten auf Normalstation und Überwachungs-/ Intensivstation. Ergebnisse Bei einer Gesamtmortalität von 19,8 % (277/1396) starben 10,6 % (93/877) der Patienten auf Normalstation und 35,5 % (184/519) der Patienten auf Intensiv- und Überwachungsstationen im klinischen Verlauf. Hierbei waren ein Alter über 60 Jahre, Adipositas, maschinelle Beatmung, NO-Therapie, ECMO-Therapie sowie akutes Nierenversagen und Apoplex im Therapieverlauf unabhängige Prädiktoren für Mortalität. Fazit Die Mortalität unseres Kollektivs auf Normal- bzw. Intensivstationen liegt im Rahmen international publizierter Daten. Sowohl die hohe Rate von Komplikationen bei schwerem Verlauf als auch die große Bedeutung einfacher Komorbiditäten kann eindrücklich gezeigt werden. Das mittlere Alter der Patienten ist mit 60 Jahren auf Normalstation und 63 Jahren auf Intensivstationen überraschend niedrig. Maximaler Patienten- und Personalschutz, eine rasche und effektive Teststrategie im Rahmen der primären Triage, standardisierte Abläufe von der Notaufnahme bis zur Intensivstation sowie eine dynamische tagesaktuelle Anpassung der Ressourcen können eine hohe Versorgungsqualität, auch während der Pandemie, sichern.

Avaliou-se o efeito de níveis de treonina digestível em rações sobre o desempenho e características de cacaça de suínos machos castrados, selecionados geneticamente, para deposição de carne na carcaça, na fase de terminação. Foram utilizados 70 animais, com peso inicial de 59,00+1,25kg, distribuídos em delineamento experimental de blocos ao acaso, com cinco tratamentos, sete repetições e dois animais por unidade experimental, sendo que na distribuição dos animais, foi adotado como critério o peso inicial. Os tratamentos foram níveis de treonina na ração (0,480; 0,504; 0,528; 0,554 e 0,572% de treonina digestível). Não houve efeito (P>0,05) dos tratamentos sobre o ganho de peso, consumo de ração e conversão alimentar. Os níveis de treonina digestível não influenciaram (P>0,05) a porcentagem de carne e a espessura de toucinho na carcaça. Conclui-se que o nível de 0,480% de treonina digestível, correspondente a relação de treonina digestível:lisina digestível de 60%, atendeu as exigências de suínos machos castrados de alto potencial para deposição de carne na carcaça dos 60 aos 95kg.An experiment was carried out to determine the effects of digestible threonine levels in ration on the performance and carcass traits of hogs genetically selected for lean deposition at finishing. Seventy barrows averaging an initial weight of 59.00+1.25kg were distributed in a randomized experimental block design with five treatments and seven replicates, being two animals per experimental unit. Initial weight was adopted as a criterion for animal distribution in the treatments, which were the levels of digestible treonine in the diet (0.480; 0.504; 0.528; 0.554; and 0.572%). No effect (P>0.05) was observed among the treatments on daily weight gain, daily feed intake, and feed conversion. Digestible threonine levels did not influence (P>0.05) lean and backfat thickness percentage. It can be concluded that the level of 0.480% of digestible treonine, corresponding to the rate digestible treonine:lysine 60%, supplied the requirments of high potential lean deposition barows from 60 to 95kg.

Um experimento foi realizado com o objetivo de avaliar os efeitos da ingestão de PB durante a gestação sobre os desempenhos produtivo e reprodutivo de fêmeas suínas gestantes de 4º ou 5º parto e de suas respectivas leitegadas. Foram utilizadas 25 matrizes mestiças (189,0±16,46 kg) com idade reprodutiva de 4º parto, distribuídas aleatoriamente em delineamento experimental inteiramente casualizado com três tratamentos (10; 13,5 e 17% de PB) e 18, 15 e 12 repetições, considerando a matriz a unidade experimental. Os ganhos em peso das porcas da cobertura aos 110 dias de gestação e da cobertura ao parto, pesos ao parto e ao desmame, o ganho em espessura de toucinho durante a gestação e a perda em espessura de toucinho durante a lactação não foram afetados pelos níveis de proteína na ração durante a gestação. O nível de 13,5% de PB na ração de gestação proporcionou o melhor balanço energético às porcas, o maior número de nascidos vivos, o maior peso médio do leitão ao desmame, o maior peso da leitegada ao parto e da leitegada ao desmame e o maior ganho de peso médio diário da leitegada. O balanço energético no processo produtivo foi mais eficiente nas matrizes que consumiram dietas com 13,5% de PB. O melhor nível de PB na ração de gestação para matrizes suínas de 4º e 5º parto é de 13,5%, que corresponde a um consumo diário de 312 g de PB.An experiment was conducted to evaluate the effects of CP intake during gestation on productive and reproductive performance of pregnant sows of 4th or 5th parturition orders. A total of 25 crossbred sows averaging 189.0±16.46 kg at 4th parity order was allotted to a completely randomized experimental design with three treatments (10, 13.5, and 17% of CP) and 18, 15 and 12 replicates, and each sow was considered an experimental unit. No treatment effects on sow weight gain from mating to 110 days of gestation and from mating to parity, on weights at parity and weaning, backfat thickness gain during gestation and backfat thickness loss during lactation were observed. The dietary CP level of 13.5% provided the best energetic balance for sows, the highest number of piglets born alive, the highest average piglet weight at weaning, the highest litter weight at parity and weaning and the highest litter average daily weight gains. The energetic balance in the productive process was more efficient in sows fed diets with 13.5% of CP. The best dietary CP level for sows of 4th or 5th parity orders in the gestation is of 13.5%, that is correspondent to a daily intake of 312 g of CP.

Scleroderma renal crisis (SRC) is an acute life-threatening manifestation of systemic sclerosis (SSc) caused by obliterative vasculopathy and thrombotic microangiopathy. Evidence suggests a pathogenic role of immunoglobulin G (IgG) targeting G-protein coupled receptors (GPCR). We therefore dissected SRC-associated vascular obliteration and investigated the specific effects of patient-derived IgG directed against angiotensin II type 1 (AT1R) and endothelin-1 type A receptors (ETAR) on downstream signaling events and endothelial cell proliferation. SRC-IgG triggered endothelial cell proliferation via activation of the mitogen-activated protein kinase (MAPK) pathway and subsequent activation of the E26 transformation-specific-1 transcription factor (Ets-1). Either AT1R or ETAR receptor inhibitors/shRNA abrogated endothelial proliferation, confirming receptor activation and Ets-1 signaling involvement. Binding of Ets-1 to the tissue factor (TF) promoter exclusively induced TF. In addition, TF inhibition prevented endothelial cell proliferation. Thus, our data revealed a thus far unknown link between SRC-IgG-induced intracellular signaling, endothelial cell proliferation and active coagulation in the context of obliterative vasculopathy and SRC. Patients’ autoantibodies and their molecular effectors represent new therapeutic targets to address severe vascular complications in SSc.

- Abstract
Background: Medulloblastoma, a genetically heterogeneous tumor, is the most frequent malignant brain tumor in children. Although several studies have been carried out, the molecular mechanism underlying medulloblastoma tumorigenesis is not completely known. microRNA (miRNA) expression profiles have been associated with development, progression, and prognosis of human cancers, including medulloblastoma. However, the role of miRNAs in pediatric medulloblastoma has been poorly explored.Methods: Global miRNA expression in 24 microdissected medulloblastoma specimens (19 pediatric and 5 adult specimens) was evaluated by microarray assay. miR-512-3p, the most differentially expressed miRNA in these two groups, was analyzed by qRT-PCR in a cohort of 51 consecutive pediatric medulloblastoma samples and 7 pediatric non-neoplastic cerebellum control samples, and its clinical significance was assessed. Further in silico miRNA prediction of target genes was performed with bioinformatics tools.Results: Compared to the controls, miR-512-3p was significantly downregulated in the pediatric medulloblastoma samples. Higher miR-512-3p was associated with incomplete degree of resection, high risk group classification, and poor overall survival. In silico analysis in an independent cohort of medulloblastoma identified that some of the miR-512-3p target genes (SMAD9, SSX2IP, MAPK10, PTCH1, CCDC6, and BMPR2) were statistically correlated with overall survival, metastasis, and death.Conclusions: For the first time, our results have shown that miR-512-3p is significantly associated with poor clinical outcome in pediatric medulloblastoma, suggesting that miR-512-3p is a potential biomarker of prognosis.

Background and Purpose: Considering the highly time-dependent therapeutic effect of endovascular treatment in patients with large vessel occlusion–associated acute ischemic stroke, prehospital identification of large vessel occlusion and subsequent triage for direct transport to a comprehensive stroke center offers an intriguing option for optimizing patient pathways. Methods: This prospective in-field validation study included 200 patients with suspected acute ischemic stroke who were admitted by emergency medical service to a comprehensive stroke center. Ambulances were equipped with smartphones running an app-based Field Assessment Stroke Triage for Emergency Destination scale for transmission prior to admission. The primary measure was the predictive accuracy of the transmitted Field Assessment Stroke Triage for Emergency Destination for large vessel occlusion and the secondary measure the predictive accuracy for endovascular treatment. Results: A Field Assessment Stroke Triage for Emergency Destination ⩾4 revealed very good accuracy to detect large vessel occlusion–related acute ischemic stroke with a sensitivity of 82.4% (95% confidence interval = 65.5–93.2), specificity of 78.3% (95% confidence interval = 71.3–84.3), and an area under the curve c-statistics of 0.89 (95% confidence interval = 0.85–0.94). Field Assessment Stroke Triage for Emergency Destination ⩾4 correctly identified 84% of patients who received endovascular treatment [73.5% specificity (95% confidence interval = 66.4–79.8)] with an area under the curve c-statistics of 0.82 (95% confidence interval = 0.74–0.89). In a hypothetical triage model of an urban setting, one secondary transportation would be avoided with every fifth patient screened. Conclusion: A smartphone app-based stroke triage completed by emergency medical service personnel showed adequate quality for the Field Assessment Stroke Triage for Emergency Destination to identify large vessel occlusion–associated acute ischemic stroke. We demonstrate feasibility of the use of a medical messaging service in prehospital stroke care. Based on these first results, a randomized trial evaluating the clinical benefit of such a triage system in an urban setting is currently in preparation. Background Current ESC guidelines for non-ST-segment elevation myocardial infarction suggest the utilization of echocardiography in patients with inconclusive initial electrocardiography and cardiac enzymes. Besides detection of alternative pathologies associated with chest pain, echocardiography can screen for wall motion abnormalities (WMA) as sign of myocardial necrosis. Purpose We evaluated the ability of the assessment of regional WMA, detected via transthoracic echocardiography, to predict the presence of culprit lesions in patients presenting with acute chest pain to the emergency department. Methods In this prospective single-centre observational cohort study, we included consecutive patients presenting to the emergency department of our University Hospital with acute chest pain, suggestive of an acute coronary syndrome, between December 2018 and August 2020. Patients with ST-elevation myocardial infarction, hemodynamic instability, or known coronary artery disease were excluded. As part of initial workup, patients received bedside echocardiography for the assessment of regional WMA by a dedicated study physician, blinded to all patients' characteristics. The primary endpoint was defined as the presence of culprit lesions as detected in subsequent invasive coronary angiography, requiring coronary revascularization therapy. Logistic regression analysis was performed in different models adjusted for traditional cardiovascular risk factors, cardiac biomarkers as well as established risk scores. Area under the receiver operating characteristics curve (AUC) was calculated to assess a potential improvement in the prediction of culprit lesions. Results Overall, 657 patients (age 58.06±18.04 years, 53% male) were included in our study. WMA were detected in 76 patients (11.6%). Patients with WMA were older (66.92±13.85 vs. 56.90±18.21 years, p Conclusion WMA strongly and independently predict the presence of culprit lesions in patients presenting with acute chest pain to the emergency department. Our results suggest that routine bedside echocardiography for assessment of WMA in emergency department may improve diagnostic algorithms in suspected acute coronary syndrome. Avaliaram-se os efeitos das fontes lipídicas (gordura de coco e óleo de soja) e dos níveis de energia digestível (3350; 3450; 3550 e 3650kcal de ED/kg de ração) sobre o desempenho produtivo e reprodutivo de 64 marrãs da raça Landrace, com peso médio inicial de 58,703±0,26kg. O delineamento experimental foi o de blocos ao acaso, em arranjo fatorial 2´4 e quatro repetições. Não houve interação entre as fontes e os níveis de energia digestível (ED). Ganho diário de peso (GDP), consumo diário de energia (CED) e conversão alimentar (CA) aumentaram de maneira linear (P0,10) o peso à puberdade, número de embriões viáveis, taxa de fertilização e mortalidade embrionária. A utilização de gordura de coco como fonte de energia proporcionou melhores resultados de GDP, enquanto que o óleo de soja foi mais eficiente para aumentar a ET. Marrãs de reposição puras da raça Landrace, que receberam ração controlada dos 60kg de peso à época da primeira cobrição, exigem o nível de 3650kcal de ED/kg de ração, para melhores resultados de desempenho e características reprodutivas, independentemente da fonte de energia utilizada.The effect of lipid sources (coconut oil and soybean oil) and digestible energy (DE) levels (3350, 3450, 3550 and 3650kcal DE/kg of diet) on the performance and reproductive traits of 64 Landrace gilts, averaging 58.703±.26kg, in a randomized block design, with four replicates, was evaluated. Daily weight gain (DWG), daily energy gain (DEG) and feed:gain ratio (F/G) linearly increased (P.10) age at puberty, number of viable embryos, fertilization rate and embryo mortality. Gilts fed on coconut oil as energy source of the diet showed the best DWG, while those fed on soybean oil were more efficient to increase BT. Reposition Landrace gilts from 60kg until breeding age (at the second heat) fed on adjust controlled diets require 3650 DE/kg of diet for better performance and reproductive results, independently of the energy source.

Foram utilizadas 216 leitoas híbridas comerciais, da mesma linhagem genética, com alto potencial para deposição de carne magra na carcaça com peso médio inicial de 66,3 ± 4,24 kg e final de 95,5 ± 2,05 kg, para avaliar o efeito de níveis de lisina da dieta. Os animais foram distribuídos em delineamento experimental de blocos ao acaso, com quatro tratamentos, 27 repetições e dois animais por unidade experimental. Os tratamentos corresponderam a dietas isoenergéticas, com diferentes níveis de lisina total (0,80; 0,90; 1,00; e 1,10%), obtidos por alterações nas proporções de milho e farelo de soja da dieta. Não foi observado efeito dos tratamentos sobre o ganho de peso diário, a taxa de crescimento em músculo e conversão alimentar em músculo. Houve efeito quadrático dos tratamentos sobre o consumo de dieta, que reduziu até o nível estimado de 0,95%, e a conversão alimentar, que melhorou até o nível de 0,97% de lisina. Observou-se efeito linear dos níveis de lisina sobre a taxa de deposição de proteína, que aumentou, e sobre a taxa de deposição de gordura, que reduziu com o aumento dos níveis de lisina na dieta. Não foi verificado efeito dos tratamentos sobre as características de comprimento de carcaça, a espessura de toucinho na região entre a última e a penúltima vértebra lombar, a espessura de toucinho a 6,5 cm da linha dorso-lombar (P2), o rendimento de carcaça, o rendimento de carne magra, o rendimento de gordura e o rendimento de pernil. Concluiu-se que leitoas geneticamente selecionadas para alta taxa de deposição de carne magra exigem 0,97% de lisina total, correspondente ao consumo diário de 24,24 g de lisina e a 0,29% de lisina total/Mcal de EM, para melhor conversão alimentar. Para maior taxa de deposição de proteína, os dados indicam que a exigência está acima de 1,1% de lisina total.

Para avaliar o efeito dos planos de nutrição, com base em níveis de lisina, sobre o desempenho e as características de carcaça de leitoas em terminação, utilizaram-se 96 leitoas híbridas comerciais de alto potencial genético para deposição de carne magra, com peso médio inicial de 65,93 ± 2,78 kg e final de 105,50 ± 2,18 kg. Os animais foram distribuídos em um delineamento experimental de blocos ao acaso, com quatro tratamentos (planos de nutrição), doze repetições e dois animais por baia, considerada a unidade experimental. Os tratamentos consistiram de planos nutricionais (estabelecidos por níveis seqüenciais de lisina) para duas diferentes faixas de peso, 65 a 95 kg e 95 a 105 kg. As rações foram isoenergéticas e os níveis de lisina foram obtidos variando a proporção entre o milho e o farelo de soja da ração. Não foi observado efeito dos tratamentos sobre o ganho de peso diário e sobre o consumo de ração diário. Houve efeito dos planos de nutrição sobre a conversão alimentar, sendo que o plano 3 propiciou melhora de 7,86% em relação ao plano 1, não diferindo dos demais. O consumo de lisina aumentou de acordo com a elevação dos níveis de lisina dos planos de nutrição. No plano 4, o consumo de lisina foi superior em 25,43%, em relação aos demais planos. Não houve diferença dos planos de nutrição sobre a taxa de conversão em músculo, conversão alimentar em músculo, comprimento de carcaça, espessura de toucinho na região entre a última e a penúltima vértebra lombar, a espessura de toucinho a 6,5 cm da linha dorso-lombar (P2), o rendimento de carcaça, o rendimento de carne magra, o rendimento de gordura e sobre o rendimento de pernil. O estudo da análise econômica indicou que o plano de nutrição 2, proporcionou o melhor retorno econômico sobre o custo da ração. Portanto, conclui-se que o plano de nutrição 3 (1,00 - 0,90% de lisina) proporcionou os melhores resultados de desempenho e características de carcaça.

298. Chest Compression Synchronized Ventilation versus Intermitted Positive Pressure Ventilation during Cardiopulmonary Resuscitation in a Pig Model.

Guidelines recommend mechanical ventilation with Intermitted Positive Pressure Ventilation (IPPV) during resuscitation. The influence of the novel ventilator mode Chest Compression Synchronized Ventilation (CCSV) on gas exchange and arterial blood pressure compared with IPPV was investigated in a pig model.In 12 pigs (general anaesthesia/intubation) ventricular fibrillation was induced and continuous chest compressions were started after 3 min. Pigs were mechanically ventilated in a cross-over setting with 5 ventilation periods of 4 min each: Ventilation modes were during the first and last period IPPV (100% O2, tidal volumes = 7 ml/kgKG, respiratory rate = 10/min), during the 2nd, 3rd and 4th period CCSV (100% O2), a pressure-controlled and with each chest compression synchronized breathing pattern with three different presets in randomized order. Presets: CCSVA: P insp = 60 mbar, inspiratory time = 205 ms; CCSVB: P insp = 60 mbar, inspiratory time = 265 ms; CCSVC: P insp = 45 mbar, inspiratory time = 265 ms. Blood gas samples were drawn for each period, mean arterial (MAP) and central venous (CVP) blood pressures were continuously recorded. Results as median (25%/75%percentiles).Ventilation with each CCSV mode resulted in higher PaO2 than IPPV: PaO2: IPPV first: 19.6(13.9/36.2)kPa, IPPV last: 22.7(5.4/36.9)kPa (p = 0.77 vs IPPV first), CCSVA: 48.9(29.0/58.2)kPa (p = 0.028 vs IPPV first, p = 0.0001 vs IPPV last), CCSVB: 54.0 (43.8/64.1) (p = 0.001 vs IPPV first, p = 0.0001 vs IPPV last), CCSVC: 46.0 (20.2/58.4) (p = 0.006 vs IPPV first, p = 0.0001 vs IPPV last). Both the MAP and the difference MAP-CVP did not decrease during twelve minutes CPR with all three presets of CCSV and were higher than the pressures of the last IPPV period.All patterns of CCSV lead to a higher PaO2 and avoid an arterial blood pressure drop during resuscitation compared to IPPV in this pig model of cardiac arrest.

Embora o acometimento do nervo fibular seja uma das neuropatias mais comuns nos membros inferiores, a lesão bilateral ainda é pouco citada na literatura. Apresentamos uma mulher jovem com doença de nervo fibular comum pouco tempo após a descompressão cirúrgica bem sucedida no membro contralateral. O principal achado é o déficit motor, com a típica marcha escarvante. A eletroneuromiografia continua sendo a propedêutica ideal, considerando atualmente novas modalidades em estudo. Mesmo que em diferentes apresentações, até o momento, a descompressão cirúrgica do nervo fibular parece ser a modalidade terapêutica de melhor resultado.

300. Práticas e cuidados maternos com a saúde bucal do filho: uma visão voltada para a rotina domiciliar

Introdução: O conhecimento, as percepções e atitudes sobre saúde bucal expressas pelas figuras maternas são importantes e merecem atenção. O planejamento de ações e a compreensão dos valores, atitudes e crenças em saúde são fenômenos sociais e biológicos vividos culturalmente. A educação em saúde bucal no ambiente domiciliar é uma opção promissora para o processo educativo devido à facilidade de aprendizagem por parte das crianças. Objetivo: Verificar o conhecimento das mães sobre saúde bucal na infância, observando as práticas e cuidados domiciliares estabelecidos por essas mulheres com a saúde oral de seu filho. Método: Foi uma pesquisa qualitativa e quantitativa que analisou os dados relacionados às práticas e cuidados maternos domiciliares com a higiene bucal de crianças, internadas num Hospital no Distrito Federal – DF. Resultados: Revelou-se 98% das mães responderam que seus filhos têm os dentes limpos quando estão em casa e 2% responderam que não. Obteve-se que 67% das mães executam os procedimentos de higienização bucal domiciliar em seus filhos, 15% é a própria criança. Através da pesquisa obteve-se que 53% das crianças já visitaram o dentista pelo menos uma vez na vida e 47% nunca foram ao dentista. Conclusão: Com o estudo proposto foi possível observar que apesar do grande percentual de crianças nunca terem ido ao dentista existe uma grande adesão às práticas de limpeza bucais domiciliares, sendo a mãe a principal figura familiar responsável pelos cuidados com seu filho.
