Advanced water treatment plants employing ultrafiltration (UF) and reverse osmosis (RO) membrane processes are frequently implemented for the production of high-quality recycled water. It is important that process performance is able to be quantified and assessed to ensure it is fit for purpose. This research utilizes size exclusion chromatography with organic carbon, organic nitrogen and UV(254) detection to determine the change in both DOC concentration and character through a UF/3 stage-RO pilot plant. It was determined that 97% of the influent DOC was removed on average to produce a water of less than 0.5 mg L(-1) as C. The UF process removed more than half of the biopolymer fraction, equating to 4.5% DOC removal, while the RO process generally removed all DOC except a small proportion of the low MW humics and acids and low MW neutral fraction. While not changing significantly in concentration, the Stage 3 RO permeate typically contained low concentrations of humic fraction, indicating a change in character and therefore a change in rejection mechanism. Overall, it was determined that while TOC monitoring is important in advanced water treatment systems, improved understanding of the character of the TOC present lends greater insight into the assessment of process performance.

Improved techniques are required for the detection of inadvertent cross-connections between recycled water and potable water systems in dual reticulation schemes. The aim of this research was to assess the potential for fluorescence spectroscopy to be developed as a tool to distinguish recycled water from potable water. Weekly grab samples of recycled and potable water were obtained over 12 weeks from within an Australian dual reticulation site and analysed for fluorescence excitation-emission matrix (EEM), dissolved organic carbon (DOC), electrical conductivity (EC), and pH. Probabilistic techniques including distribution function fitting and Monte Carlo simulation were used to assess the ability to distinguish between recycled water and potable water sample pairs and the reliability of doing so. Fluorescence EEM spectroscopy was determined to be the most effective for the reliable differentiation by monitoring the protein-like fluorescence at peak T(1)--an excitation-emission wavelength pair of λ(ex/em)=300/350 nm. While EC could distinguish between recycled and potable water, it was shown to be less sensitive and less reliable than peak T(1) fluorescence.

The capacity for removing wastewater-borne endocrine disrupting chemicals (EDCs) was investigated for two wastewater treatment plants (WWTPs) incorporating waste stabilisation ponds (WSPs) as the principal treatment technology. Samples were analysed for a number of steroidal oestrogens and androgens using liquid chromatography-tandem mass spectrometry (LC/MS/MS). Removal efficiency for steroid androgens was high for both WWTPs (93-100%) but WSP treatment was observed to be less effective for removing steroid oestrogens, particularly oestriol.

A rapid, highly sensitive method for detection of cross-connections between recycled and potable water in dual reticulation systems is required. The aim of this research was to determine the potential of fluorescence spectroscopy as a monitoring tool at three Australian dual distribution (drinking and recycled water) systems. Weekly grab samples of recycled and potable water were obtained over 12 weeks at each site and analysed for fluorescence excitation-emission matrix (EEM) spectroscopy, UV(254), dissolved organic carbon (DOC), electrical conductivity and pH. Fluorescence EEM spectroscopy was able to differentiate between recycled and potable water at each site by monitoring the protein-like fluorescence at peak T-an excitation-emission wavelength pair of lambda(ex/em) = 300/350 nm. While electrical conductivity was also able to distinguish between recycled and potable water, the differentiation was greatest when using fluorescence. For example, the peak T fluorescence in recycled water was up to 10 times that of potable water in comparison with electrical conductivity that had a maximum 5 times differentiation. Furthermore, by comparing the protein-like fluorescence at peak T and humic-like fluorescence at peak A (lambda(ex/em) = 235/426 nm), the three different recycled water systems were able to be differentiated. Overall, fluorescence shows promise as a monitoring tool for detecting cross-connections.

We study the assembly of ligated gold nanoparticles by both phenomenological modeling and computer simulations for various ligand chain lengths. First, we develop an effective nanoparticle-nanoparticle pair potential by treating the ligands as flexible polymer chains. Besides van der Waals interactions, we incorporate both the free energy of mixing and elastic contributions from compression of the ligands in our effective pair potentials. The separation of the nanoparticles at the potential minimum compares well with experimental results of gold nanoparticle superlattice constants for various ligand lengths. Next, we use the calculated pair potentials as input to Brownian dynamics simulations for studying the formation of nanoparticle assembly in three dimensions. For dodecanethiol ligated nanoparticles in toluene, our model gives a relatively shallower well depth and the clusters formed after a temperature quench are compact in morphology. Simulation results for the kinetics of cluster growth in this case are compared with phase separations in binary mixtures. For decanethiol ligated nanoparticles, the model well depth is found to be deeper, and simulations show hybrid, fractal-like morphology for the clusters. Cluster morphology in this case shows a compact structure at short length scales and a fractal structure at large length scales. Growth kinetics for this deeper potential depth is compared with the diffusion-limited cluster-cluster aggregation (DLCA) model.

A membrane bioreactor (MBR) was assessed for the removal of estrogens, androgens, and a selection of pharmaceuticals and personal care products. The biomass and aqueous components of the MBR were investigated to determine whether removal was by biodegradation or by adsorption to the biomass. Removal was monitored using chemical analysis by gas chromatography/mass spectrometry (GC-MS) as well as biological analysis using estrogenic and androgenic yeast assays. Results showed that the MBR was effective in removing the compounds of concern from raw influent with removal rates between 78 and 99%. Removal efficiencies were comparable or better than those reported for conventional activated sludge systems, which was attributed to the relatively high sludge retention time of the MBR. The biomass component showed significant concentrations of salicylic acid, triclosan, and 4-tert-octylphenol. Estrogenic and androgenic activity was also measured in the biomass. Estrone was identified as the main compound responsible for the estrogenic activity. It was concluded that the main removal pathway was biodegradation, but sorption to biomass may also be important, particularly for triclosan and 4-tert-octylphenol.

The focus of this study was to empirically estimate the specific cake resistance (SCR) by the variation in shear intensity (G) in four laboratory-scale MBRs. The control reactor (MBR(0)) was operated with aeration only while other MBRs (MBR(150), MBR(300) and MBR(450)) were operated with aeration and mechanical mixing intensities of 150, 300 and 450 rpm, respectively. It was found that the SCR was strongly correlated (R(2)=0.99) with the fouling rates in the MBRs. Moreover, the contribution of cake resistance (R(c)) to the total hydraulic resistance (R(t)) was predominant compared to the irreversible fouling resistance (R(f)). On this basis, the cake filtration model was selected as a predictive tool for membrane fouling. This model was modified by replacing the SCR with its empirical shear intensity relationship. The modified model can predict the fouling rate for a given shear intensity (G) within 80 and 250 s(-1) in a MBR system.

Objective: To investigate the prevalence of kanamycin (kan) and ampicillin (amp) resistant bacteria in public drinking water., Methods: Bacteria containing kan and amp resistant genes were amplified by PCR and further characterized by colony hybridization and transformation studies. The genus of kan and amp resistant bacteria was determined with standard methods., Results: Among the 625 drinking water samples, 400 contained kan and amp resistant bacteria and the percentage was 42.5% and 57.5%, respectively, which was further confirmed by the amplification of a 810 bp kan resistant gene and a 850 bp amp resistant gene. Of the 170 kan resistant bacteria, 90 were Gram negative and 80 were Gram positive. Of the 230 amp resistant bacteria, 160 were Gram negative while 70 were Gram positive. Salmonella, Shigella, Staphylococcus, Streptococcus, and E.coli were detected as 13%, 11%, 17%, 30%, and 29%, respectively. Bacterial strain DH5alpha transformed with plasmids isolated from kan and amp resistant bacteria confirmed that the antibiotic resistant genes were mediated by plasmids., Conclusion: Drinking water is contaminated with kan and amp resistant bacteria due to poor sanitary conditions.

We present results from detailed three-dimensional Brownian dynamics simulations of the self-assembly process in quenched short-range attractive colloids. Clusters obtained in the simulations range from dense faceted crystals to fractal aggregates which show ramified morphology on large length scales but close-packed crystalline morphology on short length scales. For low volume fractions of the colloids, the morphology and crystal structure of a nucleating cluster are studied at various times after the quench. As the volume fraction of the colloids is increased, growth of clusters is controlled by cluster diffusion and cluster-cluster interactions. For shallower quenches and low volume fractions, clusters are compact and the growth-law exponent agrees well with Binder-Stauffer predictions and with recent experimental results. As the volume fraction is increased, clusters do not completely coalesce when they meet each other and the kinetics crosses over to diffusion-limited cluster-cluster aggregation (DLCA) limit. For deeper quenches, clusters are fractals even at low volume fractions and the growth kinetics asymptotically reaches the irreversible DLCA case.

A rapid, highly sensitive and selective detector is urgently required to detect contamination events in recycled water systems - for example, cross-connection events in dual reticulation pipes that recycle advanced treated sewage effluent - as existing technologies, including total organic carbon and conductivity monitoring, cannot always provide the sensitivity required. Fluorescence spectroscopy has been suggested as a potential monitoring tool given its high sensitivity and selectivity. A review of recent literature demonstrates that by monitoring the fluorescence of dissolved organic matter (DOM), the ratios of humic-like (Peak C) and protein-like (Peak T) fluorescence peaks can be used to identify trace sewage contamination in river waters and estuaries, a situation analogous to contamination detection in recycled water systems. Additionally, strong correlations have been shown between Peak T and biochemical oxygen demand (BOD) in rivers, which is indicative of water impacted by microbial activity and therefore of sewage impacted systems. Hence, this review concludes that the sensitive detection of contamination events in recycled water systems may be achieved by monitoring Peak T and/or Peak C fluorescence. However, in such systems, effluent is treated to a high standard resulting in much lower DOM concentrations and the impact of these advanced treatment processes on Peaks T and C fluorescence is largely unknown and requires investigation. This review has highlighted that further work is also required to determine (a) the stability and distinctiveness of recycled water fluorescence in relation to the treatment processes utilised, (b) the impact of matrix effects, particularly the impact of oxidation, (c) calibration issues for online monitoring, and (d) the advanced data analytical techniques required, if any, to improve detection of contamination events.

Fluorescence excitation-emission matrix (EEM) spectroscopy was used to distinguish between two stages of reverse osmosis (RO) permeates as the first step towards investigating the potential application of fluorescence as a monitoring tool for membrane performance. The signal response of several fluorescence peaks present in Stage 1 and Stage 2 RO permeates of an advanced water treatment plant were compared. The humic-like fluorescence region was found to have the largest percentage difference between stages and therefore was the most appropriate for enabling differentiation. Increases in humic-like fluorescence did not correlate with increases in conductivity or dissolved organic carbon measurements. This suggests that fluorescence is a more selective and sensitive method for monitoring the organic composition of RO permeates than established methods. Fluorescence is therefore a promising tool for improved water quality monitoring of RO permeates.

Objective: To investigate the types of hydrocarbon ingested by children and identify factors associated with hydrocarbons ingestion., Patients and Methods: It was a hospital based case control study in which medical records of sixty seven children with hydrocarbon ingestion, admitted through emergency department between January 2001 to December 2005 of Aga Khan Hospital were reviewed. Variables such as age, sex, types of hydrocarbons, amount ingested, socioeconomic status, family size, number of children, type of containers, trend of ingestion during hot weather, length of stay at hospital along with the outcomes were evaluated., Results: Out of 67 patients, 53 (79%) were male and 14 (21%) were female. Majority of children 36 (54%) were between the ages 2-5 years. Kerosene oil 59 (88%) was the most commonly ingested hydrocarbons. Socioeconomically 48 (71%) children belonged to lower middle class. Children with large family size (> or = 3 siblings/family) were more commonly affected. Hydrocarbon were mostly 41 (61%) stored in beverages and mineral water bottles. The accident occurred in 43 (65%) during summer, whereas 34 (56%) patients had presented with fever and cough. Consolidation of lungs was found in 38 (56%) cases. Majority 53 (79%) of the patients were discharged from the hospital within the first 24 hours of admission. Male, age < 2 years, large family size, poor socioeconomic status, hot weather (afternoon and summer vacations), kerosene oil, unsafe containers were the major factors loading to hydrocarbon ingestion in this study., Conclusion: There is a need for strategic planning with parent awareness programs to reduce the hydrocarbon poisoning in our children.

Commercial feedlots for beef cattle finishing are potential sources of a range of trace chemicals which have human health or environmental significance. To ensure adequate protection of human and environmental health from exposure to these chemicals, the application of effective manure and effluent management practices is warranted. The Australian meat and livestock industry has adopted a proactive approach to the identification of best management practices. Accordingly, this review was undertaken to identify key chemical species that may require consideration in the development of guidelines for feedlot manure and effluent management practices in Australia. Important classes of trace chemicals identified include steroidal hormones, antibiotics, ectoparasiticides, mycotoxins, heavy metals and dioxins. These are described in terms of their likely sources, expected concentrations and public health or environmental significance based on international data and research. Androgenic hormones such as testosterone and trenbolone are significantly active in feedlot wastes, but they are poorly understood in terms of fate and environmental implications. The careful management of residues of antibiotics including virginiamycin, tylosin and oxytetracycline appears prudent in terms of minimising the risk of potential public health impacts from resistant strains of bacteria. Good management of ectoparasiticides including synthetic pyrethroids, macrocyclic lactones, fluazuron, and amitraz is important for the prevention of potential ecological implications, particularly towards dung beetles. Very few of these individual chemical contaminants have been thoroughly investigated in terms of concentrations, effects and attenuation in Australian feedlot wastes.

There are limited studies on the fate and levels of endocrine disrupting chemicals in sewage treatment plants in Australia. Research undertaken in Europe and North America has shown biologically significant levels of both oestrogenic and androgenic chemicals in sewage effluent. The aim of this work was to determine the oestrogenic and androgenic activities of raw and treated sewage from sewage treatment plants run by MidCoast Water, New South Wales, Australia. Oestrogenic and androgenic activities were measured using a yeast screen bioassay. Results showed that the raw effluent contained biologically significant levels of both oestrogenic (0.58-2.91 ng/l) and androgenic (216-480 ng/l) activities. Androgenic activity was significantly higher than oestrogenic activity, which was consistent with other Australian studies and was attributed to the higher levels of androgens in domestic waste from human excretion compared to oestrogens. Secondary treatment (using activated sludge) removed the majority of the oestrogenic and androgenic activity (up to 99%). Tertiary treatment by UV removed varying levels of oestrogenic (19-69%) and androgenic (5-55%) activities. A Membrane Bioreactor (MBR) at one of the STPs, which consists of an MBR followed by electrochlorination removed over 87% of the oestrogenic activity and over 98% of androgenic activity from raw sewage samples. However, levels which could be biologically significant still remained after secondary and tertiary treatment (>0.1 ng/l oestrogenic activity and >1 ng/l androgenic activity)., (Copyright (c) IWA Publishing 2008.)

Telomeres are DNA-protein complexes at the ends of eukaryotic chromosomes, the integrity of which is essential for chromosome stability. An important telomere binding protein, TTAGGG repeat factor 2 (TRF2), is thought to protect telomere ends by remodeling them into T-loops. We show that TRF2 specifically interacts with telomeric ss/ds DNA junctions and binding is sensitive to the sequence of the 3', guanine-strand (G-strand) overhang and double-stranded DNA sequence at the junction. Association of TRF2 with DNA junctions hinders cleavage by exonuclease T. TRF2 interactions with the G-strand overhang do not involve the TRF2 DNA binding domain or the linker region. However, mobility shifts and atomic force microscopy show that the previously uncharacterized linker region is involved in DNA-specific, TRF2 oligomerization. We suggest that T-loop formation at telomere ends involves TRF2 binding to the G-strand overhang and oligomerization through both the known TRFH domain and the linker region.

Objective: To identify the causes of blindness at the Ida Rieu school for the blind and deaf, Karachi, Pakistan., Methods: A cross sectional study was conducted at the Ida Rieu School for the blind and deaf. The data collected from medical record of students was entered into the WHO/PBL eye examination form for children with blindness and low vision., Results: Records of 144 pupils aged between 4-30 years were reviewed, including 67% males and 33% females. One third (31%) children had visual impairment (< 6/18-6/60) and 69% were blind (< 3/60-NPL). The commonest anatomical site was retina (41%) and whole globe (20%). The etiology was unknown in 49% cases. In 33% of cases, the data suggested hereditary cause as the etiology, 40% of cases were preventable and 13% treatable., Conclusion: Avoidable causes of blindness were seenin 53% of children, 58% of which were preventable and 19 were treatable.

[reaction: see text] Mixing of equimolar amounts of cyclobis(paraquat-p-phenylene) (CBPQT(4+)) with a bis-4-methylphenyl ether (MPE twice) of a 1,5-dioxynaphthalene (DNP) derivative in MeCN/CH(2)Cl(2) (3:1) results in the formation of a [2]pseudorotaxane which, on crystallization, yields a [4]pseudorotaxane in the solid state that is stabilized by multiple [C-H...F] interactions: a mixture of the same components in a 1:3 ratio affords a crystalline [2]pseudorotaxane after vapor diffusion of methyl-tert-butyl ether into a solution of these components in MeCN/CH(2)Cl(2) (3:1).

Trace concentrations of small soluble epoxides are suspected byproducts of drinking water ozonation. However, adequate characterization of epoxide formation is currently limited by the lack of suitable analytical methods to target these chemicals in dilute, but complex aqueous solutions. One potential approach is presented here based on aqueous-phase aminolysis. The method also employs solid-phase extraction, silylation of the solvent extract, and analysis by gas chromatography-mass spectrometry. This approach is demonstrated to be effective for the selective analysis of the epoxides 1,2-epoxybutane, epichlorohydrin, and epifluorohydrin in water with optimized method detection limits of 5-10 ng/L.

Objective: To determine the level of awareness about five common diseases, namely: Tuberculosis (TB), Typhoid, Hepatitis B, Hepatitis C and HIV/AIDS among college female students of Karachi., Methods: A cross-sectional survey of female students aged 16-21 years from three colleges selected by convenient sampling method was conducted from January to May 2004. Data was collected through a self-administered questionnaire., Results: A large number of students (71%) knew that typhoid spreads by eating contaminated food and drinking infected water. Majority (84%) knew about cough as a mode of spread for TB whereas 69% thought that TB could spread through sneeze of a TB patient. Regarding AIDS, 90% knew that it is sexually transmitted. Majority (87%) knew about the association of hepatitis B and contaminated needles; 64% were aware of hepatitis C and abuse of contaminated needles; 88% knew about the spread of HIV by the use of contaminated needles. A large number (92%) mentioned television as their main source of information., Conclusion: The general level of awareness regarding HIV/AIDS transmission was satisfactory among college girls in this study. The level of awareness of the young educated females about the modes of spread of typhoid, TB, hepatitis B and C is low. This study emphasizes the effectiveness of health education campaign regarding common infectious diseases, especially in young girls.

Telomeres protect chromosome ends by assembling unique protein-DNA complexes. TRF2 is a telomere binding protein that is involved in protecting the G-strand overhang, a 3', guanine-rich, overhang at the telomere terminus. TRF2 may protect the G-strand overhang by recognizing some organizational aspect of the telomeric single-stranded/double-stranded (ss/ds) DNA junction. This work demonstrates that TRF2, purified or in crude extracts, recognizes telomeric ss/ds DNA junctions containing wild type telomeric sequence in the ds region and a G-strand overhang with at least one telomeric repeat. Telomeric complexes containing TRF2 and pot1 assemble less efficiently when the G-strand overhang is in the form of an intramolecular G-quadruplex. However, recruitment of the DNA repair proteins, WRN, Mre11, and Ku86, is not inhibited by a G-quadruplex. This suggests that an intramolecular G-quadruplex has the potential to disrupt certain telomeric assemblies, but efficient recruitment of appropriate DNA repair proteins provides the means to overcome this obstacle.

Leishmaniasis is a major health problem worldwide. It is also a particular problem in the rural areas of Pakistan. The disease occurs in varying presentations, from the self-limited and even self-healing cutaneous forms to fatal systemic disease. Lesions of cutaneous leishmaniasis may occur anywhere on the body but the most likely sites are the exposed parts. The initial papule rapidly gives rise to an ulcer. Systemic leishmaniasis is rarer in Pakistan and invariably fatal if not treated promptly. It affects the internal body organs, particularly the spleen and the liver. Leishmaniasis is transmitted by an infected female sandfly. Cutaneous lesions are usually single and often self-healing, but a presentation with multiple ulcers resulting from multiple bites from the sandfly is not rare in Pakistan. The disease has a very long history and lesions like leishmaniasis have been described dating back to the ninth century (Balkan sore). Cutaneous leishmaniasis has been given various names in different civilizations such as "Delhi boil" in India, "Baghdad boil" in Iraq, and "saldana" in Afghanistan. The organism responsible for leishmaniasis was discovered 100 years ago but the disease has not been eradicated; rather it is on rise in many parts of the world. If control measures are not taken, it might emerge as a major health problem. Pakistan has a burden of cutaneous and visceral leishmaniasis, the mucocutaneous form being almost nonexistent. The physicians need to know the diagnostic criteria as well as the treatment of the disease. Because of a scarcity of dermatologists in the rural areas, most of these cases present to general practitioners. Control of this disease is further complicated by an inadequate supply of appropriate drugs.

The risk of endocrine disrupters to humans and wildlife is to date poorly understood, although evidence of effects is now widespread. In understanding the risk, an important step is the determination of the partitioning, as well as chemical and biochemical transformation, of compounds in the environment, the water cycle and the food chain. This is a complex task and this paper is a first step towards estimating some of these factors from a largely theoretical approach. A chemical fate model is used to predict the fate of the contraceptive drug 17alpha-ethinylestradiol (EE2). The example of the contraceptive pill is chosen to follow the journey of the drug from human ingestion and excretion to treatment in a sewage treatment plant (STP) using fugacity-based fate models, followed by discharge into a receiving river and eventually into the estuary/sea. The model predicts how EE2 will partition into the different compartments during each stage of this journey and thereby infiltrate into the food chain. The results suggest that a person would have to ingest more than 30,000 portions of fish to consume an equivalent to a single average dose of the contraceptive pill. While this scenario is highly unlikely, the biochemical consequence of the contraceptive pill is greatly significant. Furthermore, there are many identified similarly estrogenic compounds in the environment while this study only considers one. Cumulative effects of such compounds as well as degradation into other potent compounds may be anticipated. An important message in this paper is the interrelation of wastewater effluent discharge and eventual human exposure of marginally degradable and lipophilic chemicals. While at present the main concerns regarding endocrine disrupters appear to be the fear of their occurrence in drinking water sources, it is clear that the domains of wastewater treatment and discharge, water supply and contamination of food should not be treated as separate issues. The model suggests that exposure from food (contaminated by effluent) may be much more significant than from drinking water.

A conceptual model is presented for determining which currently prescribed pharmaceutical compounds are most likely to be found in sewage, and for estimating their concentrations, both in raw sewage and after successive stages of secondary sewage treatment. A ranking of the "top-50" pharmaceutical compounds (by total mass dispensed) in Australia over the 1998 calendar year was prepared. Information on the excretion ratios and some metabolites of the pharmaceuticals enabled prediction of the overall rates of excretion into Australian sewage. Mass-balance and fugacity modelling, applied to sewage generation and to a sewage treatment plant, allowed calculation of predicted concentrations of the compounds in raw, primary and secondary treated sewage effluents. Twenty nine of the modelled pharmaceutical residuals were predicted to be present in raw sewage influent at concentrations of 1 microgl(-1) or greater. Twenty of the compounds were predicted to remain in secondary effluent at concentrations of 1 microgl(-1) or greater.

An advanced water recycling demonstration plant was employed to investigate the effectiveness of a number of treatment technologies in the removal of some residuals of commonly prescribed pharmaceuticals as well as natural and synthetic hormones found in sewage. Analysis of targeted compounds was carried out by solid-phase extraction (SPE) and gas chromatography-mass spectrometry (GC-MS). Initial tests were undertaken to determine the background concentrations of the analytes during various stages of treatment. Subsequent tests, undertaken by spiking with standard solutions of the target compounds provided further information on the removal efficiencies of some selected treatment modules. The results of the study indicate that while ozonation, microfiltration and nanofiltration were partially effective, treatment by reverse osmosis was the most universally successful in the removal of the target residuals. While significantly more data is required for a full evaluation, this initial investigation suggests that reverse osmosis may be an effective means of removing a wider range of pharmaceutically active residuals and hormones from treated sewage.

Objective: To evaluate the safety and long-term efficacy of a modified transconjunctival involutional lower eyelid entropion repair., Design: Retrospective, noncomparative case series and survey., Participants: Eighty-nine consecutive patients with involutional entropion (114 eyelids)., Methods: Modified transconjunctival involutional lower eyelid entropion repair technique was performed on 114 eyelids of 89 consecutive patients over a 7-year period. All cases had a minimum of 3 months of initial office follow-up, with extended follow-up obtained via standardized telephone interviews., Main Outcome Measures: Surgical success and entropion recurrence., Results: Surgery was completed successfully with no complications in all 114 cases. All cases demonstrated correction of entropion at the 3-month office follow-up. Long-term follow-up (mean, 38.3 months; range, 4-85 months) was obtained in 75% of cases. Recurrence was noted in only one patient (two eyelids [2%])., Conclusions: Modified transconjunctival lower eyelid entropion repair is a time-efficient, safe, and efficacious technique. In contrast to a recent report using another transconjunctival technique, we found a relatively low rate of recurrence on extended follow-up.

A general procedure was developed for estimating the concentrations of pharmaceutical residues in fresh primary and secondary sewage sludge. Prescribed quantities coupled with information on the various excretion ratios of 20 pharmaceuticals and 2 of their metabolites enabled prediction of the overall rates of excretion into Australian sewage. Fugacity modelling was applied to predict concentrations of these residues in fresh primary and secondary sludge. Predicted concentrations ranged from 10(-3)-884 microg/L in primary sludge and 10(-4)-36 microg/L in secondary sludge. Overall rates of removal to sludges ranged from 1-39%. The accuracy of the model was verified by comparison to analytical data.

The number of interstitial macrophages in the testis fluctuates according to age, increasing gradually during prepubertal development to reach 15-20% in the interstitial compartment in the adult rat. These macrophages are in close morphological association with Leydig cells. Macrophage products, interleukin-1 (IL-1) and tumor necrosis factor alpha stimulate and/or inhibit steroid production in cultures of Leydig cells. We have studied the effects of macrophage products on DNA synthesis in rat Leydig cells to investigate a possible paracrine role of testicular macrophage products on the proliferation of Leydig cells. Leydig cells isolated from 10-, 20-, and 70-day-old rats were cultured for 48 h in serum-free medium, washed, and treated with different cytokines for 18 h. The medium was then removed, fresh medium containing 0.5 microCi [3H]thymidine was added, and cells were incubated for 4 h prior to determining the incorporation of [3H]thymidine into DNA. Human recombinant IL-1 beta caused a dose-dependent stimulation in the incorporation of [3H]thymidine into DNA in the Leydig cells from 10- and 20-day-old rats but had no effect on DNA synthesis in interstitial cells from adult rats. Maximum stimulation of DNA synthesis in immature Leydig cells was observed with 1-2 ng/ml IL-1 beta. Autoradiography after incubation with [3H]thymidine showed a dramatic increase in the number of labeled Leydig cells after treatment with IL-1 beta (19.27 +/- 3.77% vs. 1.44 +/- 0.52% in control cultures) indicating that IL-1 beta recruited more cells to enter the cell cycle and initiate DNA synthesis. Human recombinant IL-1 alpha and tumor necrosis factor alpha also caused significant stimulation of DNA synthesis in Leydig cells but these cytokines were much less potent (1-10%) than IL-1 beta. IL-1 beta enhanced the effects of maximally effective concentrations of growth-promoting agents previously known to stimulate DNA synthesis in immature rat Leydig cells, i.e. human CG, steroidogenesis-inducing protein, and transforming growth factor alpha plus insulin. On the basis of these results it is concluded that IL-1 may play an important role in the proliferation of Leydig cells during prepubertal development in immature rats.

Cholecystokinin (CCK)-gastrin peptides are shown to be present in the brain of all the species ranging from coelenterates to mammals. Differentiation between CCK and gastrin, presumed to be evolved from a common ancestral cerulein-like peptide, has been suggested to occur at the level of the amphibians. We examined the presence of bioassayable CCK, as determined by its potency to stimulate enzyme secretion from isolated rat pancreatic acini, in the brain of the goldfish Carassius auratus, a more primitive vertebrate than the amphibian. Among the various regions tested, the brain stem, telencephalon and spinal cord possess the highest levels of bioassayable CCK followed in decreasing order by gustatory lobes, optic tectum and duodenum. No detectable levels of CCK were found in the cerebellum. The observed bioactivity was not due to gastrin because: radioimmunoassay of the brain homogenates for gastrin revealed very low or nondetectable levels of gastrin; amylase release dose-response curves for standard CCK8 and the brain homogenate were identical; and proglumide, a competitive antagonist of CCK8, inhibited homogenate CCK-induced enzyme release with a parallel rightward shift in the dose-response curve. These observations provide evidence for the distinct presence of CCK in the brain of the goldfish suggesting that the differentiation of CCK as a distinct neuropeptide, from that of gastrin, occurs at the level of Osteichthyes (bony fish).
