Abstract. Background:. The potential impact of pre-existing coronary artery stenosis (CAS) on acute pulmonary embolism (PE) episodes remains underexplored. This study aimed to investigate the association between pre-existing CAS and the elevation of high-sensitivity cardiac troponin I (hs-cTnI) levels in patients with PE. Methods:. In this multicenter, prospective case-control study, 88 cases and 163 controls matched for age, sex, and study center were enrolled. Cases were patients with PE with elevated hs-cTnI. Controls were patients with PE with normal hs-cTnI. Coronary artery assessment utilized coronary computed tomographic angiography or invasive coronary angiography. CAS was defined as ≥50% stenosis of the lumen diameter in any coronary vessel >2.0 mm in diameter. Conditional logistic regression was used to evaluate the association between CAS and hs-cTnI elevation. Results:. The percentage of CAS was higher in the case group compared to the control group (44.3% [39/88] vs. 30.1% [49/163]; P = 0.024). In multivariable conditional logistic regression model 1, CAS (adjusted odds ratio [OR], 2.680; 95% confidence interval [CI], 1.243–5.779), heart rate >75 beats/min (OR, 2.306; 95% CI, 1.056–5.036) and N-terminal pro-B type natriuretic peptide (NT-proBNP) >420 pg/mL (OR, 12.169; 95% CI, 4.792–30.900) were independently associated with elevated hs-cTnI. In model 2, right CAS (OR, 3.615; 95% CI, 1.467–8.909) and NT-proBNP >420 pg/mL (OR, 13.890; 95% CI, 5.288–36.484) were independently associated with elevated hs-cTnI. Conclusions:. CAS was independently associated with myocardial injury in patients with PE. Vigilance towards CAS is warranted in patients with PE with elevated cardiac troponin levels.

Abstract To conduct a systematic review of the literature regarding rates and predictors of favorable seizure outcome after resective surgery for epileptic spasms (ES) in pediatric patients. The Preferred Reporting Items for Systematic Reviews and Meta‐Analyses standards were followed. We searched PubMed, EMBASE, and Cochrane CENTRAL for articles published on the prevalence or incidence of epileptic spasm since 1985. Abstract, full‐text review, and data extraction were conducted by two independent reviewers. Meta‐analysis was performed to assess overall seizure freedom rate. Subject‐level analysis was performed on a subset of studies to identify prognostic indicators. A total of 21 retrospective studies (n = 531) were included. Meta‐analysis of all studies demonstrated a pooled seizure freedom rate of 68.8%. Subject‐level analysis on 18 studies (n = 360) demonstrated a significant association between duration of spasms and recurrence of spasms after surgery, with an estimated increased risk of 7% per additional year of spasms prior to operation. Patients who underwent resective surgery that was not a hemispherectomy (i.e., lobectomy, lesionectomy, etc.) had an increased recurrence risk of 57% compared to patients who had undergone hemispherectomy. Resective surgery results in seizure freedom for the majority of pediatric patients with epileptic spasms. Patients who undergo hemispherectomy have lower risk of recurrence than patients who undergo other types of surgical resection. Increased duration of spasms prior to surgery is associated with increased recurrence risk after surgery. Plain Language Summary Children with epileptic spasms (ES) that do not respond to medications may benefit from surgical treatment. Our study reviewed existing research to understand how effective surgery is in treating ES in children and what factors predict better outcomes. Researchers followed strict guidelines to search for and analyze studies published since 1985, finding 21 studies with a total of 531 patients. They found that, on average, nearly 70% of children became seizure‐free after surgery. Further individual analysis of 360 patients showed that longer duration of spasms before surgery increased the risk of spasms returning by 7% per year. Additionally, children who had less extensive surgeries, such as removal of only a specific part of the brain, had a 57% higher risk of seizure recurrence compared to those who had a hemispherectomy, which removed or disconnected half of the brain. Overall, the study concludes that surgery can often stop seizures, especially when more extensive surgery is performed and when the surgery is done sooner rather than later.

Abstract Aims Vericiguat has been used to treat patients with heart failure with reduced ejection fraction (HFrEF) who demonstrated worsening heart failure despite treatment with other guideline‐directed medical therapies. The haemodynamic effects of vericiguat remain unclear. Methods and results This study enrolled 12 patients (median age, 63 [quartiles 53.5, 70] years; 16.7%(N=2) women) with symptomatic HFrEF (New York Heart Association functional class II–IV) who demonstrated worsening heart failure despite treatment with the four foundational guideline‐recommended therapies between March and December 2022, with follow‐ups completed in June 2023. A balloon‐tipped pulmonary artery thermodilution catheter was placed in the right internal jugular vein to perform right heart catheterisation (RHC) on day 1. Haemodynamic data were acquired before and after vericiguat intake (2.5 mg) on days 2 and 3. The data on days 2 and 3 were averaged. RHC was repeated on day 105 (37, 168). The 2022 MW6.7 Menyuan earthquake ruptured the western end of the Tianzhu seismic gap, providing an opportunity to study the regional seismogenic characteristics and seismic hazards. Here we use interferometric synthetic aperture radar (InSAR) and seismic data to study the mainshock rupture, early afterslip and the second largest aftershock of the 2022 Menyuan earthquake sequences. Our modeling results show that the mainshock ruptured the Lenglongling fault and the Tuolaishan fault with a maximum slip of ∼3 m. Rapid postseismic transient deformation occurred at the center of the Lenglongling fault. Our afterslip modeling reveals that the majority of afterslip occurred in the deeper part of the Lenglongling fault. A high-angle conjugated faulting event is found at the middle section of the Lenglongling fault. We use the stress inversion to investigate the possible triggering mechanism of the conjugated rupture event. The results indicate the maximum principal stress direction is in ∼222°, forming a ∼22° angle between the conjugated fault of second largest aftershock and the mainshock. The calculated normal stress changes indicate the region is within a pull-apart stress field, which favors such a conjugated rupturing event. Our study will help understand the rupture behavior of such kind of conjugated fault in other regions.

Microcirculatory disturbances are complex processes caused by many factors, including abnormal vasomotor responses, decreased blood flow velocity, vascular endothelial cell injury, altered leukocyte and endothelial cell interactions, plasma albumin leakage, microvascular hemorrhage, and thrombosis. These disturbances involve multiple mechanisms and interactions among mechanisms that can include energy metabolism, the mitochondrial respiratory chain, oxidative stress, inflammatory factors, adhesion molecules, the cytoskeleton, vascular endothelial cells, caveolae, cell junctions, the vascular basement membrane, neutrophils, monocytes, and platelets. In clinical practice, aside from drugs that target abnormal vasomotor responses and platelet adhesion, there continues to be a lack of multi-target drugs that can regulate the complex mechanistic links and interactions underlying microcirculatory disturbances. Natural products have demonstrated obvious positive therapeutic effects in treating ischemia/reperfusion (I/R)- and lipopolysaccharide (LPS)-induced microcirculatory disturbances. In recent years, numerous research papers on the improvement of microcirculatory function by natural products have been published in international journals. In 2008 and 2017, the first listed author of this review was invited to publish reviews in the journal of Pharmacology & Therapeutics on the improvement of microcirculatory disturbances and organ injury induced by I/R using Salvia miltiorrhiza ingredients and other natural components of compounded Chinese medicine, respectively. This review systematically summarizes the effects, targets of action, and mechanisms of natural products regarding improving I/R- and LPS-induced microcirculatory disturbances and tissue injury. Based on this summary, scientific proposals are suggested for the discovery of new drugs to improve microcirculatory disturbances in disease.

Abstract While the surface-bulk correspondence has been ubiquitously shown in topological phases, the relationship between surface and bulk in Landau-like phases is much less explored. Theoretical investigations since 1970s for semi-infinite systems have predicted the possibility of the surface order emerging at a higher temperature than the bulk, clearly illustrating a counterintuitive situation and greatly enriching phase transitions. But experimental realizations of this prediction remain missing. Here, we demonstrate the higher-temperature surface and lower-temperature bulk phase transitions in CrSBr, a van der Waals (vdW) layered antiferromagnet. We leverage the surface sensitivity of electric dipole second harmonic generation (SHG) to resolve surface magnetism, the bulk nature of electric quadrupole SHG to probe bulk spin correlations, and their interference to capture the two magnetic domain states. Our density functional theory calculations show the suppression of ferromagnetic-antiferromagnetic competition at the surface is responsible for this enhanced surface magnetism. Our results not only show counterintuitive, richer phase transitions in vdW magnets, but also provide viable ways to enhance magnetism in their 2D form.

Abstract Background Z-DNA binding protein 1 (ZBP1) is a nucleic acid sensor that is involved in multiple inflammatory diseases, but whether and how it contributes to osteoarthritis (OA) are unclear. Methods Cartilage tissues were harvested from patients with OA and a murine model of OA to evaluate ZBP1 expression. Subsequently, the functional role and mechanism of ZBP1 were examined in primary chondrocytes, and the role of ZBP1 in OA was explored in mouse models. Results We showed the upregulation of ZBP1 in articular cartilage originating from OA patients and mice with OA after destabilization of the medial meniscus (DMM) surgery. Specifically, knockdown of ZBP1 alleviated chondrocyte damage and protected mice from DMM-induced OA. Mechanistically, tumor necrosis factor alpha induced ZBP1 overexpression in an interferon regulatory factor 1 (IRF1)-dependent manner and elicited the activation of ZBP1 via mitochondrial DNA (mtDNA) release and ZBP1 binding. The upregulated and activated ZBP1 could interact with receptor-interacting protein kinase 1 and activate the transforming growth factor-beta-activated kinase 1-NF-κB signaling pathway, which led to chondrocyte inflammation and extracellular matrix degradation. Moreover, inhibition of the mtDNA-IRF1-ZBP1 axis with Cyclosporine A, a blocker of mtDNA release, could delay the progression of DMM-induced OA. Conclusions Our data revealed the pathological role of the mtDNA-IRF1-ZBP1 axis in OA chondrocytes, suggesting that inhibition of this axis could be a viable therapeutic approach for OA.

Abstract Background The effects of traction forces at different angles on impacted central incisors(ICI)with varying inverted angles (IA) may be different. The objective of this study was to analyze the biomechanical effects of different force directions (FD) on developmentally inverted ICI with multi-angle variations and to offer insights and guidance for the treatment of inverted ICI. Methods Three-dimensional finite element method was employed to simulate clinical scenarios of inverted ICI traction. As such, 0.2 N of force (direction: antero-superior angles of 90°, 100°, 110°, 120°, and 130° relative to the long axis of the inverted ICI crown) was applied to the inverted ICI with inverse angles (IA) of 40°, 30°, 20°, 10° and 0°. Inverted ICI apical displacement and Von Mises stress on periodontal ligament (PDL) and alveolar bone were compared. Results IA and FD showed minimal influence on the stress distribution in the PDL, as higher stresses were concentrated in the apical region. The higher stresses in the alveolar bone are focused on the cervical and apical regions of the tooth. In particular, IA exerts a more significant impact on stress distribution in the alveolar bone than FD. The influence of IA on the apical displacement of inverted ICI is larger than that of FD. Conclusions To promote the health of the root and periodontal tissues, it is recommended to use an angle of 100°-110° relative to the long axis of the ICI crown when dealing with a large IA (> 20°) developmentally inverted ICI. Conversely, an angle of 110°-120° can be used.

Background/purpose: Oral lichen planus (OLP) is a chronic inflammatory disease with unknown mechanisms of pathogenesis. Keratin 17 (KRT17) is a protein that regulates numerous cellular processes. This study aimed to explore the expression of KRT17 in OLP and its correlation with the severity of OLP. Materials and methods: RNA sequencing using epithelium from 5 OLP patients and 5 health control (HC) was performed, followed by functional analysis. The validation cohort of 20 OLP and 20 HC tissues were used to investigate positive area value of KRT17 by immunohistochemical analysis. Reticular, erosive and ulcerative (REU) scores were used for measuring the severity of OLP. Results: A total of 15493 genes were detected, of which 1492 genes were significantly up-regulated in OLP and 622 were down-regulated. The mRNA expression of KRT17 was elevated by 13.09-fold in OLP compared to that in HC. Pathway analysis demonstrated high KRT17 expression was associated with multiple biological processes. Abstract The correlation between lower psoas mass and the prognosis of osteoporotic vertebral compression fractures (OVCF) is still unclear. This study aims to investigate the impact of lower psoas mass on the prognosis of patients undergoing percutaneous vertebroplasty (PVP). One hundred and sixty-three elderly patients who underwent single-segment PVP from January 2018 to December 2021 were included. The psoas to L4 vertebral index (PLVI) via MRI were measured to assess psoas mass. Patients were divided into high PLVI (> 0.79) and low PLVI (≤ 0.79) groups based on the median PLVI in the cohort. The basic information (age, gender, body mass index (BMI) and bone mineral density (BMD)), surgical intervention-related elements (duration of operation, latency to ambulation, period of hospital stay, and surgical site), postoperative clinical outcomes (Visual Analog Scale (VAS) scores, Oswestry Disability Index (ODI) scores, Japanese Orthopaedic Association (JOA) scores), and incidence of secondary fractures) were compared. Abstract Numerous policies have been implemented to advance the growth of renewable energy. Nonetheless, certain policies have not yielded the anticipated impact on the progression of renewable energy development. In order to maximize the promotion effect of renewable energy policies, this study proposes a capacity allocation optimization method of wind power generation, solar power and energy storage in power grid planning under different policy objectives. First, based on the policy quantification, grey relation analysis (GRA) is used to calculate the correlation degree of the policy indicators on the planning capacity of renewable energy. Further, a multi‐objective capacity estimation model for wind, solar and energy storage is comprehensively presented. Some highly correlated policy indicators are transformed into the special constraints. And the economy and the stability of the power grid are integrated as the objective function. Meanwhile, the carbon trading and punishment for wind power and solar power abandonment are considered. Finally, the proposed model is solved by NSGA‐II‐PSO (particle swarm optimization) algorithm. The novelty of the algorithm is that the crossover operation of NSGA‐II is replaced by the position updating of particle swarm. The calculation result of the case study can effectively evaluate the optimal planning capacity of renewable energy under different policies, while ensuring the economic and the stability of the power system. The study can provide the reasonable basis and the valid analytical method for the policy formulation and the renewable energy development.

Abstract Humic substances (HSs), as the “black gold” of global agriculture, have a crucial environmental significance in cleaning harmful compounds, maintaining carbon mass balance, and increasing crop productivity. Unfortunately, the resources of HSs extracted from nature are insufficient, causing the supply, demand, and budget of the humus market to gradually increase. Given the analogous architectures of lignin and HSs, artificially regulated humification (ARH) has a great potential for directionally controlling the oxidative cleavage of lignin and/or the radical copolymerization of lignin precursors, to produce reassembled oligomers and/or polymers that can compare favorably with the function and efficacy of HSs. Herein, different ARH strategies are innovatively proposed to tackle hazardous chemicals and create humic-like biostimulators (H-LBs). We briefly describe the characteristics and functionalities of natural HSs, and summarize the latest approaches and mechanisms of lignin and its precursor-based ARH. In particular, the ARH routes of bio-composting, laccase catalysis, white-rot fungal degradation, Fenton-based advanced oxidation, and hydrothermal humification are comparatively highlighted to govern harmful substrates, enhance carbon sequestration, generate H-LBs, and practice sustainable agriculture. Furthermore, the mechanisms for promoting crop growth of H-LBs are also illustrated. Advances in modulating humification pathways may solve the technological bottlenecks of ARH in-situ and ex-situ by well-stocked lignin and its small derivatives, which are important for decontaminating contaminants, reducing CO2 emissions, and enriching H-LBs in agroecosystems.

Abstract The crimping technique is widely used in the manufacturing process of large-diameter longitudinal welded pipe. In the bending process, the springback is the critical problem affecting the shape, which is the key index of forming quality. So, crimping faces more challenges in accurate springback prediction for difficult-to-form metals such as pipeline steels and non-circular nature of the punch. This paper has established an analytical model of crimping springback for the longitudinal welded pipe. Firstly, the crimping process mathematical model which considered the impact of elastic modulus attenuation on springback calculation during plastic deformation is established, and the cross-section bending moment is calculated based on Hill’s bending theory. Secondly, a semi-analytical and semi-numerical method for calculating the crimping shape is presented. Finally, the comparison of the crimping shape of the real product shows that the calculation accuracy and efficiency of this method can meet the requirements of engineering applications. The relative error of the springback angle calculated by the analytical model is −5.84%, and the absolute error is 0.3°. This method can accurately predict the springback of longitudinal welded pipe after crimping and provide a reference for crimping process optimization.
