- Abstract
Empires and Colonies provides a thoroughgoing and lively exploration of the expansion of the seaborne empires of western Europe from the fifteenth century and how that process of expansion affected the world, including its successor, the United States. Whilst providing special attention to Europe, the book is careful to highlight the ambivalence and contradiction of that expansion. The book also illuminates connections between empires and colonies as a theme in history, concentrating on culture while also discussing the rich social, economic and political dimensions of the story. Furthermore, Empires and Colonies recognizes that whilst a study of the expansion of Europe is an important part of world history, it is not a history of the world per se. The focus on culture is used to assert that areas and peoples that lack great economic power at any given time also deserve attention. These alternative voices of slaves, indigenous peoples and critics of empire and colonization are an important and compelling element of the book. Empires and Colonies will be essential reading not only for students of imperial history, but also for anyone interested in the makings of our modern world.

Background: In 2013, the Dahlgren Memorial Library (DML) at the Georgetown University Medical Center began using text mining software to enable its clinical informationists to quickly retrieve specific, relevant information from MEDLINE abstracts while on patient rounds. Description: In 2013, DML licensed the use of the Linguamatics I2E text-mining program, and DML’s clinical informationist began using it to text mine MEDLINE abstracts on patient rounds. In 2015, DML installed I2E on a server at Georgetown and negotiated with Elsevier to obtain the right to download and text mine the full text of clinical journals in ScienceDirect to support clinical decision making. In 2016, the license agreements for the New England Journal of Medicine and the BMJ platform were modified to allow text mining. In 2018, PubMed Central open access content was added to the Linguamatics license. Results: DML’s informationists found that they were able to quickly find useful information that was not retrievable by traditional methods, and clinicians reported the information was valuable. Conclusion: The ability to text mine MEDLINE abstracts and selected journal articles on patient rounds has allowed DML’s clinical informationists to quickly search large amounts of medical literature that can be used to answer physicians’ clinical questions. DML plans to acquire additional journal articles from selected publishers in the future, which should increase the usefulness of the project. Virtual Projects are published on an annual basis in the Journal of the Medical Library Association (JMLA) following an annual call for virtual projects in MLAConnect and announcements to encourage submissions from all types of libraries. An advisory committee of recognized technology experts selects project entries based on their currency, innovation, and contribution to health sciences librarianship.

Political science - Abstract
What characterizes the dynamics of presidential popularity? Research based on the United States of America finds popularity exhibits an almost law-like cyclicality over a president’s term: high post-election “honeymoon” approval rates deteriorate before experiencing an end-of-term boost as new elections approach. We contend that cyclical approval dynamics are not specific to the USA, but rather characteristic of presidential systems more generally, despite heterogeneity in their socio-economic and political contexts. Testing this proposition requires overcoming a key empirical problem: lack of comparable data. We do so by employing time-series inputs from 324 opinion surveys from a new publicly available database—the Executive Approval Database 1.0—to craft quarterly measures of popularity across 18 Latin American contemporary presidential democracies. Our analysis strongly confirms the cyclical approval model for the region. The conclusion identifies avenues for future research on the relationships across approval, presidentialism, and electoral, institutional, and socio-economic factors afforded by the new data resource we present here.

Major histocompatibility complex (MHC) class II molecules present exogenously derived antigen peptides to CD4 T cells, driving activation of naïve T cells and supporting CD4-driven immune functions. However, MHC class II molecules are not inert protein pedestals that simply bind and present peptides. These molecules also serve as multi-functional signaling molecules delivering activation, differentiation, or death signals (or a combination of these) to B cells, macrophages, as well as MHC class II-expressing T cells and tumor cells. Although multiple proteins are known to associate with MHC class II, interaction with STING (stimulator of interferon genes) and CD79 is essential for signaling. In addition, alternative transmembrane domain pairing between class II α and β chains influences association with membrane lipid sub-domains, impacting both signaling and antigen presentation. In contrast to the membrane-distal region of the class II molecule responsible for peptide binding and T-cell receptor engagement, the membrane-proximal region (composed of the connecting peptide, transmembrane domain, and cytoplasmic tail) mediates these “non-traditional” class II functions. Here, we review the literature on the function of the membrane-proximal region of the MHC class II molecule and discuss the impact of this aspect of class II immunobiology on immune regulation and human disease.

In the Latin American context, the Dominican Republic displays high levels of civic engagement and political activism, yet they have not translated into a civil society that is able to exercise considerable influence in reforming the political system or enhancing democracy. Building on this observation, this article explores two questions. The first is if there is tendency toward dual participation in civic and political organizations, which reduces civil society’s ability to pressure political parties and the State. The second refers to the nature of the connections that civil society participants have with the State and with political parties. The analysis demonstrates that an important segment of the population that participates in civic associations also participates in political parties and in clientelist networks.

Background Techniques to preserve the sub-valvular apparatus in order to reduce morbidity and mortality following mitral valve replacement have been frequently reported. However, it is uncertain what impact sub-valvular apparatus preservation techniques have on long-term outcomes following mitral valve replacement. This study investigated the effect of sub-valvular apparatus preservation on long-term survival and quality of life following mitral valve replacement. Methods A microsimulation model was used to compare long-term survival and quality-adjusted life years following mitral valve replacement after conventional valve replacement and sub-valvular apparatus preservation. Probabilistic sensitivity analysis and alternative analysis were performed to investigate uncertainty associated with the results. Results Our Analysis suggests that patients survive longer if the sub-valvular apparatus are preserved (65.7% SD 1.5%, compared with 58.1% SD 1.6% at 10 years). The quality adjusted life years gained over a 10 year period where also greater after sub-valvular apparatus preservation. (6.54 QALY SD 0.07 QALY, compared with 5.61 QALY, SD 0.07 QALY). The superiority of preservation techniques was insensitive to patient age, parameter or model uncertainty. Conclusion This study suggests that long-term outcomes may be improved when the sub-valvular apparatus are preserved. Given the lack of empirical data further research is needed to investigate health-related quality of life after mitral valve replacement, and to establish whether outcomes differ between preservation techniques.

ABSTRACT Objective: Previous series have demonstrated that Clomiphene Citrate (CC) is an effective treatment to increase Total Testosterone (TT) in Late Onset Hypogonadism (LOH) patients. However, what happens to TT levels after ending CC treatment is still debatable. The objective of this study is to evaluate TT levels 3 months after the discontinuation of CC in patients with LOH who were previously successfully treated with the same drug. Materials and Methods: Twenty-seven patients with LOH that were successfully treated (achieved TT levels >11nmol/l) with CC 50mgs daily for 50 days were prospectively recruited in our Andrological outpatient clinic. CC was then stopped for 3 months and TT levels were measured at the end of this period. Results: Mean TT level before discontinuation of CC was 22.7±8.1nmol/L (mean±SD). Three months after discontinuation, mean TT level significantly decreased in all patients, 10.2±3.9nmol/l (p

- Abstract
First published in 1992, Theater and World is a detailed exploration of Shakespeare's representation of history and how it affects the relation between theatre and world. The book focuses primarily on the Second Tetralogy (Richard II, Henry IV Part I, Henry IV Part II, and Henry V) and includes a wealth of analysis and interpretation of the plays. In doing so, it explores a wide range of topics, including the relation between literary and theatrical representations and the world; the nature of illusion and reality; genre; the connection between history and fiction (especially plays); historiography and literary criticism or theory; poetry and philosophy; and irony, both rhetorical and philosophical.Theater and World continues to have lasting relevance for anyone with an interest in Shakespeare's words and his representation of history in particular.

- Abstract
First published in 1996, Explorations in Difference explores how contemporary debates over identity and difference come into play within the workings of cultural, legal, and political institutions. The book brings together a variety of perspectives on the meanings and implications of difference in the context of postmodern theory. It is divided into two parts: ‘Theoretical Accounts', which establishes a context for postmodern inquiries into difference, and ‘Instances', which provides application to particular issues. Highly interdisciplinary, Explorations in Difference continues to have lasting relevance and will appeal to those with an interest in postmodern difference and its implications.

- Abstract
This book of original essays explores three important areas in comparative literature and history and in cultural studies: the boundaries between history and fiction;women as writers and subjects; and the connection between the early modern, modern and postmodern.New history and new literary studies look at innovative ways to see past cultures in a new light. Traditional methods are used to new ends and writers who are familiar within their cultures are translated to other cultures. This study promotes an expanded understanding of our cultural artifacts in a rapidly changing present. It discusses English-speaking culture in the early modern period in the context of other European cultures and relates Europe to other parts of the world, most notably America. After grounding the discussion of culture in history, identity, dialogue as a genre that crosses the boundaries between philosophy and fiction, the rhetoric of prefaces to historical collections, cosmographies and histories that share something with the techniques of literary and forensic rhetoric, the book proceeds to discuss two central issues in cultural studies today: gender and postmodernity. The final section of the book provides a general assessment through early modern texts of modernity and postmodernity.
