259. Diamorphine pharmacokinetics and conversion factor estimates for intranasal diamorphine in paediatric breakthrough pain:systematic review

Abstract

BackgroundIntranasal diamorphine is a potential treatment for breakthrough pain but few paediatric data are available to assist dose estimation.AimTo determine an intranasal diamorphine dose in children through an understanding of pharmacokinetics.DesignA systematic review of the literature was undertaken to seek diamorphine pharmacokinetic parameters in neonates, children and adults. Parenteral and enteral diamorphine bioavailability were reviewed with respect to formation of the major metabolite, morphine. Clinical data quantifying equianalgesic effects of diamorphine and morphine were reviewed.Review sourcesPubMed (1960–2020); EMBASE (1980–2020); IPA (1973–2020) and original human research studies that reported diacetylmorphine and metabolite after any dose or route of administration.ResultsThe systematic review identified 19 studies: 16 in adults and 1 in children and 2 neonatal reports. Details of study participants were extracted. Age ranged from premature neonates to 67 years and weight 1.4–88 kg. Intranasal diamorphine bioavailability was predicted as 50%. The equianalgesic intravenous conversion ratio of morphine:diamorphine was 2:1. There was heterogeneity between pharmacokinetic parameter estimates attributed to routes of administration, lack of size standardisation, methodology and pharmacokinetic analysis. Estimates of the pharmacokinetic parameters clearance and volume of distribution were reduced in neonates. There were insufficient paediatric data to characterise clearance or volume maturation of either diamorphine or its metabolites.ConclusionsWe estimate equianalgesic ratios of intravenous morphine:diamorphine 2:1, intravenous morphine:intranasal diamorphine 1:1 and oral morphine:intranasal diamorphine of 1:3. These ratios are based on adult literature, but are reasonable for deciding on an initial dose of 0.1 mg/kg in children 4–13 years.

Abstract

ObjectivesNo randomised controlled trials have been conducted for breakthrough pain in paediatric palliative care and there are currently no standardised outcome measures. The DIPPER study aims to establish the feasibility of conducting a prospective randomised controlled trial comparing oral and transmucosal administration of opioids for breakthrough pain. The aim of the current study was to achieve consensus on design aspects for a small-scale prospective study to inform a future randomised controlled trial of oral morphine, the current first-line treatment, versus transmucosal diamorphine.MethodsThe nominal group technique was used to achieve consensus on best practice for mode of administration, dose regimen and a range of suitable pain intensity outcome measures for transmucosal diamorphine in children and young people with breakthrough pain. An expert panel of ten clinicians in paediatric palliative care and three parent representatives participated. Consensus was achieved when agreement was reached and no further comments from participants were forthcoming.ResultsThe panel favoured the buccal route of administration, with dosing according to the recommendations in the Association for Paediatric Palliative Medicine formulary (fifth Edition, 2020). The verbal Numerical Rating Scale was selected to measure pain in children 8 years old and older, the Faces Pain Scale-Revised for children between 4 and 8 years old, and Face, Legs, Activity, Cry and Consolability (FLACC)/FLACC-Revised as the observational tools.ConclusionsThe nominal group technique allowed consensus to be reached for a small-scale, prospective, cohort study and provided information to inform the design of a randomised controlled trial.

264. Surface CO2 Exchange Dynamics across a Climatic Gradient in McKenzie Valley: Effect of Landforms, Climate and Permafrost

Abstract

Northern regions are experiencing considerable climate change affecting the state of permafrost, peat accumulation rates, and the large pool of carbon (C) stored in soil, thereby emphasizing the importance of monitoring surface C fluxes in different landform sites along a climate gradient. We studied surface net C exchange (NCE) and ecosystem respiration (ER) across different landforms (upland, peat plateau, collapse scar) in mid-boreal to high subarctic ecoregions in the Mackenzie Valley of northwestern Canada for three years. NCE and ER were measured using automatic CO2 chambers (ADC, Bioscientific LTD., Herts, England), and soil respiration (SR) was measured with solid state infrared CO2 sensors (Carbocaps, Vaisala, Vantaa, Finland) using the concentration gradient technique. Both NCE and ER were primarily controlled by soil temperature in the upper horizons. In upland forest locations, ER varied from 583 to 214 g C·m−2·year−1 from mid-boreal to high subarctic zones, respectively. For the bog and peat plateau areas, ER was less than half that at the upland locations. Of SR, nearly 75% was generated in the upper 5 cm layer composed of live bryophytes and actively decomposing fibric material. Our results suggest that for the upland and bog locations, ER significantly exceeded NCE. Bryophyte NCE was greatest in continuously waterlogged collapsed areas and was negligible in other locations. Overall, upland forest sites were sources of CO2 (from 64 g·C·m−2·year−1 in the high subarctic to 588 g C·m−2·year−1 in mid-boreal zone); collapsed areas were sinks of C, especially in high subarctic (from 27 g·C·m−2 year−1 in mid-boreal to 86 g·C·m−2·year−1 in high subarctic) and peat plateaus were minor sources (from 153 g·C·m−2·year−1 in mid-boreal to 6 g·C·m−2·year−1 in high subarctic). The results are important in understanding how different landforms are responding to climate change and would be useful in modeling the effect of future climate change on the soil C balance in the northern regions.

Abstract

In North India, political leaders are referred to as netās, and the term netāgirī is broadly and pejoratively used to describe the self-promotion, political maneuvering, and public rhetoric in which politicians engage. However, my ethnographic fieldwork in the state of Uttarakhand, India, shows that local divinities can also be netās: they vie for their constituents’ support, make decisions that materially impact people’s lives, and threaten to use force in implementing those decisions. These “political divinities” are routinely encountered as possessed dancers in large-scale public rituals in this region. In this article, I focus on how political divinities affect, and are affected by, audiences in tangible and far-reaching ways. I argue that public possession rituals open up a highly charged zone for inherently fluid, situational, and pragmatic negotiations between humans and divinities. While anthropological studies of possession view it as a sociopolitical event that trades in power relations, this article calls for a rhetorical approach to possession, which foregrounds possession as a way of persuading particular audiences of certain ways of thinking and acting in matters of collective importance.

Abstract

Spontaneous coronary dissections are a well described entity which are often difficult to treat. Intramural hematoma is one type of coronary dissection. Previous case reports have described the treatment for angiographically visible dissection. We describe the first report of diffuse intramural hematoma visualized only on intravascular ultrasound with no angiographically obvious evidence of dissection treated with multivessel drug eluting stenting. This case highlights the importance of a high level of suspicion and atypical presentations of coronary dissection, and the use of multimodality imaging with intravascular ultrasound and computerized tomography for both diagnosis and therapy.

280. 13[C]-urea breath test as a novel point-of-care biomarker for tuberculosis treatment and diagnosis.

Abstract

BACKGROUND:Pathogen-specific metabolic pathways may be detected by breath tests based on introduction of stable isotopically-labeled substrates and detection of labeled products in exhaled breath using portable infrared spectrometers. METHODOLOGY/PRINCIPAL FINDINGS:We tested whether mycobacterial urease activity could be utilized in such a breath test format as the basis of a novel biomarker and diagnostic for pulmonary TB. Sensitized New-Zealand White Rabbits underwent bronchoscopic infection with either Mycobacterium bovis or Mycobacterium tuberculosis. Rabbits were treated with 25 mg/kg of isoniazid (INH) approximately 2 months after infection when significant cavitary lung pathology was present. [(13)C] urea was instilled directly into the lungs of intubated rabbits at selected time points, exhaled air samples analyzed, and the kinetics of delta(13)CO(2) formation were determined. Samples obtained prior to inoculation served as control samples for background (13)CO(2) conversion in the rabbit model. (13)CO(2), from metabolic conversion of [(13)C]-urea by mycobacterial urease activity, was readily detectable in the exhaled breath of infected rabbits within 15 minutes of administration. Analyses showed a rapid increase in the rate of (13)CO(2) formation both early in disease and prior to treatment with INH. Following INH treatment, all evaluable rabbits showed a decrease in the rate of (13)CO(2) formation. CONCLUSIONS/SIGNIFICANCE:Urea breath testing may provide a useful diagnostic and biomarker assay for tuberculosis and for treatment response. Future work will test specificity for M. tuberculosis using lung-targeted dry powder inhalation formulations, combined with co-administering oral urease inhibitors together with a saturating oral dose of unlabeled urea, which would prevent the delta(13)CO(2) signal from urease-positive gastrointestinal organisms.

Abstract

Background: Life participation is an outcome of critical importance to patients receiving peritoneal dialysis (PD). However, there is no widely accepted or validated tool for measuring life participation in patients receiving PD.Methods: Online consensus workshop to identify the essential characteristics of life participation as a core outcome, with the goal of establishing a patient-reported outcome measure for use in all trials in patients receiving PD. Thematic analysis of transcripts was performed.Results: Fifty-six participants, including 17 patients and caregivers, from 15 countries convened via online videoconference. Four themes were identified: reconfiguring expectations of daily living (accepting day-to-day fluctuation as the norm, shifting thresholds of acceptability, preserving gains in flexibility and freedom), ensuring broad applicability and interpretability (establishing cross-cultural relevance, incorporating valued activities, distinguishing unmodifiable barriers to life participation), capturing transitions between modalities and how they affect life participation (responsive to trajectory towards stable, reflecting changes with dialysis transitions) and maximising feasibility of implementation (reducing completion burden, administrable with ease and flexibility).Conclusions: There is a need for a validated, generalisable outcome measure for life participation in patients receiving PD. Feasibility, including length of time to complete and flexible mode of delivery, are important to allow implementation in all trials that include patients receiving PD.

National efforts are underway to prepare the UK National Health Service (NHS) for the COVID-19 pandemic; however, the efficacy of these interventions is unknown. In view of this, a cross-sectional survey of front-line healthcare workers (HCWs) at two large acute NHS hospital trusts in England was undertaken to assess their confidence and perceived level of preparedness for the virus. The survey found that there has been moderate success in readying HCWs to manage COVID-19, but that more still needs to be done, particularly in relation to educating HCWs about laboratory diagnostics. Abstract

Intravenous thrombolysis with alteplase bolus followed by infusion is a global standard of care for patients with acute ischaemic stroke. We aimed to determine whether tenecteplase given as a single bolus might increase reperfusion compared with this standard of care.
