253. Efficacy and safety of radotinib in chronic phase chronic myeloid leukemia patients with resistance or intolerance to BCR-ABL1 tyrosine kinase inhibitors

Abstract

Radotinib (IY5511HCL), a novel and selective BCR-ABL1 tyrosine kinase inhibitor, has shown pre-clinical and phase I activity and safety in chronic myeloid leukemia. This phase II study investigated the efficacy and safety of radotinib in Philadelphia chromosome-positive chronic phase-chronic myeloid leukemia patients with resistance and/or intolerance to BCR-ABL1 tyrosine kinase inhibitors. Patients received radotinib 400 mg twice daily for 12 cycles based on results from the phase I trial. The primary end point was rate of major cytogenetic response by 12 months. A total of 77 patients were enrolled. Major cytogenetic response was achieved in 50 (65%; cumulative 75%) patients, including 36 (47%) patients with complete cytogenetic response by 12 months. Median time to major cytogenetic response and complete cytogenetic response were 85 days and 256 days, respectively. Major cytogenetic response and complete cytogenetic response rates were similar between imatinib-resistant and imatinib-intolerant patients, but were higher in patients without BCR-ABL1 mutations. Overall and progression-free survival rates at 12 months were 96.1% and 86.3%, respectively. All newly-occurring or worsening grade 3/4 hematologic abnormalities included thrombocytopenia (24.7%) and anemia (5.2%); grade 3/4 drug-related non-hematologic adverse events included fatigue (3.9%), asthenia (3.9%), and nausea (2.6%). The most common biochemistry abnormality was hyperbilirubinemia (grade 3/4 23.4%), and 12 of 18 cases were managed with dose modification. Study findings suggest radotinib is effective and well tolerated in chronic phase-chronic myeloid leukemia patients with resistance and/or intolerance to BCR-ABL1 tyrosine kinase inhibitors and may represent a promising alternative for these patients. Abstract

A prototype CMOS (complementary metal oxide semiconductor) imaging system has been built and the possibility of applying to the application to astronomical observations has been investigated. The CCD (charge coupled device) image sensor has been the mainstay of image capture and astronomical imaging for the last 30 years, but CMOS devices have shown rapidly increasing success and have been adapted to many commercial imaging systems. Although the photometric performances and system noise of CMOS sensors are lower than that of CCD image sensors, CMOS imaging system can be used to obtain general image capture for astronomical applications.

Abstract

Korea Astronomy Observatory (KAO) is working to retrofit its 1m robotic telescope in collaboration with a company (ACE, Astronomical Consultants & Equipment). The telescope system is being totally refurbished to make a fully automatic telescope which can operate in both interactive and fully autonomous robotic modes. Progress has been made in design and manufacturing of the telescope mount, mechanics, and optical performance system tests are being made for re-configured primary and secondary mirrors. The optical system is designed to collect 80% incident light within 0.5 arcsec with f/7.5 Ritchey-Chretien design. The telescope mount is an equatorial fork with a friction drive system. The design allows fully programmable tracking speeds with typical range of 15 arcsec/sec with accuracy of +/-5 arcsec/hour. The mount system has integral pointing model software to correct for refraction, and all mechanical errors and misalignments. The pointing model will permit positioning to better than 30 arcsec RMS within 75o from zenith and 45 arcsec RMS elsewhere on the sky. The software is designed for interactive, remote and robotic modes of operation. In interactive and remote mode the user can manually enter coordinates or retrieve them from a computer file. In robotic mode the telescope controller downloads the coordinates in the order determined by the scheduler. The telescope will be equipped with a CCD camera and will be accessible via the internet.

256. Development of CCD Imaging System Using Thermoelectric Cooling Method

Abstract

We developed low light CCD imaging system using thermoelectric cooling method collaboration with a company to design a commercial model. It consists of Kodak KAF-0401E (768x512 pixels) CCD chip,thermoelectric module manufactured by Thermotek. This TEC system can reach an operative temperature of -25deg. We employed an Uniblitz VS25S shutter and it has capability a minimum exposure time 80ms. The system components are an interface card using a Korea Astronomy Observatory (hereafter KAO) ISA bus controller, image acquisition with AD9816 chip, that is 12bit video processor. The performance test with this imaging system showed good operation within the initial specification of our design. It shows a dark current less than 0.4e-/pixel/sec at a temperature of -10deg, a linearity 99.9+/-0.1%, gain 4.24e-adu, and system noise is 25.3e- (rms). For low temperature CCD operation, we designed a TEC, which uses a one-stage peltier module and forced air heat exchanger. This TEC imaging system enables accurate photometry (+/-0.01mag) even though the CCD is not at 'conventional' cryogenic temperatures (140K). The system can be a useful instrument for any other imaging applications. Finally, with this system, we obtained several images of astronomical objects for system performance tests.

Abstract

BackgroundInebilizumab is an anti-CD19 antibody approved for the treatment of neuromyelitis optica spectrum disorder (NMOSD) in adults with aquaporin-4 autoantibodies. The relationship between B-cell, plasma-cell (PC), and immunoglobulin depletion with longitudinal reductions in NMOSD activity after inebilizumab treatment was characterised post hoc in an exploratory analysis from the N-MOmentum study (NCT02200770).MethodsPeripheral blood CD20+ B cells, PC gene signature, and immunoglobulin levels were assessed throughout N-MOmentum (follow-up ≥2.5 years); correlations with clinical metrics and magnetic resonance imaging (MRI) lesion activity were assessed.FindingsInebilizumab induced durable B-cell and PC depletion within 1 week versus placebo. Although no association was observed between B-cell counts at time of attack and NMOSD activity, depth of B-cell depletion after the first dosing period correlated with clinical outcomes. All participants receiving inebilizumab demonstrated a robust long-term therapeutic response, and participants with ≤4 cells/μL after the first 6-month dosing interval had persistently deeper B-cell depletion, lower annualised attack rates (estimated rate [95% CI]: 0.034 [0.024-0.04] vs 0.086 [0.056-0.12]; p = 0.045), fewer new/enlarging T2 MRI lesions (0.49 [0.43-0.56] vs 1.36 [1.12-1.61]; p

270. A longitudinal brain magnetic resonance imaging study of neuromyelitis optica spectrum disorder.

Abstract

Brain involvement is commonly seen in patients with neuromyelitis optica spectrum disorder (NMOSD). However, little is known about the chronic changes of acute brain lesions on MRI over time. Here, our objective was to evaluate how acute brain MRI lesions in NMOSD changed on follow-up MRI. We reviewed the MRIs of 63 patients with NMOSD who had acute brain lesions and follow-up MRI over an interval of at least 3 months. Of the 211 acute brain lesions, 24% of lesions disappeared completely on T2-weighed images (WI) and a decrease in size ≥50% on T2-WI was observed in 58% of lesions on follow-up MRI. However, 47% of lesions revealed focal T1-hypointensity and, in particular, 18% showed focal cystic changes. Cystic changes were observed most commonly in corticospinal tract and corpus callosal lesions whereas the vast majority of lesions in the cerebellum, basal ganglia and temporal white matter resolved completely. MRI remission on T2-WI occurred in 82% of lesions, while approximately half of the lesions presented foci of T1-hypointensity, which may be considered a severe tissue injury over time. The extent of brain injury following an acute brain lesion in NMOSD may depend on the location of the lesion.

274. Period Change of W UMa Type Contact Binary AB And

Abstract

The CCD photometric observations of W UMa-type eclipsing binary AB And were made from September 1994 to October 1996. New four primary minimum times were obtained form these observations. The analysis of times of minimum light for AB And confirms other previous studies that the orbital period of AB And have been changing as a form of sinusoidal variation. In this paper, we calculated the new orbital elements with linear and nonlinear quadratic term, and the best fit equation is derived with the assumption that the period variation of AB And changes sinusoidal pattern. From the sinusoidal term of this orbital element, we calculate period variation as 92 years with amplitude of 0d.059. However this result considering only sinusoidal term was not satisfied with our recent observations. Thus, by assuming another parabolic period variation with the sinusoidal pattern, we derived the best fit orbital elements. From the quadratic coefficient of this orbital elements, we calculated the secular variation of 0.73 seconds, and from the sinusoidal term, the period variation turned out to be 62.9 years with amplitude of 0d.024. If we assume only the sinusoidal period variation of AB And, the period has to be decreased with 10 years. However if we consider quadratic term with the sinusoidal period variation of the light elements, the period is expected to be increased. Therefore long-term observations of this binary system are required to confirm this issue.

Abstract

When a human performs a given specific task, it has been known that the central nervous system controls modularized muscle group, which is called muscle synergy. For human-robot interface design problem, therefore, the muscle synergy can be utilized to reduce the dimensionality of control signal as well as the complexity of classifying human posture and motion. In this paper, we propose an approach to design a human-robot interface which enables a human operator to transfer a kinodynamic control command to robotic systems. A key feature of the proposed approach is that the muscle synergy and corresponding activation curve are employed to calculate a force generated by a tool at the robot end effector. A test bed for experiments consisted of two armband type surface electromyography sensors, an RGB-d camera, and a Kinova Gen2 robotic manipulator to verify the proposed approach. Abstract

In the Monte Carlo (MC) burnup analyses, the uncertainty of a tally estimate at a burnup step may be induced from four sources: the statistical uncertainty caused by a finite number of simulations, the nuclear covariance data, uncertainties of number densities, and cross-correlations between the nuclear data and the number densities. In this paper, the uncertainties of kinf, reaction rates, and number densities for a PWR pin-cell benchmark problem are quantified by an uncertainty propagation formulation in the MC burnup calculations. The required sensitivities of tallied parameters to the microscopic cross-sections and the number densities are estimated by the MC differential operator sampling method accompanied by the fission source perturbation. The uncertainty propagation analyses are conducted with two nuclear covariance data—ENDF/B-VII.1 and SCALE6.1/COVA libraries—and the numerical results are compared with each other.

Abstract

Background Since it was suggested that B cells play a role in the pathogenesis of chronic graft-versus-host disease, rituximab, an anti-CD20 monoclonal antibody targeting B cells, has been shown to be effective in steroid-refractory, chronic graft-versus-host disease. However, most of the data were from small numbers of patients or retrospective analyses. We, therefore, conducted a multicenter phase II study to confirm the efficacy of this treatment strategy that targets B cells.Design and Methods We diagnosed and evaluated chronic graft-versus-host disease according to the National Institute of Health criteria for clinical trials on this condition. The treatment consisted of weekly intravenous infusions of rituximab for 4 weeks followed by monthly rituximab for 4 months. We evaluated the patients’ responses and monitored their disease activity until their final visit, which was on day 365. We also assessed the patients’ subsequent quality of life and serum levels of B-cell-activating factor of the tumor necrosis factor family.Results Among 37 patients enrolled (median age, 29 years; range 8–57 years), 32 patients responded to rituximab with 8 complete and 24 partial responses. Twenty-one patients maintained their response for 1 year, so their steroid treatment was discontinued or its dose reduced (21/37, or 56.8%), and their scores representing quality of life were improved although these changes were not statistically significant. The responses were better for clinical manifestations of the skin, oral cavity and musculoskeletal system (response rate, 71.4–100%) than for other organs. However, infectious complications and primary disease relapse accounted for the majority of treatment failure. The pre-treatment serum level of B cell-activating factor of the tumor necrosis factor family was not associated with better treatment outcome (P=0.147).Conclusions Rituximab could improve clinical responses and quality of life of patients with steroid-refractory chronic graft-versus-host disease, although such patients may need active prophylaxis against infection.

298. Evaluating Predictive Uncertainty under Distributional Shift on Dialogue Dataset

Abstract

In open-domain dialogues, predictive uncertainties are mainly evaluated in a domain shift setting to cope with out-of-distribution inputs. However, in real-world conversations, there could be more extensive distributional shifted inputs than the out-of-distribution. To evaluate this, we first propose two methods, Unknown Word (UW) and Insufficient Context (IC), enabling gradual distributional shifts by corruption on the dialogue dataset. We then investigate the effect of distributional shifts on accuracy and calibration. Our experiments show that the performance of existing uncertainty estimation methods consistently degrades with intensifying the shift. The results suggest that the proposed methods could be useful for evaluating the calibration of dialogue systems under distributional shifts.

299. An Archaeological Discourse Analysis of Physical Education as Challenging in South Korea Primary Schools

Abstract

South Korean society tends to undervalue Physical Education (PE) and overemphasizes academic subjects such as Korean, English, mathematics and science. This study problematizes the challenges within PE in the South Korean primary school context, where teachers find it difficult to deliver quality PE. Data were collected from archives and interviews with 11 South Korean primary school teachers to identify the challenges of PE that they faced. An archaeological discourse analysis, based on a Foucauldian framework, was employed to trace how the discourse of PE as challenging arose. Findings highlight not only the dominant discursive constructions of the challenges facing primary school PE but also the possible historical conditions intricately connected with social structures and institutions, surrounding primary school PE and beyond the individual's control, in and outside the South Korean school system. The findings were: (a) PE as lacking equipment and facilities and school principals holding the budget allocation authority (b) PE as inconsequential and PE with little or no role in students' entry in university (c) PE as accident-prone and absence of legal safeguards and social support for teachers. This paper provides readers with a fresh analytical insight in the area of PE research.
