Abstract

Background: Atrial fibrillation (AF) is one of the most prevalent cardiac arrhythmias associated with substantially increased risks of ischemic stroke and thromboembolism. Oral anticoagulants (OACs) are the cornerstone of AF management and effectively prevent AF-related stroke. As new non-vitamin K antagonist OACs (NOACs) have become available, the landscape of stroke prevention in AF has changed. However, there are considerable gaps between daily clinical practice and current guideline-based recommendations for anticoagulant therapy in Japan. Consequently, little is known about the real-world setting and the current use of NOACs, especially by practitioners in Japan. Methods: We conducted a prospective, observational study in 3847 patients with AF who were enrolled in clinics and hospitals located in Kanagawa Prefecture from September 2013 through March 2015. The participating centers included practitioners (small clinics), medium-sized hospitals, and university hospitals. The primary endpoints were epidemiologic characteristics, status of treatment with anticoagulants and antiplatelet agents, outcomes, and adverse events, including cerebrovascular disease, bleeding, and death. Results: The mean CHADS2 score was 1.81±1.27, the mean CHADS2-Vasc score was 3.02±1.58, and the mean HAS-BLED score was 2.23±1.06, respectively. The usage rate of warfarin was 44.2% overall, and the usage rate of NOACs was 33.5%. Conclusions: The results of the study are expected to serve as the basis for providing clinical practice guidance to healthcare institutions in Japan, with the ultimate goals of better characterizing the appropriate use of OACs and providing clinical decision support to physicians to facilitate the design of appropriate therapeutic strategies and the selection of anticoagulants for the management of AF.

クライエントとソーシャルワーカーとの関係はソーシャルワーク実践の基本とされるが，近年ではクライエントのスピリチュアリティへの配慮が求められるなど，改めて両者の関係性を検討する必要性が生じている。本稿では，ソーシャルワークの実践に関する先行研究を整理し，共感と信頼，パートナーシップといった関係性，スピリチュアリティへの配慮について概観した。その上で，西田哲学の概念を用いて両者の関係性を再考した結果，ソーシャルワーカーには，我執を棄ててクライエントと人格的に応答し，スピリチュアリティを契機に，自己の内で行為的直観的にクライエントと合一しつつも，常に反省するあり方が求められることが明らかとなった。, The relationship between the client and the social worker is the basis of social work practice. In recent years, social workers in social work practices have been required to consider the spirituality of the client, and it is necessary to reexamine the relationship between the two. In this paper, we analyzed previous studies on the practice of social work and surveyed relationships such as empathy, trust and partnership, and consideration for spirituality. Based on that, I reconsidered the relationship between the client and the social worker using the concepts of Kitaro Nishida's philosophy. As a result, social workers are expected to give up selfattachment and respond to their clients in a way according to social workers' conscience, and unite act-intuitively with client within social workers' self, triggered by spirituality, while constantly reflecting on themselves. 258. Clinical characteristics and outcomes of Japanese atrial fibrillation patients with poor medication adherence: A sub-analysis of the GENERAL study

Abstract

Oral anticoagulation therapy is essential for preventing stroke in patients with atrial fibrillation (AF). However, poor anticoagulant adherence may hamper medication safety and effective prevention of stroke.GENERAL is a prospective cohort study of AF patients taking rivaroxaban prescribed by general practitioners in Japan. In this study, anticoagulant adherence was calculated as the proportion of days covered (PDC), and patients were retrospectively divided into two groups: good adherence (PDC ≥80 %) and poor adherence (80 %).Of 5680 patients in the GENERAL study, the poor adherence group consisted of 223 patients (3.9 %). Baseline clinical characteristics were almost comparable regarding age (PDC ≥80 % vs.80 %: 73.9 vs. 74.0 years, p = 0.92) and sex (male 64.6 % vs. 66.8 %, p = 0.52). The PDC80 % group more often had various co-morbidities, and had significantly higher CHADSPoor anticoagulant adherence, as measured by PDC80 %, was associated with higher incidence of stroke or systemic embolism and major bleeding in the GENERAL study.

261. Cytoneme-mediated signaling essential for tumorigenesis.

Abstract

Communication between neoplastic cells and cells of their microenvironment is critical to cancer progression. To investigate the role of cytoneme-mediated signaling as a mechanism for distributing growth factor signaling proteins between tumor and tumor-associated cells, we analyzed EGFR and RET Drosophila tumor models and tested several genetic loss-of-function conditions that impair cytoneme-mediated signaling. Neuroglian, capricious, Irk2, SCAR, and diaphanous are genes that cytonemes require during normal development. Neuroglian and Capricious are cell adhesion proteins, Irk2 is a potassium channel, and SCAR and Diaphanous are actin-binding proteins, and the only process to which they are known to contribute jointly is cytoneme-mediated signaling. We observed that diminished function of any one of these genes suppressed tumor growth and increased organism survival. We also noted that EGFR-expressing tumor discs have abnormally extensive tracheation (respiratory tubes) and ectopically express Branchless (Bnl, a FGF) and FGFR. Bnl is a known inducer of tracheation that signals by a cytoneme-mediated process in other contexts, and we determined that exogenous over-expression of dominant negative FGFR suppressed tumor growth. Our results are consistent with the idea that cytonemes move signaling proteins between tumor and stromal cells and that cytoneme-mediated signaling is required for tumor growth and malignancy.

263. Cubital tunnel syndrome with small occult ganglion: A case report of bike rider

Abstract

Cubital tunnel syndrome, the second-most common peripheral compression neuropathy, is associated with dynamic pressure in the cubital tunnel with the elbow flexion position. Medial elbow ganglion originated from the ulnohumeral joint causing cubital tunnel syndrome has been reported. This report describes the case of a 48-year-old man who developed numbness in his left ring and little finger after prolonged motorbike touring. He gradually showed decreased hand grip strength with medial elbow joint pain. Ultrasonography and magnetic resonance images revealed small occult ganglion at the medial side of elbow joint. Surgical resection of ganglion and ulnar nerve decompression relieved the ulnar neuropathy symptom. Prolonged motorbike riding while maintaining the elbow flexion position might exacerbate the symptom of cubital tunnel syndrome in patients with even a small space-occupying lesion such as the small occult ganglion.

Abstract

Planning elevator installation in high-rise buildings, it is important to co-create with not only elevator specialists but also multiple stakeholders such as building owners and architects. Therefore, we should share current issues and find consensus on measures among the multiple stakeholders. Then, we also should prototype to solve the issues, and make effects visible. To co-create the planning effectively, we have developed a people flow simulator that enables us simulation of lining up behavior for elevator inside high-rise building. It simulates based on a condition, which include a building layout and traffic demands, defined by a planning. It shows an impact of changes in building conditions and elevator specifications with an easy-to-understand 3D animation. It allows us easily to find issues from the planning and discuss measures with multiple stakeholders. Additionally, we verified an accuracy of developed simulator comparing actual index which calculated from the logs and simulated index which calculated by the simulator. As a result of accuracy evaluation, we have concluded that the simulator has enough accuracy.

Abstract

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P

Abstract

Production of reactive oxygen species (ROS) and consequent glutathione oxidation are associated with various physiological processes and diseases, including cell differentiation, senescence, and inflammation. GFP-based redox sensors provide a straight-forward approach to monitor ROS levels and glutathione oxidation within a living cell at the subcellular resolution. We utilized palmitoylated versions of cytosolic glutathione and hydrogen peroxide sensors (Grx1-roGFP2 and roGFP2-Orp1, respectively) and demonstrated a unique redox environment near biological membranes. In HeLa cells, cytosolic glutathione was practically completely reduced (EGSH/GSSG = â 333Â mV) and hydrogen peroxide level was under the detectable range. In contrast, the cytoplasmic milieu near membranes of intracellular vesicles exhibited significant glutathione oxidation (EGSH/GSSG > â 256Â mV) and relatively high H2O2 production, which was not observed for the plasma membrane. These vesicles colocalized with internalized EGFR, suggesting that H2O2 production and glutathione oxidation are characteristics of cytoplasmic surfaces of the endocytosed vesicles. The results visually illustrate local redox heterogeneity within the cytosol for the first time.

Due to the superfluous consciousness of ambient noise and sound insulation, many music college facilities employ a style in which the exterior appears as a closed box and the lesson rooms are lined up along corridors in a jail‐cell‐like manner in the interior. In this project, we tried to break down such an image. By exposing the volume of the lesson rooms to the exterior, small, uneven, courtyard‐like spaces between neighboring rooms were created, and the volume of the building was broken down. As a result, the characteristic “porous” volume arrangement was born. For such adjustments, it was more appropriate to use a new computer‐based method of design that incorporates three‐dimensional feedback loops. The porous figures embed a sense of continuity and surprise, as if one were wandering around a village. This resulted in the creation of rich spaces and enhanced presence as a whole.

Abstract

In this review, we summarize the recent progress in thermal conductivity analysis of nanocellulose materials called cellulose nanopapers, and compare them with polymeric materials, including neat polymers, composites, and traditional paper. It is important to individually measure the in-plane and through-plane heat-conducting properties of two-dimensional planar materials, so steady-state and non-equilibrium methods, in particular the laser spot periodic heating radiation thermometry method, are reviewed. The structural dependency of cellulose nanopaper on thermal conduction is described in terms of the crystallite size effect, fibre orientation, and interfacial thermal resistance between fibres and small pores. The novel applications of cellulose as thermally conductive transparent materials and thermal-guiding materials are also discussed.
