Type 2 diabetes (T2D) is a complex metabolic disorder with no cure and high morbidity. Exposure to inorganic arsenic (iAs), a ubiquitous environmental contaminant, is associated with increased T2D risk. Despite growing evidence linking iAs exposure to T2D, the factors underlying inter-individual differences in susceptibility remain unclear. This study examined the interaction between chronic iAs exposure and body composition in a cohort of 75 Diversity Outbred mice. The study design mimics that of an exposed human population where the genetic diversity of the mice provides the variation in response, in contrast to a design that includes untreated mice. Male mice were exposed to iAs in drinking water (100 ppb) for 26 weeks. Metabolic indicators used as diabetes surrogates included fasting blood glucose and plasma insulin (FBG, FPI), blood glucose and plasma insulin 15 min after glucose challenge (BG15, PI15), homeostatic model assessment for [Formula: see text]-cell function and insulin resistance (HOMA-B, HOMA-IR), and insulinogenic index. Body composition was determined using magnetic resonance imaging, and the concentrations of iAs and its methylated metabolites were measured in liver and urine. Associations between cumulative iAs consumption and FPI, PI15, HOMA-B, and HOMA-IR manifested as significant interactions between iAs and body weight/composition. Arsenic speciation analyses in liver and urine suggest little variation in the mice's ability to metabolize iAs. The observed interactions accord with current research aiming to disentangle the effects of multiple complex factors on T2D risk, highlighting the need for further research on iAs metabolism and its consequences in genetically diverse mouse strains.

International audience; With the development of clinical databases and the ubiquity of EHRs, physicians and researchers alike have access to an unprecedented amount of data. Complexity of the available data has also increased since clinical reports are also included and require frameworks with natural language processing capabilities in order to process them and extract information not found in other types of documents. In the following work we implement a data processing pipeline performing phenotyping, disambiguation, negation and subject prediction on such reports. We compare it to an existing solution routinely used in a children’s hospital with special focus on genetic diseases. We show that by replacing components based on rules and pattern matching with components leveraging deep learning models and fine-tuned word embeddings we obtain performance improvements of 7%, 10% and 27% in terms of F1 measure for each task. The solution we devised will help build more reliable decision support systems.

Trials have shown that decision rules meant to improve use of computed tomographic pulmonary angiography in patients with suspected pulmonary embolism in the emergency department decrease how frequently clinicians use this imaging. This study used real-world data from 26 emergency departments in 6 countries to address this issue. Visual Abstract. Temporal Trends in the Use of Computed Tomographic Pulmonary Angiography for Suspected Pulmonary Embolism in the Emergency Department: Multiple decision rules have been developed to improve the use of computed tomographic pulmonary angiography in patients with suspected pulmonary embolism in the emergency department. And clinical trials have shown that all of these decision rules decrease how frequently clinicians use this type of imaging. The study described in this article used real-world data from 26 emergency departments in 6 European countries to address the same issue and found something different. Background: Recently, validated clinical decision rules have been developed that avoid unnecessary use of computed tomographic pulmonary angiography (CTPA) in patients with suspected pulmonary embolism (PE) in the emergency department (ED). Objective: To measure any resulting change in CTPA use for suspected PE. Design: Retrospective analysis. Setting: 26 European EDs in 6 countries. Patients: Patients with CTPA performed for suspected PE in the ED during the first 7 days of each odd month between January 2015 and December 2019. Measurements: The primary end points were the CTPAs done for suspected PE in the ED and the number of PEs diagnosed in the ED each year adjusted to an annual census of 100 000 ED visits. Temporal trends were estimated using generalized linear mixed regression models. Results: 8970 CTPAs were included (median age, 63 years; 56% female). Statistically significant temporal trends for more frequent use of CTPA (836 per 100 000 ED visits in 2015 vs. 1112 in 2019; P < 0.001), more diagnosed PEs (138 per 100 000 in 2015 vs. 164 in 2019; P = 0.028), a higher proportion of low-risk PEs (annual percent change [APC], 13.8% [95% CI, 2.6% to 30.1%]) with more ambulatory management (APC, 19.3% [CI, 4.1% to 45.1%]), and a lower proportion of intensive care unit admissions (APC, −8.9% [CI, −17.1% to −0.3%]) were observed. Limitation: Data were limited to 7 days every 2 months. Conclusion: Despite the recent validation of clinical decision rules to limit the use of CTPA, an increase in the CTPA rate along with more diagnosed PEs and especially low-risk PEs were instead observed. By analyzing the responses of 1075 fantasy sports players, the paper highlights the differences among players of the four leading North American team sports of football, baseball, basketball, and hockey. Motivations for participation vary in each of these sports. Whereas the study confirms that fantasy football is the most prominent among the four, it also highlights key characteristics that define fantasy baseball, basketball, and hockey players. In particular, the study demonstrates that for fantasy football players the main driving force in their participation centers on general sociability, a form of cultural capital and shared connection that is common among friends at the bar and the office. In contrast, for fantasy hockey players, participation in the game is more about reinforcement within their group, a solidification among a self-selected and devoted cluster of fantasy hockey players that revels in its confined, almost cult-like, associativity.

Dès la fin du mois de février 2020, les urgentistes français ont été confrontés à une situation inédite et complexe dans la gestion des cas les plus sévères d’infections pulmonaires associées au nouveau coronavirus (SARSCoV- 2). Les informations en provenance de Chine et les recommandations initiales de l’Organisation mondiale de la santé ont rapidement amené à considérer l’intubation et la ventilation mécanique précoce des malades atteints par la pneumonie de la Covid-19. Or, dès la fin du mois de mars 2020, grâce aux retours d’expérience et de prise en charge, d’abord de la part des réanimateurs et urgentistes italiens, puis espagnols, les pratiques et les recommandations concernant les modalités d’oxygénation et de ventilation des patients Covid-19 ont évolué. Le caractère exceptionnel de cette pandémie et la grande adaptabilité des services de Samu/Smur de France, en l’espace de quelques semaines, pour prendre en charge ces patients oxygénodépendants, justifient que nous en fassions le retour d’expérience, et ce, d’autant plus que nous sommes exposés à un risque de recrudescence d’infections respiratoires graves associées au SARS-CoV-2 à court terme, risquant de saturer une nouvelle fois notre système de santé. Nous détaillons donc ici le retour d’expérience des prises en charge médicales préhospitalières concernant principalement les supports d’oxygénation et de ventilation mécanique.

To investigate the role of glutathione transferases (GSTs) in the metabolism of inorganic arsenic (iAs), we compared the disposition of iAs and its metabolites in wild-type mice and mice lacking genes encoding GST-P, -M and -T after exposure to 100 ppb iAs in drinking water. We found no differences between the two genotypes in the concentrations of total arsenic or arsenic species in urine, liver, and kidneys. No genotype-dependent differences were found in proportions of arsenicals in the tissues, and only small differences were observed in the urine. Thus, under these conditions, GST-P, -M and -T did not play a significant role in iAs metabolism in mice.

Inorganic arsenic (iAs) is an environmental diabetogen, but mechanisms underlying its diabetogenic effects are poorly understood. Exposures to arsenite (iAs(III)) and its methylated metabolites, methylarsonite (MAs(III)) and dimethylarsinite (DMAs(III)), have been shown to inhibit glucose stimulated insulin secretion (GSIS) in pancreatic β-cells and isolated pancreatic islets. GSIS is regulated by complex mechanisms. Increase in ATP production through metabolism of glucose and other substrates is the ultimate trigger for GSIS in β-cells. In the present study, we used metabolomics to identify metabolites and pathways perturbed in cultured INS-1 832/13 rat insulinoma cells and isolated murine pancreatic islets by exposures to iAs(III), MAs(III) and DMAs(III). We found that the exposures perturbed multiple metabolites, which were enriched primarily in the pathways of amino acid, carbohydrate, phospholipid and carnitine metabolism. However, the effects of arsenicals in INS-1 832/13 cells differed from those in the islets and were exposure specific with very few overlaps between the three arsenicals. In INS-1 832/13 cells, all three arsenicals decreased succinate, a metabolite of Krebs cycle, which provides substrates for ATP synthesis in mitochondria. Acetylcarnitine was decreased consistently by exposures to arsenicals in both the cells and the islets. Acetylcarnitine is usually found in equilibrium with Acetyl-CoA, which is the central metabolite in catabolism of macronutrients and the key substrate for Krebs cycle. It is also thought to play an antioxidant function in mitochondria. Thus, while each of the three trivalent arsenicals perturbed specific metabolic pathways, which may or may not be associated with GSIS, all three arsenicals appeared to impair mechanisms that support ATP production or antioxidant defense in mitochondria. These results suggest that impaired ATP production and/or mitochondrial dysfunction caused by oxidative stress may be the mechanisms underlying the inhibition of GSIS in β-cells exposed to trivalent arsenicals.

Embryonic stem cells (ESCs) are defined by their ability to self-renew and the potential to differentiate into all tissues of the developing organism. We previously demonstrated that deleting the catalytic SET domain of the Set1A/complex of proteins associated with SET1 histone methyltransferase (Set1A/COMPASS) in mouse ESCs does not impair their viability or ability to self-renew; however, it leads to defects in differentiation. The precise mechanisms by which Set1A executes these functions remain to be elucidated. In this study, we demonstrate that mice lacking the SET domain of Set1A are embryonic lethal at a stage that is unique from null alleles. To gain insight into Set1A function in regulating pluripotency, we conducted a CRISPR/Cas9-mediated dropout screen and identified the MOZ/MORF (monocytic leukaemia zinc finger protein/monocytic leukaemia zinc finger protein-related factor) and HBO1 (HAT bound to ORC1) acetyltransferase complex member ING5 as a synthetic perturbation to Set1A. The loss of Ing5 in Set1AΔSET mouse ESCs decreases the fitness of these cells, and the simultaneous loss of ING5 and in Set1AΔSET leads to up-regulation of differentiation-associated genes. Taken together, our results point toward Set1A/COMPASS and ING5 as potential coregulators of the self-renewal and differentiation status of ESCs.

291. The compressive strength of earth-hemp blocks tested with different densities, earth types, and cementitious binders

- Abstract
The building sector needs to shift toward the use of materials that have low-embodied energy, minimize operational-energy, and minimize the amount of waste upon disposal. Here, we report on a series of experiments on low-density earth-hemp blocks, which can be implemented as an insulation for buildings. Earth-hemp finds a similar usage to hempcrete/hemp-lime, yet the use of raw earth as a binder allows to dramatically decrease the embodied energy. The set presented here evidences that pure earth-hemp with high content in clay minerals reaches higher compressive strength (0,33 MPa) than equivalents with hydraulic binder, for similar thermal conductivity (0,07 W/m.K).Earth-hemp samples were characterized in terms of compressive strength in order to test the influence of density, earth type, incorporation of mineral additives, and amount of water used for creating the blocks. Two types of natural earths were investigated, which differ in their clay content: a surficial loess with 25 wt.% clay minerals and a quarried paleosoil with high clay content (65 wt.%). For each earth type, 4 types of mineral additives were investigated in order to test whether they can have a stabilizing effect: Portland cement, aerial-lime, gypsum-plaster and a MgO-based cement. The binders (i.e. earth + additive) were created with replacement of earth by mineral additives at 0, 4, 8, and 20 wt. %. For each type of binder, 3 densities of the resulting earth-hemp samples were produced (250, 280, 340 kg/m3). Additionally, two series of this set of samples were produced using a low amount of added water (150 wt.% water/hemp) and high amount of added water (370 wt.% water/hemp).Samples using the earth with high clay content have compressive strengths up to twice as high as those with low clay content. This result is expected since clay minerals are the main agent of binding in earth materials. Also expected was the increase in compressive strength with sample density, which is directly correlated to the amount of binder. More interestingly, the dataset also exhibits the negative effect of mineral additives: a trend of decreasing compressive strength with amount of incorporated mineral additive is visible, independently of the type of additive and earth type. In between additive types, the compressive strengths of samples mixed with MgO-based cement and gypsum-plaster are better than those mixed with Portland cement and aerial lime. Additionally, samples produced using a low amount of added water are much less resistant than those with a high amount of added water for every sample tested. Finally, samples using pure earth with high clay content and high amount of incorporated water are the most resistant, and reach compressive strengths of 0.33 MPa for a density of 340 kg/m3, which is slightly stronger than existing commercial lime-hemp blocks.

292. Geometry of overdeepenings obtained through three-dimensional gravity modelling

- Abstract
We investigated the formation mechanism of tunnel valleys, by producing 3D models of bedrock topography using gravimetry. We obtained the cross-sectional geometry of tunnel valleys in the Swiss foreland, near Bern. The combination of information about the densities of the sedimentary fill and of the bedrock together with borehole data and gravity surveys along profiles across the valleys served as input for our 3D gravity modelling software, Prisma. This finally allowed us to model the gravity effect of the Quaternary fill of the overdeepenings and to produce cross-sectional geometries of the overdeepenings. We focused on two sections situated in the Gürbe valley and in the Aare valley. We determined a density of 2’500 kg/m3 for the Upper Marine Molasse bedrock, and with Prisma we obtained a bulk density of kg/m3 for the Quaternary infill. Our gravity surveys across the valleys yielded a maximum residual anomaly of -2.9 mGal for the Gürbe valley and -4.1 mGal for the Aare valley. The application of our Prisma model showed that these anomalies can be explained by Quaternary suites with a thickness of 160 m and 235 m for the infill of the Gürbe and Aare valleys, respectively. The high-resolution information about the cross-sectional geometry of the tunnel valley flanks, from the application of Prisma, allowed us to infer a two-step formation process of the overdeepened trough. A first glaciation, during MIS 6 or before, would have deepened the trough. And a second glaciation, during the Last Glacial Maximum (MIS 2), would have widened the valleys. We explain this pattern by the differences between the ice thicknesses of the LGM and MIS 6 glaciers and by the relatively low erodibility of the Upper Marine Molasse bedrock. The Molasse units indeed comprise tender and porous sandstones and offer a lower erosional resistance than the Quaternary infill, which consists of cohesive and thus competent glacio-lacustrine marls. This probably offered ideal conditions for the thick and thus erosive MIS 6 glaciers to erode deeply into the Molasse bedrock. In contrast, the lacustrine fill of this trough possibly prevented the thinner and thus less erosive LGM or MIS 2 glaciers to further incise the bedrock. The consequence was that erosion of the LGM glaciers mainly occurred on the lateral sides, thereby resulting in a widening of the tunnel valleys. Finally, we apply this approach to the remaining gravity profiles, to create a 3D model of the geometry of the overdeepening network near Bern.

The diagnosis of PE remains difficult in 2023 because the signs and symptoms are not sensible nor specific. The consequences of potential diagnostic errors can be dramatic, whether by default or by excess. Furthermore, the achievement of a simple diagnostic strategy, based on clinical probability assessment, D-dimer measurement and computed tomography pulmonary angiography (CTPA) leads to a new challenge for PE diagnosis: over-testing. Indeed, since the 2000s, the wide availability of CTPA resulted in a major increase in investigations with a mod I confirm erate increase in PE diagnosis, without any notable improvement in patient outcomes. Quite the contrary, the complications of anticoagulation for PE increased significantly, and the long-term consequences of imaging diagnostic radiation is an important concern, especially the risk of breast cancer for young women. As a result, several strategies have been proposed to fight over-testing. They are mostly based on defining a subgroup of patients for whom no specific exam should be required to rule-out PE and adjusting the D-dimer cutoff to allow the exclusion of PE without performing CTPA. This narrative review presents the advantages and limitations of these different strategies as well as the perspective in PE diagnosis.

The size of grains in gravel and conglomerate deposits is most easily measured on photos taken from related outcrops. However, the occlusion of grains by the sedimentary matrix or other grains, and possible distortions of photos, could introduce a bias in such datasets. Here, we explore the uncertainties associated with datasets where the lengths of the grainswere measured on photos. To this end, we analysed coarse-grained (>2mm) fluvial material from a gravel pit (Bern, Switzerland). We compared grain size data collected from digital photos with the results where the same material was measured with a calliper and mechanically sieved. Our analyses reveal that the percentile values such as the D16, D50 and D84 of datasets where the grains' longest visible axes were measured on digital photos best correlate to the corresponding percentile values of data collected through sieving. We also find that the longest visible axes of grains measured on digital photos are c. 17 % smaller than the lengths of the intermediate b-axes of grains measured with a calliper. We therefore suggest to measure the longest visible axes on digital photos, and to correct the data by a corresponding factor such as +17 % for the target grain size percentiles.

- Abstract
The nature of dark matter and properties of neutrinos are among the most pressing issues in contemporary particle physics. The dual-phase xenon time-projection chamber is the leading technology to cover the available parameter space for Weakly Interacting Massive Particles (WIMPs), while featuring extensive sensitivity to many alternative dark matter candidates. These detectors can also study neutrinos through neutrinoless double-beta decay and through a variety of astrophysical sources. A next-generation xenon-based detector will therefore be a true multi-purpose observatory to significantly advance particle physics, nuclear physics, astrophysics, solar physics, and cosmology. We present a description of the GBAR positron (e+) trapping apparatus, which consists of a three stage Buffer Gas Trap (BGT) followed by a High Field Penning Trap (HFT), and discuss its performance. The overall goal of the GBAR experiment is to measure the acceleration of the neutral antihydrogen (H¯) atom in the terrestrial gravitational field by neutralising a positive antihydrogen ion (H¯+), which has been cooled to a low temperature, and observing the subsequent H¯ annihilation following free fall. To produce one H¯+ ion, about 1010 positrons, efficiently converted into positronium (Ps), together with about 107 antiprotons (p¯), are required. The positrons, produced from an electron linac-based system, are accumulated first in the BGT whereafter they are stacked in the ultra-high vacuum HFT, where we have been able to trap 1.4(2) × 109 positrons in 1100 s.

- Abstract
DUNE is a dual-site experiment for long-baseline neutrino oscillation studies, neutrino astrophysics and nucleon decay searches. ProtoDUNE Dual Phase (DP) is a 6 × 6 × 6 m3 liquid argon time-projection-chamber (LArTPC) that recorded cosmic-muon data at the CERN Neutrino Platform in 2019–2020 as a prototype of the DUNE Far Detector. Charged particles propagating through the LArTPC produce ionization and scintillation light. The scintillation light signal in these detectors can provide the trigger for non-beam events. In addition, it adds precise timing capabilities and improves the calorimetry measurements. In ProtoDUNE-DP, scintillation and electroluminescence light produced by cosmic muons in the LArTPC is collected by photomultiplier tubes placed up to 7 m away from the ionizing track. In this paper, the ProtoDUNE-DP photon detection system performance is evaluated with a particular focus on the different wavelength shifters, such as PEN and TPB, and the use of Xe-doped LAr, considering its future use in giant LArTPCs. The scintillation light production and propagation processes are analyzed and a comparison of simulation to data is performed, improving understanding of the liquid argon properties.
