256. Diabetes and hyperglycaemia among hospitalised patients with <scp>COVID</scp> ‐19 in Western Sydney: a retrospective cohort study

Abstract

Diabetes has been recognised as a major risk factor for COVID-19 mortality and hospital complications in earlier studies.To examine the characteristics of hospitalised COVID-19 patients with diabetes and the impact of diabetes and hyperglycaemia on hospital outcomes.This was a retrospective cohort study. Admission glucose levels, HbA1c, diabetes status and hospital outcomes were determined for subjects admitted from June to November 2021 by matching a pathology data set, a clinical data set and the hospital administrative database. The outcomes of interest were death, intensive care unit (ICU) admission and length of stay (LOS).There were 1515 individuals admitted with COVID-19 with 49 deaths (3.2%) and 205 (13.5%) ICU admissions. The median length of hospital stay was 3.7 days. Three hundred and ten patients (20%) had diabetes, with 46 (15%) newly diagnosed. Patients with diabetes had a higher mortality than patients who did not have diabetes (8% vs 2%, P 0.001), were more likely to be admitted to ICU (20% vs 12%, P = 0.001) and have longer median LOS stay (6.6 (interquartile range (IQR) 2.9-12.5) vs 2.9 (IQR 0.5-7.1) days, P 0.001). In multivariate models, neither diabetes nor admission glucose predicted death. Admission glucose level but not diabetes was an independent predictor of ICU admission and LOS.There is a high prevalence of diabetes among patients hospitalised with COVID-19, with worse outcomes. In contrast to previous studies, the association of diabetes with mortality was not significant when adjusted for other variables. This is possibly related to the benefits of vaccination and current medical and ICU interventions.

Abstract

The use of social media platforms to convey public opinions and attitudes has exponentially increased over the last decade on topics related to health. In all these social media postings related to the pandemic, specific attention has been focused on healthcare professionals, specifically nurses.This study aimed to explore how the keyword 'nurse' is located in COVID-19 pandemic-related tweets during a selected period of the pandemic in order to assess public perception.Tweets related to COVID-19 were downloaded from Twitter for the period January 1st, 2020, to November 11th, 2021. Sentiment analysis was used to identify opinions, emotions, and approaches expressed in tweet which included 'nurse', 'COVID-19', and 'pandemic' as either keyword or hashtags.A total of 2,440,696 most used unique words in the downloaded 582,399 tweets were included and the sentiment analysis indicated that 24.4% (It is encouraging to note that with the advent of major health crises, public perceptions on social media, appears to portray an image of nurses which reflects the professionalism and values of the profession.

Abstract

Metabolic rate is a key parameter in fish energy budgets that strongly influences the output of bioenergetics models. In this study, we tested the hypothesis that metabolic rate varies with growth history of age-1 largemouth bass Micropterus nigricans Cuvier, 1828. Two groups of fish were fed alternating maintenance or ad libitum rations of fathead minnow Pimephales promelas Rafinesque, 1820, so that over a 9-week period, initial and ending size of fish was similar. After 9 weeks, oxygen consumption was measured using static, closed respirometry. Although final body weight was similar between the two groups (means, 104–108 g), specific oxygen consumption for fish fed maintenance rations (0.094 mg O2 g−2 h−1) was 38% less than that measured for fish fed ad libitum (0.152 mg O2 g−2 h−1). Bioenergetics estimates of food consumption were similar to observed values for fish fed ad libitum (∼7% error), but for fish fed maintenance rations, the model overestimated food consumption by 65%. By accounting for changes in metabolic rate owing to reduced feeding, error in model estimates of food consumption was reduced. These findings shed new insight into factors associated with consumption-dependent error in bioenergetics models and highlight the importance of feeding history on metabolic rate of fish. Incorporating growth-dependent metabolism into bioenergetics models can improve model accuracy and allow fisheries biologists to make more informed decisions regarding fish growth and energetics. Abstract

Rationale: The prevalence and diagnostic utility of bronchodilator responsiveness (BDR) in a real-life setting is unclear. Objective: To explore this uncertainty in patients aged ⩾12 years with physician-assigned diagnoses of asthma, asthma and chronic obstructive pulmonary disease (COPD), or COPD in NOVELTY, a prospective cohort study in primary and secondary care in 18 countries. Methods: The proportion of patients with a positive BDR test in each diagnostic category was calculated using 2005 (ΔFEV1 or ΔFVC ⩾12% and ⩾200 ml) and 2021 (ΔFEV1 or ΔFVC >10% predicted) European Respiratory Society/American Thoracic Society criteria. Measurements and Main Results: We studied 3,519 patients with a physician-assigned diagnosis of asthma, 833 with a diagnosis of asthma + COPD, and 2,436 with a diagnosis of COPD. The prevalence of BDR was 19.7% (asthma), 29.6% (asthma + COPD), and 24.7% (COPD) using 2005 criteria and 18.1%, 23.3%, and 18.0%, respectively, using 2021 criteria. Using 2021 criteria in patients diagnosed with asthma, BDR was associated with higher fractional exhaled nitric oxide; lower lung function; higher symptom burden; more frequent hospital admissions; and greater use of triple therapy, oral corticosteroids, or biologics. In patients diagnosed with COPD, BDR (2021) was associated with lower lung function and higher symptom burden. Conclusions: BDR prevalence in patients with chronic airway diseases receiving treatment ranges from 18% to 30%, being modestly lower with the 2021 than with the 2005 European Respiratory Society/American Thoracic Society criteria, and it is associated with lower lung function and greater symptom burden. These observations question the validity of BDR as a key diagnostic tool for asthma managed in clinical practice or as a standard inclusion criterion for clinical trials of asthma and instead suggest that BDR be considered a treatable trait for chronic airway disease. Abstract

Abstract Background Prescribing medicines for older adults in care homes is known to be sub-optimal. Whilst trials testing interventions to optimise prescribing in this setting have been published, heterogeneity in outcome reporting has hindered comparison of interventions, thus limiting evidence synthesis. The aim of this study was to develop a core outcome set (COS), a list of outcomes which should be measured and reported, as a minimum, for all effectiveness trials involving optimising prescribing in care homes. The COS was developed as part of the Care Homes Independent Pharmacist Prescribing Study (CHIPPS). Methods A long-list of outcomes was identified through a review of published literature and stakeholder input. Outcomes were reviewed and refined prior to entering a two-round online Delphi exercise and then distributed via a web link to the CHIPPS Management Team, a multidisciplinary team including pharmacists, doctors and Patient Public Involvement representatives (amongst others), who comprised the Delphi panel. The Delphi panellists (n = 19) rated the importance of outcomes on a 9-point Likert scale from 1 (not important) to 9 (critically important). Abstract

Slow sand filters (SSF) are a simple water treatment technology providing an important alternative to conventional drinking water treatment. SSF are extensive in terms of carbon cost and chemical use but require a large land area and are complex to operate, as periodic cleaning is required to prevent filter clogging. Therefore, redundant SSF beds are required to enable water production to occur during long cleaning downtimes. Underwater skimming (UWS) is a cleaning innovation where the foulant layer (containing sand and particles) is removed using a skimmer consisting of a shrouded blade mounted on a vehicle platform. Sand, particles, and biofilm are skimmed prior to ex situ washing of the recovered sand. In this Viewpoint, we posit that the introduction of an in situ underwater skimmer operated remotely can substantially help to offset the aforementioned challenge of downtime, with its associated loss of production, enabling the technology to operate more efficiently and remain a pertinent and advantageous process option within modern water treatment facilities or possibly resource constrained settings. Otherwise, this resilient biotechnological process could be replaced by chemical and energy-intensive processes which increase the entropy of water treatment more than SSF. The anticipated benefits and challenges of UWS of SSF are discussed.

Abstract

The 25Al(p,¿) reaction has long been highlighted as a possible means to bypass the production of 26Al cosmic ¿ rays in classical nova explosions. However, uncertainties in the properties of key resonant states in 26Si have hindered our ability to accurately model the influence of this reaction in such environments. We report on a detailed ¿-ray spectroscopy study of 26Si and present evidence for the existence of a new, likely l=1, resonance in the 25Al + p system at Er=153.9(15) keV. This state is now expected to provide the dominant contribution to the 25Al(p,¿) stellar reaction rate over the temperature range, T˜0.1-0.2 GK. Despite a significant increase in the rate at low temperatures, we find that the final ejected abundance of 26Al from classical novae remains largely unaffected even if the reaction rate is artificially increased by a factor of 10. Based on new, galactic chemical evolution calculations, we estimate that the maximum contribution of novae to the observed galactic abundance of 26Al is ˜0.2M¿. Abstract

During the slow neutron capture process in massive stars, reactions on light elements can both produce and absorb neutrons thereby influencing the final heavy element abundances. At low metallicities, the high neutron capture rate of 16O can inhibit s-process nucleosynthesis unless the neutrons are recycled via the 17O(α,n)20Ne reaction. The efficiency of this neutron recycling is determined by competition between the 17O(α,n)20Ne and 17O(α,γ)21Ne reactions. While some experimental data are available on the former reaction, no data exist for the radiative capture channel at the relevant astrophysical energies.The 17O(α,γ)21Ne reaction has been studied directly using the DRAGON recoil separator at the TRIUMF Laboratory. The reaction cross section has been determined at energies between 0.6 and 1.6 MeV Ecm, reaching into the Gamow window for core helium burning for the first time. Resonance strengths for resonances at 0.63, 0.721, 0.81 and 1.122 MeV Ecm have been extracted. The experimentally based reaction rate calculated represents a lower limit, but suggests that significant s-process nucleosynthesis occurs in low metallicity massive stars.

290. An experimental investigation into filter ripening : contact filtration of lowland reservoir water

Abstract

Pilot rapid gravity filters, treating lowland reservoir water, were operated to examine chemical, physical and biological influences on filter ripening. Ripening occurred if, after backwashing, filtrate turbidities showed an initial peak, then improvement to a minimum value. Backwashing using collapse pulsing prevented biological fouling of media, and ensured constant starting head losses for three years. Ripening was examined using sand roughing filtration and dual media contact filtration with pre-ozonation and iron (III) sulphate coagulation. Turbidity removal data from sand filters showed that ripening behaviour varied with seasonal changes in applied loading. Biological maturation of filter media over several months caused improved turbidity removals. Subsequently, ripened turbidity removals varied proportionally with water temperature. Within a filter run ripened turbidity removal was independent of load at constant flow rate, but varied inversely with flow rate. Dual media filters showed ripening behaviour in roughing and contact filtration modes. Iron dosing produced superior turbidity removals. A novel coagulation control method was devised, based on the shape of the filtrate turbidity curves. Ripening time varied inversely with coagulant dose, but higher doses caused turbidity breakthrough. A temporary initial overdose could control ripening. Pre-ozonation enhanced roughing and contact filtration, but did not change the time taken to achieve maximum removals. With contact filtration ripened removals were a fixed proportion of the applied turbidity in winter, but filtrate turbidity was independent of load in summer, achieving 0.2 NTU. Flow rates of 12, 17 and 22 m.h−1 produced identical ripening curves, indicating a complex relationship between flow rate and particle capture. This was confirmed with data showing inconsistent relationships between turbidity, particle loading and ripening, and poorer ripening behaviour on longer filter runs. Further detailed particle size data were required. Six definitions of ripening were examined but none was satisfactory as ripening behaviour changed with chemical treatment and seasonal factors.

Abstract

Background: mHealth applications assist workflow, help move towards equitable access to care, and facilitate care delivery. They have great potential to impact care in low-resource countries, but have significant ethical concerns pertaining to patient autonomy, safety, and justice. Objective: To achieve consensus among stakeholders on how to address concerns pertaining to autonomy, safety, and justice among mHealth developers and users in low-resource settings, in particular for the application of image-based consultation for diagnostic support. Methods: A consensus approach was taken during a three-day workshop using a purposive sample of global mHealth stakeholders (n = 27) professionally and geographically spread. Throughout a series of introductory talks, group brainstorming, plenary reviews, and synthesis by the moderators, lists of actions were generated that address the concerns engendered by mHealth applications on autonomy, justice and safety, taking into account the development, implementation, and scale-up phases of an mHealth application lifecycle. Results: Several types of actions were recommended; key ones among them included building in risk mitigation measures from the development stage, establishing inclusive consultation processes, using open sources platform whenever possible, training all clinical users, and bearing in mind that the gold standard of care is face-to-face consultation with the patient. Recommendations of patient, community and health system participation and of governance were identified as cutting across the mHealth lifecycle. Conclusion: Priorities agreed-upon at the meeting echo those put forward concerning other domains and locations of application of mHealth. Those more forcefully articulated are the need to adopt and maintain participatory processes as well as promoting self-governance. They are expected to cut across the mHealth lifecycle and are prerequisites to the safeguard of autonomy, safety and justice.
