Background: Since the advent of navigational systems in neurosurgery, various implications have been introduced for them in spine and brain practices. Although, the range of surgeries in which these systems are being used is getting wider over time, their application is becoming more specific in certain situations. Methods: This means that defining specific indications for their usage is not as easy as it was previously thought. Brief reviewing of the available literature showed various navigation systems proposed and used in neurosurgery. Results: In this study, we review case selection criteria, techniques, and the results of neuronavigation applications in the patients underwent neurosurgical operations in the Sina hospital (Tehran, Iran) during 2011. Conclusions: The findings of our survey and experiences prove the efficacy and advantages of this technique that reduces the risk of neurovascular damage, neural tissue manipulation, operation time, and bleeding. The present study reports a very rare case of giant intradiploic angiolipoma. To our knowledge this is the second case reported of an intradiploic angiolipoma that occurred in the calvarium. We present a short controversial discussion regarding the new aspects in differential diagnosis of skull mass lesions according to the findings of this very rare case. Keywords: Skull, Bone, Lesions

Background: The present study was aimed to review the articles published by Iranian neurosurgeons regarding their experiences during Iraq-Iran 8-year conflict and compare it with reports from other conflicts. Methods: The relevant papers and studies, published up to December 2011, were searched from three databases including MEDLINE, and two Iranian databases of Iran Medex and Scientific Information Database with the keywords Iran, Iraq, conflict, battle, war, traumatic aneurysm (TA), post-traumatic epilepsy (PTE), brain infection, penetrating head wound (PHW), cerebrospinal fluid (CSF) leakage, spine injury and peripheral nerve injury. Results: A total of twenty-eight papers were found that presented PHW, development of TA, infections, PTE and peripheral nerve injuries. There were two different protocols for management of PHWs: radical surgery and minimal debridement protocol. The overall CNS infection rate was 10%. The cumulated incidence of TA was 6%. Conclusions: Conservative minimal debridement of the wounds was indicated in patients with small entrance wounds, or those with GCS greater than or equal 8 and no progressive neurological deficit. To diagnose TA before rupture angiography was performed in patients with shells or bone fragments, passed through the crowded vasculature, or with large/delayed hematoma, or if the surgeon had high index of suspicion based on neuroimaging and early debridement surgery. Surgery in a well-equipped nearby hospital after quick and safe evacuation of the victims by trained-salvaging-ancillary groups, administration of broad-spectrum antibiotics and proper antiepileptic drugs decreased the morbidity and mortality of casualties after PHW in war situations. The biases of the case selection, data collection, confounders and decreasing biases by conducting blinded controlled clinical trials were discussed. One of the most applicable methods for utilization of wind energy is the use of doubly fed induction generator (DFIG). This paper presents two various strategies for control of DFIG in wind energy conversion system. Two most common and best control methods are implemented vector control and direct torque control. The studied control methods and their performance have been compared and analyzed based on the results of stimulation. The results indicate the fact that although direct torque control method is a bit slow at the start time; due to less usage of machine parameters, less complexity of control algorithm and improvement of transient response speed of the system, this method presents better performance compared to vector control method.

Multiple primary brain tumors are commonly observed in patients with a history of brain radiation therapy or neurofibromatosis. The concomitant presence of 2 different types of brain tumors in a single location or chamber is a very rare clinical presentation in the absence of such a predisposing factor. The authors report on the case of a 16-year-old boy presenting with different types of brain tumors in 2 ventricular chambers concomitantly. This boy had a medium-sized colloid cyst of the third ventricle and a large fibrillary astrocytoma fungating from the brainstem into the floor of the fourth ventricle. The lesions were successfully excised in 2 separate surgeries. Radiotherapy was used as the adjuvant mode of therapy. There has been no sign of tumor recurrence after 16 months of follow-up. Clinical awareness and recognition of such a combination of tumors is important because they will dictate special treatment strategies depending on the individual biological aggressiveness of each tumor.

Background: It is almost an accepted scenario that trigeminal neuralgia (TN) occurs when there is a kind of vascular compression on the root entry zone of trigeminal nerve at pons. There are occasional reports about trigeminal neuralgia as the presenting sign of intracranial tumors but temporal glioma has rarely been included in the list. Case Description: We report three cases of temporal lobe glioma which presented with trigeminal neuralgia as the initial manifestation and review the relevant literature briefly. The patients were 19-, 20-, and 31-year-old males who presented with partially controlled TN. The tumor mass could be detected in paraclinical evaluations when the usual modalities of therapy for facial pain in our community were not effective. Excisional surgery led in full pain control in all the cases. Two of the patients died because of tumor recurrence after a year and the other one is being treated by adjuvants. Conclusion: We add these types of intracranial tumors to the list of the etiologies for TN and the possible mechanisms for the initiation of pain in these types of intracranial tumors are discussed.

Background: The literature does not offer the rate of protection provided by different types of helmets used, especially as it applies to developing countries. We hypothesize that standard versus nonstandard types of helmets might differ in the rate of complications of head and neck trauma occurring in victims of motorcycle accidents. Here we report the rate of occurrence, the type of injuries and differences thereof in standard and nonstandard helmet bearers, and its relevance to protection from serious injury. Methods: The data were gathered from a data set of motorcycle accident victims admitted to the emergency department of Sina Hospital (Teheran/Iran). A cross-sectional study was designed for a 6-month period of time, June to December 2007. Variants analyzed included: demographics, types of helmets used, level of education of the victims (as in: being trained for using helmets and status of holding a valid driving license). The latter variants were evaluated for possibly influencing the outcome of the injured motorcyclists using either kind of helmets. Results: Among a total of 576 injured motorcyclists who had head, face, or neck injuries, 432 (75%) were using some kind of helmet. A total of 144 (25%) of the injured patients were admitted to the neurosurgical emergency service. There were 100 patients whose data sheets contained all variables which could be included in the pilot analysis of this cohort. Discussion: All 100 subjects were male patients with the age range of 32 ± 11 years. Twenty-five percent were using standard helmets at the time of accident, 43% had no cranio-facio-cervical injury except very mild skin abrasions, and 23% had facial injury, including skin lacerations needing sutures, two nasal bone fractures, and no maxillofacial damage. Among the patients using standard helmets, 44% had head injuries which needed to be taken care of (mostly nonoperatively), while 61% using nonstandard helmets had head trauma (P > 0.05). The other variables did not reach a significant value affecting the use of either standard or nonstandard helmets in prevention of craniofacial damages. Conclusion: This pilot analysis (comprising the data from 100 cases of motorcycle accidents) could not demonstrate statistically significant differences in injury patterns of different types of helmets and variants influencing their respective use. However, it can lead the way for further analysis of larger and more comprehensive head trauma databases regarding factors contributing to the issue of head injury.

Osteoid osteoma affects the spine in only 10% of cases. More than 50% of the spinal cases involve the lumbar and cervical vertebrae. Involvement of C-1 and C-2 vertebrae has previously been reported only very rarely in the published literature. The authors report 4 cases of upper cervical osteoid osteoma, 1 involving C-1 and the other 3 C-2, and they discuss different aspects of management in similar cases. The patients were 14, 17, 35, and 46 years old, and all presented with neck pain and various degrees of painful limitation of head rotation not ameliorated by ordinary analgesics. Radionuclide isotope bone scans, CT scanning, and MR imaging were helpful preoperative diagnostic modalities. The first attempt at eradication of the lesions failed in 2 cases and the lesions could be excised totally at a second approach. Postoperatively, the patients all became pain free and gained full range of neck motion. There has been no tumor recurrence and no sign of instability in short- to medium-term follow-up. Among the several etiologies mentioned for neck pain and torticollis, osteoid osteoma of the first 2 cervical vertebrae should be considered as a possible but rare cause. Even though different kinds of management have been mentioned for osteoid osteoma, resection of the lesion remains the best option for achieving a cure.

Background/Subject: A rare case of osteoblastoma of the third cervical vertebra (C3) occurring in a 16-year-old adolescent is presented. He was admitted suffering from neck pain for more than 3 years. Method: Plain X-rays, technetium bone scanning, CT scan and MRI indicated an expansile, partially sclerotic lesion of the C3 involving the superior facet and pedicle on the right side. The lesion was excised through a posterior approach. Result: Even though the 18-month follow-up is still a very short interval in our case, the subject’s neck pain has resolved completely and there has been no sign of tumor recurrence in the recently taken imaging. There has been no clinical or radiologic sign of instability in the follow-up investigations. To the best of our knowledge, this is the third case of C3 osteoblastoma reported in the literature, highlighting the predilection of the reported cases to occur in 14- to 16-year-old boys. Conclusion: A full investigation is indicated in youngsters suffering from persisting neck pain and complete resection of the lesion can prevent recurrence and malignant transformation. Long-term follow-up is needed to declare a lifelong cure of the disease.

Background Burr-hole is the most frequently used neurosurgical treatment for chronic subdural hematoma (CSDH). Few data can be found in the literature confirming the impact of using drainage after evacuation and irrigation of the cavity containing CSDH on the outcome of the patients. It is not clear whether installing such a drainage system would reduce the recurrence rate. Objective The aim of this study was to compare the postoperative recurrence rates and the chance of occurrence of other possible complications between a group treated by burr-hole irrigation without drainage (BI-D) and burr hole-irrigation with drainage (BI+D). Materials and Methods Forty patients experiencing CSDH were included in a randomized controlled trial conducted between June 2007 and July 2009. All underwent surgery and were analyzed in this preliminary report. Twenty patients were assigned to BI+D group and the other 20 to the BD-I group. Inclusion and exclusion criteria are defined, and the cases happened to be well matched. All the epidemiologic, clinical, radiologic, procedure-related, and outcome variables were saved in the data sheaths and analyzed by the use of SPSS v 14. The patients were followed by an independent observer, who visited the patients at 1- and 6-month intervals. Results There were 28 male and 12 female patients (2.3:1) with the age range between 18 and 96 years (mean, 67 years; SD, 18 years). Glasgow coma scale scores varied between 9 and 15 in 37 patients and less than 9 in the other 3 patients. A history of head trauma was established in 25 patients. Limb weakness, loss of consciousness, and headache were the most common presentations. Recurrence occurred in one patient (5%) in BI+D and in none of the patients in BI-D group after 1-month follow up, without significant statistical difference ( P = 0.31). At the end of 6 months there was one more recurrence (5%) in BI-D group, and the recurrence rates became equal. Both patients who experience a recurrence used antiplatelet drugs, and the one in BI-D group also had diabetes. The morbidity and mortality rates were greater in BI+D group, but these finding were not statistically significant ( P = 0.37 and 0.73, respectively). There were no significant differences between the two surgical approaches regarding their relation with the recurrence of CSDH, the patient's morbidity, or mortality. The relative risk was greater for BI+D when the primary and secondary outcome measures were taken into account, but it was not significant statistically. Conclusion Type of surgical technique does not seem to be a main variable improving the outcome of such patients and may act as a confounding factor. Age, neurological status, and comorbidities seem to have more significant impact upon the surgical outcome.

The authors report the clinical features, imaging and surgical findings, and follow-up of 5 rare cases of schwannoma of the middle fossa with possible origin from the greater superficial petrosal nerve (GSPN). All patients presented to a single neurosurgical institution. The study design was a prospective follow-up of 5 cases of schwannomas of the middle fossa that most likely originated from the GSPN. The presenting features were burning pain in the eyes in 2 cases, epilepsy and behavioral changes in 2 cases, and headache in 1 case. Imaging studies showed a subtemporal mass on the anterosuperior aspect of the petrous bone in all cases. An enhancing tail on the MR images was highly suggestive of the origin of the tumor from the GSPN. All tumors were removed through a subtemporal extra- or intradural approach. Partial to complete peripheral facial nerve palsy was encountered after surgery in 3 cases, which recovered completely. Dry eye was the long-term permanent deficit in all cases. Mid- to long-term follow-up of the cases has not revealed any tumor recurrence. The 5 cases of schwannoma of the middle fossa with possible origin from GSPN were managed successfully, and their clinical presentation, differential diagnosis, and management are discussed.

Nonmissile low-velocity penetrating brain injuries are unusual among civilian population. Work-related penetrating head wounds are rarer among this group. Here, we report two rare cases of penetrating head wounds caused by industrial grinder tool. The patients (both men) were struck to the head by detached blades of the grinder tool. Depressed fractures, penetrated bone fragments, and dural lacerations were all treated by the routine fashion. Paranasal sinus damage and further cerebrospinal fluid leakage were also treated conservatively. The reported cases are unexpected variants of injuries that neurosurgeons may come across and their management is still beyond the scope of textbooks. Skill and experience of the general neurosurgeons can handle the cases properly but prevention of such incidents using safer and guarded instruments should be stressed to manufacturers.

Cerebral hydatidosis is a rare larval infection of the central nervous system affecting mostly the children and young adults. The authors report a case of a 5-year-old girl harboring a large right frontoparietal hydatid cyst. The imaging studies were not diagnostic for ‘cerebral hydatidosis’ and were interpreted as ‘brain abscess’. Attempting for simple cannulation of the lesion through a burr hole drained a clear fluid. The cannula was removed promptly but the cyst wall came out little by little through the hole in a single piece which turned out to be the entire wall of a hydatid cyst. She was treated by antihelmintic medications for 6 months and there has been no sign of recurrence after a year. Odd paraclinical imaging characteristics and unique accidental surgical event makes this case the first reported pediatric brain hydatidosis with spontaneous complete delivery of the cyst through a single burr hole which can certainly be considered a wrong attempt if undertaken intentionally.

Pilomyxoid astrocytoma (PMA) is a recently described tumor typically occurring in the hypothalamic-chiasmatic region of very young children. PMA is characterized by a more aggressive course than pilocytic astrocytoma and exhibits certain differing histological features. We report a PMA in an adolescent patient with visual field disturbance. Imaging studies revealed enlargement of the sella turcica due to a homogenously enhancing sellar and suprasellar mass identifiable both on CT scans and MRI. We believe that PMA may be included in the list of differential diagnoses of the lesions expanding the sella turcica.

Child abuse with sewing needle is a rare but well-known homicide attempt threatening the life of victims. Information about diagnosis and treatment of such cases either in the acute or chronic phases is lacking and ambiguous in the literature. This study intends to report the experience of 6 victims of homicide attempt who presented to the authors in different decades of life and were managed in different ways. This may deliver some evidences to the literature regarding management of further cases encountered by neurosurgeons.The authors had the chance of managing 6 patients referred to their trauma center harboring one or more sewing needles within their cranium. There were 3 male and 3 female patients, with 2 patients in their first decade of life, and the others, each in either decade of life. The youngest was 6, and the eldest 51 year old. The elder patients were having vague headaches, for which a plain skull x-ray or CT of the brain lead to the diagnosis of persisting intracranial foreign bodies. Chronic headache was the main complaint of the patients. Four patients underwent surgical removal of the sewing needle, and 2 are being followed.Among the 4 patients who underwent surgery, 1 died after a short period of 'akinetic mutism.' Headache and limb paresthesia improved 6 months after the operation in 2 cases, and the other 1 remained unchanged. The cases under observation have been doing well. Biochemical analysis of the rusted needle showed a composite of oxidant form of some of the elements of needle such as Fe, Mn, and Cr.In spite of standard algorithms proposed for management of penetrating head wounds, selection of the best treatment in the victims harboring sewing needles in their brain needs close cooperation between neurosurgeons, pediatricians, psychiatrists, and social workers. Furthermore, there is no absolute indication for removing sewing intracranial needles detected in the later decades of life.

Background: Sphenoid wing meningiomas extending to the orbit (ePMSW) are currently removed through several transcranial approaches. Presenting the largest surgical cohort of hyperostosing ePMSW with the longest follow up period, we will provide data supporting minilateral orbitotomy with excellent exposure for wide resection of all compartments of the tumor. Methods: A retrospective survival analysis is made of the data cumulated prospectively during a period of 34 years, including 88 cases of ePMSW with a mean follow up period of 136.4 months. The impact of preoperative variables upon different outcome measures is evaluated. Standard pterional craniotomy was performed in 12 patients (C) while the other 76 cases underwent the proposed modified lateral miniorbitotomy (LO). Results: There were 31 men and 57 women. The age range varied between 12 and 70 years. Patients presented with unilateral exophthalmos (Uex) ranging between 3 and 16 mm. Duration of proptosis before operation varied between 6 months and 16 years. The status of visual acuity (VA) prior to operation was: no light perception (NLP) in 16, light perception (LP) up to 0.2 in 3, 0.3–0.5 in 22, 0.6–0.9 in 24, and full vision in 23 patients. Postoperatively, acceptable cosmetic appearance of the eyes was seen in 38 cases and in 46 mild inequality of < 2 mm was detected. Four cases had mild enophthalmos (En). Among those who had the worst VA, two improved and one became almost blind after operation. The cases with VA in the range of 0.3–0.5 improved. Among those with good VA (0.5 to full vision), 2 became blind, vision diminished in 10, and improved or remained full in the other 35 cases. Tumor recurrence occurred in 33.3% of group C and 10.5% of group LO (P = 0.05). The major determinant of tumor regrowth was the technique of LO (P = 0.008). Conclusion: Using LO technique, the risky corners involved by the tumor is visualized from the latero‑inferior side rather than from the latero‑superior avenue. Background: There are well-known complications for shunt procedures. Shunt fracture or disconnection is the second most frequent cause of shunt malfunction in children. Shunt disconnection is not a common cause of shunt malfunction in the early period after installation, especially in the adulthood. Case Description: Fracture of the proximal (inlet) connector of a flat-based shunt installed for decompression of a large posterior fossa arachnoid cyst in a 31-year-old female with signs of increased intracranial pressure led to recurrence of her symptoms 6 months after surgery. Conclusion: Awareness of the possibility of fracture site in the junction of the inlet connector of flat bottom shunt systems is warranted and can be diagnosed by three-dimensional computed tomography (3D CT) imaging without performing shunt series study.

Objective The aim of this study was to present 4 rare cases of medullary compressive lesions presenting with IH, to discuss the indications for neurosurgical decompression in such cases, and to review the possible etiologies of IH briefly. Methods The reported cases include (1) a 19-year-old adolescent boy and a 21-year-old man with Chiari I malformation and cervical syrinx compressing or tightening the cervico-medullary junction; (2) a 35-year-old man with ependymoma extending from the lower half of the fourth ventricle to C1/C2 junction and compressing this part of the medulla; and (3) a 16-year-old adolescent boy who was referred to the emergency department in severe distress because of IH, occurring as a result of a distal PICA aneurysm located on the floor of the lower triangle of the fourth ventricle. Results Surgical decompression in all the cases was successful in withholding the hiccups. Conclusion The dorsal medullary area is not an unusual location for genesis of hiccup. When encountering a case of IH, appropriate CNS imaging studies should be included in the evaluation protocol.

Chronic subdural hematoma (CSDH) is one of the most frequent types of intracranial hemorrhage. We evaluated the independent association between abnormal CT findings in CSDH and both Glasgow coma score (GCS) on admission as a measure of consciousness and Glasgow outcome score (GOS) at discharge as a measure of outcome.One hundred and sixteen consecutive patients with CSDH were studied. The variables considered were brain atrophy, hydrocephalus, hematoma location, midline shift, and hematoma density. After univariate analysis and evaluation of the role of age as a confounder in bivariate analysis, variables that had significant association with Glasgow scores were included in the final multivariate model.Brain atrophy was the only variable with significant independent association (p0.001; OR=77.214; 95% CI=8.336-715.212) with unfavorable outcome as defined by GOS. On the other hand, hydrocephalus (p=0.042; OR=6.503; 95% CI=1.072-39.449) and higher hematoma densities (p=0.027; OR=2.597; 95% CI=1.116-6.024) independently and significantly increased the risk of severe consciousness impairment when the group with GCS12 was considered as the reference category.Brain atrophy independently increases the risk of unfavorable outcome after CSDH. Hydrocephalus and higher hematoma densities independently increase the risk of severe consciousness impairment after CSDH.

The differential diagnosis for a dorsal midline mass presenting in a newborn encompasses a wide range of pathological conditions, including spinal dysraphisms, tumoral overgrowths including teratoma and hamartoma, disturbances in regression of fetal tail, and pseudotail formation. To present (a) three rare cases of human tails and one case of tethered cord due to a midline anomaly resembling part of a human limb, (b) to discuss different kinds of clinical and pathological conditions which may be encountered in such newborns, (c) to hypothesize a theory about genesis of these congenital lesions, and (d) to remind drawbacks in preoperative evaluations and surgical management of these newborns. Four newborns with dorsal midline malformations initially diagnosed as heterotopias are presented. Appropriate preoperative investigations and proper surgical interventions leading to resection of the lesions were performed. All four were skin-covered lesions containing well-differentiated cellular elements of fat, vascular, muscular, bony, and cartilaginous origin. Midline spinal dysraphism was detectable both pre- and intraoperatively only in one case in which cord untethering was also performed along with the first surgical intervention. In the other three "tailed cases," midline bone defect could not be detected in the available X-ray films; consequently, simple excision of the tail-like lesions was done. The first patient had to be explored for repeat untethering of the cord after 4 years, but the others have not yet developed any sign of tethering during an average period of 12 years follow-up. Morphological diagnosis of these lesions is not easy, and the attending pediatrician and neurosurgeon should be familiar with the differential diagnosis of such lesions and be prepared for possible time-consuming operation mandatory to achieve total resection of the lesion in a newborn under general anesthesia. Reviewing the possible theories regarding the genesis of such anomalies, it is hypothesized and suggested that all similar cases could have been of hamartomatous origin rather than defects of embryogenesis.

I read with interest the manuscript by Saidinia S et al. (1) about the neuro-manifestations of osteopetrosis. In these communications, the authors have succeeded in forwarding a well-developed discussion of different aspects of the illness and presented their experience with cases, referred to their Neurosurgical department and treated surgically by their group. Even though the information transferred by the authors is classically acceptable but there are delicate points to be reminded so that the readers might not be misled.

During military conflicts, surviving victims traditionally underwent large enough craniectomy or craniotomy to achieve extensive debridement of the in-driven bone, shell fragments, and debris. Meanwhile, the completely as well as the partially devitalized adjacent brain tissue would also be debrided. This might have led to additional neurologic deficit or lesser degree of recovery of functions.We studied the efficacy and safety of much more limited or even absence of surgical intervention in a selected group of wartime head-wounded patients. Among 1,150 war victims with penetrating head wounds referred to two major centers affiliated with Tehran University of Medical Sciences during the 8-year period spanning the Iran-Iraq conflict, there were 191 head-wounded patients who underwent either no surgical intervention or a very limited debridement. We have attempted to follow up as closely as possible for more than 8 years. Ninety-nine of them fulfilled the criteria to be included in this series. Close follow-up for development of cerebral infections, functional recovery, or development of new neurologic deficits and epilepsy were performed. At least 3 contrast-enhanced CT scans were taken for each patient. All patients were victims of the battle-field areas with low- to moderate-velocity missile or shell fragment injury. The Glasgow Coma Scale (GCS) scores of the patients were between 8-14 when visited by the first attending neurosurgery staff. The factors considered to be effective in the outcome analysis were the entry point, the number of the in-driven bone fragments, the retained shell fragments, the brain compartments affected, and the paranasal sinus or skull base involvement with or without cerebrospinal fluid (CSF) fistula.There were 13 unilobar, 44 bilobar, 4 trilobar, 38 transventricular, and 3 skull base lesions. The number of retained metal fragments varied between 1 to 6. The number of retained bone fragments varied between 1 to 5 in 73 victims and more than five chips in other cases. The outcome was good (back to work) in 90 patients and poor (severely disabled) in 4. Five patients died because of severity of brain damage and meningitis. The effect of the different enumerated variables upon the outcome was measured using chi(2) and Fisher exact test, which was nonsignificant for all of the variables except for the orbitofacial entry point of injury (p = 0.00006).This study seems to indicate that not only is reoperation for retained bone or shell fragments unnecessary, but surgeons having modern neuroradiological instruments available to detect the proper explanation for changes in GCS of the patients may decide not to proceed with any surgical intervention in a good number of patients. As a result of the "no surgical treatment protocol," one can preserve cerebral tissue without exposing the patient to undue risk of seizures, infection, or death by leaving behind some or all the in-driven bone and shell fragments.

Background: Mycotic cerebral aneurysms are uncommon. We intend to report the first case of multiple mycotic cerebral aneurysms due to Brucella infection that were treated surgically. Case Description: A 34-year-old man with neurobrucellosis presented with intracerebral haemorrhage (ICH). Three mycotic aneurysms were detected in the vicinity of middle cerebral artery (MCA). Medical treatment failed to treat them and aneurysms had to be managed surgically. Conclusion: Brucella-related cerebral mycotic aneurysm has rarely been reported. This is the first report of three mycotic aneurysms occurring in a young man with neurobrucellosis treated surgically.

BACKGROUND Growing skull fractures rarely develop in the skull base region. To the best of our knowledge, only two similar cases have been reported in the English literature. This rare complication, which can occur even after a mild head injury, can produce exophthalmos and threaten the vision. METHODS The clinical and radiological findings of expanding leptomeningeal cysts extending into the orbit in nine patients referred to the department of Neurosurgery are presented. After appropriate investigations confirming the presence of the expansile retroglobal lesion, surgical exploration was performed via fronto-lateral or the preferred fronto-basal approach. The variable findings are denoted and the relevant literature is also reviewed. RESULTS Frontobasal head injuries play an important role in pathogenesis of these traumatic expansile leptomeningeal cysts. In this series of nine young girls, 6.65% (six out of nine) injuries happened in the first decade of life with an interval of 2 to 12 months (mean = 6.7, SD = 9.7). High-resolution coronal view computed tomography (CT) scanning with bone density window images, and high intensity cystic lesions visible on T2-weighted coronal MR images were diagnostic clues. CONCLUSIONS Growing fractures of the anterior skull base may complicate the natural course of healing of any minor frontobasal head injury, especially during childhood. Good quality imaging is mandatory in cases of progressive unilateral exophthalmos. Proper surgical intervention will lead to a good cosmetic result.

The authors report four very rare radiation-associated tumors (or radiation-induced tumors; RITs) of the central nervous system (CNS) and review the literature on this topic. The purpose of this study was to determine the possible relationship between the harmful effects of radiation therapy, the shortest and the longest interval between the time of irradiation and the occurrence of the secondary tumor, and possible predisposing factors. The tumorigenic effects of therapeutic irradiation of the CNS have been mentioned in the literature, but the authors' literature search did not disclose either many reports of cases such as their own or a satisfactory and concise discussion on the different aspects of the late and catastrophic complications of this method of adjunct therapy to the CNS. Four rare cases of RIT in three patients are presented: a unique case of intradural meningioma of the cervical spine, which was irradiated successfully only for the patient to present with a new high-grade cerebral astrocytoma 4 years later, a paraventricular cavernoma and a fronto-temporo-orbital chondrosarcoma. These second RITs became symptomatic in the 17th, 16th and 15th years of life, respectively, in these young patients. The primary lesions were ependymomas, two in the IV ventricle and one in the left hemisphere. The time intervals between radiation and secondary tumor presentation were 14 and 18 years, 9 years and 28 months, in the order in which these patients presented. All the patients survived the second operation except the one with chondrosarcoma, who died in spite of repeated surgical interventions and adjunct therapies. It is concluded that the development of secondary RITs does not necessarily require a very long time interval; that although sarcomas are the most common RITs of the CNS in childhood and adolescence, benign and other rare and curable lesions may also occur in the field or vicinity of the field of radiation; and that in view of the possibility of occurrence of different types of RITs after varying time intervals in a single patient, whole-life followup of similar patients is mandatory.

Objective To evaluate the correlation between admission Glasgow Coma Scale (GCS) and Glasgow Outcome Scale (GOS) in chronic subdural hematoma (CSDH). Methods 128 consecutive patients (107 men and 21 women) with CSDH were studied and the correlation between GCS on admission and postoperative GOS was determined.
