Dagens Svenska elnät har ett stort behov av renovering och upprustning vilket ger ett stort investeringsbehov under de närmast kommande åren. Det gör att det är viktigt att finna nya entreprenörer utöver de som idag redan finns. Hitta en väg för att bli mer attraktiv och intressant för dessa entreprenörer. Denna rapport belyser två metoder som uppdragsgivare har att förhålla sig till vid projektering. Det ena är Elektrisk Byggnads Rationalisering (EBR) det andra är Allmän Material- och Arbetsbeskrivning (AMA). Systemet med EBR är ett tryggt system som grundar sig på mångårig statistik över installationer, renoveringar och underhåll. Där har man fördelar med standardiserade komponenter som är kostnadsekonomiska och skapar en kvalitetssäkring av utförda arbeten. AMA är ett referensverk för uppbyggandet av projekt från grunden. AMA är friare i sin struktur när man skall projektera för en anläggning. Man behöver inte förhålla sig till mer strikta arbetssätt. Denna rapport ser till skillnader i EBR/AMA och ser till vad som är bra förändringar för att attrahera nya intressenter på marknaden. Vad som framöver går att förändra och samtidigt påverka vid projektering och upphandlingsunderlag., Today's Swedish power grid has a major need for renovation and upgrading, which gives a big investment requirement over the next few years. This makes it important to find new entrepreneurs in addition to those already available today. Find a way to become more attractive and interesting for these entrepreneurs. This report highlights two methods that outsourcer have to relate to. First method is Electrical Building Rationalization (EBR) and the other method is general material and work description (AMA). The system of EBR is a secure system based on long-term statistics from installations, renovations and maintenance. There the advantages of standardized components are that they are cost-effective and create a quality of work performed. AMA is a reference tool for building projects from scratch. It's freer in its structure when planning a facility. One does not need relate to more strict approach. This report looks at the differences in EBR / AMA and looks at what are good changes to attract new actors in the market. What can be changed in the future and at the same time affecting projecting design and procurement documentation.

254. Recycling of concrete for sustainable road construction : Why are proven methods not currently used?

- Abstract
This report aims to investigate why proven methods for recycling concrete waste as road construction material are not practiced in Sweden. An additional objective is to investigate how concrete is handled as a waste product and whether it would be environmentally friendly and financially beneficial to clients and contractors. Information has been extracted via interviews conducted with experts from various positions within the civil engineering industry. Additional information was obtained through literature studies and questionnaires sent and received via email. Results which were frequently mentioned by engineering professionals included the extra expense of transporting and processing crushed concrete, parties involved in the design and construction processtend to follow traditional methods of using tried and tested virgin materials, the assumption of responsibility for structural failure due to alternative materials and general lack of knowledge surrounding crushed concrete as a construction material. Conclusions are that crushed concrete is suitable for construction of subbases in roads and base courses of cycle/pedestrian paths. Traditionally used virgin materials are generally less expensive than crushed concrete. Existing legislation makes the use of recycled construction material difficult. Awareness and education regarding recycled concrete, as a construction material, should be increased.

Julien Poinssot (1844-1900), Louis Poinssot (1879-1967) et Claude Poinssot (1928-2002), archéologues de père en fils, ont été, à des titres divers, des acteurs et des témoins privilégiés de l’histoire de l’archéologie en Tunisie. Entre les années 1870 et 1990, ils ont réuni un ensemble documentaire exceptionnel et original sur l’histoire de l’Afrique du Nord, de l’Antiquité au XXe siècle. Constitué d’une bibliothèque de plus de 4 500 volumes, de 25 mètres-linéaires d’archives issues de leurs recherches et de celles de leurs confrères – Bernard Roy (1846-1919), Paul Gauckler (1866-1911), Alfred Merlin (1876-1965) – le fonds Poinssot offre aux chercheurs d’aujourd’hui une documentation sans équivalent sur l’archéologie française en Afrique du Nord. Les contributions présentées ici explorent l’activité de trois générations de chercheurs, mus autant par le culte de l’archéologie que par le respect du document. Elles mettent au jour les multiples aspects de l’activité scientifique de cette famille d’archéologues, tout autant que les contraintes intellectuelles, matérielles et économiques qui pesaient sur elle.

Increased operational risk in recent years has been enhanced by the creation of products and services ever more complex financial innovations, increased competition, etc.. 272. Primary Biliary Cirrhosis Presenting with Gastritis, Hyperlipidemia and Marked Weight Loss.

We report an unusual presentation of primary biliary cirrhosis. We present the case of a 31-year-old Indian woman who presented to the hospital with non specific complaints of gastritis, reduced appetite and marked weight loss and no complaints of pruritus. Serum liver enzymes were elevated. Cytoplasmic pattern of antinuclear antibodies was found on immunofluorescence microscopy accompanied by positive anti smooth muscle antibody. Endoscopy revealed esophageal varices. Following a liver biopsy, the diagnosis of primary biliary cirrhosis was made. It is important to consider the diagnosis of hepatic cirrhosis when patient presents with gastritis, reduced appetite and marked weight loss in both the presence and absence of ascites. We examine Van Wynsberghe and Robbins (JAMA 25:719-735, 2019) critique of the need for Artifcial Moral Agents (AMAs) and its rebuttal by Formosa and Ryan (JAMA 10.1007/s00146-020-01089-6, 2020) set against a neo-Aristotelian ethical background. Neither Van Wynsberghe and Robbins (JAMA 25:719-735, 2019) essay nor Formosa and Ryan’s (JAMA 10.1007/s00146-020-01089-6, 2020) is explicitly framed within the teachings of a specifc ethical school. The former appeals to the lack of “both empirical and intuitive support” (Van Wynsberghe and Robbins 2019, p. 721) for AMAs, and the latter opts for “argumentative breadth over depth”, meaning to provide “the essential groundwork for making an all things considered judgment regarding the moral case for building AMAs” (Formosa and Ryan 2019, pp. 1–2). Although this strategy may beneft their acceptability, it may also detract from their ethical rootedness, coherence, and persuasiveness, characteristics often associated with consolidated ethical traditions. Neo-Aristotelian ethics, backed by a distinctive philosophical anthropology and worldview, is summoned to fll this gap as a standard to test these two opposing claims. It provides a substantive account of moral agency through the theory of voluntary action; it explains how voluntary action is tied to intelligent and autonomous human life; and it distinguishes machine operations from voluntary actions through the categories of poiesis and praxis respectively. This standpoint reveals that while Van Wynsberghe and Robbins may be right in rejecting the need for AMAs, there are deeper, more fundamental reasons. In addition, despite disagreeing with Formosa and Ryan’s defense of AMAs, their call for a more nuanced and context-dependent approach, similar to neo-Aristotelian practical wisdom, becomes expedient.

Black lung disease is a crippling occupational lung disease experienced by coal miners throughout the world. However, this disease was not always recognized by the medical profession and required significant efforts on the part of miners’ unions to force mainstream recognition. The historiography on the subject is limited, especially with respect to the relationship between organized medicine and organized labor. This work further explores this relationship, particularly how this relationship differed between the parties in Wales and in West Virginia. In doing so, it portrays a more detailed picture of the fight for black lung recognition as well as highlights the different relationships between medicine and miners in the two regions.

Background Hospitalized persons who inject drugs are at a greater risk of adverse hospital outcomes including discharge against medical advice, inpatient illicit drug use, overdose, and death. However, there are limited data on the frequency and outcomes of these events in the United States. Methods This retrospective analysis included patients with injection-related infections receiving a protocol for injection drug use (IDU) at University of Alabama at Birmingham Hospital from 2016 to 2017. In-hospital IDU was suspected or reported drug usage plus confirmatory drug screen, and documented discharges "against medical advice" were deemed patient-directed discharges (PDD). We analyzed the frequency of and associations between in-hospital IDU, PDD, 30-day readmission, and deaths (between 2016 and 2019) using McNemar's tests. Logistic regression models evaluated the association between PDD, in-hospital IDU, readmission, and death. Results Overall, 83 patients met inclusion criteria: 28 (34%) with in-hospital IDU, 12 (14%) PDD, 9 (11%) died, and 12 (14%) 30-day readmission. In-hospital IDU was significantly associated with PDD (P = .003), 30-day readmission (P = .005), and death (P = .0003). Patient-directed discharges and 30-day readmission were not significantly associated with death nor with each other. Conclusions In a cohort of patients receiving inpatient care for injection-related infections, illicit drug use, PDD, 30-day readmissions, and death were common. Furthermore, patients who use illicit drugs while hospitalized are significantly more likely to leave early, be readmitted, and/or die. We must design models of care that prevent adverse outcomes, including drug use and PDD, to reduce barriers to evidence-based treatment of infections. [ABSTRACT FROM AUTHOR]

Background : The detection of anti-mitochondrial antibodies (AMA) is considered a hallmark in diagnosing primary biliary cholangitis (PBC). The most important AMA-subtype is AMA-M2 directed against the E2-subunit of pyruvate dehydrogenase. It is common clinical interpretation that lack of M2 due to immunoblotting (IB) indicates absence of specific auto-reactivity. We aimed to define whether M2-IB confirmation is linked to clinical outcomes. Methods : Our cohort comprised 302 patients who tested positive for AMA with indirect immunofluorescence between 2006 and 2015. One hundred and eighty-four subjects (60.9%; male n = 29 [15.8%]) were tested M2-positive by confirmatory IB, whereas 118 subjects were IB-M2-negative (39.1%; male n = 25 [21.2%]). The natural history of 236 patients (78.1%) was evaluated by clinical follow-up (FU) assessing causes of death, leading health condition and response to PBC standard therapy if applicable. Results : Mean time to FU was 6.8 years. Twenty-eight M2-positive patients (15.2% of 184) and 28 M2-negative patients (23.7% of 118) had died (p = 0.0958). Thirty-four M2-positives (18.5%) and 32 M2-negatives (27.1%) were not available for FU. According to the clinical course by the time of FU, subjects were allocated to one of four groups: a) 34 patients had known PBC with n = 16 having an adequate and 18 an inadequate treatment response, b) 1 de novo PBC was detected, c) 13 were AMA-positive without biochemical evidence of PBC and d) 9 subjects were tested AMA-negative at FU. These numbers were comparable to M2-positive subjects with similar long-term clinical outcome. Conclusion : Our data suggest that the clinical value of confirmatory M2 immunoblotting in the diagnostic routine of PBC is overestimated as the clinical course appears not to be related to the test result.

Background & Aims The role of liver transplantation (LT) for the relief of fatigue in patients with primary biliary cirrhosis (PBC) is unclear, and while many centers exclude fatigue as an indication for transplantation, there have been no studies to prospectively evaluate the impact of LT on fatigue. We aimed at assessing the severity of fatigue in LT candidates with PBC and the impact of LT on fatigue. Methods In a prospective, longitudinal study, we used the PBC-40 questionnaire in 49 adult patients with PBC at listing and at 6, 12, and 24 months after LT and in two sex- and age-matched cohorts of community controls and non-transplanted PBC patients. Correlation analysis was used to assess the relationship between liver function and fatigue. ANOVA was used to compare the variation of fatigue score before and after LT. Results There was no correlation between MELD and fatigue before LT (r2 = 0.01). The dissertation aims at showing the differences in dealing with the death issue in characteristic works of art of both Czech and world literature. Above all, it pursues the comparison of two novels Život střídá smrt ("Life Becomes Death") by Petr Prouza and Nebe nemá dno ("Heaven is Bottomless") by Hana Andronikova. Both unique books belong to key Czech novels with the topic of death. Moreover, both of them deal with death from the point of personal experience, which means they are strongly autobiographical. The dissertation concentrates on creative processes and topical aspects, grasps autobiographical elements of both novels and all the identities and differences in the approach to the same topic. Therefore, the dissertation is devoted to listing the works of art dealing with death, further on to the analyses, interpretation and comparison of the novels Život střídá smrt ("Life Becomes Death") and Nebe nemá dno ("Heaven is Bottomless") together with the reflexion of reader's experience.

Através deste trabalho pretende-se explicar quais as vantagens da utilização da ferramenta de Benchmarking na administração pública. Para isso serão abordadas as inovações que Portugal desenvolveu e as inovações que Portugal ajuda a implementar noutros países. Em complemento, serão ainda abordadas as inovações de outros países adotadas por Portugal e mais tarde implementadas. O objeto deste estudo é a Agência para a Modernização Administrativa (AMA) e é através das suas atividades que podemos concluir o que Portugal faz bem, o que faz de melhor e em que é que está na vanguarda no que diz respeito à Administração Pública. A new approach for the Azimuthal Mode Analysis (AMA) of over- and subsampled sound fields in flow ducts of turbomachines using circumferential sensor arrays is presented which is based on Compressed Sensing. Compressed Sensing is a framework to solve underdetermined systems of linear equations under the assumption that the solution vector is sparse. In the context of AMA, such sparse mode spectra often occur in the analysis of tonal sound field components generated by rotor-stator interaction. Important features such as stability and accuracy are investigated by analysis of a wide range of simulated and measured pressure data and comparison with commonly applied methods. Also two different optimisation approaches for the design of circumferential sensor arrays are introduced which allows a reduction of the number of sensors for large modal ranges if only a limited number of modes are dominant. Furthermore, the remaining mode amplitudes are estimated exploiting the deconvolution property of the applied Compressed Sensing algorithm.

Lawmakers mandated MIPS as part of the bipartisan 2015 Medicare Access and CHIP Reauthorization Act (MACRA) ending the sustainable growth rate formula that had repeatedly threatened to cause deep cuts in Medicare payments to doctors. On a slide presentation before the vote, the MedPAC staff said MIPS cannot succeed. Most of the people experience digestive problems like constipation, diarrhoea, acid eructations, loss of appetite etc. at some stage of life. These are the acute conditions that appear due to Agnimandya (reduced power of digestion, assimilation and metabolism). Conditions that may cause Agnimandya includes changes in diet and lifestyle and chronic diseased conditions. When the Agnimandya or the conditions that may cause Agnimandya persist for longer duration it turns into a life threatening disease. When Doshas (body humours) get aggravated, they affect the Agni (Power of digestion, assimilation and metabolism) and thus the food taken is not digested properly forming the Ama (morbid material). Ama when formed is accumulated in the body over the period of time, forming roots of many diseases. In the present case study the patient has Agnimandya, because of the chronic metabolic conditions. The Agnimandya and consequent nutritional deficiencies are addressed, while preparing the treatment protocol. The central focus of Ayurvedic treatment is the management of Agni and the Ama. The diet was advised on the line of Samsarjana Krama (Sequential administration of liquid diet to normal diet to kindle the Agni or digestive power) and the effect of treatment was analysed after 1 month. Amavata is a disease caused due to the vitiation or aggravation of Vayu associated with Ama. Vitiated Vayu circulates the Ama all over the body through Dhamanies, takes shelter in the Shleshma Sthana (Amashaya, Sandhi, etc.), producing symptoms such as stiffness, swelling, and tenderness in small and big joints, making a person lame. The symptoms of Amavata are identical to rheumatism, which include rheumatoid arthritis and rheumatic fever. It is observed that rheumatism is an autoimmune disorder, which is among the collagen disorders having strong and significant parlance with Amavata. Various drug trials were already carried out on Amavata, yet there is a lacuna in the management of Amavata. Hence, in the present clinical study, 28 patients were selected and kept on 'Amrita Ghrita'. All the patients were investigated for complete blood count (CBC), rheumatoid arthritis (RA) titer, Antistreptolysin O (ASO) titer, C-reactive protein (CRP) titer, platelet count, urine routine, and microscopic, before and after treatment. The collected data was distributed according to age, sex, and prakruti, and a t-test was applied for the clinical assessment of the subjective and objective parameters of 'Amrita Ghrita,' and it has shown significant reduction in the positivity of the RA titer (t > 5.09, at the 0.001% level), ASO titer (t > 4.08, at the 0.001% level), and CRP titer (t > 4.82, at the 0.001% level), and weight gain (t > 5.12, at the 0.001% level), as also an increase in Hb% (t >9.22, at the 0.001% level), and platelet count (t> 5.90, at the 0.001% level), and decrease in ESR (t > 9.70, at the 0.001% level).

298. "I'm Leaving": Factors That Impact Against Medical Advice Disposition Post-Trauma.

Background: Discharge against medical advice (AMA) is an important, yet understudied, aspect of health care-particularly in trauma populations. AMA discharges result in increased mortality, increased readmission rates, and higher health care costs., Objective: The goal of this analysis was to determine what factors impact a patient's odds of leaving the hospital prior to treatment., Methods: We performed a retrospective analysis of the National Trauma Data Bank on adult trauma patients (older than 14 years) from 2013 to 2015. Of the 1,770,570 patients with known disposition, excluding mortality, 24,191 patients (1.4%) left AMA. We ascertained patient characteristics including age, sex, race, ethnicity, insurance status, ETOH, drug use, geographic location, Injury Severity Score (ISS), injury mechanism, and anatomic injury location. Multivariate logistic regression models were used to determine which patient factors were associated with AMA status., Results: Uninsured (odds ratio [OR] 2.72; 95% confidence interval [CI] 2.58-2.86) or Medicaid-insured (OR 2.50; 95% CI 2.37-2.63) trauma patients were significantly more likely to leave AMA than patients with private insurance. Compared to white patients, African-American patients (OR 1.06; 95% CI 1.02-1.11) were more likely, and Native-American (OR 0.62; 95% CI 0.52-0.75), Asian (OR 0.59; 95% CI 0.49-0.69), and Hispanic (OR 0.80; 95% CI 0.75-0.85) patients were less likely, to leave AMA when controlling for age, sex, ISS, and type of injury., Conclusions: Insurance status, race, and ethnicity are associated with a patient's decision to leave AMA. Uninsured and Medicaid patients have more than twice the odds of leaving AMA. These findings demonstrate that racial and socioeconomic disparities are important targets for future efforts to reduce AMA rates and improve outcomes from blunt and penetrating trauma., (Copyright © 2020 Elsevier Inc. 299. Metabolic Dysfunction in Myalgic Encephalomyelitis/Chronic Fatigue Syndrome Not Due to Anti-mitochondrial Antibodies.

Metabolic profiling studies have recently indicated dysfunctional mitochondria in myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). This includes an impaired function of pyruvate dehydrogenase complex (PDC), possibly driven by serum factor(s), which leads to inadequate adenosine triphosphate generation and excessive lactate accumulation. A reminiscent energy blockade is likely to occur in primary biliary cholangitis (PBC), caused by anti-PDC autoantibodies, as recently proposed. PBC is associated with fatigue and post-exertional malaise, also signifying ME/CFS. We herein have investigated whether ME/CFS patients have autoreactive antibodies that could interfere with mitochondrial function. We found that only 1 of 161 examined ME/CFS patients was positive for anti-PDC, while all PBC patients (15/15) presented significant IgM, IgG, and IgA anti-PDC reactivity, as previously shown. None of fibromyalgia patients (0/14), multiple sclerosis patients (0/29), and healthy blood donors (0/44) controls showed reactivities. Anti-mitochondrial autoantibodies (inner and outer membrane) were negative in ME/CFS cohort. Anti-cardiolipin antibody levels in patients did not differ significantly from healthy blood donors. Purpose – This article aims to provide an overview of the popular website Reddit. Design/methodology/approach – In many cases, items that are popular on Reddit become viral, appearing on other social media sites and in the news several days later. Findings – The site’s content is supplied by its users, and the popularity of that content is also determined by the membership. While the site can be described simply as an aggregator of user provided content, this simple description does not adequately capture the essence of the Reddit community and the impact of that community and its generated content on society.
