Amaç: Bu çalışmada amaç; diyafram kalınlığının weaning üzerine etkilerini ultrasonografi (USG) ile değerlendirmektir.Gereç ve Yöntem: Prospektif olarak yapılan bu çalışmada; hastaların demografik özellikleri, yoğun bakıma yatış ve mekanik ventilasyon endikasyonları, ek hastalıkları, diyafram kalınlığı, Sıralı Organ Yetmezliği Değerlendirmesi (SOFA) skoru ve Basitleştirilmiş Akut Fizyoloji Skoru (SAPS) II, mortalite oranı, periferik oksijen satürasyonu (SpO2), IMV süresi, dakika hacmi, solunum sayısı (RR), ekspiratuar hacimleri, RSBI değerleri kaydedildi.Hastalar invaziv mekanik ventilatör (IMV) basınç desteği (PS) modunda 30 dakika boyunca spontan olarak ventile edildi . Diyafram kalınlığı (DT), B moduna ayarlanmış 7-10 MHz lineer ultrason probu kullanılarak ölçüldü. Sağ hemidiyafram, 8. ve 10. interkostal boşluklar arasındaki midaksiller çizgide diyafram ve göğüs kafesinin birleştiği bölgede görüntülendi. Ölçümler sonrasında hastalar stabil hale geldikten sonra 1 saat boyunca T-tüpe alındı. T-tüpte spontan solunum denemeleri (SBT)'ni başarıyla tamamlayan hastalar ekstübe edildi. Hastalar başarılı ve başarısız weaning olmak üzere 2 gruba ayrıldı.Bulgular: Toplam 72 hasta çalışmaya dahil edilme kriterlerini sağlayarak değerlendirilmeye alındı. Hastaların 4 tanesi 24 saatten daha kısa süre IMV desteği aldığı için çalışma dışı bırakıldı. Diyafram kalınlık indeksi (DTI) ve diyafram kalınlaşma fraksiyonu (DTF) oranlarında gruplar arasında bir farklılık gözlenmedi. İnspirasyon sonunu diyafram kalınlığı (DTV) ve ekspirasyon sonunu diyafram kalınlığı (DFRV) olarak tanımlanan diyaframın kalınlık ölçümleri başarılı weaning grubunda daha yüksekti. Weaning başarısını tahmin etmek için DTV'nin en iyi eşik değeri 0.72, DFRV değeri 0.669 olarak bulundu.Sonuç: Weaning başarısı diyafram fonksiyonları, diyafram kalınlık parametreleri ve akciğer kapasiteleri ile yakından ilişkilidir. USG, diyaframı değerlendirmek için kullanılabilecek pratik, hasta başı uygulanabilen, noninvaziv bir yöntemdir. DTV ve DFRV weaning sürecinde başarıyı tahmin edebilecek parametreler arasındadır. DTF ve DTI weaning başarısını öngörmede yetersiz bulundu., Purpose: The aim of this study; to evaluate the effects of diaphragm thickness on weaning with ultrasonography (USG).Materials and Methods: In this prospective study; demographic characteristics of the patients, indications for hospitalization in the intensive care unit and mechanical ventilation, comorbidity, diaphragm thickness, Sequential Organ Failure Assessment (SOFA) score and Simplified Acute Physiology Score (SAPS) II, mortality rate, peripheral oxygen saturation (SpO2), invasive mechanical ventilator (IMV) duration, minute volume, respiratory rate (RR), expiratory volumes (VTE), RSBI values were recorded. The patients were ventilated spontaneously for 30 minutes in pressure support (PS) mode in IMV. Diaphragm thickness (DT) was measured using a 7-10 MHz linear ultrasound probe set to B mode. The right hemidiaphragm was visualized in the midaxillary line between the 8th and 10th intercostal spaces, at the junction of the diaphragm and rib cage. After the measurements were stabilized, the patients were taken to the T-tube for one hour. The patients who successfully completed T-tube spontaneous breathing trials (SBT) were extubated. The patients were divided into two groups as successful and unsuccessful weaning.Results: A total of 72 patients were evaluated by meeting the inclusion criteria. Four of the patients were excluded from the study because they received IMV support for less than 24 hours. There was no difference between the groups in the ratios of diaphragm thickness index (DTI) and diaphragmatic thickening fraction (DTF). The thickness measurements of the diaphragm, defined as diaphragm thickness at the end of inspiration (DTV) and diaphragm thickness at the end of expiration (DFRV), were higher in the successful weaning group. The best threshold value of DTV to predict weaning success was found to be 0.72 and DFRV value to be 0.669.Conclusion: Weaning success is closely related to diaphragm functions, diaphragm thickness parameters and lung capacities. USG is a practical, bedside, noninvasive method that can be used to evaluate the diaphragm. DTV and DFRV are among the parameters that can predict success in the weaning process. DTF and DTI were found to be insufficient in predicting weaning success.

Herein, we report a controllable pathway to accelerate the polymerization kinetics of dopamine using ultrasound as a trigger. The use of ultrasound was demonstrated to dramatically accelerate the slow liquid phase reaction kinetics and increase the deposition rate of the polydopamine coating on the surface of polymeric membranes.

Patients, who have higher fibrosis and necroinflammatory activity scores, are at high risk for hepatocellular carcinoma and cirrhosis. Therefore, it is important to distinguish between active and inactive hepatitis in this group. Materials and Methods: Sixty-six treatment-naive, non-cirrotic, HBe antigen (HBeAg)-negative and a PNALT and a level of a hepatitis B virus (HBV) DNA level of >= 2000 IU/mL were included in this study. lshak's scoring system was used for histopathological evaluation. Chronic hepatitis was defined as a fibrosis score of higher than/equal to 2 and/or a histological activity index score of higher than 4. Results: The percentage of patients diagnosed with advanced fibrosis score and high necroinflammatory activity was 65% and 48%, respectively. Purpose- The purpose of this study is to analyze the relationship between social media adoption (SMA) and export performance (EP) inTurkey as an emerging market economy. This paper also examines the role that the adoption of social media plays in firm export performanceand whether this role is moderated by firm size.Methodology- The study employs the panel tobit model to investigate the relation between SMA and EP. The Borsa Istanbul ManufacturingIndex, comprised of 150 firms, constitutes our sample frame. The timeframe for our dataset covers the period of 2010-2020. While analyzingthe determinants of EP, R&D intensity, marketing intensity, firm size, firm age, internationalization age and sector are control variables, andSMA is independent variable. The SMA was proxied by Social Media Index (SMI), Facebook, Instagram, Twitter, and YouTube dummies.Following Slover-Linett and Stoner (2011), we choose a simple criterion to define an official social media presence. A social media presencewas considered official if linked from the institutional website, directly from the homepage or from the secondary level of the website. SMIis defined as the sum of Facebook, Instagram, Twitter, and YouTube dummies. Facebook dummy assumes the value of '1' when the firm hasa Facebook account in a particular year, or '0' otherwise. Instagram, Twitter and YouTube dummies were defined in the same way.Findings- The analysis reveals that the coefficient for SMI is positive and statistically significant. The results also showed that the coefficientfor the interaction between SMI and firm size is positive and significant. Moderation examines under what conditions, the SMI-EP relationvaries. The firm size has a positive and significant moderating role on the relation between SMI and EP. Although, the coefficients forFacebook and Instagram are positive and significant, the coefficients for Twitter and Youtube are insignificant. The findings also indicate thatwhile R&D intensity is negatively and statistically significant, marketing intensity and firm size are positively, and statistically significant andinternationalization age is negatively and statistically significant for firms with a high level of EP. We were unable to find support for anassociation between EP and firm age.Conclusion- The results suggest a positive effect of social media adoption on export performance and this relationship is moderated by firmsize. Turkish companies that adopt social media benefit from higher levels of export performance. The result of this study contributes to theexisting literature pointing to the importance of social media adoption for exporting firms in emerging markets.

Background The site frequency spectrum summarizes the distribution of allele frequencies throughout the genome, and it is widely used as a summary statistic to infer demographic parameters and to detect signals of natural selection. The use of high-throughput low-coverage DNA sequencing data can lead to biased estimates of the site frequency spectrum due to high levels of uncertainty in genotyping. Results Here we design and implement a method to efficiently and accurately estimate the multidimensional joint site frequency spectrum for large numbers of haploid or diploid individuals across an arbitrary number of populations, using low-coverage sequencing data. The method maximizes a likelihood function that represents the probability of the sequencing data observed given a multidimensional site frequency spectrum using genotype likelihoods. Notably, it uses an advanced binning heuristic paired with an accelerated expectation-maximization algorithm for a fast and memory-efficient computation, and can generate both unfolded and folded spectra and bootstrapped replicates for haploid and diploid genomes. On the basis of extensive simulations, we show that the new method requires remarkably less storage and is faster than previous implementations whilst retaining the same accuracy. When applied to low-coverage sequencing data from the fungal pathogen Neonectria neomacrospora, results recapitulate the patterns of population differentiation generated using the original high-coverage data. Conclusion The new implementation allows for accurate estimation of population genetic parameters from arbitrarily large, low-coverage datasets, thus facilitating cost-effective sequencing experiments in model and non-model organisms.

Producing block co-polymer-based nanofiltration (NF) membranes with sharp molecular weight cutoffs via an efficient method exhibiting persistent size-based separation quality is challenging. In this study, this challenge was addressed by reporting a facile approach to fabricate pentablock co-polymer (PBC)-based thin-film composite (TFC) NF membranes. The PBC, consisting of temperature-responsive Pluronic F127 (PEO-b-PPO-b-PEO) middle blocks and pH-responsive poly(N,N-(diethylamino)ethyl methacrylate) end blocks, were synthesized by atom-transfer radical polymerization. This polymer was then attached electrostatically to the surface of polysulfone/sulfonated polyether-sulfone support membranes fabricated using a non-solvent-induced phase separation technique. The conformational changes of the PBC chains in response to pH and temperature determined the, pure water flux and neutral solute (PEG 1000) rejection performance of TFC membranes. Permeability of the membranes increased from 13.0 +/- 0.63 to 15.9 +/- 0.06 L/m(2).h bar and from 6.7 +/- 0.00 to 13.9 +/- 0.07 L/m(2).h.bar by changing the solution pH from 4 to 8.5 and temperature from 4 to 25 degrees C, respectively. The pH- and temperature-responsive conformational changes did not affect the PEG 1000 rejection and membrane pore radius, which remained constant at similar to 89% and similar to 0.9 nm, respectively. This important finding was attributed to the high grafting density of co-polymer chains, resulting in spatial limitations among the grafted chains. The pore size of similar to 0.9 nm achieved with the proposed membrane design is the smallest size reported so far for membranes fabricated from block copolymers. TFC membranes demonstrated high stability and maintained their flux and rejection values under both static (storage in an acidic solution for up to 1 month) and dynamic (filtering PEG 1000 solution over 1 week) conditions. Pentablock copolymers enable a NF membrane with a sharp molecular weight cutoff suitable for size-selective separations. The membrane fabrication technique proposed in this study is a scalable and promising alternative that does not involve complex synthetic routes.

Aim The aim of the present study was to investigate the familial and somatic mutations as well as polymorphisms of TP53 gene in patients with uterine leiomyoma. Methods The study included 35 women with histologically diagnosed as uterine leiomyomas at the Gynecology Department of Adnan Menderes University Faculty of Medicine. Tissue and blood samples were analyzed for mutations and polymorphisms of TP53 gene by next generation sequencing (Miseq-Illumina). Acquired data was compared with the normal data in Ensembl database. Data from 1000 genome project and data from exome sequencing analyses in Intergen Genetic Diagnosis Center (Ankara) were used as controls for polymorphism analyses. Results There were no mutations in tissue and blood samples. However, when the polymorphisms were evaluated, a significant difference was found in NM_000546.5(TP53):c.215C > G (p.Pro72Arg) polymorphism between the study and control groups. The results indicated that P72R/P72R genotype increased the risk of leiomyoma development by 6.3 fold (95% confidence interval [CI]: 2.880-13.793). There was a negative correlation between P72R/WT genotype and leiomyoma development (OR = 0.261, 95% CI: 0.114-0.596). P72R/P72R genotype was statistically higher in the patients with leiomyoma compared with the controls and 1000 genomes from Asian, European and World populations. Conclusion The results of the present study suggested that P72R/P72R genotype may be associated with development of uterine leiomyoma in the Turkish population in the Western part of the country.

In the words of Lazzarato, the subjectivities produced for 'socialmachines' are now produced with the help of the globalizing world in areas suchas consumption, communication, social media and cinema. Every field wherevisual production is produced that is also the area of subjectivity production.In this context, the mainstream cinema, along with the subjects andsubjectivities that own produces, speaks to the consciousness of individuals or(as Lazzarato expresses) even to the 'feeling, vision and ways of thinking ofpre-individual components, perception forms etc.'The subjectivity which transferred tothe spectator with help of narrative style and the characters created in thefilms does not always serve capitalist subjectivity; sometimes, thissubjectivity is criticized. Although the number of such films is small, theseexamples have been frequently encountered recently. These films not only depictthe situation of the individual in modern capitalism but also try to constructnew forms of subjectivity, as Jacques Ranciere points out, by causing a radicalmetamorphosis, a subjective liberation. In this article, we aim to describe a new operator J_(s,a,μ)^(δ,λ) via convolution. Moreover, we aim to present a new subclass C_Σm (τ;β) related to m-fold symmetric bi-univalent functions in the open unit disk Θ={z∈C∶|z| ˂ 1 }. Finally, an estimate related to the Hankel determinant for functions in C_Σm (τ;β) are given.

The inference of ploidy levels from genomic data is important to understand molecular mechanisms underpinning genome evolution. However, current methods based on allele frequency and sequencing depth variation do not have power to infer ploidy levels at low-and mid-depth sequencing data, as they do not account for data uncertainty. Here we introduce HMMploidy, a novel tool that leverages the information from multiple samples and combines the information from sequencing depth and genotype likelihoods. We demonstrate that HMMploidy outperforms existing methods in most tested scenarios, especially at low-depth with large sample size. We apply HMMploidy to sequencing data from the pathogenic fungus Cryptococcus neoformans and retrieve pervasive patterns of aneuploidy, even when artificially downsampling the sequencing data. We envisage that HMMploidy will have wide applicability to low-depth sequencing data from polyploid and aneuploid species.

Amaç: Bu çalışmada erektil disfonksiyon nedeniyle ikinci basamak tedavide intrakavernozal alprostadil uygulanarak tedavi edilen hastalarda tedavinin 15 soruluk Uluslararası Erektil Fonksiyon Endeksi (IIEF-15) anketi ile ölçülen cinsel fonksiyonlar üzerine etkisi araştırılmıştır.Gereç ve Yöntemler: Haziran 2018 – Ekim 2019 arasında erektil disfonksiyon nedeniyle tedavi gören, birinci basamak tedavi modalitelerinden yetersiz yanıt alınan, ikinci basamak tedavide intrakavernozal alprostadil uygulanan ve 12 haftadan daha uzun süre tedaviye devam eden hastaların verileri retrospektif olarak incelenerek 21 hasta çalışmaya dahil edildi. Hastaların yaşları, ek hastalıkları, ilaç kullanım süreleri, ilaca bağlı yan etkiler, uygulama öncesi penil Doppler ultrasonografi (USG) parametreleri ve ilaç kullanımı öncesi ve 12 hafta sonrası ereksiyon fonksiyonları IIEF-15 formu ile değerlendirildi. İntrakavernozal alprostadil’e 5 μg dozdan başlandıktan sonra doz titrasyonu neticesinde 10 μg dozunda devam edildi. Bulgular: Tedavi öncesi yapılan penil Doppler USG de hastaların ortalama peak sistolik akım hızı (PS) değeri 24,4 ±15,1 cm/sn, ortalama end diastolik akım hızı (ED) değeri 4,3±4,5 cm/sn olarak ölçüldü. IIEF skorları 12. haftada ölçülmesine rağmen ortalama ilaç kullanım süreleri 21,9±16,6 hafta olarak ölçüldü. Erektil Fonksiyon Skoru uygulama öncesi 9.2±5.3 iken uygulama sonrası 20.33±6.2 olarak tespit edildi. Orgazmik İşlev Skoru uygulama öncesi 3,2±2,5, uygulama sonrası ise 7,5±2,4 olarak tespit edildi. Cinsel İstek Skoru uygulama öncesi 3,72±1,48, uygulama sonrası 8,9±1,4 olarak tespit edildi. Cinsel Memnuniyet Skoru uygulama öncesi 3,7±2,8, uygulama sonrası ise 9,1±2,7 olarak tespit edildi. Genel Memnuniyet Skoru uygulama öncesi 4±1.7 iken uygulama sonrası 8.1±1.7 olarak tespit edildi (Tüm parametreler p, Objective: In patients treated through the application of intracavernosal alprostadil in second-line treatment due to erectile dysfunction, the effect of the treatment on sexual functions measured with a 15-question International Index of Erectile Function (IIEF-15) questionnaire was investigated in this study.Material and Methods: Twenty-one patients treated due to erectile dysfunction between June 2018-October 2019, taking inadequate response from first-line treatment modalities, were applied intracavernosal alprostadil in second-line treatment and continued it for more than 12 weeks were included in the study, retrospectively. Ages, comorbid diseases, drug usage duration, drug-related side effects, penile Doppler ultrasonography (USG) parameters before application and the erection functions before and 12 weeks after drug use were evaluated with IIEF-15 form for the patients. Intracavernosal alprostadil was started with a dose of 5 μg and was continued with a dose of 10 μg as the result of dose titration.Results: The mean peak systolic blood flow velocity (PS) value of the patients was 24.4 ±15.1 cm/sec, and the mean end-diastolic flow velocity (ED) value was measured as 4.3±4.5 cm/sec based on the pre-treatment penile Doppler USG.Although the IIEF scores were measured in the 12th week, mean drug usage durations were measured as 21.9±16.6 weeks. Erectile Function Score was 9.2±5.3 before application; it was detected as 20.33±6.2 after the application. Orgasmic Function Score was 3.2±2.5 before application; it was detected as 7.5±2.4 after the application. Objectives: The mechanisms of acute liver failure resulting from epilepsy are poorly defined and appear to be multifactorial, including hypoxia and steatosis. The role of inflammation on liver damage after seizures is unclear. This study aimed to investigate the relationship between proinflammatory cytokines and liver damage after pentylenetetrazole-induced acute and chronic epilepsy models in rats.Materials and methods: Male Wistar albino adult rats (n = 18), weighing 220–240 g, were used in the experiment. Bu araştırmanın amacı ebelerin yetkileri ilgili görüşlerini değerlendirmek amacıyla yapılmıştır. Nitel araştırma yöntemi tekniklerinden görüşme tekniği kullanılmış olup fenomenolojik bir araştırmadır. Örneklemi; maksimum çeşitlilik örnekleme tekniğiyle belirlenen Türkiye’nin Akdeniz bölgesinde bir şehir hastanesi doğumhane biriminde çalışan 11 ebe oluşturmuştur. Veriler yarı yapılandırılmış form ile bireysel görüşme ve ses kaydı alınarak toplanmış, içerik analizi yoluyla çözümlenmiştir. Nitel verilerin analizi sonucu 5 tema, 12 alt tema, 38 kod oluşturulmuştur. Bu çalışmada ebeler çalıştıkları doğumhane ortamının gürültülü, yoğun, stresli olarak bildirmişlerdir. İlaveten mesleklerini severek yaptıklarını, anne ve bebek sağlığından sorumlu olduklarını dile getirmişlerdir. Ayrıca ebelerin yetkilerini; gebeyi karşılama, gebenin yatışını yapma, gebenin ilk değerlendirmesini yapma, gebeye tedaviyi uygulama, doğum öncesi takibini yapma, doğum yaptırma ve doğum sonrası bir saat anne ve bebek takibi olarak belirtmişlerdir. Ebeler doğumhaneye yatış yapma, reçete yazma, indüksiyon uygulama yetkilerinin kendilerine tanımlanmasının uygun olacağını da düşünmektedirler. Ebelerin belirttiği yetki alanlarının; Sağlık Meslek Mensupları ile Sağlık Hizmetlerinde Çalışan Diğer Meslek Mensuplarının İş ve Görev Tanımlarına Dair Sayı: 29007 ve 2014 belirlediği Yönetmeliği’nin yetkinlik alanlarıyla orantılı olduğu saptanmıştır. Lisans eğitimlerinde ve hizmet içi eğitimlerde ebelerin yetkilerine yönelik eğitimler verilerek bilgilerinin güncellenmesi sağlanmalıdır. The interview technique, a qualitative research method technique, was used in this phenomenological research. The sample, determined using the maximum diversity sampling technique, consisted of 11 midwives working in the delivery ward of a city hospital in the Mediterranean Region of Turkey. Data were collected using a semi-structured form by means of individual interviews and voice recordings and analyzed by content analysis. 5 themes, 12 sub-themes and 38 codes were created as a result of qualitative data analysis. In the present study, the midwives reported that the delivery ward environment where they worked was noisy, busy and stressful. Additionally, they stated that they loved their profession and were responsible for the health of the mother and baby. Furthermore, they stated the following as within the competency of midwives; welcoming the pregnant woman, carrying out the admission procedure, performing the first evaluation of the pregnant woman, administering treatment to the pregnant woman, prenatal follow-up, delivering the baby and following up the mother and baby for one hour after birth. The midwives also thought that it would be appropriate for them to be given the authority to admit pregnant women to the delivery ward, write prescriptions and induce labor. The competency areas stated by the midwives are consistent with the competency areas of the Regulation specified in Numbers 29007 and 2014 of Job and Job Descriptions of Healthcare Professionals and Other Professionals Working in Healthcare Services. The knowledge of midwives should be updated through education during undergraduate education and in-service training. Laws involving midwifery and regulations should be made. Compulsory professional liability insurance for midwives should also be included in these regulations.
