Multimodal alignment facilitates the retrieval of instances from one modality when queried using another. In this paper, we consider a novel setting where such an alignment is between (i) instruction steps that are depicted as assembly diagrams (commonly seen in Ikea assembly manuals) and (ii) video segments from in-the-wild videos; these videos comprising an enactment of the assembly actions in the real world. To learn this alignment, we introduce a novel supervised contrastive learning method that learns to align videos with the subtle details in the assembly diagrams, guided by a set of novel losses. To study this problem and demonstrate the effectiveness of our method, we introduce a novel dataset: IAW for Ikea assembly in the wild consisting of 183 hours of videos from diverse furniture assembly collections and nearly 8,300 illustrations from their associated instruction manuals and annotated for their ground truth alignments. We define two tasks on this dataset: First, nearest neighbor retrieval between video segments and illustrations, and, second, alignment of instruction steps and the segments for each video. We report the discovery of a low-mass $z=5.200\pm 0.002$ galaxy that is in the process of ceasing its star formation. The galaxy, MACS0417-z5PSB, is multiply imaged with magnification factors $\sim40$ by the galaxy cluster MACS J0417.5-1154, observed as part of the CAnadian NIRISS Unbiased Cluster Survey (CANUCS). Using observations of MACS0417-z5PSB with a JWST/NIRSpec Prism spectrum and NIRCam imaging, we investigate the mechanism responsible for the cessation of star formation of the galaxy, and speculate about possibilities for its future. Using spectrophotometric fitting, we find a remarkably low stellar mass of $\rm{M_*}=4.3\pm^{0.9}_{0.8} \times 10^{7} \rm{M_{\odot}}$, less than 1% of the characteristic stellar mass at $z\sim5$. We measure a de-lensed rest-UV half-light radius in the source plane of $30\pm^{7}_{5}$ pc, and measure a star formation rate from H$\alpha$ of $0.14\pm^{0.17}_{0.12}$ $\rm{M_{\odot}/yr}$. We find that under the assumption of a double power law star formation history, MACS0417-z5PSB has seen a recent rise in star formation, peaking $\sim10-30$ Myr ago and declining precipitously since then. Together, these measurements reveal a low-mass, extremely compact galaxy which is in the process of ceasing star formation. We investigate the possibilities of mechanisms that have led to the cessation of star formation in MACS0417-z5PSB, considering stellar and AGN feedback, and environmental processes. We can likely rule out an AGN and most environmental processes, but leave open the possibility that MACS0417-z5PSB could be a star forming galaxy in the lull of a bursty star formation history., Comment: 9 pages, 4 figures, 1 table

254. Deep Declarative Dynamic Time Warping for End-to-End Learning of Alignment Paths

This paper addresses learning end-to-end models for time series data that include a temporal alignment step via dynamic time warping (DTW). Existing approaches to differentiable DTW either differentiate through a fixed warping path or apply a differentiable relaxation to the min operator found in the recursive steps used to solve the DTW problem. We instead propose a DTW layer based around bi-level optimisation and deep declarative networks, which we name DecDTW. By formulating DTW as a continuous, inequality constrained optimisation problem, we can compute gradients for the solution of the optimal alignment (with respect to the underlying time series) using implicit differentiation. An interesting byproduct of this formulation is that DecDTW outputs the optimal warping path between two time series as opposed to a soft approximation, recoverable from Soft-DTW. We show that this property is particularly useful for applications where downstream loss functions are defined on the optimal alignment path itself. This naturally occurs, for instance, when learning to improve the accuracy of predicted alignments against ground truth alignments. We evaluate DecDTW on two such applications, namely the audio-to-score alignment task in music information retrieval and the visual place recognition task in robotics, demonstrating state-of-the-art results in both., Comment: ICLR 2023 (Poster)

255. OGLE-2016-BLG-1195 AO: Lens, Companion to Lens or Source, or None of the Above?

We systematically investigate the claim by Vandorou et al. (2023) to have detected the host star of the low mass-ratio ($q<10^{-4}$) microlensing planet OGLE-2016-BLG-1195Lb, via Keck adaptive optics (AO) measurements $\Delta t=4.12\,$yr after the peak of the event ($t_0$). If correct, this measurement would contradict the microlens parallax measurement derived from Spitzer observations in solar orbit taken near $t_0$. We show that this host identification would be in $4\,\sigma$ conflict with the original ground-based lens-source relative proper-motion measurements. By contrast, Gould (2022) estimated a probability $p=10\%$ that the ``other star'' resolved by single-epoch late-time AO would be a companion to the host or the microlensed source, which is much more probable than a 4$\,\sigma$ statistical fluctuation. In addition, independent of this proper-motion discrepancy, the kinematics of this host-identification are substantially less probable than those of the Spitzer solution. Hence, this identification should not be accepted, pending additional observations that would either confirm or contradict it, which could be taken in 2023. Motivated by this tension, we present two additional investigations. We explore the possibility that Vandorou et al. (2023) identified the wrong ``star'' (or stellar asterism) on which to conduct their analysis. We find that astrometry of KMT and Keck images favors a star (or asterism) lying about 175 mas northwest of the one that they chose. We also present event parameters from a combined fit to all survey data, which yields, in particular, a more precise mass ratio, $q=(4.6\pm 0.4)\times 10^{-5}$. Finally, we discuss the broader implications of minimizing such false positives for the first measurement of the planet mass function, which will become possible when AO on next-generation telescopes are applied to microlensing planets., Comment: 31 pages, 7 figures, 3 tables, 2 appendices, submitted to AAS Journals

256. 3DInAction: Understanding Human Actions in 3D Point Clouds

We propose a novel method for 3D point cloud action recognition. Understanding human actions in RGB videos has been widely studied in recent years, however, its 3D point cloud counterpart remains under-explored. This is mostly due to the inherent limitation of the point cloud data modality -- lack of structure, permutation invariance, and varying number of points -- which makes it difficult to learn a spatio-temporal representation. To address this limitation, we propose the 3DinAction pipeline that first estimates patches moving in time (t-patches) as a key building block, alongside a hierarchical architecture that learns an informative spatio-temporal representation. We show that our method achieves improved performance on existing datasets, including DFAUST and IKEA ASM. Code is publicly available at https://github.com/sitzikbs/3dincaction.

257. Learning to Select Camera Views: Efficient Multiview Understanding at Few Glances

Multiview camera setups have proven useful in many computer vision applications for reducing ambiguities, mitigating occlusions, and increasing field-of-view coverage. However, the high computational cost associated with multiple views poses a significant challenge for end devices with limited computational resources. To address this issue, we propose a view selection approach that analyzes the target object or scenario from given views and selects the next best view for processing. Our approach features a reinforcement learning based camera selection module, MVSelect, that not only selects views but also facilitates joint training with the task network. Experimental results on multiview classification and detection tasks show that our approach achieves promising performance while using only 2 or 3 out of N available views, significantly reducing computational costs. Furthermore, analysis on the selected views reveals that certain cameras can be shut off with minimal performance impact, shedding light on future camera layout optimization for multiview systems. We complete the publication of all microlensing planets (and ``possible planets'') identified by the uniform approach of the KMT AnomalyFinder system in the 21 KMT subprime fields during the 2019 observing season, namely KMT-2019-BLG-0298, KMT-2019-BLG-1216, KMT-2019-BLG-2783, OGLE-2019-BLG-0249, and OGLE-2019-BLG-0679 (planets), as well as OGLE-2019-BLG-0344, and KMT-2019-BLG-0304 (possible planets). The five planets have mean log mass-ratio measurements of $(-2.6,-3.6,-2.5,-2.2,-2.3)$, median mass estimates of $(1.81,0.094,1.16,7.12,3.34)\, M_{\rm Jup}$, and median distance estimates of $(6.7,2.7,5.9,6.4,5.6)\, {\rm kpc}$, respectively. The main scientific interest of these planets is that they complete the AnomalyFinder sample for 2019, which has a total of 25 planets that are likely to enter the statistical sample. We find statistical consistency with the previously published 33 planets from the 2018 AnomalyFinder analysis according to an ensemble of five tests. Of the 58 planets from 2018-2019, 23 were newly discovered by AnomalyFinder. Within statistical precision, half of all the planets have caustic crossings while half do not (as predicted by Zhu et al. 2014), an equal number of detected planets result from major-image and minor-image light-curve perturbations, and an equal number come from KMT prime fields versus subprime fields., Comment: 17 tables, 20 figures, submitted to AAS journals

We present the results of a systematic search for candidate quiescent galaxies in the distant Universe in eleven $JWST$ fields with publicly available observations collected during the first three months of operations and covering an effective sky area of $\sim145$ arcmin$^2$. We homogeneously reduce the new $JWST$ data and combine them with existing observations from the $Hubble\,Space\,Telescope$. We select a robust sample of $\sim80$ candidate quiescent and quenching galaxies at $3 < z < 5$ using two methods: (1) based on their rest-frame $UVJ$ colors, and (2) a novel quantitative approach based on Gaussian Mixture Modeling of the $NUV-U$, $U-V$, and $V-J$ rest-frame color space, which is more sensitive to recently quenched objects. We measure comoving number densities of massive ($M_\star\geq 10^{10.6} M_\odot$) quiescent galaxies consistent with previous estimates relying on ground-based observations, after homogenizing the results in the literature with our mass and redshift intervals. However, we find significant field-to-field variations of the number densities up to a factor of $2-3$, highlighting the effect of cosmic variance and suggesting the presence of overdensities of red quiescent galaxies at $z>3$, as it could be expected for highly clustered massive systems. Importantly, $JWST$ enables the robust identification of quenching/quiescent galaxy candidates at lower masses and higher redshifts than before, challenging standard formation scenarios. We report the analysis of microlensing event OGLE-2017-BLG-1038, observed by the Optical Gravitational Lensing Experiment, Korean Microlensing Telescope Network, and Spitzer telescopes. The event is caused by a giant source star in the Galactic Bulge passing over a large resonant binary lens caustic. The availability of space-based data allows the full set of physical parameters to be calculated. However, there exists an eightfold degeneracy in the parallax measurement. The four best solutions correspond to very-low-mass binaries near ($M_1 = 170^{+40}_{-50} M_J$ and $M_2 = 110^{+20}_{-30} M_J$), or well below ($M_1 = 22.5^{+0.7}_{-0.4} M_J$ and $M_2 = 13.3^{+0.4}_{-0.3} M_J$) the boundary between stars and brown dwarfs. A conventional analysis, with scaled uncertainties for Spitzer data, implies a very-low-mass brown dwarf binary lens at a distance of 2 kpc. Compensating for systematic Spitzer errors using a Gaussian process model suggests that a higher mass M-dwarf binary at 6 kpc is equally likely. A Bayesian comparison based on a galactic model favors the larger-mass solutions. We systematically inspect the microlensing data acquired by the KMTNet survey during the previous seasons in order to find anomalous lensing events for which the anomalies in the lensing light curves cannot be explained by the usual binary-lens or binary-source interpretations. From the inspection, we find that interpreting the three lensing events OGLE-2018-BLG-0584, KMT-2018-BLG-2119, and KMT-2021-BLG-1122 requires four-body (lens+source) models, in which either both the lens and source are binaries (2L2S event) or the lens is a triple system (3L1S event). Following the analyses of the 2L2S events presented in \citet{Han2023}, here we present the 3L1S analysis of the KMT-2021-BLG-1122. It is found that the lens of the event KMT-2021-BLG-1122 is composed of three masses, in which the projected separations (normalized to the angular Einstein radius) and mass ratios between the lens companions and the primary are $(s_2, q_2)\sim (1.4, 0.53)$ and $(s_3, q_3) \sim (1.6, 0.24)$. By conducting a Bayesian analysis, we estimate that the masses of the individual lens components are $(M_1, M_2, M_3)\sim (0.47\,M_\odot, 0.24\,M_\odot, 0.11\,M_\odot)$. The companions are separated in projection from the primary by $(a_{\perp,2}, a_{\perp,3})\sim (3.5, 4.0)$~AU. The lens of KMT-2018-BLG-2119 is the first triple stellar system detected via microlensing., Comment: 8 pages, 7 figures, 2 tables

The global network of gravitational-wave observatories now includes five detectors, namely LIGO Hanford, LIGO Livingston, Virgo, KAGRA, and GEO 600. These detectors collected data during their third observing run, O3, composed of three phases: O3a starting in April of 2019 and lasting six months, O3b starting in November of 2019 and lasting five months, and O3GK starting in April of 2020 and lasting 2 weeks. In this paper we describe these data and various other science products that can be freely accessed through the Gravitational Wave Open Science Center at https://gwosc.org. This work aims to assess how well a model performs under distribution shifts without using labels. While recent methods study prediction confidence, this work reports prediction dispersity is another informative cue. Confidence reflects whether the individual prediction is certain; dispersity indicates how the overall predictions are distributed across all categories. Our key insight is that a well-performing model should give predictions with high confidence and high dispersity. That is, we need to consider both properties so as to make more accurate estimates. To this end, we use the nuclear norm that has been shown to be effective in characterizing both properties. Extensive experiments validate the effectiveness of nuclear norm for various models (e.g., ViT and ConvNeXt), different datasets (e.g., ImageNet and CUB-200), and diverse types of distribution shifts (e.g., style shift and reproduction shift). We show that the nuclear norm is more accurate and robust in accuracy estimation than existing methods. Furthermore, we validate the feasibility of other measurements (e.g., mutual information maximization) for characterizing dispersity and confidence. Protoclusters of galaxies have been found in the last quarter century. However, most of them have been found through the overdensity of star-forming galaxies, and there had been no known structures identified by multiple spectroscopically confirmed quiescent galaxies at $z>2.5$. In this letter, we report the discovery of an overdense structure of massive quiescent galaxies with the spectroscopic redshift $z=2.77$ in the COSMOS field, QO-1000. We first photometrically identify this structure as a $4.2\sigma$ overdensity with 14 quiescent galaxies in $7\times4\ {\rm pMpc^2}$ from the COSMOS2020 catalog. We then securely confirm the spectroscopic redshifts of 4 quiescent galaxies by detecting multiple Balmer absorption lines with Keck/MOSFIRE. All the spectroscopically confirmed members are massive ($\log{(M_\star/M_\odot)}>11.0$) and located in a narrow redshift range ($2.76
68$ times denser than in the general field. In addition, we confirm that they form a red sequence in the $J-K_s$ color. This structure's halo mass is estimated as $\log{(M_{\rm halo}/M_\odot)}>13.2$ from their stellar mass. Similar structures found in the IllustrisTNG simulation are expected to evolve into massive galaxy clusters with $\log{(M_{\rm halo}/M_\odot)}\geq14.8$ at $z=0$. These results suggest that QO-1000 is a more mature protocluster than the other known protoclusters. We present the observations and analysis of a high-magnification microlensing planetary event, KMT-2022-BLG-0440, for which the weak and short-lived planetary signal was covered by both the KMTNet survey and follow-up observations. The binary-lens models with a central caustic provide the best fits, with a planet/host mass ratio, $q = 0.75$--$1.00 \times 10^{-4}$ at $1\sigma$. The binary-lens models with a resonant caustic and a brown-dwarf mass ratio are both excluded by $\Delta\chi^2 > 70$. The binary-source model can fit the anomaly well but is rejected by the ``color argument'' on the second source. From Bayesian analyses, it is estimated that the host star is likely a K or M dwarf located in the Galactic disk, the planet probably has a Neptune-mass, and the projected planet-host separation is $1.9^{+0.6}_{-0.7}$ or $4.6^{+1.4}_{-1.7}$ au, subject to the close/wide degeneracy. This is the third $q < 10^{-4}$ planet from a high-magnification planetary signal ($A \gtrsim 65$). Together with another such planet, KMT-2021-BLG-0171Lb, the ongoing follow-up program for the KMTNet high-magnification events has demonstrated its ability in detecting high-magnification planetary signals for $q < 10^{-4}$ planets, which are challenging for the current microlensing surveys., Comment: MNRAS accepted

The growth of cities creates challenges to biodiversity and social justice. Researchers addressing these challenges often do so in silos; their efforts persistently separate justice for non-humans and humans. We analyze this separation through two concepts—luxury effect and green gentrification—that each explores urban greenspace and justice, but from different angles. The luxury effect implicitly targets justice for non-humans by exploring biodiversity in cities and finds that wealth explains the distribution of biodiversity. Green gentrification explicitly targets justice for humans by examining how new greenspaces affect people and finds that such greenspaces often displace vulnerable people. Using scientometric analyses, we show the disjunction between these concepts. We draw on socio-ecological justice concepts to suggest that disregarding relationships and conflicts among humans and non-humans in concepts does not eliminate them in practice, but stalls progress toward just cities. We urge scholars to simultaneously focus on justice for humans and non-humans.

The COVID-19 pandemic profoundly disrupted society and raised concerns about the well-being of older adults. Today, many people have abandoned ‘pandemic protocols’ but questions remain about the pandemic's longer term impacts on older adults. We used digitally mediated photovoice to explore older adults’ well-being in the context of their everyday lives, early in the pandemic and again two years later. Nine apartment-dwelling older adults who lived in high-density neighbourhoods in Vancouver, Canada participated in our study. We holistically considered how five dimensions of well-being were interrelated to shape experiences: material, relational, subjective, psychological, and environmental. We explored positive and negative aspects of these dimensions in relation to two key themes: (i) negotiating comfort, purpose and confinement at home, and (ii) navigating neighbourhood places amidst pandemic disruptions. Material resources to meet one's needs (e.g., communication technology, hobbies) and access to public and private greenspaces were assets. However, some participants experienced increased barriers to maintaining the five overlapping dimensions of well-being we examined; these participants viewed their homes negatively, and engaged less with their neighbourhoods over time. We emphasize how older adults’ well-being during the pandemic was a dynamic and, at times, precarious process. Understanding how interrelated dimensions shape older adult well-being might ultimately inform practices and policies that promote age-friendly cities.

AbstractIn humans, exposure to early life adversity has profound implications for susceptibility to developing neuropsychiatric disorders later in life. Studies in rodents have shown that stress experienced during early postnatal life can have lasting effects on brain development. Glucocorticoids and sex steroids are produced in endocrine glands and the brain from cholesterol; these molecules bind to nuclear and membrane-associated steroid receptors. Unlike other steroids that can also be made in the brain, neurosteroids bind specifically to neurotransmitter receptors, not steroid receptors. The relationships among steroids, neurosteroids, and stress are multifaceted and not yet fully understood. However, studies demonstrating altered levels of progestogens, androgens, estrogens, glucocorticoids, and their neuroactive metabolites in both developmental and adult stress paradigms strongly suggest that these molecules may be important players in stress effects on brain circuits and behavior. In this review, we discuss the influence of developmental and adult stress on various components of the brain, including neurons, glia, and perineuronal nets, with a focus on sex steroids and neurosteroids. Gaining an enhanced understanding of how early adversity impacts the intricate systems of brain steroid and neurosteroid regulation could prove instrumental in identifying novel therapeutic targets for stress-related conditions.

La expansión de la urbanización en la provincia de Buenos Aires condujo a la fragmentación de ecosistemas naturales, lo que ha generado la necesidad comunitaria de habitar entornos saludables. Como consecuencia, las/os ciudadanas/os han tomado la iniciativa de organizarse y establecer redes para preservar pequeños reductos de naturaleza. Mediante el análisis de tres casos (Reserva Natural Provincial Santa Catalina, Reserva Urbana de la Defensa de El Palomar “Isla Verde” y Paisaje Protegido de Interés Provincial Camino de las Flores) exploramos el papel crucial de la participación ciudadana en la protección de estas áreas verdes urbanas y sus desafíos. Observamos denominadores comunes: comunidades organizadas con una identidad arraigada a su barrio, activistas y académicos/as e investigadores/as que se comprometen con su defensa. Estas, constituyen espacios de resistencia y resignificación, con una respuesta colectiva orientada en conquistar derechos y promover la biodiversidad. Concluimos que la actual crisis ambiental exige la preservación activa de espacios verdes urbanos donde la comunidad asume un papel protagónico y enfrenta desafíos territoriales y disputas políticas.

Abstract The ecosystem system services framework has potential to help clarify wildlife management challenges at the single species scale, but existing methods struggle to capture the complex values and tradeoffs at play in human–wildlife interactions. We worked with community scientists to gather and use stories (n = 150) as a source of ecosystem services data about living alongside eastern coyotes (Canis latrans var) in rural Vermont, United States. Our a priori ecosystem service assessment showed that human–coyote interactions can have simultaneous positive and negative human well‐being impacts at both the sample scale and for individual interviewees. Our research identified emergent themes that lent insight into how interviewees justified different kinds of relationships with coyotes. We applied a tradeoffs lens to evaluate three policy options based on their potential to mitigate ecosystem disservices and maximize ecosystem services from human–coyote interactions. We found that ecosystem services assessment rooted in a dataset of stories revealed policy‐relevant understanding of value conflicts and alignments at the sample scale without overshadowing the nuances of individual interviewees' experiences.

Background: Charles Bonnet syndrome (CBS) refers to the presence of visual hallucinations experienced by people, without cognitive, or psychiatric deficits that are related to sight loss. This study surveyed younger adults (18–60 years) with visual impairments, to assess the impact of anxiety, depression, loneliness, and the COVID-19 lockdowns on their visual hallucinations. Objectives: To examine the association between depression, anxiety, loneliness, the COVID-19 lockdowns, and visual hallucinations in younger adults with CBS. Design: An online survey was used with an opportunistic sample of people with sight loss. Methods: A survey assessed the frequency, duration, and valence of visual hallucinations using a five-point Likert scale, and assessed anxiety, depression, and loneliness using the Hospital Anxiety and Depression Scale and UCLA (University of California, Los Angeles) loneliness scale, respectively. Results: Twenty-nine young adults (21 female), aged 22–59 years with vision loss from a range of causes, who experience visual hallucinations, were included in the survey. The majority (76%) of participants had experienced hallucinations within the past week, with 83% stating they occurred frequently or very frequently. Introduction. The global incidence and prevalence of spine disorders are increasing with population growth. Traumatic spine injury and non-traumatic spine disorders are life-changing conditions. Despite growing literature about spine disorders, we found little published Asian epidemiological data. This study aimed to thoroughly understand the epidemiology of patients with spine disorders in our institution. Methodology. This study utilized a descriptive retrospective cohort study design, and included patients with spine disorders admitted from January 1, 2016, to December 31, 2022. The patient records were retrieved, and data was collected according to the demographic profile, level of spinal disorders, type of management, and mortality rate. Results. Of 474 patients with spinal disorders admitted to our institution, most were young and older adults at 31.4% and 36.3%, respectively. Most were males at 70.3%. Traumatic spinal disorders were more common at 66.2%. Infection was the most common non-traumatic disorder at 56.9%. The cervical spine was most affected by traumatic etiology (56.1%), while the thoracic spine was most affected by non-traumatic causes (57.5%). Among non-traumatic cases, infectious etiology, particularly tuberculosis, accounted for the highest number, followed by degenerative causes and tumors (74.7%, 70.0%, and 25.8%, respectively). Surgical management was primarily used for traumatic spinal disorders, while the majority of non-traumatic cases received conservative treatment (55.1% and 72.5%, respectively). There has been a steady decrease in mortality for spinal disorders for the past seven years. Conclusion. Both traumatic and non-traumatic etiologies of spine disorders show a steady decrease in mortality rate, which may indicate an improvement in the hospital’s orthopaedic spine service. The reduced mortality rates indicate improvement in spine care in the locality and can be used to advocate for public health measures.

Background Depression is common in people with dementia, and negatively affects quality of life. Aims This paper aims to evaluate the cost-effectiveness of an intervention for depression in mild and moderate dementia caused by Alzheimer's disease over 12 months (PATHFINDER trial), from both the health and social care and societal perspectives. Method A total of 336 participants were randomised to receive the adapted PATH intervention in addition to treatment as usual (TAU) (n = 168) or TAU alone (n = 168). Health and social care resource use were collected with the Client Service Receipt Inventory and health-related quality-of-life data with the EQ-5D-5L instrument at baseline and 3-, 6- and 12-month follow-up points. Principal analysis comprised quality-adjusted life-years (QALYs) calculated from the participant responses to the EQ-5D-5L instrument. Results The mean cost of the adapted PATH intervention was estimated at £1141 per PATHFINDER participant. From a health and social care perspective, the mean difference in costs between the adapted PATH and control arm at 12 months was −£74 (95% CI −£1942 to £1793), and from the societal perspective was −£671 (95% CI −£9144 to £7801). The mean difference in QALYs was 0.027 (95% CI −0.004 to 0.059). At £20 000 per QALY gained threshold, there were 74 and 68% probabilities of adapted PATH being cost-effective from the health and social care and societal perspective, respectively. Conclusions The addition of the adapted PATH intervention to TAU for people with dementia and depression generated cost savings alongside a higher quality of life compared with TAU alone; however, the improvements in costs and QALYs were not statistically significant.

Background The National Health Service (NHS) is suffering from a workforce crisis of mental and physical sickness and attrition following the COVID-19 pandemic. An in-depth understanding of healthcare workers’ (HCWs) experiences during the pandemic is required to understand the impacts on their mental health in this challenging work environment. This qualitative study explores HCWs’ concerns during the COVID-19 pandemic—expressed in real time during an active mental health crisis.Design This study involved analysis of data from ‘Shout’, a text message-based, UK-wide mental health support service which, during the pandemic, was advertised to HCWs specifically. Pseudorandom sampling of scripts of anonymised text message conversations between HCWs and Shout Volunteers from April 2020 to March 2021 was undertaken, with data fully anonymised by Shout before researchers accessed them on a secure purpose-built platform. Following application of exclusion and inclusion criteria, 60 conversations were coded to develop a thematic framework and analysed using grounded theory, with subthemes triangulated to create final themes. Quotes extracted from this process were then synthesised for publication.Results Three themes emerged from the data: (1) poor mental health, subthemes—(a) overwhelming negative feelings or emotional distress experienced, and; (b) active crisis/resurgent symptoms. (2) Negative work experiences, subthemes: (a) negative NHS work culture and expectations; (b) inadequate structures and arrangements for support; (c) trauma at work, and (d) abuse at work. (3) The impact of the COVID-19 pandemic, subthemes: (a) additional work pressure and (b) isolation and risk.Conclusion This study explores the challenges and mental health concerns in HCWs during an active crisis. Organisational stressors, mental health provision and additional resources for HCWs to recover from the pandemic remain a vital issue in current NHS service provision.

IntroductionSepsis is a complex clinical syndrome characterized by a heterogenous host immune response. Historically, static protein and transcriptomic metrics have been employed to describe the underlying biology. Here, we tested the hypothesis that ex vivo functional TNF expression as well as an immunologic endotype based on both IFNγ and TNF expression could be used to model clinical outcomes in sepsis patients.MethodsThis prospective, observational study of patient samples collected from the SPIES consortium included patients at five health systems enrolled over 17 months, with 46 healthy control patients, 68 ICU patients without sepsis, and 107 ICU patients with sepsis. Whole blood was collected on day 1, 4, and 7 of ICU admission. Outcomes included in-hospital and 180-day mortality and non-favorable discharge disposition defined by skilled nursing facility, long-term acute care facility, or hospice. Whole blood ELISpot assays were conducted to quantify TNF expression [stimulated by lipopolysaccharide (LPS)] and IFNγ expression (stimulated by anti-CD3/CD28 mAb), which were then used for assignment to one of four subgroups including an ‘immunocompetent’, ‘immunosuppressed endotype’, and two ‘mixed’ endotypes.ResultsWhole blood TNF spot-forming units were significantly increased in septic and CINS patients on days 4 and 7 compared to healthy subjects. In contrast, TNF expression per cell on days 1, 4, and 7 was significantly lower in both septic and critically ill non-septic (CINS) patients compared to healthy subjects. Early increases in total TNF expression were associated with favorable discharge disposition and lower in-hospital mortality. ‘Immunocompetent’ endotype patients on day 1 had a higher proportion of favorable to non-favorable discharges compared to the ‘immunosuppressed’ endotype. Similarly, ‘immunocompetent’ endotype patients on day 4 had a higher in-hospital survival compared to the ‘immunosuppressed’ endotype patients. Finally, among septic patients, decreased total TNF and IFNγ expression were associated with 180-day mortality.ConclusionsIncreased ex vivo whole blood TNF expression is associated with improved clinical outcomes. Further, the early ‘immunocompetent’ endotype is associated with favorable discharge and improved in-hospital and 180-day survival. The ability to functionally stratify septic patients based on blood cell function ex vivo may allow for identification of future immune modulating therapies.

Long-term wildlife research and monitoring programs strive to maintain consistent data collections and analytical methods. Incorporating new techniques is important but can render data sets incongruent and limit their potential to discern trends in demographic parameters. Integrated population models (IPMs) can address these limitations by combining data sources that may span different periods into a unified statistical framework while providing a holistic view of population dynamics. We developed an IPM in a Bayesian framework for grizzly bears (Ursus arctos) in the Greater Yellowstone Ecosystem. We coupled demographic data with multiple, independent population count data to link annual changes in abundance with vital rates over 4 decades (1983–2023). Abundance increased threefold from an estimated 270 individuals in 1984 to 1030 individuals in 2023. Parameter estimates indicated survival of bears ≥2 years of age was high, contributing to robust population growth during the 1980s (λ = 1.023 [50 % interquartile range = 0.993–1.082]) and 1990s (λ = 1.064 [1.023–1.103]). A slowing of population growth started around 2000 (2000s: λ = 1.030 [0.989–1.068]) and continued into the 2010s (λ = 1.021 [0.985–1.057]), due primarily to reductions in survival of bears
