Purpose: To identify extension routes of craniofacial malignancies and formulate a surgical treatment plan based thereupon. Material and Methods: We retrospectively reviewed the medical records of 253 patients with craniofacial malignancies who underwent surgical treatment at the Romodanov Neurosurgery Institute from 2002 through 2022. Of the 253 patients, 112 had a primary tumor, and 141, a secondary tumor. Preoperative Karnofsky performance scores ranged from 50 to 70 points. Patients underwent neurological and ophthalmological status assessment, as per routine protocols. Results: Epithelial malignancies were the most common (53.7%), whereas anaplastic meningioma and embryonal malignancies were rather uncommon (1.2% and 0.4%, respectively) craniofacial malignancies. The presence of certain clinical symptoms was associated primarily with tumor origin and extension. A high rate of general brain and rhinological symptoms in our study sample was caused by a high percentage of intracranial and paranasal sinus tumors. Craniofacial malignancies most commonly originate from the midline (particularly, anterior midline skull base). Ethmoidal labyrinth was the most common site of origin (45.0%), followed by a sphenoid sinus (12.2%), pterygopalatine and infratemporal fossae (9.9%), whereas the cavernous sinus and olfactory fossa were the least common sites of origin (0.4% and 1.2%, respectively). Craniofacial tumors extended most commonly intracranially (transdurally, epidurally, via adhesion to the dura mater, and/or cavernous sinus growth) or intraorbitally. Anterior craniofacial resection (bifrontal craniotomy with combined with either lateral rhinotomy or supraorbital advancement; or a subcranial approach) was the most common surgical treatment. Postoperative cerebrospinal fluid rhinorrhea and infectious complications (meningitis and meningoencephalitis) were the most frequent complications. The overall postoperative mortality rate was 2.0%. Conclusion: First, compared to the transcranial and facial approaches, the craniofacial resection is advantageous in terms of the radicality of tumor excision. Second, the subcranial approach is preferable to the bifrontal approach in the presence of marked extracranial tumor component, whereas the transbasal Derome approach is effective in the presence of marked extracranial and/or intracranial tumor components. Finally, both the orbitozygomatic and infratemporal approaches allow for the radicality of excision of lateral skull base malignancies, but the latter approach is associated with a lower rate of complications.

Objective: Cancer is a leading cause of death globally, prompting numerous efforts to find effective treatments. HeLa cells, derived from Henrietta Lacks’ cancerous squamous cells, have played a crucial role in cancer research due to their origin, resistance, and rapid growth. They are particularly useful for studying ways of cellular death triggering, or apoptosis, without an immune response. Thus, the objective of this paper was to review the latest publications on the subject of HeLa apoptosis so that a brief view to be available on the otherwise so extended subject.

Brain tumours are heterogeneous and are classified comprehensively into molecular subtypes based on genetic alterations. Glioblastoma rapid progression, drug resistance, and recurrence have been scientifically linked to several factors, including its rapid growth rate, loss of apoptosis, pro-survival signalling, molecular heterogeneities and hallmark features to infiltrate vital brain structures. Because of the growing demand for design and development of delivery systems to overcome the existing limitations with the current therapeutic strategies, researchers are exploiting multifaceted aspects of nanotechnology to improve delivery of the drug payload. Firstly, nanotechnology procedures can improve the drug delivery methods with the help of nanoparticles (NPs) based nanovectors that can efficiently cross blood-brain barrier. Secondly, NPs also improve the cellular uptake of the drug as they can efficiently bind with the cell surface. Thirdly, NPs make the delivery of siRNAs and peptides possible, which can suppress the resistance of glioblastoma against TMZ or other chemo-preventive drugs. Fourthly, the use of metal NPs increases the efficiency of scanning or magnetic resonance imaging (MRI) procedures as they can produce contrasts in it. Lastly, NPs make it possible to use highly targeted co-administered strategies like chemoprevention and near infrared (NIR) or radiotherapy (RT). Hence, nanotechnology offers several promising solutions against glioblastoma by countering it on many fronts.

Malignant struma ovarii (MSO) is an extremely rare monodermal ovarian teratoma. Preoperative diagnosis and intraoperative freezing diagnosis are exceedingly difficult due to the rarity of the disease and its clinically noncharacteristic manifestations with less than 200 reports in the current literature. In this paper, a case of MSO (papillary carcinoma) with hyperthyroidism was discussed in terms of its epidemiology, clinicopathology, molecular features, treatment, and prognosis.

The aim of this study was to compare the pre- and postoperative function of patients qualifying for resection of malignant and nonmalignant primary brain tumors to determine the relationship among tumor type, function, and the course of rehabilitation after surgery. This single-center, prospective, observational study recruited 92 patients requiring prolonged postoperative rehabilitation during their inpatient stay, who were divided into a nonmalignant tumor group (n = 66) and a malignant tumor group (n = 26). Functional status and gait efficiency were assessed using a battery of instruments. Motor skills, postoperative complications, and length of hospital stay (LoS) were recorded and compared between groups. The frequency and severity of postoperative complications, the time needed to attain individual motor skills, and the proportion of patients losing independent gait (~30%) were similar between groups. However, paralysis and paresis were more frequent in the malignant tumor group before surgery (p < 0.001). While nonmalignant tumor patients deteriorated more according to all scales after surgery, patients with malignant tumors were still characterized by worse ADL, independence, and performance at discharge. Worse functional outcomes in the malignant tumor group did not affect LoS or rehabilitation. Patients with malignant and nonmalignant tumors have similar rehabilitation needs, and patient expectation—especially those with nonmalignant tumors—should be appropriately managed.

Breast phyllodes tumors (PTs) are biphasic, with epithelial and stromal components. Although the PT incidence is low (approximately 1% of all breast tumors), its clinical outcomes are unpredictable, and malignant PTs often progress rapidly. No effective treatment is currently available, thus resulting a high mortality rate from malignant PTs. PT cell lines must be established to facilitate the study of PTs. Herein, we established six PT cell lines through continuous passage or cell immortalization. We characterized these PT cell lines through in vitro functional assays, malignant PT marker detection and short tandem repeat identification. Benign PT cell lines (SYSH-BPT-01 and SYSH-BPT-02) were transfected with human papillomavirus 16 E6/E7, and two malignant PT cell lines (SYSH-MPT-01 and SYSH-MPT-02) were transfected with Simian virus 40 large T antigen. Two malignant PT cell lines (SYSH-MPT-03 and SYSH-MPT-04) were established through continuous passage. All malignant PT cell lines showed greater proliferation, colony formation, migration, invasion and collagen contraction ability than the benign PT cell lines. Moreover, the expression levels of malignant PT markers (α-smooth muscle actin and fibroblast activation protein) and short tandem repeat identification indicated that each PT cell line was identical to the parental primary cells. We successfully established PT cell lines that preserved the features of primary cells. These cell lines may serve as ideal experimental models for studying the function of breast PTs, thus opening new possibilities for PT drug screening and therapeutic target validation.

Background: Ovarian cancer ranks second among gynecological cancers worldwide. This study aimed to compare the glucose transporter-1 (GLUT-1) expression in benign, borderline, and malignant ovarian epithelial tumors and evaluate GLUT-1 expression as a diagnostic tool for distinguishing tumors in the ovary. Materials and Methods: This descriptive-analytical cross-sectional study analyzed 69 pathological samples of patients diagnosed with ovarian epithelial tumors who underwent oophorectomy. Immunohistochemical staining was performed using GLUT-1 antibody. The intensity of cell membrane staining and the proportion of positive neoplastic cells were graded to score immunostaining. Gastrointestinal stromal tumors (GIST) are rare tumors accounting for less than 1% of primary neoplasia of the digestive tract. They can also occur outside the GIT, where they are called extra gastrointestinal stromal tumors (EGIST), which are extremely rare tumors. GISTS and EGISTS have similar histo-pathological and molecular profiles. Though these are known to be FDG avid, some of the atypical GISTS can be FDG negative or only minimally FDG avid.Here, an unusual case of a 57-year-old female with FDG avid solitary chest wall mass is presented, which after biopsy and immune-histochemistry was proven as EGIST. She underwent en block resection of the tumor and is presently on adjuvant treatment with Imatinib mesylate. This case report highlights the importance of considering EGIST as a differential diagnosis of a solitary chest wall mass and the utility of 18F- FDG PET/CT in its management.

Primary cardiac tumors are exceptionally rare, with malignant tumor occurrences ranging from 0.0017% to 0.28%. Among these, primary cardiac angiosarcoma (PCA) stands as the most prevalent malignancy, primarily impacting the right cardiac system. In this case report, we present the instance of a 44-year-old woman who recently exhibited acute chest discomfort and was subsequently diagnosed with a microangiosarcoma within the right atrium and superior vena cava. Diagnostic modalities including chest x-rays, CT, MRI, and PET-CT were instrumental in pinpointing the tumor's location and nature. Surgical excision followed by pathological and immunological examinations confirmed the diagnosis. The patient's recovery post-surgery has been encouraging, with successful follow-up chemoradiotherapy administered. Despite advancements, devising optimal strategies for enhancing patient survival and quality of life in angiosarcoma cases remains a pressing research challenge.

Multiple myeloma is a common malignant bone-based disease. Pleural effusions reported in these patients remain rare. It is commonly due to congestive heart disease, pulmonary embolism, nephrotic syndrome or a second neoplasia. The true myelomatous pleural effusion resulting from a direct tumoral invasion of the pleural are extremely rare. We report here the case of a massive pleural effusion revealing multiple myeloma in a 71-year-old patient. The chest ultrasound showed a massive pleural effusion in the left side with a multinodular thickening of the pleura. The medical thoracoscopy showed a grape-cluster appearance. The diagnosis was made by pleural guided biopsy revealing abnormal plasma cells with an intense positive CD 138 (plasma cell marker) and MUM1 (multiple myeloma oncogene1) staining with a light kappa chain in the protein electrophoresis associated with a myeloma. Unfortunately, our patient died one month after the initial diagnosis. We present also a review of the recent literature in order to highlight the clinical presentations of the myelomatous pleural effusion, the diagnostic tools, the therapeutic strategies as well as the outcomes.

Simple Summary: Renal cell carcinoma (RCC) is frequently diagnosed at the early localized stage as an incidental finding (about 60% of cases). Imaging procedures (ultrasound, CT, MRI) represent the only way to diagnose RCC, but they are not always reliable for the discrimination between malignant and benign tumors, in particular when the renal mass is small (<4 cm) because they demonstrate low diagnostic specificity. The quantitative analysis of contrast-enhanced CT in venous phase using radiomics could provide additional information for the accurate characterization of small renal masses (SRMs). (1) Background and (2) Methods: In this retrospective, observational, monocentric study, we selected a cohort of eighty-five patients (age range 38–87 years old, 51 men), enrolled between January 2014 and December 2020, with a newly diagnosed renal mass smaller than 4 cm (SRM) that later underwent nephrectomy surgery (partial or total) or tumorectomy with an associated histopatological study of the lesion. The radiomic features (RFs) of eighty-five SRMs were extracted from abdominal CTs bought in the portal venous phase using three different CT scanners. Lesions were manually segmented by an abdominal radiologist. Image analysis was performed with the Pyradiomic library of 3D-Slicer. A total of 108 RFs were included for each volume. A machine learning model based on radiomic features was developed to distinguish between benign and malignant small renal masses. The pipeline included redundant RFs elimination, RFs standardization, dataset balancing, exclusion of non-reproducible RFs, feature selection (FS), model training, model tuning and validation of unseen data. (3) Results: The study population was composed of fifty-one RCCs and thirty-four benign lesions (twenty-five oncocytomas, seven lipid-poor angiomyolipomas and two renal leiomyomas). The final radiomic signature included 10 RFs. The average performance of the model on unseen data was 0.79 ± 0.12 for ROC-AUC, 0.73 ± 0.12 for accuracy, 0.78 ± 0.19 for sensitivity and 0.63 ± 0.15 for specificity. Purpose: There are limited data on the impact of caregiving for patients with malignant pleural mesothelioma (MPM) on the caregiver. We aimed to identify the demographic characteristics of these caregivers, the caregiving activities they perform and how caregiving burden impacts their work productivity and overall activity. Methods: This cross-sectional study collected data from caregivers of patients with MPM across France, Italy, Spain and the United Kingdom January-June 2019. Caregiver demographics, daily caregiving tasks and the impact of caregiving on physical health was collected via questionnaire. The Zarit Burden Interview (ZBI) was used to assess caregiver burden and the Work Productivity and Activity Impairment questionnaire (WPAI) assessed impairment at work and during daily activities. Analyses were descriptive. Results: Overall, 291 caregivers provided data. Caregivers were mostly female (83%), living with the patient (82%) and their partner/spouse (71%). Caregivers provided over five hours of daily emotional/physical support to patients. ZBI scores indicated 74% of caregivers were at risk of developing depression. Employed caregivers had missed 12% of work in the past seven days, with considerable presenteeism (25%) and overall work impairment (33%) observed. Overall, the mean activity impairment was 40%. Conclusion: Caregivers provide essential care for those with MPM. We show caregiving for patients with MPM involves a range of burdensome tasks that impact caregivers' emotional health and work reflected in ZBI and WPAI scores. We report a unique case of a suspected recurrent intracranial epidermoid cyst (EDC) that was found on pathology to have undergone malignant transformation to squamous cell carcinoma (SCC) approximately 25 years after initial resection. Additionally, we performed a systematic review including 94 studies reporting intracranial EDC to SCC transformation. Ninety-four studies were included in our systematic review. PubMed, Scopus, Cochrane Central, and EMBASE were searched in April 2020 for studies regarding histologically confirmed SCC arising within an EDC. Kaplan-Meier estimations were used to estimate time to event including survival, and log rank tests were used to test for significance. All analyses were conducted using STATA 14.1 (StataCorp, College Station, Texas, USA); tests were two-sided, and statistical significance was defined using the alpha threshold of 0.05. The overall median time to transformation was 60 months (95% confidence interval {CI}, 12–96). Transformation time was significantly shorter in the no surgery group (10 months, 95% CI undefined) versus the other 2 groups (60 months, 95% CI, 12–72 in surgery only and 70 months, 95% CI, 9–180 in surgery + adjuvant therapy group, both P < 0.01). Overall survival was significantly longer in the surgery + adjuvant therapy group (13 months, 95% CI, 9–24) versus the other 2 groups (3 months, 95% CI, 1–7 in surgery only and 6 months, 95% CI, 1–12 in the no surgery group, both P < 0.01). We report a rare case of delayed malignant transformation of an intracranial EDC to SCC, occurring nearly 25 years after initial resection. Transformation time in the no-surgery group was statistically significantly shorter as compared to the surgery only and surgery + adjuvant therapy groups. A perivascular epithelioid cell tumor (PEComa) is an uncommon mesenchymal tumor composed of perivascular epithelioid cells. These tumor cells show variable immunoreactivity for both melanocytic and myogenic markers. Occurrence of PEComa has been reported at various anatomical sites, including the gynecological tract, uterus being the most common. Although most patients have sporadic PEComas, a subset may be associated with the inactivation of TSC1 or TSC2 genes and the occurrence of TFE3 gene fusions. However, a relationship between PEComas and other tumors is rare. We report a 41-year-old female patient with malignant PEComa who was admitted to the hospital with a complaint of vaginal bleeding. Because she had previously been diagnosed with colorectal and breast carcinomas at an early age, we performed a comprehensive genetic analysis to identify molecular alterations present in her background that unveiled multiple malignancy predispositions. Next-generation sequencing (NGS) analysis revealed two heterozygous germline pathogenic variants in the ATM and TP53 genes and a heterozygous variant of unknown significance (VUS) in the BRCA2 gene. The patient was diagnosed with the Li-Fraumeni Syndrome owing to the medical and family history and also the presentation of a pathogenic mutation of the TP53 gene. There are very few case reports in the literature describing PEComa in the Li-Fraumeni syndrome, and this is the first report of a uterine PEComa in a patient with Li-Fraumeni syndrome. Breast cancer, a leading cause of female mortality worldwide, poses a significant health challenge. Recent advancements in deep learning techniques have revolutionized breast cancer pathology by enabling accurate image classification. Various imaging methods, such as mammography, CT, MRI, ultrasound, and biopsies, aid in breast cancer detection. Computer-assisted pathological image classification is of paramount importance for breast cancer diagnosis. This study introduces a novel approach to breast cancer histopathological image classification. It leverages modified pre-trained CNN models and attention mechanisms to enhance model interpretability and robustness, emphasizing localized features and enabling accurate discrimination of complex cases. Our method involves transfer learning with deep CNN models—Xception, VGG16, ResNet50, MobileNet, and DenseNet121—augmented with the convolutional block attention module (CBAM). The pre-trained models are finetuned, and the two CBAM models are incorporated at the end of the pre-trained models. The models are compared to state-of-the-art breast cancer diagnosis approaches and tested for accuracy, precision, recall, and F1 score. The confusion matrices are used to evaluate and visualize the results of the compared models. They help in assessing the models' performance. The test accuracy rates for the attention mechanism (AM) using the Xception model on the "BreakHis" breast cancer dataset are encouraging at 99.2% and 99.5%. Background and Objective: Ovarian masses are one of the most common clinical conditions among women, which if not treated properly may be developed into malignant. However, not ovarian masses are progressing into malignant, and early diagnosis and treatment generally cure them without much destruction. The rate of occurrence highly depends on the demographic and obstetric profile of women. Since demographic profile varies it becomes essential to carry out detailed studies of a particular region to understand the prevalence of ovarian masses. Here, such a retroprospective study was carried out to determine the prevalence of ovarian masses in the Rajkot district. Methodology: A total of 44 cases with ovarian masses were taken into consideration for the determination of the effect of various parameters and their association with ovarian masses. The patients were analyzed for nonneoplastic, benign, and malignant tumors. Patients were analyzed based on demographic location, obstetric profile, and clinical presentations. Results: Based on the entire study, it was found that women in a middle age group are more prone to ovarian masses as compared to the older age group. Not only this but higher parity is also associated with increased chances of ovarian masses. Among nonneoplastic tumors occurrence of the serious cyst was highest. While in the case of benign tumors, mucinous cystadenoma and teratoma both were found predominant. However, no significant difference was observed in the patients with nonneoplastic and benign tumors. Among 44 cases, only one case with serous papillary adenocarcinoma was found. Conclusion: Based on the results of the study, it was found that women of Rajkot district in the age group 30–50 years are more prone to ovarian masses. [ABSTRACT FROM AUTHOR]

The incidence of renal mass detection has increased during recent decades, with an increased diagnosis of small renal masses, and a final benign diagnosis in some cases. To avoid unnecessary surgeries, there is an increasing interest in using radiomics tools to predict histological results, using radiological features. We performed a narrative review to evaluate the use of radiomics in renal mass characterization. Conventional images, such as computed tomography (CT) and magnetic resonance (MR), are the most common diagnostic tools in renal mass characterization. Distinguishing between benign and malignant tumors in small renal masses can be challenging using conventional methods. To improve subjective evaluation, the interest in using radiomics to obtain quantitative parameters from medical images has increased. Several studies have assessed this novel tool for renal mass characterization, comparing its ability to distinguish benign to malign tumors, the results in differentiating renal cell carcinoma subtypes, or the correlation with prognostic features, with other methods. In several studies, radiomic tools have shown a good accuracy in characterizing renal mass lesions. A machine can learn from experience without explicit programming thanks to computer education. It is an area within AI. Deep learning, a kind of machine learning, infers critical features for image processing via multiple layer processing and mathematical operations based on artificial neural networks. In the field of healthcare, which encompasses medicine and dentistry, artificial intelligence has several applications. Early melanoma skin cancer identification is necessary for effective therapy. Melanoma, among the various types of skin cancer, has recently gained international recognition as the most deadly one since it is much more likely to spread to other body regions if detected and treated quickly. Clinical diagnosis of various ailments is increasingly using non-invasive medical computer vision or medical image processing. These methods offer an automatic image processing tool that makes it possible to examine the lesion quickly and precisely. The procedures used in this study included building a database of dermoscopy images, preprocessing, segmenting using thresholding, extracting statistical features using asymmetry, border, colour, diameter, etc., and choosing features based on the total dermoscopy score, principal component analysis (PCA), and convocation neural network classification (CNN). There are multiple indications to evaluate the bone marrow in canines and felines, persistent or unexplained abnormalities in the blood count and serum biochemistry, accompanied by non-specific signs and symptoms. They can be reactive or marrow disorders. The objective was to present an overview of the most frequent indications for the morphological evaluation of bone marrow in canines and felines reported in the literature. An approach was made to malignant and benign disorders with bone marrow involvement in canines and felines and the frequency is presented through a search of original articles in the databases PubMed, ScienceDirect, Scopus, SciELO, and LILACS, using MeSh terms: bone marrow neoplasms, bone marrow examination, congenital bone marrow failure syndromes, bone marrow failure disorders, combined with the Boolean operator AND with the words dog and cat. Articles that applied bone marrow as an aid for the differential diagnosis of infections such as leishmaniasis, ehrlichiosis and anaplasmosis were found. However, most of the publications are aimed at diagnosing malignant processes, such as myeloproliferative and lymphoproliferative disorders. It is concluded that the evaluation of the bone marrow is of great importance, resulting in clarifying the diagnosis and evaluating the staging of some diseases, as well as their prognosis and follow-up. In Malaysia, nasopharyngeal carcinoma is the 5th (4.0 %) most prevalent malignant tumor and it is the 3rd highest in male for cancer occurrence rate (8.4 %). Therefore, discovery of novel anti-cancer herbal drugs is of importance. In this study, the cytotoxic effect of traditional Chinese herbal prescription Xiao Xian Xiong decoction which made up of Huanglian (Coptis chinensis Franch.), Banxia (Pinellia ternata (Thunb.) Breit.), and Gualou (Trichosanthes kirilowaii Maxim) was tested. The cytotoxic effect Xiao Xian Xiong decoction was studied on 8 nasopharyngeal cancer cell lines (TWO-1, TWO-4, HONE-1, SUNE-1, CNE-2, HK-1, CNE-1 and C666-10). Global gene expression analysis was conducted using nCounter XT gene expression assay. The half-maximal inhibitory concentration values obtained for Xiao Xian Xiong decoction-treated HONE-1 and CNE-2 cell lines were 88.55 and 92.42 µg/ml respectively. Global gene expression showed Xiao Xian Xiong decoction significantly downregulated genes that are associated with cell growth and proliferation, whereas upregulated genes that are associated with induced cell cycle arrest and apoptosis in HONE-1 cells. Introduction: Per- and poly-fluoroalkyl substances (PFASs) are a large, complex group of synthetic chemicals humans can be exposed to from occupational or environmental sources. In this systematic review and meta-analysis, we examined the association between PFAS exposure, particularly Perfluorooctanoic Acid (PFOA), and Perfluorooctane Sulfonic Acid (PFOS), and risk of kidney, liver, and testicular cancer. Methods: We systematically searched PubMed to identify cohort and case-control studies reported after the Monograph of the International Agency for Research on Cancer and the Toxicological Profile of the Agency for Toxic Substances and Disease Registry. We assessed the quality of the studies by using a modified version of the Newcastle-Ottawa Scale (NOS). Forest relative risk (RR) plots were constructed for liver, kidney, and testicular cancer. We conducted stratified analyses by geographic region, study design, quality score, outcome, years of publication, exposure source, and PFAS type. A random-effects model was used to address heterogeneity between studies. Results: Fifteen studies, including ten cohort studies, three case-control studies nested in a cohort, and two case-control studies were included after removing duplicate and irrelevant reports. We found an association between overall PFAS exposure and the risk of kidney cancers (RR=1.18, 95% CI =1.05-1.32; I =52.8%, 11 studies). Also, we showed an association between high-level exposure to PFAS and kidney cancer (RR=1.74, 95% CI =1.23-2.47; p=0.005) and testicular cancer (RR=2.22, 95% CI =1.12-4.39; p=0.057). There was no association with liver cancer. We found no heterogeneity by geographical region, PFAS type, study design, outcome, quality score, year of publication, or exposure source. Only two studies reported results among women. Conclusions: We detected an association between overall PFAS exposure and kidney cancer and high doses of PFAS with testicular cancer. In this report, we detail a case of new-onset resistant hypertension leading to hypertensive emergency complicated by myocardial infarction and congestive heart failure secondary to underlying cortisol-producing metastatic adrenocortical carcinoma. Case Report: A 57-year-old woman with no past medical history presented with generalized weakness and weight gain. Her blood pressure was 239/141 with a pulse of 117. Other vital signs were normal. A physical exam was notable for obesity and lower-extremity edema. Initial serum investigations were notable for leukocytosis, hypokalemia, metabolic alkalosis, and elevated troponin and BNP. An ECG showed anterolateral ST depression and left ventricular hypertrophy. A coronary angiogram revealed no coronary artery disease. Her ejection fraction was 25% by echocardiogram. Further investigation revealed severely elevated serum cortisol levels. CT scans were notable for left adrenal mass with evidence of hepatic, lung, and bone metastasis. A liver biopsy confirmed metastatic adrenocortical carcinoma. The patient was started on antihypertensives and a steroidogenesis inhibitor, with improvement in her blood pressure. She received palliative chemotherapy but later elected to pursue hospice care. Conclusions: This report highlights the potential for underlying cortisol excess and adrenocortical carcinoma as a potential secondary etiology of resistant hypertension and hypertensive crisis. Due to the aggressive nature of this tumor, as demonstrated in this patient, a high index of suspicion and prompt attention are required for patients presenting with these clinical manifestations. Background: Ameloblastic carcinoma and metastasising ameloblastoma are rare epithelial odontogenic tumours with aggressive features. Distinguishing between these two lesions is often clinically difficult but necessary to predict tumour behaviour or to plan future therapy. Here, we provide a brief review of the literature available on these two types of lesions and present a new case report of a young man with an ameloblastoma displaying metastatic features. We also use this case to illustrate the similarities and differences between these two types of tumours and the difficulties of their differential diagnosis. Case presentation: Our histopathological analyses uncovered a metastasising tumour with features of ameloblastic carcinoma, which developed from the ameloblastoma. We profiled the gene expression of Wnt pathway members in ameloblastoma sample of this patient, because multiple molecules of this pathway are involved in the establishing of cell polarity, cell migration or for epithelial–mesenchymal transition during tumour metastasis to evaluate features of tumor behaviour. Indeed, we found upregulation of several cell migration–related genes in our patient. Moreover, we uncovered somatic mutation BRAF p.V600E with known pathological role in cancerogenesis and germline heterozygous FANCA p.S858R mutation, whose interpretation in this context has not been discussed yet. Conclusions: In conclusion, we have uncovered a unique case of ameloblastic carcinoma associated with an alteration of Wnt signalling and the presence of BRAF mutation. Background. Classification of phyllodes tumors is challenging due unclear diagnostic criteria, recently addressed by consensus review criteria. Herein, we reviewed all malignant phyllodes tumor resections and reclassified them based on the consensus guidelines, correlating with outcome. We hypothesize that application of criteria would result in a significant proportion being "down-graded" to either borderline or benign phyllodes tumor. Methods. Primary resections of malignant phyllodes tumor were reviewed by four AP board-certified, breast fellowship-trained pathologists. Morphologic variables delineated in consensus guidelines (ie stromal cellularity, cellular atypia, tumor border, presence of heterologous elements, presence of stromal overgrowth) were evaluated. Following review, cases were reclassified as benign, borderline, or malignant. Results. Upon reclassification, 20% (5/20) cases were "down-graded" to borderline phyllodes tumor while 80% (15/20) remained malignant phyllodes tumor. Two morphologic features were statistically significant including broadly infiltrating tumor border in 80% (12/15) of malignant phyllodes tumors compared to none in borderline phyllodes tumor (0/5) (p = 0.004) and stromal overgrowth in 67% (10/15) of malignant phyllodes tumor compared to none in borderline phyllodes tumors (0/5) (p = 0.03). Upon review of the pathology reports, 30% (6/20) contained all 5 histomorphologic variables delineated in the consensus review criteria. Malignant phyllodes tumor resulted in five cases with recurrence (33.3%, 5/15) and three cases with metastases (20.0%, 3/15) and borderline phyllodes tumor resulted in one case with recurrence (20.0%, 1/5) and no metastases (0/5). Conclusion. The consensus guidelines for phyllodes tumor are useful for subclassification. We hypothesize that standardize reporting of the histomorphologic variables may lead to better consensus. Lung cancer is the uncontrolled growth of cells that originates in the lung parenchyma or cells that line the air passages. These cells divide rapidly to form malicious tumors. This paper proposes a multi-task ensemble of three dimensional (3D) deep neural network (DNN) based model, namely: pre-trained EfficientNetB0, BiGRU-based SEResNext101, and the proposed LungNet. The ensemble model performs binary classification and regression tasks to accurately classify the benign and malignant pulmonary nodules. This study also explores the attribute importance and proposes a domain knowledge-based regularization technique. The proposed model is evaluated on the public benchmark LIDC-IDRI dataset. Through a comparative study, it was shown that when coefficients generated by the random forest (RF) are used in the loss function, the proposed ensemble model offers a better prediction capability of the accuracy of 96.4% compared to the state-of-the-art methods. In addition, the receiver operating characteristic curves show that the proposed ensemble model has better performance than the base learners. Purpose: This study aims to investigate three different image processing methods on quantitative parameters of IVIM sequence, as well as apparent diffusion coefficients and simple perfusion fractions, for benign and malignant liver tumors. Materials and methods: IVIM images with 8 b-values (0–1000 s/mm2) and 1.5 T MRI scanner in 16 patients and 3 healthy people were obtained. Next, the regions of interest were selected for malignant, benign, and healthy liver regions (50, 56, and 12, respectively). Then, the bi-exponential equation of the IVIM technique was fitted with two segmented fitting methods as well as one full fitting method (three methods in total). Using the segmented fitting method, diffusion coefficient (D) is fixed with a mono-exponential equation with b-values that are greater than 200 s/mm2. The perfusion fraction (f) can then be calculated by extrapolating, as the first method, or fitting simultaneously with the pseudo-diffusion coefficient (D*) as the second method. In the full fitting method, as the third method, all IVIM parameters were obtained simultaneously. The mean values of parameters from different methods were compared in different grades of lesions. Results: Our results indicate that the image processing method can change statistical comparisons between different groups for each parameter. The D value is the only quantity in this technique that does not depend on the fitting process and can be used as an indicator of comparison between studies (P < 0.05). The most effective method to distinguish liver lesions is the extrapolated f method (first method). This method created a significant difference (P < 0.05) between the perfusion parameters between benign and malignant lesions. Conclusion: Using extrapolated f is the most effective method of distinguishing liver lesions using IVIM parameters. Early diagnosis and treatment are the keys to managing patients with breast cancer. Identifying patients even before they present with symptoms is made possible through screening methods. Thermography is a tool for screening carcinoma breast to reduce the associated morbidity and mortality. This paper proposes a novel feature selection using a machine learning algorithm, namely, the Greedy search optimization algorithm. This algorithm is applied to compare various features selection techniques. These techniques are sequential backward (SBS), sequential forward feature selection, and exhaustive feature selection techniques. It is concluded from this comparison that sequential backward feature selection is the best technique for breast cancer diagnosis. Our average score of SBS comes 88.5714%, with a computational time of 87.4 s. For classification purposes, we have used an artificial neural network. The classification result varies according to the age group with the physiology of the breast. Considering this, we have selected features age-group wise by sequential backward technique. Purpose: To evaluate and compare the value of conventional ultrasound-based superb microvascular imaging (SMI) and color Doppler flow imaging (CDFI) in the diagnosis of malignant thyroid nodule by meta-analysis. Methods: The literature included in the Cochrane Library, PubMed, and Embase were searched by using "superb microvascular imaging (SMI), color Doppler flow imaging (CDFI), ultrasound, thyroid nodules" as the keywords from inception through February 1, 2023. According to the inclusion and exclusion criteria, the clinical studies using SMI and CDFI to diagnose thyroid nodules were selected, and histopathology of thyroid nodules was used as reference standard. The diagnostic accuracy research quality assessment tool (QUADAS-2) was used to evaluate the quality of included literature, and the Review Manager 5.4 was used to make the quality evaluation chart. The heterogeneity test was performed on the literature that met the requirements, the combined sensitivity, specificity, positive likelihood ratio, and negative likelihood ratio were pooled, and a comprehensive ROC curve analysis was performed. Meta-DiSc version 1.4, StataSE 12, and Review Manager 5.4 software were used. Results: Finally, 13 studies were included in this meta-analysis. A total of 815 thyroid malignant nodules were assessed. All thyroid nodules were histologically confirmed after SMI or CDFI. The combined sensitivity, specificity, PLR, NLR, DOR, and area under the SROC curve of SMI for the diagnosis of malignant thyroid nodules were 0.80(95%CI: 0.77-0.83), 0.79(95%CI: 0.77-0.82), 4.37(95% CI: 3.0-6.36), 0.23(95%CI: 0.15-0.35), 22.29(95%CI: 12.18-40.78), and 0.8944, respectively; the corresponding values of CDFI were 0.62(95%CI: 0.57-0.67), 0.81(95%CI: 0.78-0.85), 3.33(95%CI: 2.18-5.07), 0.41(95%CI: 0.27-0.64), 8.93 (95%CI: 3.96-20.16), and 0.8498. Conclusion: The diagnostic efficiency of SMI for malignant thyroid nodules is better than CDFI, and SMI technology can provide significantly more information on vascularity, make up for the deficiency of CDFI, and has better clinical application value. Background: The oral cavity is the most common site of cancer in the Indian subcontinent contributing to one-third of all cancers. The prognosis of oral dysplasia and malignant lesions can be predicted by both clinicopathological parameters and molecular markers. Aims and Objectives: The present study was undertaken to examine and compare the expression profile of cell cycle regulatory proteins p63 and Ki-67 in normal dysplastic and malignant lesions of oral mucosa and its correlation with prognosis. Settings and Design: This was a retrospective case-control study in a tertiary referral center. Materials and Methods: We evaluated and compared the expression profile of antigens p63 and Ki-67 on 60 cases of histologically proven oral dysplastic and malignant lesions oral squamous cell carcinoma (OSCC - 35 cases), oral epithelial dysplasia (OED - 15 cases), and 10 cases of benign and inflammatory oral lesions. Statistical Analysis: Mean and standard deviation were calculated and ANOVA statistical test was used to compare the data. Results: The mean labeling index of p63 and KI-67 was higher in OSCC than OED and minimum in inflammatory (controls) groups. Simple Summary: Epithelioid hemangioendothelioma (EHE) is a very rare malignant vascular neoplasm with unpredictable clinical course due to its remarkable heterogeneity. Large cohort studies with molecularly/immunohistochemically confirmed EHE are scarce, and what determines survival has been controversial. This retrospective nationwide cohort study addresses EHE epidemiology and aims to identify clinical and histopathological parameters with prognostic significance, thereby providing useful insights into the clinical behavior of this rare cancer. Our findings emphasize the aggressive behavior of EHE, demonstrated by lower 1- and 5-year overall survival rates compared to those in the current literature. Moreover, we confirmed the usefulness of the risk stratification model by Shibayama for unifocal disease and showed that multifocal and metastatic disease have no survival differences, indicating that multifocality is early metastatic disease. Tumors in the head and neck area have a higher propensity for lymph node metastases, entailing consideration of lymphadenectomy. Epithelioid hemangioendothelioma (EHE) is an extremely rare vascular sarcoma with variable aggressive clinical behavior. In this retrospective study, we aimed to investigate prognostic factors based on clinicopathologic findings in a molecularly/immunohistochemically confirmed nationwide multicenter cohort of 57 EHE cases. Patients had unifocal disease (n = 29), multifocal disease (n = 5), lymph node metastasis (n = 8) and/or distant metastasis (n = 15) at the time of diagnosis. The overall survival rate was 71.4% at 1 year and 50.7% at 5 years. Survival did not correlate with sex, age or histopathological parameters. No survival differences were observed between multifocal and metastatic disease, suggesting that multifocality represents early metastases and treatment options are limited in comparison to unifocal disease. In unifocal tumors, survival could be predicted using the risk stratification model of Shibayama et al., dividing the cases into low- (n = 4), intermediate- (n = 15) and high- (n = 3) risk groups. No clinical or histopathological parameters were associated with progressive unifocal disease course. Lymph node metastases at the time of diagnosis occurred in 14.0% of the cases and were mainly associated with tumor localization in the head and neck area, proposing lymph node dissection. In conclusion, our results demonstrate the aggressive behavior of EHE, emphasize the prognostic value of a previously described risk stratification model and may provide new insights regarding tumor focality, therapeutic strategies and prognosis. Malignant peritoneal mesothelioma is a rare and extremely malignant tumor. It is difficult for clinicians to diagnose early for its insidious onset and lack of specific clinical manifestations, laboratory indicators and imaging characteristics. At present, the accurate diagnosis dependes on the pathological examination and immunochemistry. In this paper, we report a case of malignant peritoneal mesothelioma found by the laparoscopic surgery due to "hydrofallopian tube". This is a female patient of childbearing age who had primary infertility for 3 years. The patient had no history of asbestos exposure, unclear etiology, atypical clinical manifestations. The diagnosis of malignant peritoneal mesothelioma was very difficult before histopathological examination. We analyze retrospectively the medical records of the patient and review of the previous literature. Introduction: Cytological examination of body fluids is a commonly performed investigation that provides information about inflammatory and malignant lesions, aiding in the diagnosis of effusion aetiology and potential differential diagnoses. The cell block technique, combined with cytospin study, offers an additional advantage in such cases. Aim: To compare the diagnostic utility of cell block method and cytospin method in detecting malignancy in body fluids. Materials and Methods: This cross-sectional observational study was conducted for a period of two years at Bharati Vidyapeeth (DTU) Medical College Hospital and Research Centre in Pune, Maharashtra, India. A total of 100 body fluid samples were analysed, including 73 pleural fluids, 24 ascitic fluids, and 3 pericardial fluids. Each fluid sample was divided into two equal parts: one processed for cytospin (CYTOTEK), and the other processed to create a cell block. Four Immunohistochemistry (IHC) markers (Thyroid Transcription Factor-1 (TTF-1), Cytokeratin 7 (CK7), Cytokeratin 20 (CK20), and Calretinin) were applied as needed. Data analysis was performed using the Statistical Package for Social Sciences (SPSS) version 25.0, and results were calculated using the Chi-square test. Results: Of the total 100 fluid samples, 65 were negative, 17 were positive, and 18 were suspicious for malignancy using the cytospin method. In comparison, the cell block method identified 63 samples as negative, 29 as positive, and 8 as suspicious for malignancy. The cell block method exhibited a higher sensitivity (100.00%) and specificity (94.03%) than the cytospin method (81.82% and 88.06%, respectively) in this study. Conclusion: Cell block method accurately diagnosed 10 cases as positive for malignancy that were initially deemed suspicious on cytospin smear. Therefore, the cell block method proved to be a superior diagnostic tool for malignancy compared to cytospin smear. Given that cell blocks allow for longer storage and additional analysis such as IHC and microarray, they should be adopted in addition to cytospin for effusion cytology. [ABSTRACT FROM AUTHOR]

Background: It is difficult to select patients who will benefit from endobronchial treatment (ET) in malignant central airway obstruction (MCAO). We aimed to determine the tumor-related factors that affect the success of MCAO treatment. Methods: ETs for MCAO between March 2019 and June 2021 were analyzed retrospectively. The relationships between the success of the procedure and the percentage of endoluminal obstruction, tumor size, and type of lesion were evaluated. Results: Totally, 220 ETs were administered to 205 patients. Treatment was significantly more successful for the patients with pure endobronchial lesions than those with mixed lesions. The success rate was significantly lower when the tumor size was greater than 54.5 mm and the degree of endoluminal stenosis exceeded 92%; the area under the curve was 0.734 (0.625-0.842; P = 0.001) and 0.733 (0.597-0.870; P = 0.001), respectively. There was no difference in the procedural success between lung cancer and extrathoracic malignancies and tumor treatment before the procedure. Conclusion: Mixed lesions, tumor size over 54.5 mm, and a degree of stenosis over 92% are risk factors for unsuccessful endoluminal obstruction procedures. Carcinoma breast is the commonest cancer among women. Various authors have studied breast cancer with Contrast-Enhanced Ultrasound (CEUS) with promising results. Despite promising results, the additional cost of post-processing software limits its availability. In this study, we evaluated the utility of CEUS in differentiating malignant from benign breast lesions on regular ultrasound equipment without the use of dedicated software. We performed CEUS in 121 women with 121 breast lesions. CEUS was done by creating a custom preset on existing ultrasound equipment with the help of an application specialist authorized by the vendor. Lesions were evaluated qualitatively without the use of any commercial software. The pattern of enhancement i.e. homogenous, heterogeneous, peripheral, or no enhancement, and the number of penetrating vessels i.e., few or multiple were recorded. Results were compared with histopathological diagnosis. There were a total of 121 breast lesions. The study showed sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 86.67 %, 54.10 %, 65 %, and 80.49% respectively for differentiating benign vs malignant lesions on the basis of the pattern of contrast enhancement. Using penetrating vessels for differentiating malignant lesions from benign lesions, the sensitivity, specificity, PPV, and NPV were found to be 64%, 67.86%, 78.05%, and 51.35% respectively. CEUS is useful in differentiating malignant from benign breast lesions. It can be easily performed by creating a custom preset on standard ultrasound equipment without the use of expensive software. The aim of this study was to evaluate the use of visually-enhanced lesion scope (VELscope) as an adjuvant diagnostic tool along with clinical examination. VELscope device which works on the principle of tissue fluorescence and helps in diagnosis or identification of suspected dysplastic lesions. The use of VELscope in dentistry is contributory as it aids in the detection of dysplastic changes in oral potentially malignant disorders and appropriate surgical margins for resection, VELscope-guided biopsy procedures, indicative of bacterial, fungal, and viral infections, and also inflammatory lesions. The cases discussed in the report are clinical cases where VELscope has been used as an accessory tool to enhance the clinical efficacy of diagnosis. There are different domains of cases where the use of VELscope has been assessed, also cases where VELscope can be beneficial to identify most preferred site of biopsy, aiding in the identification of fungal, bacterial presence, and evaluation of premalignant lesions and different oral carcinomas. Both the advantages, limitations, and significance of the device in each clinical condition have been discussed. VELscope is a handheld, noninvasive adjuvant device which can be used along with conventional clinical conditions for enhanced diagnostic accuracy. It can also be used as a screening tool for the early detection of oral carcinoma and dysplastic lesions in mass screening and tertiary centers. Further studies are required to establish the efficacy of VELscope in the clinical practice of oral medicine and radiology. Increasing incidence of oral carcinoma is a worrying scenario. Early detection of these conditions can be assessed by various diagnostic methods which broadly includes conventional staining dyes such as Lugol's iodine, methylene blue, toluidine blue, or light-based detection such as chemiluminescence, VELscope, Vizilite, or optical-based system such as colposcopy and optical coherence tomography. VELscope can be one of the tool for assessing dysplastic lesions at an early stage. Brain tumours are heterogeneous and are classified comprehensively into molecular subtypes based on genetic alterations. Glioblastoma rapid progression, drug resistance, and recurrence have been scientifically linked to several factors, including its rapid growth rate, loss of apoptosis, pro-survival signalling, molecular heterogeneities and hallmark features to infiltrate vital brain structures. Because of the growing demand for design and development of delivery systems to overcome the existing limitations with the current therapeutic strategies, researchers are exploiting multifaceted aspects of nanotechnology to improve delivery of the drug payload. Firstly, nanotechnology procedures can improve the drug delivery methods with the help of nanoparticles (NPs) based nanovectors that can efficiently cross blood-brain barrier. Secondly, NPs also improve the cellular uptake of the drug as they can efficiently bind with the cell surface. Thirdly, NPs make the delivery of siRNAs and peptides possible, which can suppress the resistance of glioblastoma against TMZ or other chemo- preventive drugs. Fourthly, the use of metal NPs increases the efficiency of scanning or magnetic resonance imaging (MRI) procedures as they can produce contrasts in it. Lastly, NPs make it possible to use highly targeted co-administered strategies like chemoprevention and near infrared (NIR) or radiotherapy (RT). Aim and Objective: The objective of our study was to build a convolutional neural network (CNN) model and detection and classification of benign and malignant radiolucent lesions in orthopantomogram (OPG) by implementing CNN. Method: Two basic CNN models were implemented on Anaconda with Python 3 on 64-bit, CNN-I for detection of radiolucency and CNN-II for classification of radiolucency into benign and malignant lesions. One hundred fifty eight OPG with radiolucency and 115 OPG without radiolucency was used for training and validation of CNN models. Data augmentation was performed for the training and validation dataset. The evaluation of the performance of both CNN by new data consisting (60 OPG images) 30 benign and 30 malignant lesions. Statistical Analysis: Performed using SPSS (Statistical package for social science) 20.0 version. The descriptive statistics was performed. The Cohen kappa correlation coefficient was used for assessment of reliability of the diagnostic methods. P < .05 was considered statistically significant. Determination of sensitivity, specificity, positive and negative predictive value was also performed. Result: CNN-I showing sensitivity for detection of the benign lesion is 76.6% and sensitivity for the malignant lesion is 63.3% with overall sensitivity is 70%. CNN-II showing sensitivity for classification of the benign lesion is 70% and for classification of the malignant lesion is 63.3% with overall classification sensitivity is 66.6%. The kappa correlation coefficient value for diagnosis made by CNN-II is 0.333 and P < .05. Among the deadliest diseases is skin cancer, particularly melanoma. There is a lot of overlap between various skin lesions like melanoma and moles in the color skin photos. The challenge of discovery and diagnosis is made more challenging by nevus. To prevent unnecessary effort, time, and human life loss, an accurate automated method for skin lesion classification is required. This study offers a technique for categorizing skin lesions automatically. our system relies on the predictions of algorithm called CNN (Convolutional Neural Network). CNN are mainly used for finding patterns in images to identify classes, and categories and it learns from the data. The source of dataset is International Skin Imaging Collaboration (ISIC) and we chose to work with total 2836 images, from this experiment we got the accuracy of 85 to 91%. [ABSTRACT FROM AUTHOR]

Rationale: Sclerosing variant of adenoid cystic carcinoma (SACC) is a rare variant of adenoid cystic carcinoma (ACC) with only two cases in the literature. In this article, we report two cases of SACCs and attempt to explain the reason for sclerosis and its association with the disease outcome. Patient Concerns: The first patient is a 43-year-old Dravidian Indian female with a chief complaint of pain and swelling in the right posterior cheek region of four months' duration. The second patient is an 8-year-old boy with a well-defined swelling in the left cheek region. Diagnosis: Both these cases were diagnosed as sclerosing variants of ACC. Treatment: The tumour was surgically excised for both patients. Outcomes: Both the patients had no signs of residual disease/recurrence. Struma ovarii is a monodermal ovarian teratoma. This is an ovarian germ cell tumour with more than 50% ovarian tissue. It is a rare tumour, accounting for only 3-5% of all ovarian teratomas. Malignant transformation occurs very rarely, only in about 0.3% cases. The authors report a case of struma ovarii with a malignant transformation to papillary carcinoma thyroid. Not much information is present in the literature about this rare entity. Due to the rarity of this condition, there is a lack of uniformity of histological criteria of malignancy and its management is not universally accepted by physicians. Here, the authors present a rare case of struma ovarii with malignant transformation to papillary carcinoma thyroid. A 62-year-old female presented with a left ovarian mass. On histopathological examination of the surgical specimen of hysterectomy it was found that the ovary had a neoplastic growth with morphological features of papillary carcinoma thyroid. The final diagnosis was struma ovarii with papillary thyroid carcinoma. Postoperative period was uneventful for the patient. However, follow-up was not done.

Abstract Background CNS TB roughly correlates to 1–10% of all intracranial tumors. The spectrum of CNS-TB is wide and can present as tuberculous meningitis, tubercular abscess, tuberculoma, or hypertrophic pachymeningitis. Tuberculomas are usually located at the corticomedullary junction and periventricular region, as expected for hematogenous dissemination. They are mostly infratentorial in children and supratentorial in adults. They may present months to years after infection. Observations Here we have a 55-year-old female who presented with a history of headaches, sudden loss of consciousness, and seizures. Initial imaging showed a right parietal mass lesion appearing to be malignant on initial imaging which had a significant mass effect and midline shift. The patient was operated on for the same and histopathology showed chronic granulomatous lesion and tissue staining s/o tuberculosis. Lessons So a diagnostic option for infective etiology should be kept even for an intracranial supratentorial mass lesion.

Contarini syndrome refers to the presence of bilateral pleural effusions with different characteristics resulting from distinct aetiologies on each side. The syndrome was named after Francesco Contarini, the 95th Doge of Venice, who had right-sided transudative effusion possibly due to heart failure, and left-sided exudative effusion due to parapneumonic effusion. While bilateral pleural effusions typically exhibit similar characteristics and are secondary to the same underlying cause, such as heart failure or malignancy, there have been rare reports of bilateral effusions with different characteristics. In this case report, we present the case of a 53-year-old female with known breast carcinoma who presented with right-sided exudative pleural effusion and left-sided transudative pleural effusion, each with distinct aetiologies, thus establishing Contarini syndrome. This report highlights the importance of considering the possibility of two separate aetiologies for bilateral pleural effusions, which should not be overlooked in clinical practice.

We hypothesized that small molecule transcriptional perturbation could be harnessed to target a cellular dependency involving protein arginine methyltransferase 5 (PRMT5) in the context of methylthioadenosine phosphorylase (MTAP) deletion, seen frequently in malignant pleural mesothelioma (MPM). Here we show, that MTAP deletion is negatively prognostic in MPM. In vitro, the off-patent antibiotic Quinacrine efficiently suppressed PRMT5 transcription, causing chromatin remodelling with reduced global histone H4 symmetrical demethylation. Quinacrine phenocopied PRMT5 RNA interference and small molecule PRMT5 inhibition, reducing clonogenicity in an MTAP-dependent manner. This activity required a functional PRMT5 methyltransferase as MTAP negative cells were rescued by exogenous wild type PRMT5, but not a PRMT5E444Q methyltransferase-dead mutant. We identified c-jun as an essential PRMT5 transcription factor and a probable target for Quinacrine. Our results therefore suggest that small molecule-based transcriptional perturbation of PRMT5 can leverage a mutation-selective vulnerability, that is therapeutically tractable, and has relevance to 9p21 deleted cancers including MPM.
