Purpose: To study the anesthetic and hemodynamic effects of etomidate-remifentanil combination treatment in laparoscopic surgery. Methods: Patients scheduled for gynecological laparoscopic surgery (n = 120) were assigned to test and control groups (60 patients each). Etomidate combined with remifentanil anesthesia was used in the test group, while propofol-remifentanil combination anesthesia was used in the control group. The effect of anesthesia on awakening time, extubation time, pain relief time, systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) of patients before and after the extubation were observed and recorded for the two groups. Results: Excellent rating was 98.0 % in the test group, and was superior to the corresponding rating of 86.0 % in the control group. Anesthesia time, awakening time, extubation time and pain relief time were markedly shorter in the test group than in controls (p < 0.05). However, there were no significant differences in SBP, DBP and HR of patients with tracheal intubation between the two groups (p > 0.05). The results were similar in patients with laparoscopic placement. After laparoscopic placement and tracheal extubation, significant decreases in SBP and HR in the test group were seen, relative to control patients (p < 0.05). Conclusion: The anesthetic effect of etomidate combined with remifentanil is superior to that of propofol and remifentanil, and ensures stability of hemodynamic parameters such as SBP, DBP and HR during the period of anesthesia. Abstract Agonists and antagonists for galanin receptor subtypes GalR1-3 can be used as putative therapeutics targets for the treatment of various human diseases. However, effects of galanin and its N-terminal fragments on myocardial ischemia/reperfusion injury remain unclear. This study was designed to assess the ability of the full-length galanin (GWTLNSAGYLLGPHAIDNHRSFSDKHGLT-NH2, G1), the natural fragments WTLNSAGYLL-NH2 (G2) and WTLNSAGYLLGPHA (G3), and their modified analogs WTLNAAGYLL (G4) and WTLNSAGYLLGPβAH (G5) to limit acute myocardial infarction in rats in vivo. The peptides G2-5 were synthesized by the automatic solid phase method using Fmoc technology, purified by preparative HPLC and identified by 1H NMR spectroscopy and MALDI −TOF mass spectrometry. The peptides G1-5 were administered by i.v. bolus injection at the onset of reperfusion at doses of 0.25, 0.50, 1.0, 2.0 or 3.0 mg/kg. The optimal doses of the peptides G1-5 significantly reduced the infarction area and decreased the activity of CK-MB and LDH in blood plasma at the end of reperfusion compared with the control. Among the peptides studied, G5 showed high efficacy in reducing the infarct size and the activity of necrosis markers in blood plasma with no significant effect on hemodynamic parameters. The results suggest that a novel agonist for galanin receptors G5 may be a promising tool for the treatment of myocardial ischemia/reperfusion (I/R) injury. Objectives: Anxiety regarding dental procedures is a universal challenge for every patient and the treating dental surgeon. Measurement of heart rate, blood pressure, and oxygen saturation is now commonly accepted by the scientific community as an accurate and objective measurement of the patient's anxiety compared to other subjective assessments. The aim of this study was to assess the influence of educational videos demonstrating transalveolar extraction of mandibular third molars on hemodynamic parameters. Materials and Methods: The study was designed as a prospective clinical trial. The modified dental anxiety scale was used to assess the preoperative anxiety of the patient. Participating patients were divided randomly into two groups (verbal and video group). Each patient's hemodynamic parameters such as heart rate, blood pressure, and oxygen saturation were measured in the waiting area, sitting on the dental chair, incision, bone drilling, tooth elevation, suturing, and in the postoperative area. All the analysis was done using the SPSS version 18 software. Results: Overall, there were no significant differences between the two groups with respect to hemodynamic parameters. The mean heart rate and blood pressure of patients between both groups were comparatively consistent and did not very much from the onset of procedure to the end. On the other hand, oxygen saturation levels were statistically significantly higher in the video group at the onset of incision and drilling. Conclusions: Videos can be interpreted in different ways by patients. Overall, hemodynamic parameters are overall not influenced with videos. [ABSTRACT FROM AUTHOR]

Impedance cardiography (ICG) is a non-invasive tool for assessing the hemodynamic parameters. It has been used for diagnosing several cardiovascular diseases, such as heart failure, cardio-myopathy, and valvular diseases. Particularly, the valvular heart disease is characterized by the damage in one of the four heart valves: the mitral, aortic, tricuspid or pulmonary valves. The mitral valve insufficiency and the aortic valve stenos are the most frequent valve diseases in the world. In this paper, we propose to diagnosis the mitral valve insufficiency using the impedance cardiography technique. The study group consisted of 40 subjects (20 control subjects and 20 patients with mitral insufficiency). A parameter “I” is calculated from the impedance cardiogram waveform and it is used to differentiate control subjects from patients with mitral insufficiency. The parameter “I” was related significantly to the abnormalities of the impedance cardiogram waveform. We aim to investigate the correlations between hemodynamic parameters, penile rigidity grading, and the therapeutic effects of phosphodiesterase type 5 inhibitors using color Doppler flow imaging after intracavernosal injection in patients with erectile dysfunction. This study involved 164 patients. After intracavernosal injection with a mixture of papaverine (60 mg), prostaglandin E 1 (10 mg), and lidocaine (2%, 0.5-1 ml), the penile vessels were assessed using color Doppler flow imaging. Penile rigidity was classified based on the Erection Hardness Score system as Grades 4, 3, 2 or 1 (corresponding to Schramek Grades V to II). Then, the patients were given oral sildenafil (50-100 mg) and scored according to the International Index of Erectile Function (IIEF-5) questionnaire. The number of patients with penile rigidities of Schramek Grades II to V was 14, 18, 21, and 111, respectively. The IIEF-5 score was positively correlated with the refilling index of the penile cavernosal artery (r = 0.79, P< 0.05), the peak systolic velocity (r = 0.45, P< 0.05), and penile rigidity (r = 0.75, P< 0.05), and was negatively correlated with the end diastolic velocity (r = −0.74, P< 0.05). For patients with erectile dysfunction, both the IIEF-5 score after sildenafil administration, which is correlated with penile rigidity, and the hemodynamic parameters detected using color Doppler flow imaging may predict the effects of phosphodiesterase type 5 inhibitor treatment and could provide a reasonable model for the targeted-treatment of erectile dysfunction.

Background and Objective: The usual method of assessing depth of anesthesia, which is not sensitive and specific enough, is measuring hemodynamic parameters, autonomic changes and subjective symptoms including movement, sweating and lacrimation. This study aimed to determine the relationship between the depth of anesthesia and hemodynamic indices. Material and Methods: This correlational study was conducted on 57 patients undergoing elective hernia surgery in Shahid Beheshti Hospital ,Sabzevar in summer 2014. An identical anesthesia technique was used for all patients. Depth of anesthesia was monitored quantitatively by bispectral index (BIS) at 5-minute intervals, with simultaneous recording of heart rate and blood pressure. We analyzed the data using Anova and correlation coefficient. Results: The mean age was 45.54±13.46. The patients were males (n=36) and females (n= 21). Only 59.6 % of the patients experienced a normal depth of anesthesia. Hemodynamic parameters were not significantly correlated with anesthetic depth except for heart rate in the first 5 minutes (P= 0.013). Conclusion: Given that hemodynamic parameters have some limitations in determining the acceptable depth of anesthesia, we recommend using the monitors based on brain signal processing.

The investigations involved 290 patients (85 males and 205 females). The data of patients with non-alcoholic fatty liver disease (NAFLD) (steatosis), combined with overweight, and the data of control group patients with normal body weight without liver disease have been compared. Examined patients were divided into four age groups (according to World Health Organization criterion): 1st age group — from 25 to 34 years, 2nd — from 35 to 44 years, 3rd — from 45 to 59 years, 4th — from 60 to 75 years. Structural and hemodynamic parameters of great vessels of the neck, obtained in duplex investigation (ultrasound sonography in B-regime, color flow mapping and Doppler investigation), were analyzed. The changes of structural and hemodynamic parameters of great vessels of the neck (intima-media complex thickening, reduction in the linear velocity of blood flow) were determined. NAFLD (steatosis), associated with increased body weight, has a direct impact on the structural and hemodynamic state of great vessels of the neck. A correlation between NAFLD (steatosis), combined with increased body weight, and indicator of blood velocity — peak systolic flow velocity (p < 0.05) has been revealed. Violation of the structure and hemodynamics of the main vessels of the neck, detected during the complex duplex examination, is the result of direct impact of NAFLD (steatosis) and increased body weight and leads to the development of cardiovascular risk, mainly in groups of middle-aged and elderly patients.

Background: The tracheal tube is always considered to be the gold standard for laparoscopic surgeries. As conventional laryngoscopy guided endotracheal intubation evokes significant hypertension and tachycardia, we have used I-gel, second generation extraglottic airway device, in an attempt to overcome these drawbacks. We conducted this study to compare haemodynamic changes during insertion, efficacy of ventilation, and complications with the use of I-gel when compared with endotracheal tube (ETT) in laparoscopic surgeries. Materials and Methods: A total of 60 American Society of Anaesthesiologists physical status I and II adult patients undergoing elective laparoscopic surgeries were randomly allocated to one of the two groups of 30 patients each: Group-A (I-gel) in which patients airway was secured with appropriate sized I-gel, and Group-B (ETT) in which patients airway was secured with laryngoscopy - guided endotracheal intubation. Ease, attempts and time for insertion of airway device, haemodynamic and ventilatory parameters at different time intervals, and attempts for gastric tube insertion, and perioperative complications were recorded. Results: There was significant rise in pulse rate and mean blood pressure during insertion with use of ETT when compared to I-gel. Furthermore, time required for I-gel insertion was significantly less when compared with ETT. However ease and attempts for airway device insertion, attempts for gastric tube insertion and efficacy of ventilation were comparable between two groups. Conclusion: We concluded that I-gel requires less time for insertion with minimal haemodynamic changes when compared to ETT. I-gel also provides adequate positive-pressure ventilation, comparable with ETT. Hence I-gel can be a safe and suitable alternative to ETT for laparoscopic surgeries.

Background: Information obtained from arterial pulse waveforms (APW) can be useful for characterizing the cardiovascular system. To achieve this, it is necessary to know the detailed characteristics of APWs in different states of an organism, which would allow APW parameters (APW-Ps) to be assigned to particular (patho)physiological conditions. Therefore, our work aimed to characterize 35 APW-Ps in rats under the influence of isoflurane (ISO) and Zoletil/xylazine (ZO/XY) anesthesia and to study the effect of root extract from Acanthopanax senticosus (ASRE) in these anesthetic conditions., Methods: The right jugular vein of anesthetized rats was cannulated for the administration of ASRE and the left carotid artery for the detection of APWs from which 35 APW-Ps were evaluated., Results: We obtained data on 35 APW-Ps, which significantly depended on the anesthesia, and thus, they characterized the cardiovascular system under these two conditions. ASRE transiently modulated all 35 APW-Ps, including a transient decrease in systolic and diastolic blood pressure (BP) and heart rate or increases in pulse BP, dP/dt max , and systolic and diastolic areas. Whereas the transient effects of ASRE were similar, the extract had prolonged disturbing effects on the cardiovascular system in rats under ZO/XY but not under ISO anesthesia. This negative effect can result from the disturbance caused by ZO/XY anesthesia on the cardiovascular system., Conclusions: We characterized 35 APW-Ps of rats under ISO and ZO/XY anesthesia and found that ASRE contains compounds that can modulate the properties of the cardiovascular system, which significantly depended on the status of the cardiovascular system. Objective: Takayasu arteritis (TA) is a difficult disease to deal with because there are neither reliable clinical signs, laboratory biomarkers, nor a single noninvasive imaging technique that can be used for early diagnosis and disease activity monitoring. Knowledge of aortic hemodynamics in TA is lacking. This study aimed to fill this gap by assessing hemodynamics in patients with TA using image-based computational fluid dynamics (CFD) simulations., Methods: Eleven patients with TA were included in the present study. Patient-specific geometries were reconstructed from either clinical aortic computed tomography angiography or magnetic resonance angiography studies and coupled with physiological boundary conditions for CFD simulations. Key anatomical and hemodynamic parameters were compared with a control group consisting of 18 age- and sex-matched adults without TA who had healthy aortas., Results: Compared with controls, patients with TA had significantly higher aortic velocities (0.9 m/s [0.7, 1.1 m/s] vs 0.6 m/s [0.5, 0.7 m/s]; P  = .002), maximum time-averaged wall shear stress (14.2 Pa [9.8, 20.9 Pa] vs 8.0 Pa [6.2, 10.3 Pa]; P  = .004), and maximum pressure drops between the ascending and descending aorta (36.9 mm Hg [29.0, 49.3 mm Hg] vs 28.5 mm Hg [25.8, 31.5 mm Hg]; P  = .004). These significant hemodynamic alterations in patients with TA might result from abnormal anatomical features including smaller arch diameter (20.0 mm [13.8, 23.3 mm] vs 25.2 mm [23.3, 26.8 mm]; P  = .003), supra-aortic branch diameters (21.9 mm [18.5, 24.6 mm] vs 25.7 mm [24.3, 28.3 mm]; P  = .003) and descending aorta diameter (14.7 mm [12.2, 16.8 mm] vs 22.5 mm [19.8, 24.0 mm]; P  < .001)., Conclusions: CFD analysis reveals hemodynamic changes in the aorta of patients with TA. The applicability of CFD technique coupled with standard imaging assessments in predicting disease progression of such patients will be explored in future studies. Future large cohort study with outcome correlation is also warranted., Clinical Relevance: Based on patient-specific computational fluid dynamics simulations, the present retrospective study revealed significant difference in aortic hemodynamics between the patients with and without Takayasu arteritis (TA). To the best of our knowledge, this study is the first to evaluate hemodynamic conditions within TA, demonstrating the potential of computational flow modeling in capturing abnormal hemodynamic forces, such as high wall shear stress, resulted from irregular morphological changes. Objectives: Arterial stiffness has emerged as a surrogate marker of cardiovascular disease. We investigated the role of myocardial performance and hemodynamic parameters in arterial stiffness in patients with rheumatoid arthritis (RA), which is accompanied by excess cardiovascular risk.Design: Arterial stiffness was evaluated with pulse wave velocity (PWV) in RA patients and controls. Cardiac and hemodynamic characterization was based on impedance cardiography. Cardiovascular risk factors, inflammatory markers and disease-related parameters were assessed.Results: PWV (8.2 ± 2.1 vs 7.4 ± 1.4 m/s, p = .016) was higher among RA patients (n = 104) compared to controls (n = 52). In the RA group, PWV correlated with markers of cardiac contractibility (acceleration and velocity index), myocardial blood flow (cardiac output and stroke volume), preload (thoracic fluid content) and afterload (systemic vascular resistance) (p < .05 for all). PWV tended to increase with decreasing oxygen delivery to the myocardium (r = 0.055), as well as with shortening of the ejection duration of the left ventricle (p = .058). However, these associations no longer remained significant after adjustment for classical cardiovascular risk factors, inflammation and corticosteroid use, which were independently associated with PWV.Conclusions: Among patients with RA, arterial stiffness appears as the composite of cardiovascular risk factors and inflammation, while corticosteroid use emerges as an additional adverse factor. Background: The objective is to study the time-dependent hemodynamic effects of altiazem PP-180 and the blood circulation parameters in patients with arterial hypertension of the II degree in outpatient polyclinic conditions. Methods: The study included 78 patients with essential II grade AH aged 42 to 73 years (mean age of 57.65 ± 0.6). Among them were 28 men and 50 women. The duration of the disease ranged from 4 to 23 years (an average of 9.2 ± 0.12). All the groups involved in the research were divided into two subgroups: the main group and the control group. The control group included 30 patients, who took A-180 once a day at non-fixed hours. The main group included 48 patients, who were divided into random subgroups depending on the time they took A-180 once a day. Within 3 days prior to treatment and during the 10-day treatment course, the medical staff measured all the pateints' heart rate and blood pressure by NC Korotkoff every 3 hours, 6 times a day. The patients' heart rate and blood pressure were alternatively self-measured. Prior to treatment and at the end of the 10-day course of therapy with altiazem PP-180, all the patients involved in the research underwent ECG examination and echocardiography on the Aloka machine (Japan). Results: The most favorable hemodynamic support for the hypotensive effect of altiazem PP-180 was observed when patients took it at 7 a.m., 10 a.m. and 10 p.m. in their normal mode of work and rest. When patients took altiazem PP-180 at 1 p.m, 4 p.m. and 7 p.m., its hypotensive effect was due to less favorable hemodynamic support. Conclusions: When taken at different hours of the day - both fixed and non-fixed hours - altiazem PP-180 caused a distinct hypotensive effect in patients with the II degree AH in outpatient polyclinic conditions. Aim: Present study was done to evaluate the influence of addition of sufentanil to bupivacaine 0.5% heavy, on various characteristics of subarachnoid block, when given to parturients. Methodology: The present prospective randomized clinical study of 60 patients was carried out in the Department of Anesthesiology, Government Medical College and SSG Hospital, Baroda. Spinal anesthesia was given in lumbar intervertebral space L3-L4, with midline approach, using 23 G spinal needle. Patients were randomly divided into two groups, to receive either inj bupivacaine heavy 0.5% (Group B) or inj bupivacaine heavy 0.5% plus 10 μg sufentanyl (Group BS). Various parameters monitored were vital parameters, sensory block, motor block, neonatal outcome, intra-operative complications, postoperative analgesia and postoperative complications. Results: The mean time for onset of sensory block was 78.46 ± 2.32 sec in Group B and 37.93 ± 1.39 sec in Group BS. The mean onset of motor block in Group B was 59.2 ± 2.76 sec while in Group BS it was 51.93 ± 1.48 sec. The difference was statistically significant. Patients in Group B were alert (grade 0) intra-operatively whereas majority of patients in Group BS had grade II sedation, denotes that they were sleepy but arousable. Conclusion: Addition of 1ml (10 μg) sufentanil to 2 ml of bupivacaine heavy (0.5%) intrathecally hastens the onset and prolongs the duration of sensory and motor blockade. Acetylcholine is the main transmitter of the parasympathetic vagus nerve. According to the cholinergic anti-inflammatory pathway (CAP) concept, acetylcholine has been shown to be important for signal transmission within the immune system and also for a variety of other functions throughout the organism. The spleen is thought to play an important role in regulating the CAP. In contrast, the existence of a “non-neuronal cardiac cholinergic system” that influences cardiac innervation during inflammation has been hypothesized, with recent publications introducing the heart instead of the spleen as a possible interface between the immune and nervous systems. To prove this hypothesis, we investigated whether selectively disrupting vagal stimulation of the right ventricle plays an important role in rat CAP regulation during endotoxemia. We performed a selective resection of the right cardiac branch of the Nervus vagus (VGX) with a corresponding sham resection in vehicle-injected and endotoxemic rats. Rats were injected with lipopolysaccharide (LPS, 1 mg/kg body weight, intravenously) and observed for 4 h. Intraoperative blood gas analysis was performed, and hemodynamic parameters were assessed using a left ventricular pressure-volume catheter. Rat hearts and blood were collected, and the expression and concentration of proinflammatory cytokines using quantitative reverse transcription polymerase chain reaction and enzyme-linked immunosorbent assay were measured, respectively. Four hours after injection, LPS induced a marked deterioration in rat blood gas parameters such as pH value, potassium, base excess, glucose, and lactate. The mean arterial blood pressure and the end-diastolic volume had decreased significantly. Further, significant increases in blood cholinesterases and in proinflammatory (IL-1β , IL-6, TNF-α) cytokine concentration and gene expression were obtained. Right cardiac vagus nerve resection (VGX) led to a marked decrease in heart acetylcholine concentration and an increase in cardiac acetylcholinesterase activity. Without LPS, VGX changed rat hemodynamic parameters, including heart frequency, cardiac output, and end-diastolic volume. In contrast, VGX during endotoxemia did not significantly change the concentration and expression of proinflammatory cytokines in the heart. In conclusion we demonstrate that right cardiac vagal innervation regulates cardiac acetylcholine content but neither improves nor worsens systemic inflammation. [ABSTRACT FROM AUTHOR]

This randomized controlled study (pretest, posttest, test control group) was conducted with the aim of determining the effect ofmusic on the severity of dyspnea, anxiety, and hemodynamic parameters in patients with dyspnea. The research was conducted in the Chest Diseases Service of a public hospital in the western region of Turkey. The number of patients included in the sample was determined by power analysis, and as a result, 60 patients were accepted, 30 in the music group and 30 in the control group. Sixty percent of the patients who took part in the study were male, their mean age was 61.21 ± 1.13 years, and their mean dyspnea severity before the intervention was 62.68 ± 22.28. The difference between the mean dyspnea severity and anxiety scores of the music group patients before and after the application of music was found to be statistically significant (P G .05), but no statistically significant differencewas found in hemodynamic parameters (P > .05). In light of the research findings, it was concluded that music had an effect on anxiety and the severity of dyspnea in patients with dyspnea but that it had no effect on hemodynamic parameters. Ischemic hepatitis is an important, yet underdiagnosed pathological condition seen in either cardiology or hepatology clinics or intensive care units. The main causes are severe heart failure, circulatory and septic shock. Close monitoring of biological tests (AST, ALT, LDH) together with hemodynamic parameters (blood pressure, cardiac output and central venous pressure) allow for rapid and accurate diagnosis. This study aimed to investigate the effect of glutamate-evoked masseter muscle pain on intramuscular oxygenation during rest and sustained elevated muscle activity ( SEMA). Seventeen healthy individuals participated in two sessions in which they were injected with glutamate and saline in random order. Each session was divided into three, 10-min periods. During the first (period 1) and the last (period 3) 10-min periods, participants performed five intercalated 1-min bouts of masseter SEMA with 1-min periods of 'rest'. At onset of the second 10-min period, glutamate (0.5 ml, 1 M; Ajinomoto, Tokyo, Japan) or isotonic saline (0.5 ml; 0.9%) was injected into the masseter muscle and the participants kept the muscle relaxed in a resting position for 10 min (period 2). The hemodynamic characteristics of the masseter muscle were recorded simultaneously during the experiment by a laser blood-oxygenation monitor. The results demonstrated that glutamate injections caused significant levels of self-reported pain in the masseter muscle; however, this nociceptive input did not have robust effects on intramuscular oxygenation during rest or SEMA tasks. Interestingly, these findings suggest an uncoupling between acute nociceptive activity and hemodynamic parameters in both resting and low-level active jaw muscles. Thyroid hormone deficiency during fetal life (fetal hypothyroidism) causes intrauterine growth restriction (IUGR). Fetal hypothyroidism (FH) could attenuate normal cardiac functions in the later life of the offspring rats. The aim of this study was to evaluate the contribution of myomiR network and its target gene expression in cardiac dysfunction in fetal hypothyroid rats. Six Pregnant female rats were divided into two groups: Control consumed tap water, and the hypothyroid group received water containing 0.025% 6-propyl-2-thiouracil during gestation. Hearts from male offspring rats in adulthood (month 3) were tested with Langendorff apparatus for measuring hemodynamic parameters. Expressions of miR-208a, -208b, and -499 and its target genes including thyroid hormone receptor 1 (Thrap1), sex-determining region Y-box 6 (Sox6), and purine-rich element-binding protein β (Purβ) were measured by qPCR. FH rats had lower LVDP (%20), +dp/dt (%26), −dp/dt (%20), and heart rate (%21) than controls. FH rats at month 3 had a higher expression of β-MHC (190%), Myh7b (298%), and lower expression of α-MHC (36%) genes in comparison with controls. FH rats at month 3 had a higher expression of miR-499 (520%) and miR-208b (439%) and had lower expression of miR-208a (74%), Thrap1 (47%), Sox6 (49%), and Purβ (45%) compared with controls. Our results showed that thyroid hormone deficiency during fetal life changes the pattern of gene expression of myomiR network and its target genes in fetal heart, which, in turn, resulted in increased β-MHC expression and associated cardiac dysfunction in adulthood. Purpose: Although hemodynamic instability is expected during the post-cardiac arrest period, the implications of hemodynamic parameters for outcomes remain unclear. Each phase of targeted temperature management (TTM) affects hemodynamic responses differently. This study aims to investigate the association of hemodynamic parameters with outcomes in patients receiving TTM after cardiac arrest.Methods: The study prospectively enrolled patients who were treated with TTM (33°C for 24h) after cardiac arrest. We assessed clinical and hemodynamic variables at pre-specified time points in survivors and non-survivors as well as among those with favorable vs. poor neurologic outcomes at discharge.Results: The study analyzed the records of 95 patients who completed TTM; at discharge 54 (57%) were alive and 21 (22%) had favorable neurological outcomes. Heart rate(HR) at 24h>93/min, cardiac index (CI) at 12h<2.5l/min/m2 and lower average of mean arterial blood pressure(MAP) at 36h and 48h were independently associated with in-hospital mortality (P<0.05 by Cox regression analysis respectively). MAP at 48h between 84-110mm Hg and lower HR at 48h were significantly associated with better neurological outcome at discharge (P<0.05 for by logistic regression analysis respectively).Conclusions: Our results indicate that lower MAP and HR more than 93/min are associated with in-hospital mortality during the initial 48h after ROSC. CI at 12h<2.5l/min/m2 is associated with survival but not with neurological outcome. One of the important parts of the cardiac system is aorta which is the fundamental channel and supply of oxygenated blood in the body. Diseases of the aorta represent critical cardiovascular bleakness and mortality around the world. This study aims at investigation of hemodynamic parameters in a two-dimensional axisymmetric model of three-layer grafted aorta using fluid–structure interaction (FSI). It assumes that a damaged part of aorta, which may happen as a result of some diseases like aneurysm, dissection and post-stenotic dilatation, is replaced with a biomaterial graft. Four types of grafts materials so-called Polyurethane, Silicone rubber, Polytetrafluoroethylene (PTFE) and Dacron are considered in the present study. The assumption of linear elastic and isotropic material is set for the both aorta's wall and aforementioned grafts. Blood is considered as an incompressible and Newtonian fluid. The results indicate higher displacement in Polyurethane and silicone rubber in comparison with other two. Furthermore, results reveal that blood flow velocity has slightly higher values in PTFE and Dacron grafted models compared to Polyurethane and Silicone rubber ones. Even though there are some differences in hemodynamic patterns in these grafted models, they are not considerable as much as von Mises stresses across the graft-aorta intersections are. Patients receiving mechanical ventilation, due to stress, anxiety and stimulations of tracheal intubation, experience agitation that threatens the process of mechanical ventilation and also can cause dyspnea, confusion and asthenia. Agitation control by the medications and physical restraints, brings unpleasant side effects as well. Therefore, this study was performed to determine the effect of aromatherapy with lavender oil on agitation and hemodynamic parameters in mechanically ventilated patients. This quasi-experimental study was performed over 40 patients under mechanical ventilation. All patients who met inclusion criteria, were recorded their hemodynamic parameters and agitation severity before the intervention and then two drops of 2% lavender essential oil were added to 200 ml of distilled water and the aromatherapy was done within one hour. Hemodynamic and agitation parameters recorded every 15 minutes during the first hour and every 30 minutes during the second and third hours. Agitation severity and hemodynamic parameters were compared before and after aromatherapy and were analyzed by statistical methods [descriptive and inferential]. The findings showed that the scores of agitation and hemodynamic parameters, before and after aromatherapy with lavender were significant (P < 0.001). According to the results of this study, aromatherapy with lavender oil is effective on agitation and hemodynamic parameters in patients undergoing mechanical ventilation; therefore, it is suggested to be used as one of the non-drug and uncomplicated interventions in independent nursing duties to control patient’s agitation

Mechanisms of development, growth, and rupture of a cerebral aneurysm have been studied by computational fluid dynamics (CFD), calculating hemodynamic parameters, such as wall shear stress, oscillatory shear index (OSI), and relative residence time (RRT). It is well known that the upstream velocity profile and upstream vessel shape influence the computational results. However, few studies have dealt with cases involving a bifurcation upstream of a cerebral aneurysm. Furthermore, the fluid-dynamic effects of multiple structural elements of upstream vessel shape, such as a bifurcation and a bend, on the results of CFD remain unknown. The purpose of this study was to elucidate the fluid-dynamic effects of multiple structural elements of upstream vessel shape such as bifurcation and bend on blood flow and hemodynamic parameters inside an aneurysm. Computations were performed for blood flow in a cerebral aneurysm with upstream sequential elements of a bifurcation, a bend, and a straight section, using four models with different upstream boundaries, i.e., before the bifurcation, after the bifurcation before the bend, after the bend, and after the straight section just before the cerebral aneurysm. The results were compared to elucidate the effect of each upstream structural element on the blood flow and hemodynamic parameters in the cerebral aneurysm. Differences in OSI and RRT resulting from changes in the velocity distribution were observed locally in the aneurysm between the models. It was also found that the effect of the bifurcation on the velocity distribution was greater than that of the bend.

There is considerable interest in the pleiotropic effects of statins and their potential role in the treatment of pulmonary hypertension. Previous experimental findings indicate that a combination of lipophilic statins with phosphodiesterase type-5 inhibitor, sildenafil, can offer preventive effects on rat monocrotaline-induced pulmonary hypertension. The present study is aimed to assess whether therapeutic regimen provides any benefits. Seven days after pulmonary hypertension induction, hydrophilic rosuvastatin and sildenafil were given for 14 days to male Wistar outbred rats. Right ventricular pressure, right ventricle mass and three biomarkers were evaluated after 21 days: brain natriuretic peptide, high-density lipoprotein cholesterol and vascular endothelial growth factor. The present study demonstrates that administration of hydrophilic statin with sildenafil results in reduction of pulmonary vascular remodeling and right ventricular pressure. The results of biochemical measurements may suggest that statins play a positive role in right ventricle function or the process of angiogenesis in pulmonary hypertension development.

AIM: To observe the application effect of hyperbaric oxygen in the patients with optic atrophy and influence degree for the hemodynamic parameters.METHODS: Fifty patients with optic atrophy in our hospital from January 2012 to January 2014 were objected, they were randomly divided into control group(conventional optic atrophy treatment group)and observation group(conventional treatment and hyperbaric oxygen treatment group), each group was 25 cases. Statistical analysis of two group before and after treatment eyesight, vision acuity, visual field defect and ophthalmic artery, central retinal artery blood flow parameters were undergone.RESULTS: The sight, visual field sensitivity and field vision defect of observation group were all better than those of control group at first, second and third course after the treatment, arteriae ophthalmica and arteriae centralis retinae EDV and PSV were all higher than those of control group, PI and RI were all lower than those of control group were all significant differences(PCONCLUSION: The application effect of hyperbaric oxygen in the patients with optic atrophy is better, and the influence of treatment method for the ocular hemodynamic parameters are more active.

Background: Following head injury, cardiopulmonary functions are impaired and this disturbs the oxygenation transport pathway. Expanding cardiopulmonary physical therapy to encompass the oxygen transport system as a whole has implication for treatment as well as assessment and treatment outcome. Therefore, the aim of the study is to assess the oxygenation level in head injury patients with relation to body positioning in the intensive care unit (ICU). Methodology: Thirty consecutive patients with head injury with hemodynamically stable were included from the surgical ICU, ages ranging from 15 to 50 years. Noninvasive vital parameters (oxygen saturation [SpO2], pulse rate [PR], respiratory rate [RR], and blood pressure [BP]) were observed and recorded in different body positions at regular intervals of 5 min for 15 min in each position. Results: There was increment in SpO2 value in all positions from 0 min to end of 15 min in supine (98.63 ± 0.36-98.73 ± 0.30), right-side lying (98.77 ± 0.30-98.93 ± 0.20), left-side lying (98.73 ± 0.29-99.03 ± 0.24), and recline sitting (30°-70°) (99.03 ± 0.24-99.50 ± 0.22). However, there was statistically significant increment in recline sitting (30°-70°) compared to other positions (P = 0.036) while other parameters (PR, RR, and BP) were getting stabilized at lower values at end of 15 min in every positions tested. Conclusion: We conclude that upright position bring about significant increase in arterial SpO2 compared to any other positions. We investigated whether indices of the reflex regulation of the cardiovascular system, i.e., muscle ergoreflex activity (ERGO) and characteristics of the autonomic balance, differ from each other while measured in the vertical vs. horizontal positions in healthy humans. Ten young healthy men (mean age 28 ± 1 years; BMI 23.6 ± 0.8 kg/m) were examined. In each subject, the ERGO (ventilatory response to a control handgrip exercise, with repetitive isometric contractions at 50% of the maximal strength), baroreflex sensitivity (BRS, the sequence method), heart rate variability (HRV), and a few hemodynamic parameters were measured in two positions, horizontal and vertical. In the latter position, the heart rate, and diastolic and mean blood pressures (BP) were higher, while the stroke volume was smaller (all P < 0.05) with no differences in the systolic BP, cardiac output, and systemic vascular resistance (all P > 0.2). The HRV time domains, as well as BRS, decreased, whereas HRV-LF increased (all P < 0.05) in the vertical position as compared to the horizontal one. There were no differences between other HRV parameters (all P > 0.2) in the two examined positions. Minute ventilation during the exercise increased, while ERGO was reduced in the vertical position (all P < 0.05). Thus, the body position noticeably affects both reflex regulation of the cardiovascular system and autonomic balance. This observation has to be taken into account during physiological testing. Objective(s): Particulate matter (PM) exposure can promote cardiac ischemia and myocardial damage. The effects of PM10 on hemodynamic parameters, lipid peroxidation, and infarct size induced by ischemia-reperfusion injury and the protective effects of vanillic acid (VA) in isolated rat heart were investigated. Materials and Methods: Eighty male Wistar rats (250-300 g) were divided into 8 groups (n=10): Control, Sham, VAc, VA, PMa (0.5 mg/kg PM, intratracheal instillation), PMb (2.5 mg/kg PM, intratracheal instillation), PMc (5 mg/kg PM, intratracheal instillation), and PMc + VA (5 mg/kg PM, intratracheal instillation; and 10 mg/kg vanillic acid, gavage for 10 days). PM10 was instilled into the trachea in two stages, within 48 hr. After isolating the hearts and transfer to a Langendorff apparatus, hearts were subjected to 30 min ischemia and 60 min reperfusion. Hemodynamic parameters (±dp/dt, LVSP, LVDP, and RPP), production of lipid peroxidation (MDA), and infarct size were assessed. Results: A significant decrease in ±dp/dt, LVSP, LVDP and RPP occurred in PM groups. A significant increase in MDA and myocardial infarct size occurred in PM groups. A significant increase in LVDP, LVSP, ±dp/dt, RPP and decrease in infarct size, MDA, and myocardial dysfunction was observed in groups that received vanillic acid after ischemia-reperfusion. Conclusion: It was demonstrated that PM10 increases MDA, as well as the percentage of cardiac infarct size, and has negative effects on hemodynamic parameters. Cardiac involvement is common in rheumatoid arthritis. Subendocardial viability ratio (SEVR) is a non-invasive measure of microvascular coronary perfusion, yet it remains unclear whether it is affected in rheumatoid arthritis patients. We additionally sought predictors of SEVR in rheumatoid arthritis among a wide range of disease-related parameters, cardiac and hemodynamic factors, and markers of atherosclerosis, arteriosclerosis, and endothelial dysfunction. SEVR was estimated in rheumatoid arthritis patients and healthy controls by applanation tonometry, which was also used to evaluate arterial stiffness (pulse wave velocity and augmentation index). In the rheumatoid arthritis group, carotid intima-media thickness (cIMT) was additionally estimated by ultrasound, cardiac and hemodynamic parameters by impedance cardiography, and endothelial dysfunction by measurement of asymmetric dimethylarginine (ADMA). In a total of 122 participants, SEVR was lower among 91 patients with rheumatoid arthritis compared to 31 controls (141.4 ± 21.9 vs 153.1 ± 18.7%, p = 0.009) and remained so among 29 rheumatoid arthritis patients without hypertension, diabetes, or cardiovascular diseases, compared to the control group (139.7 ± 21.7 vs 153.1 ± 18.7%, p = 0.013). SEVR did not significantly correlate with arterial stiffness, cIMT, ADMA, or disease-related parameters. Multivariate analysis revealed gender ( p = 0.007), blood pressure ( p = 0.028), heart rate ( p = 0.025), cholesterol levels ( p = 0.008), cardiac index ( p < 0.001) and left ventricular ejection time ( p = 0.004) as independent predictors of SEVR among patients with rheumatoid arthritis. Patients with rheumatoid arthritis exhibit lower values of SEVR compared to healthy individuals. Cardiac and hemodynamic parameters, rather than functional indices of endothelial and macrovascular dysfunction, may be useful as predictors of myocardial perfusion in rheumatoid arthritis. The aim of this investigation was to evaluate the recovery pattern of a whole-body compression garment on hemodynamic parameters and on autonomic nervous system (ANS) activity after a swimming performance. Ten young male athletes were recruited and tested in 2 different days, with and without wearing the garment during the recovery phase. After a warm-up of 15 minutes, athletes were instructed to perform a maximal 400-m freestyle swimming event, and then time series of beat-to-beat intervals for heart rate variability (HRV), baroreflex sensitivity (BRS), and hemodynamic parameters were recorded for 90 minutes of recovery. The vagally mediated high frequency (HF) power of R-R intervals, NN50, and pNN50 showed a faster recovery due to the costume; meanwhile, the low frequency (LF) spectral component of HRV (LFRR) index of sympathetic modulation of the heart and the LF:HF ratio and BRS alpha index (αLF) were augmented in control than in garment condition. When athletes wore the swimsuit, cardiac output was increased and the returning of the blood to the heart, investigated as stroke volume, was kept constant because of the reduction of the total peripheral resistances. During control condition, heart rate (HR) was restored back to baseline value 20 minutes later with respect to garment condition, confirming that the swimsuit recover faster. The effectiveness of the swimsuit on ANS activity after a maximal aerobic performance has been shown with a greater recovery in terms of HRV and hemodynamic parameters. I conducted a prospective repeated-measure study in the general surgery intensive care unit to investigate the associations among acute postoperative pain, analgesic therapy, and hemodynamic parameters. I selected 33 patients and recorded 84 episodes of pain. I measured intensity of pain and hemodynamic parameters after patients were transferred from the postanesthesia care unit to the general surgery intensive care unit, immediately before analgesic therapy and at 15, 30, and 45 minutes after analgesic therapy. Acute pain increased systolic (SBP), diastolic (DBP), and mean arterial blood pressure (MAP); pulse rate (PR); and arterial oxygen saturation. Fifteen minutes after analgesic therapy, SBP and PR decreased, and DBP, MAP, and oxygen saturation increased. Thirty minutes after therapy, SBP, MAP, and PR decreased, and DBP and oxygen saturation increased. Forty-five minutes after therapy, SBP, MAP, and PR decreased, and DBP and oxygen saturation increased. I saw no significant hemodynamic parameter changes during postoperative episodes of pain. Human error is one of the leading causes of industrial accidents at modern manufacturing production sites. An adaptive asynchronous human-machine system (Async-HMS) was previously proposed as a new mechanism for preventing the reduction of cognitive level by controlling the actual operation period of machines that lead to monotonous behavior. The objective of this paper is to assess Async-HMS from the aspects of person's current capacity for performing processing resources. A dual-task method consisting of a synchronization task with three periodic fluctuations and four levels of difficulties of mental arithmetic tasks was conducted with Async-HMS on a PC. The three periodic fluctuations are constant periodic fluctuation (Cnst), perceptible periodic fluctuation (Supraliminal), and imperceptible periodic fluctuation (Subliminal). The task performance and the psychophysiological state were assessed through the dual-task method. In an ANOVA analysis, the time shared fraction, which is an index of processing resources devoted to an arithmetic task, had no significant effect on the synchronization task. The mean blood pressure in Subliminal had a significant increase compared with that in Cnst and Supraliminal. These results indicate that subliminal fluctuation evokes sympathetic hyperactivity without affecting processing resources. Implementation of subliminal operation period into industrial machines performing periodic behavior might inhibit monotony and, therefore, prevent human error. [ABSTRACT FROM AUTHOR]

Background: Lumbar spine surgery demands intense analgesia. Preemptive multimodal analgesia (MMA) is a novel approach to attenuate the stress response to surgical stimulus. Aims: The aim of the study was to assess the intraoperative morphine consumption in patients undergoing lumbar spine surgery. Patients and Methods: A randomized, prospective, double-blind study involving 42 patients belonging to the American Society of Anesthesiologists Class I and II scheduled to undergo elective lumbar spine surgery were allocated into two groups of 21 each. Group A (study group) received injection diclofenac sodium, paracetamol, clonidine, and skin infiltration with bupivacaine adrenaline and Group B (control group) received paracetamol and skin infiltration with saline adrenaline. Preemptive analgesia was practiced in both the groups. Intraoperative morphine consumption was documented. Statistical Methods: Intraoperative morphine consumption between the two groups was compared using Mann-Whitney U-test. Postextubation sedation score between the two groups was compared using Chi-square test and presented as number and percentage. P < 5% was considered statistically significant. Results: Intraoperative morphine consumption was significantly low in the study group (P < 0.001). Postextubation sedation score was comparable between the two groups. Conclusion: Preemptive MMA has demonstrated significant morphine sparing effect intraoperatively in patients undergoing lumbar spine surgeries. [ABSTRACT FROM AUTHOR]

Background: Lumbar spine surgery is associated with hemodynamic variations at intubation, change of position, and skin incision. A balanced anesthesia with multimodal analgesia (MMA) is necessary to attenuate these changes. Aim: To assess the relative effectiveness of preemptive MMA compared with the conventional analgesic regime in suppressing the hemodynamic response to endotracheal intubation, prone positioning, and skin incision. Settings and Design: A randomized, prospective study involving 42 patients belonging to the American Society of Anesthesiologists Physical Status 1 and II scheduled to undergo elective lumbar spine surgery were allocated into two groups of 21 each. Materials and Methods: Forty-two patients were randomly allocated into Groups A and B. Group A (study group) received diclofenac, paracetamol, clonidine, and bupivacaine with adrenaline skin infiltration and Group B (control group) injection paracetamol and saline with adrenaline skin infiltration. Statistical Analysis Used: Hemodynamic parameters (heart rate [HR], systolic blood pressure [SBP], diastolic blood pressure [DBP], and mean arterial pressure [MAP]) between the groups following intubation, prone position, and skin incision were noted and compared using repeated measure analysis of variance. One sample t-test was used to compare the standard mean concentration with the means of the study and control groups. P < 5% being considered statistically significant. Results: In the study group, HR, SBP, DBP, and MAP were lower at intubation and change of position as compared to the control group and were statistically significant. Hemodynamic stability is very much important during induction of general anesthesia. So, this study was planned to compare hemodynamic parameters during induction by thiopentone and propofol. This prospective study was conducted after approval from institutional ethics committee in non-hypertensive patients of ASA grade I and II aged between 18-60 yrs of either sex, admitted for different surgical procedure. The patients were randomized into group P (propofol) and group T (thiopentone). Heart rate, both systolic (SBP) and diastolic blood pressure (DBP) were recorded during induction and at 1 minutes, 2 minutes, 3 minutes and 4 minutes interval after intubation. Thirty patients in each group were included during study period. After induction, there was fall in both mean SBP and DBP after an increase during intubation which was more in group T (p<0.05). The statistically significant difference was observed only at 1min between two groups for SBP (P value <0.05). The mean heart rate was almost similar at pre induction time in both the groups (p>0.05). There was rise in heart rate during intubation in both the groups, thereafter heart rate started decreasing. The fall was similar in both the group at any given point of observation (P value >0.05). Both propofol and thiopentone alter the blood pressure and heart rate during induction in surgical patients which are more pronounced in thiopentone but these changes return close to baseline value earlier in case of propofol. Background: Subsequent to coronary angiography, procedures performed to prevent bleeding may cause pain in the patient. In this study, we aimed to determine the effect of acupressure on pain level and hemodynamic parameters in patients undergoing coronary angiography., Method: In this prospective, a two-arm (1:1), randomized controlled trial was conducted, with 124 patients undergoing coronary angiography included. The randomly assigned study group ( n  = 62) received acupressure on the LI4 (on the dorsum of the hand, between the 1st and 2nd metacarpal bones), PC6 (three fingers above the wrist), and LI11 (at the lateral end of the transverse cubital crease) points for 15 min 2 h after angiography, while the control group ( n  = 62) received no acupressure. Data were collected using the visual pain scale (VAS) and hemodynamic parameters [systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), respiratory rate (RR), and peripheral oxygen saturation (SpO 2 )], monitoring form before, immediately after, and at 10, 20, and 30 min after acupressure., Results: In the study, it was found that patients had moderate pain after coronary angiography (study group: 5.02 ± 2.27; control group: 3.98 ± 1.82). When the groups were compared, it was found that the VAS score of the study group before angiography was significantly higher than that of the control group, but lower than the control group immediately after acupressure, and at 10, 20, and 30 min after acupressure. In addition, it was determined that acupressure was significantly higher in DBP and RR in the study group compared to the control group at 20 and 30 min; it was not effective in terms of SBP, HR, and SpO 2 values., Conclusion: The results of the study indicated that patients reported moderate pain after coronary angiography, and that acupressure was effective in reducing the pain level, but affected only the DBP and RR hemodynamic parameters. Phthalic selenoanhydride (R-Se) solved in physiological buffer releases various reactive selenium species including H 2 Se. It is a potential compound for Se supplementation which exerts several biological effects, but its effect on the cardiovascular system is still unknown. Therefore, herein we aimed to study how R-Se affects rat hemodynamic parameters and vasoactive properties in isolated arteries. The right jugular vein of anesthetized Wistar male rats was cannulated for IV administration of R-Se. The arterial pulse waveform (APW) was detected by cannulation of the left carotid artery, enabling the evaluation of 35 parameters. R-Se (1-2 µmol kg -1 ), but not phthalic anhydride or phthalic thioanhydride, transiently modulated most of the APW parameters including a decrease in systolic and diastolic blood pressure, heart rate, dP/dt max relative level, or anacrotic/dicrotic notches, whereas systolic area, dP/dt min delay, dP/dt d delay, anacrotic notch relative level or its delay increased. R-Se (~10-100 µmol L -1 ) significantly decreased the tension of precontracted mesenteric, femoral, and renal arteries, whereas it showed a moderate vasorelaxation effect on thoracic aorta isolated from normotensive Wistar rats. The results imply that R-Se acts on vascular smooth muscle cells, which might underlie the effects of R-Se on the rat hemodynamic parameters.
