Ectopic tooth is not uncommon and usually occurs in the palate and maxillary sinus. We report a case of ectopic tooth located in the nasal cavity, a rare site. The mass depicted by CT was highly attenuated, and central lucency was observed.

Although the eruption of teeth into the nasal cavity is uncommon, the identification of such teeth can be important since they have the potential to cause considerable morbidity

Eighteen human molars with enamel pearls ranging in diameter from 0.8 to 2.7 mm were sectioned, acid-etched, and processed for SEM observation. In addition to pearl enamel, the specimens contained crown enamel for comparison. All pearls were of the composite type. The enamel layer reached maximal thicknesses of between 0.3 and 0.7 mm opposite the tip of the dentinal cone. The enamel structure was normal, but more variable and irregular than crown enamel. The prism course was often irregular throughout the whole thickness of enamel. Distinct Hunter-Schreger bands were absent. Prisms and interprism were occasionally difficult to identify as separate entities. The packing of crystals seemed to be less tight in pearl enamel. The scarcity of Retzius lines was attributed to the method of preparation. Prism cross-striations with a periodicity ranging between 1.3 and 4.5 μm were frequently encountered. A superficial prism-free zone with a regular 1.7-2.5-μm striation was present in localized regions of many pearls. The formation time of a medium-sized pearl was calculated to be about 11.5 months. A variety of cross-cut prism configurations was observed. Occasional hypoplastic lesions and hypomineralized areas were encountered. In conclusion, pearl enamel possesses most of the structural attributes of crown enamel, but in general its organization seems to be less orderly. Enamel pearls represent developmental disturbances in position and timing more than in structure.

Four cases have been presented involving malpositioned premolars and molars that were brought into the arch. From the cases presented, it appears that aggressive surgical intervention to redirect ectopic premolars creates significant secondary problems. Interference with the bone surrounding the ectopic tooth may compromise the adjacent teeth and bone level. Pressure against the root of the impacted tooth may cause resorption. If the buccal or labial plate is removed, orthodontic movement will be impeded. Specific biochemical changes in bone are induced by the application of orthodontic forces. In these cases, creating space with coiled spring appliances resulted in remarkable reorientation and proper eruption of ectopic, impacted teeth. When surgical intervention is required in cases involving ectopic teeth, close collaboration between orthodontist and oral and maxillofacial surgeon is imperative to achieve successful results without negative sequelae.

Background The etiology of ectopic third molars located in the sigmoid notch of the mandible is unclear. Only a few cases have been reported. The aim of this article is to discuss the etiology as well as treatment options and diagnostic imaging techniques. Methods A PubMed and Medline search of the literature from 1965 to 2015 to ectopic third molars in the mandibular notch was performed. Furthermore, a clinical case provided by the authors is reported. Results Among the eight reviewed cases, two male and six female patients were affected that ranged from 25 to 62 years of age (mean 48.4). Pain and swelling in the preauricular region or trismus but also the absence of symptoms was reported. Only in two of the summarized articles an extra-oral access for the removal of the tooth was used. The etiology seems to be individually different, however dentigerous cysts and chronic inflammation seem to play an important role in their appearance. While previous diagnostic reports described two-dimensional diagnostic imaging, currently the three-dimensional imaging is common for preoperative surgical planning with respect to removing ectopic molars. Conclusions Ectopic third molars in the mandible are a rare condition. The etiology seems to be individually different. Nowadays, three-dimensional imaging is common for preoperative surgical planning.

Ectopic eruption of teeth occurs in a variety of locations.Commonly seen in palate and maxillary sinus, they have alsobeen reported in nasal cavity, orbit, mandibular condyle,coronoid process, and facial skin.Here, we report a rare case of recurrent sinusitis caused byectopic tooth in right ethmoid sinus and successful endoscopicremoval of it. The symptoms of the patient disappeared aftersurgery. Such case has not been yet reported in the Englishliterature.Ectopic tooth is an uncommon cause of sinusitis. It shouldbe considered in patients presenting with recurrent sinusitis unresponsiveto medical treatment. Computed tomography of thesinuses is the modality of choice for diagnosis. Endoscopic sinussurgery is the best method for management of such cases.
