Abstract

Background The role of primary tumour surgery in pancreatic neuroendocrine tumours (PNETs) with unresectable liver metastases is controversial and international guidelines do not recommend surgery in such cases. Resectability of the primary tumour has never been considered in outcome comparisons between operated and non-operated patients. Methods From two institutional prospective databases of patients affected by PNET and unresectable liver metastases, 63 patients who underwent a left-pancreatectomy at diagnosis were identified and compared with a group of 30 patients with a potentially resectable but not-resected primary tumour located in the body or tail. The endpoint was overall survival (OS). Results The two groups significantly differed at baseline with regard to liver tumour burden Ki-67 labelling index, site of pancreas, results of the 18 FDG PET-CT and age. In the operated patients, surgical morbidity comprised 7 cases of pancreatic fistula. Postoperative mortality was nil. Median OS for patients undergoing left-pancreatectomy was 111 months vs 52 for the non operated patients (p = 0.003). At multivariate analysis after propensity score adjustment, no surgery as well as liver tumour burden>25% and higher Ki-67 index were associated with an increased risk of death during follow-up. In patients with unresectable primary tumour, OS was similar in comparison to that in the resectable but non-resected patients, and significantly worse than that in the resected patients (p = 0.032). Conclusion In PNETs located in the body or tail and diffuse liver metastases distal pancreatectomy may be justified in selected patients. Abstract

Coronary angioplasty and coronary artery bypass grafting (CABG) are both major techniques for the management of coronary artery disease, but CABG is associated with a lower incidence of repeat revascularization. Recent studies comparing angioplasty with stenting vs CABG have yielded conflicting results, with some suggesting improved survival with stenting, and others the opposite. We thus undertook a systematic overview of the randomized trials comparing stenting vs CABG in coronary artery disease.MEDLINE (January 1986-February 2003), ISI Current Contents, the Cochrane Controlled Trial Register, LILACS and the American Heart Association, American College of Cardiology, European Society of Cardiology, and Transcatheter Cardiovascular Therapeutics conference proceedings were among the databases we searched. Abstraction was performed in a non-blinded manner on pre-specified forms. The random-effect odds ratios for death, myocardial infarction, stroke, repeat revascularization, and symptomatic angina were computed for the longest available follow-up.Nine randomized trials (3283 patients, representing only 6% of all screened subjects) with an average follow-up of 28 months were included in the analysis, while four studies were excluded because they were still unpublished, ongoing, or with non-systematic stenting. No study used drug-eluting stents. The odds ratios for stenting vs CABG were 0.82 (95% confidence interval-CI 0.57-1.18, p = 0.3) for the occurrence of death, non-fatal myocardial infarction or stroke, 4.6 (95% CI 3.5-5.9, p0.00001) for repeat revascularization, and 2.3 (95% CI 1.8-2.8, p0.00001) for symptomatic angina. Heterogeneity tests were not statistically significant. The results of sensitivity analysis were similar even after stratification for single vessel, off-pump, single center or high-quality studies.Overall and event-free survival after conventional stenting for coronary artery disease are similar to those after CABG, but surgery is still associated with a significantly lower incidence of repeat revascularization and symptoms. The role of next-generation drug-eluting stents in widening the indications for stenting and overcoming restenosis will need to be assessed in future observational and randomized studies comparing stenting vs CABG.

Abstract

AIMS To compare the efficacy and safety of lanoteplase, a single-bolus thrombolytic drug derived from alteplase tissue plasminogen activator, with the established accelerated alteplase regimen in patients presenting within 6 h of onset of ST elevation acute myocardial infarction. METHODS AND RESULTS 15,078 patients were recruited from 855 hospitals worldwide and randomized in a 2:1 ratio to receive either lanoteplase 120 KU. kg(-1)as a single intravenous bolus, or up to 100 mg accelerated alteplase given over 90 min. The primary end-point was all-cause mortality at 30 days and the hypothesis was that the two treatments would be equivalent. By 30 days, 6.61% of alteplase-treated patients and 6.75% lanoteplase-treated patients had died (relative risk 1.02). Total stroke occurred in 1.53% alteplase- and 1.87% lanoteplase-treated patients (ns); haemorrhagic stroke rates were 0.64% alteplase and 1.12% lanoteplase (P=0.004). The net clinical deficit of 30-day death or non-fatal disabling stroke was 7.0% and 7.2%, respectively. By 6 months, 8.8% of alteplase-treated patients and 8.7% of lanoteplase-treated patients had died. CONCLUSION Single-bolus weight-adjusted lanoteplase is an effective thrombolytic agent, equivalent to alteplase in terms of its impact on survival and with a comparable risk-benefit profile. The single-bolus regimen should shorten symptoms to treatment times and be especially convenient for emergency department or out-of-hospital administration.

206. Risk of misdiagnosis and overtreatment in patients with main pancreatic duct dilatation and suspected combined/main-duct intraductal papillary mucinous neoplasms.

Abstract

Background Segmental/diffuse dilatation of the main pancreatic duct (MPD) is the typical feature of combined/main-duct intraductal papillary mucinous neoplasms (CMD-IPMNs). MPD dilation in IPMNs may be also expression of mucus hypersecretion/obstructive chronic pancreatitis (OCP). The aim of this study was to evaluate the presence and extension of MPD involvement by tumor/OCP and assess the risk of overtreatment. Methods Retrospective analysis of suspected CMD-IPMNs resected between January 2009 and October 2014 were included. Pathologic correlations among MPD dilatation, IPMN, and OCP was searched. Results Overall, 93 patients were resected for suspected CMD-IPMNs. At pathology, CMD-IPMNs were found in 69 patients (74%). Branch-duct IPMNs (BD-IPMNs) were found in 8 cases (9%), pancreatic ductal adenocarcinoma (PDAC) in absence of IPMN in 9 (10%), cystic neuroendocrine tumor (NET G2) in 1 (1%), serous cystadenoma in 2 (2%), and OCP alone/mucinous metaplasia in 4 patients (4%). Overall, 18 patients (19%) underwent an overtreatment because unnecessary (2 BD-IPMNs, 2 serous cystadenomas, and 4 OCPs only) or too extensive resections (9 CMD-IPMNs and 1 PDAC with associated OCP). In these, total pancreatectomy was the most common procedure (67%). Median size of MPD in IPMN-involved area was 12 mm compared with 7 mm when only OCP was found ( P < .05). Conclusion There is a considerable risk of overtreatment in patients with a preoperative morphologic diagnosis of CMD-IPMNs. Partial pancreatectomy with margin examination should be performed instead of upfront total pancreatectomy. Radiologic observation can be considered in asymptomatic patients with “worrisome” MPD dilatation (5–9 mm) and lacking other high-risk stigmata. [ABSTRACT FROM AUTHOR]

Abstract

Software as a service (SaaS) approach is a demand-driven application that provides network-based access to an integrated portfolio of applications across the virtual value chain of an enterprise. Therefore, the aim of the present paper is to investigate the organisational technological and economic factors, which influence the adoption of SaaS for customer relationship management (CRM) applications. The theoretical framework refers to transaction cost theory (TCT) and resource-based view (RBV) theory. The research is based on the case study method and analyses four companies of big dimensions, which have adopted the SaaS, approach at least for three years. The results show the importance of some organisational factors such as the optimisations of marketing processes, while other factors such as costs are strongly connected to the reduction of financial risks more than to the costs control in a strict meaning.

Abstract

Vasopressin is currently recommended in the management of patients with cardiac arrest, but its efficacy is still incompletely established. We systematically reviewed randomized trials comparing vasopressin to control treatment in the management of cardiac arrest in humans and animals. Two human and 33 animal studies were retrieved. At pooled analysis vasopressin appeared equivalent to adrenaline (epinephrine) in the management of human cardiac arrest (N=240), with, respectively 63 (78/124) vs 59% (68/116) ROSC (P=0.43), and 16 (20/124) vs 14% (16/116) survival to hospital discharge (P=0.52). In animal trials (N=669) vasopressin appeared instead significantly superior to both placebo (ROSC, respectively 93 [98/105] vs 19% [14/72], P<0.001) or adrenaline (ROSC, respectively 84 [225/268] vs 52% [117/224], P<0.001). In conclusion, vasopressin is superior to both placebo or adrenaline in animal models of cardiopulmonary resuscitation. Evidence in humans is still limited and confidence intervals estimates too wide to reliably confirm or disprove results obtained in experimental animal settings. © 2003 Elsevier Ireland Ltd. Abstract

A multicentre, randomised, open trial with a 2 x 2 factorial design was conducted to compare the benefits and risks of two thrombolytic agents, streptokinase (SK, 1·5 MU infused intravenously over 30-60 min) and alteplase (tPA, 100 mg infused intravenously over 3 h) in patients with acute myocardial infarction admitted to coronary care units within 6 h from onset of symptoms. The patients were also randomised to receive heparin (12 500 U subcutaneously twice daily until discharge from hospital, starting 12 h after beginning the tPA or SK infusion) or usual therapy. All patients without specific contraindications were given atenolol (5-10 mg iv) and aspirin (300-325 mg a day). The end-point of the study was the combined estimate of death plus severe left ventricular damage. 12 490 patients were randomised to four treatment groups (SK alone, SK plus heparin, tPA alone, tPA plus heparin). No specific differences between the two thrombolytic agents were detected as regards the combined end-point (tPA 23·1%; SK 22·5%; relative risk 1·04, 95% Cl 0·95-1·13), nor after the addition of heparin to the aspirin treatment (hep 22·7%, no hep 22·9%; RR 0·99, 95% Cl 0·91-1·08). The outcome of patients allocated to the four treatment groups was similar with respect to baseline risk factors such as age, Killip class, hours from onset of symptoms, and site and type of infarct. The rates of major in-hospital cardiac complications (reinfarction, post-infarction angina) were also similar. The incidence of major bleeds was significantly higher in SK and heparin treated patients (respectively, tPA 0·5%, SK 1·0%, RR 0·57, 95% Cl 0·38-0·85; hep 1·0%, no hep 0·6%, RR 1·64, 95% Cl 1·09-2·45), whereas the overall incidence of stroke was similar in all groups. SK and tPA appear equally effective and safe for use in routine conditions of care, in all infarct patients who have no contraindications, with or without post-thrombolytic heparin treatment. The 8·8% hospital mortality of the study population (compared with approximately 13% in the control cohort of the GISSI-1 trial) indicates the beneficial impact of the proven acute treatments for AMI.

219. Unregulated Pollutants from Tampered Two-wheelers

Abstract

A fleet of 9 two-wheelers (5 mopeds and 4 motorcycles) was tested in the Vehicle Emissions Laboratory of the European Commission Joint Research Centre to evaluate the effect of several tampering procedures on the tailpipe exhaust emissions. Vehicles performed prescribed driving cycles (ECE-R47 and WMTC) on a chassis dynamometer in a climatic test cell, and raw as well as diluted emissions were collected, the latter following the European legislation. After testing the vehicles in their original configuration, an intrusive tampering was applied and tests were repeated for comparison of the results. We found that the impact of tampering is vehicle- and compound-specific with a prevalence of negative effects on the exhaust emissions (regulated and unregulated). Remarkably in some cases, the tampering for improved vehicles’ performance increased the total hydrocarbons and carbon monoxide up to 27 and 15 times with respect to the original vehicles in hot engine conditions. Nitrogen oxides increased up to a factor of 2 when the tampering involved the intake air or hotter/leaner engine conditions. The results for unregulated compounds included ammonia (up to a 30 fold increase), nitrogen dioxide (up to 11 times), nitrous oxide (up to 2.5 times), acetaldehyde (up to 2.4 times), formaldehyde (up to 4 times), ethanol (up to 3.4 times) and aromatic compounds (up to 2 times). We show that tampering aimed at improving the vehicle's performance can have deleterious effects on the exhaust emissions, thus posing serious concerns on air quality.

Most of mortality worldwide occurs because of cancer diseases. Nanostructures are the new compounds that have become one of the most important technologies for using in different fields over the past two years especially in medicine. In between, nanotechnology has the potential to cancer detection and therapy. This study is a review of prospects in applications of nano-materials for cancer detection and treatment. We have summarized the nano-materials (metal nanospheres, nanorods, nanoshells and nanotubes) in medical applications targeting cancer. We also discuss advances in established nanoparticle technologies such as liposomes, polymer micelles, and functionalization about tumor targeting, controlled release and drug delivery. This paper will discuss the therapeutic applications of different nano-materials with a major focus on their applications for the treatment of cancer. Briefly, the toxicity of conventional nanostructures was also mentioned in this paper.

Abstract

The formation of biofilm (Biofouling) in different surface is the great concern in types of fields, especially in medical and health system as well as in membrane technology. The present study deals with the synthesis and characterization of Al2O3 nanopowders with antibacterial activity which can be a potentially utilized material for biocompatible implants. Nanostructure was synthesized based on sol-gel method and then, crystallite size, and microstructural and morphological characterization of nanostructure were determined by X-ray diffraction, electron-microscopic techniques - scanning electron microscopy (SEM) and transmission electron microscopy (TEM). According to X-ray diffraction, the value of particle size for Al2O3 nanopowders is 20.85 nm. In the following, the antibacterial activity of Al2O3 nanoparticles was assessed on three gram positive and three gram negative bacteria by radial diffusion assay and measurement of minimum inhibitory concentration (MIC). The toxicity of Al2O3 nanopowders on blood cells was also assessed. The results showed that this nanostructure has potent antibacterial activity against gram positive and gram negative bacteria. The synthesized Al2O3 nanopowders showed the antimicrobial activity against antibiotic resistant bacterium, Staphylococcus aureus. Significant antibacterial activity of this nanostructure was seen to have a greatest effect on Bacillus cereus with the MIC value of 9.2 μg/ml; while, among bacterial strains, Salmonella typhimurium was investigated to be the most resistant one with the MIC of 35.6 μg/ml. Al2O3 nanopowders showed no toxicity on blood cells. according to acquired data in this study, Al2O3 nanopowders may be a good material for inhibition of biofilm formation.

Abstract: In recent years, there has been a considerable interest in bioactive peptides derived from food proteins which might have beneficial effects on human health. Zizyphus jujuba is a medicinal plant with well-demonstrated biological functions for which various bioactive compounds except antioxidant peptides have been reported. Therefore, the aim of this study was to determine the antioxidant activity of Z. jujuba-derived protein hydrolysates and the purified peptides. Based upon this study, it was revealed that the hydrolysates prepared from Z. jujuba fruit possessed antioxidant effects. Among the prepared hydrolysates, trypsin hydrolysate with the highest antioxidant activity was fractionated using reverse-phase high-performance liquid chromatography (RP-HPLC). The most potent antioxidant peptides, named fractions F3 and F6, identified as VGQHTR (MW: 678.36±0.3Da) and GWLK (MW: 482.27±0.3Da), respectively, using tandem mass spectrometry. This study demonstrated that the derived hydrolysates and the purified peptides from Z. jujuba proteins can prevent oxidative reactions and might be underutilised for food preservation and medicinal purposes. Employing five country-level databases, it explores how different combinations of technology diffusion (that is, social media penetration) and social conditions (functional literacy and attitude toward science) affect the level of vaccine coverage. The analyses are based on two built ad-hoc longitudinal datasets of 36 and 40 countries worldwide, employing a fuzzy-set qualitative comparative analysis (fs-QCA) approach. The findings highlight that, at the country level, the absence of high social media penetration, combined with an above-average level of functional literacy and positive attitude toward science, enable vaccine coverage to reach the level recommended by health institutions and authorities. Solid estimates describing the clinical course of SARS-CoV-2 infections are still lacking due to under-ascertainment of asymptomatic and mild-disease cases. In this work, we quantify age-specific probabilities of transitions between stages defining the natural history of SARS-CoV-2 infection from 1965 SARS-CoV-2 positive individuals identified in Italy between March and April 2020 among contacts of confirmed cases. Infected contacts of cases were confirmed via RT-PCR tests as part of contact tracing activities or retrospectively via IgG serological tests and followed-up for symptoms and clinical outcomes. In addition, we provide estimates of time intervals between key events defining the clinical progression of cases as obtained from a larger sample, consisting of 95,371 infections ascertained between February and July 2020. We found that being older than 60 years of age was associated with a 39.9% (95%CI: 36.2–43.6%) likelihood of developing respiratory symptoms or fever ≥ 37.5 °C after SARS-CoV-2 infection; the 22.3% (95%CI: 19.3–25.6%) of the infections in this age group required hospital care and the 1% (95%CI: 0.4–2.1%) were admitted to an intensive care unit (ICU). The corresponding proportions in individuals younger than 60 years were estimated at 27.9% (95%CI: 25.4–30.4%), 8.8% (95%CI: 7.3–10.5%) and 0.4% (95%CI: 0.1–0.9%), respectively. The infection fatality ratio (IFR) ranged from 0.2% (95%CI: 0.0–0.6%) in individuals younger than 60 years to 12.3% (95%CI: 6.9–19.7%) for those aged 80 years or more; the case fatality ratio (CFR) in these two age classes was 0.6% (95%CI: 0.1–2%) and 19.2% (95%CI: 10.9–30.1%), respectively. The median length of stay in hospital was 10 (IQR: 3–21) days; the length of stay in ICU was 11 (IQR: 6–19) days. 245. COVID-19 and Guillain-Barré syndrome: A single-center prospective case series with a 1-year follow-up

Abstract

Single reports of Guillain-Barré syndrome (GBS) have been reported worldwide during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic. While case reports are likely to be biased toward uncommon clinical presentations, systematic assessment of prospective series can highlight the true clinical features and spectrum. In this prospective, observational study, we included all consecutive patients who developed GBS. In patients with SARS-CoV-2 infection as antecedent, the time-gap between the infection and GBS onset had to be ≤30 days. The referral was a neurological University Research Hospital, in the Italian Region more severely involved by the pandemic, and hospitalizing both COVID+ and non-COVID neurological diseases. Clinical, laboratory, cerebrospinal fluid, and electromyographic features of GBS diagnosed between March 2020 and March 2021 were compared to a retrospective series of GBS diagnosed between February 2019 and February 2020 (control population). Nasopharyngeal swab was still positive at GBS onset in 50% of patients. Mild-to-moderate COVID-related pneumonia, as assessed by X-ray (6 patients) or X-ray plus computerized tomography (2 patients) co-occurred in 6 of 10 patients. GBS diagnosed during the pandemic period, including 10 COVID-GBS and 10 non–COVID-GBS, had higher disability on admission (P= .032) compared to the GBS diagnosed between February 2019 and 2020, possibly related to later hospital referral in the pandemic context. Compared to non–COVID-GBS (n = 10) prospectively diagnosed in the same period (March 2020–2021), post–COVID-GBS (n = 10) had a higher disability score on admission (P= .028), lower sum Medical Research Council score (P= .022) and lymphopenia (P= .025), while there were no differences in GBS subtype/variant, severity of peripheral involvement, prognosis and response to treatment. Cerebrospinal fluid search for SARS-CoV-2 RNA and antiganglioside antibodies were negative in all COVID+ patients. Temporal clustering of cases, coinciding with the waves of the pandemic, and concomitant reduction of the incidence of COVID-negative GBSs may indicate a role for SARS-CoV-2 infection in the development of GBS, although the association may simply be related to a bystander effect of systemic inflammation; lack of prevalence of specific GBS subtypes in post–COVID-GBS also support this view. GBS features and prognosis are not substantially different compared to non–COVID-GBS.

Abstract

In patients with dilated cardiomyopathy (DCM) of different aetiologies, a variable frequency of improvement in the left ventricular (LV) systolic function has been reported, while in patients with a ‘classic’ idiopathic DCM, the frequency of improvement is still under debate, and clinical and haemodynamic predictors of recovery of the LV function are needed. The aim of the present study was to determine the frequency of improvement in the LV systolic function in idiopathic DCM and to identify predictors of reversibility of the impaired LV contractility. A sample of 98 consecutive patients with idiopathic DCM was retrospectively evaluated. Echocardiographic and Doppler measurements were directly taken from the routine echo-report. LV systolic function was assessed semiquantitatively using a score index (SFSI). According to the improvement in the LV systolic function, the patients were divided into group 1 patients with improvement, and group 2 patients without improvement. During a follow-up of at least 12 months, 19 patients (19%) showed an improvement, with a significant increase in the mean SFSI; all these group 1 patients survived without heart transplant; in group 2, 18 patients (23%) died and 3 (4%) received a heart transplant. Patients in group 1 had a significantly shorter duration of symptoms (P = 0.0045), a younger age (P = 0.006), a shorter DtE (P = 0.04), a lower SFSI (P < 0.01), a worse NYHA class (P < 0.001) and more frequently had a history of hypertension (P < 0.0001). The same variables were significant predictors of improvement at the univariate analysis. At the multivariate logistic regression analysis, a shorter duration of symptoms (P = 0.02), a history of hypertension (P = 0.003), and a worse NYHA class (P = 0.01) were independent predictors of improvement. A relatively large percentage of patients with an idiopathic DCM will have a marked improvement in the LV systolic function. This is more likely to happen in the presence of a short duration of symptoms and a history of hypertension. After an improvement, the prognosis is excellent. [ABSTRACT FROM PUBLISHER]

The paper discusses some ways in which vagueness and its phenomena may be thought to impose certain limits on our knowledge and, more specifically, may be thought to bear on the traditional philosophical idea that certain domains of facts are luminous, i.e., roughly, fully open to our view. The discussion focuses on a very influential argument (due to Tim Williamson) to the effect that almost no such interesting domains exist. Many commentators have felt that the vagueness unavoidably inherent in the description of the facts that are best candidates for being luminous plays an illicit role in such argument. The paper centres around the idea that vagueness brings with itself the prima facie plausibility of soritical principles. Using the diagnostics of sharpenings, it is first pointed out that, despite certain considerations to the contrary, the margin‐for‐error principle required by the anti‐luminosity argument may well derive all of its plausibility from an underlying soritical principle. The notion of confidence that is relevant to the argument is then isolated and sharply distinguished from the notion of subjective probability. Against this background, it is argued that the reasoning about confidence involved in the argument in favour of the problematic margin‐for‐error principle is fallacious in the same way in which sorites reasoning is. This reveals the possibility of having reliable knowledge even at the penumbral limit with falsity, a possibility for which a concrete formal model is constructed. The model in turn permits a deeper appreciation of the role played in the dialectic by the distinction between confidence and subjective probability as well as by confidence requirements on knowledge. It is concluded that careful heeding of vagueness and its phenomena, far from forcing new and surprising limits on our knowledge, actually removes one of the main barriers – unreliability – often thought to stand in its way.
